Rotational stability is key for optimizing postoperative visual outcomes because even a small degree of rotation of a toric intraocular lens (IOL) from its target axis can result in a significant reduction in astigmatic correction. This systematic review and meta-analysis evaluated the rotational stability of toric IOLs of different lens models and haptic designs. All published studies and clinical trials that investigate postoperative rotation of toric IOLs were searched and evaluated. Quality of studies was assessed using the Methodological Index for Nonrandomized Studies scale. A single-arm metaanalysis was performed in R4.3.1 software with subgroup analysis performed based on lens model and haptic design. 51 published studies of 4863 eyes were included in the meta-analysis. The pooled mean absolute rotation of all toric IOLs was 2.36 degrees (95% CI, 2.08-2.64). Postoperative rotation is dependent on many aspects of lens material and design. Methods: The review evaluates mathematical models in tear film dynamics and their limitations, discusses contact lens wear models, and highlights computational mechanical models. It also explores computational techniques, customization of models based on individual blinking dynamics, and non-invasive diagnostic tools like high-speed cameras and advanced imaging technologies. Results: Mathematical models provide insights into tear film dynamics but face challenges due to simplifications. Contact lens wear models reveal complex ocular physiology and design aspects, aiding in lens development. Computational mechanical models explore eye biomechanics, often integrating tear film dynamics into a Multiphysics framework. While different computational techniques have their advantages and disadvantages, non-invasive tools like OCT and thermal imaging play a crucial role in customizing these Multiphysics models, particularly for contact lens wearers. Purpose: Defocus Incorporated Soft Contact (DISC) lenses, a commonly used type of multifocal lens in clinical practice, may slow down myopia progression by inducing myopic retinal defocus. The purpose of this study was to explore whether the induced defocus across the retina could be affected by visual environments encountered in the real world, such as differences in viewing distance and ambient illuminance. Methods: In this cross‐over trial, 30 myopic adults wore both DISC lenses and single vision contact (SVC) lenses in random order. An open‐view Hartmann–Shack scanning wavefront sensor was used to measure defocus at different retinal locations along the horizontal meridian under four experimental conditions: far target (3 m) and near targets (0.33 m) under scotopic (<1 lux) or photopic (~300 lux) conditions. Results: The results showed that DISC lenses induced more myopic retinal defocus than SVC lenses in all conditions (all p < 0.05), except for the scotopic near target. In addition, for DISC lenses, the defocus was greater in the photopic than the scotopic conditions for both the far and near targets (both p < 0.05). Conclusion: In conclusion, the retinal defocus induced by these multifocal lenses was dependent on both visual distance and ambient illuminance, indicating that the visual conditions might affect the anti‐myopia efficacy of these devices. Purpose: Employing "relative plus" (Add) power to extend the functional vision range is a primary method to correct presbyopia with contact lenses. Simultaneous vision contact lenses are typically associated with visual disturbances at higher Add powers, often resulting in compromised vision and necessitating specialized fitting methods. Among mature individuals suffering from presbyopia, we evaluated the visual performance of a catenary curve-based extended depth of focus (EDOF) optical profile contact lens with a simplified fitting process. Methods: Mature individuals suffering from presbyopia with Add requirements of +2.00 D or more were recruited. Monocular and binocular visual acuities were obtained across optical vergences ranging from − 4.00 D to +2.00 D to generate defocus curves for best spectacle-corrected distance vision (baseline) and center-distance, catenary curve-based contact lenses (catenary). A subjective questionnaire evaluating the lenses was employed. Results: Twenty-four mature individuals suffering from presbyopia, average age 59.2 (range: 51– 68 years) and average Add requirement of +2.24 D (range +2.00 D to +2.50 D) were enrolled. Under high-contrast conditions, the catenary lens provided functional binocular vision (0.30 logMAR or better) at all optical vergences from distance to − 3.50 D (equivalent to 28 cm). Participants demonstrated a significant improvement (p < 0.05) in binocular visual acuity while wearing the catenary lens with an imposed defocus of − 1.50 D to − 4.00 D (equivalent to object distances from 66 cm to 25 cm). Subjective ratings with the catenary lens were equivalent to those documented at baseline. Conclusion: The catenary curve-based lenses provided a full range of functional vision while maintaining clear distance vision for individuals suffering from advanced presbyopia. Comparison with previous results involving this lens indicates that these findings are also generalizable to wearers with lower Add requirements. Purpose: This study addressed the utilisation of machine learning techniques to estimate tear osmolarity, a clinically significant yet challenging parameter to measure accurately. Elevated tear osmolarity has been observed in contact lens wearers and is associated with contact lens‐induced dry eye, a common cause of discomfort leading to discontinuation of lens wear. Methods: The study explored machine learning, regression and classification techniques to predict tear osmolarity using routine clinical parameters. The data set consisted of 175 participants, primarily healthy subjects eligible for soft contact lens wear. Various clinical assessments were performed, including symptom assessment with the Ocular Surface Disease Index and 5‐Item Dry Eye Questionnaire (DEQ‐5), tear meniscus height (TMH), tear osmolarity, non‐invasive keratometric tear film break‐up time (NIKBUT), ocular redness, corneal and conjunctival fluorescein staining and Meibomian glands loss. Results: The results revealed that simple linear regression was insufficient for accurate osmolarity prediction. Instead, more advanced regression models achieved a moderate level of predictive power, explaining approximately 32% of the osmolarity variability. Notably, key predictors for osmolarity included NIKBUT, TMH, ocular redness, Meibomian gland coverage and the DEQ‐5 questionnaire. In classification tasks, distinguishing between low (<299 mOsmol/L), medium (300–307 mOsmol/L) and high osmolarity (>308 mOsmol/L) levels yielded an accuracy of approximately 80%. Key parameters for classification were similar to those in regression models, emphasising the importance of NIKBUT, TMH, ocular redness, Meibomian glands coverage and the DEQ‐5 questionnaire. Conclusions: This study highlights the potential benefits of integrating machine learning into contact lens research and practice. It suggests the clinical utility of assessing Meibomian glands and NIKBUT in contact lens fitting and follow‐up visits. Machine learning models can optimise contact lens prescriptions and aid in early detection of conditions like dry eye, ultimately enhancing ocular health and the contact lens wearing experience. Purpose: To determine if factors pertaining to corneal thickness and elevation were linked to the prescribed back optic zone radius and overall diameter in keratoconus (group I), and cross-linked keratoconus (group II), cases successfully fitted with rigid contact lenses. Patients and Methods: A review of cases adapted to contact lens wear (Rose K™) and assessed using Pentacam™ (for corneal anterior surface topography, astigmatism, elevation, and topographic thickness variation) was undertaken to determine the significance of any correlation with comfort, lens usage, grading of keratoconus, corrected distance visual acuity (with glasses and contacts), refractive error, prescribed lens powers, back optic zone radius and overall diameter values were assessed. Results: Chief findings were, comfort was rated highly, lenses were worn for more than 10 hours/day in 75% of cases. RCL corrected distance logMAR acuities (median, mode, inter-quartile ranges) were 0.11, 0.12 (0.08– 0.13) and 0.08, 0.08 (0.06– 0.11) in groups I and II, respectively. Multiple linear regression revealed significant correlations (p< 0.01) between the [A] back optic zone radius, the thinnest value of corneal thickness (x1) and anterior corneal surface elevation (x2). The respective r2 values were 0.471 in group I and 0.512 in group II. [B] overall diameter, x1 and x2. The respective r2 values were 0.282 in group I and 0.505 in group II. Conclusion: RCLs were well-tolerated in both groups. The r2 values imply there is a 50% chance of correctly predicting the suitable back optic zone radius in both groups, a 50% chance of correctly predicting the overall diameter in group II and 28% in group I cases using just x1 and x2. The thinnest value of corneal thickness and anterior corneal surface elevation could be used to quickly select the BOZR and OD during RCL fitting. Identification of the baseline chracteristics for children undergoing orthokeratology with relatively fast myopia progression can allow a more accurate determination of the risk/benefit ratio. This study aimed to investigate if baseline corneal biomechanics can classify relatively slow and fast myopia progression in children. Children aged six to 12 years with low myopia (0.50 to 4.00 D) and astigmatism (less than or equal to 1.25 D), were recruited. Participants were randomised to be fitted with orthokeratology contact lenses with a conventional compression factor (0.75 D, n = 29) or an increased compression factor (1.75 D, n = 33). Relatively fast progressors were defined as participants who had axial elongation of 0.34 mm or above per 2 years. A binomial logistic regression analysis and a classification and regression tree model were used in the data analysis. The corneal biomechanics were measured with a bidirectional applanation device. The axial length was measured by a masked examiner. As there were no significant between-group differences in the baseline data (all p > 0.05), data were combined for analysis. The mean ± SD axial elongation for relatively slow (n = 27) and fast (n = 35) progressors were 0.18 ± 0.14 mm and 0.64 ± 0.23 mm per 2 years, respectively. The area under the curve (p2area1) was significantly higher in relatively fast progressors (p = 0.018). The binomial logistic regression and classification and regression tree model analysis showed that baseline age and p2area1 could differentiate between slow and fast progressors over 2 years. Purpose: To improve the effectivity of patient-specific finite element analysis (FEA) to predict refractive power change (RPC) in rigid Ortho-K contact lens fitting. Novel eyelid boundary detection is introduced to the FEA model to better model the effects of the lid on lens performance, and stress and strain outcomes are investigated to identify the most effective FEA components to use in modelling. Methods: The current study utilises fully anonymised records of 249 eyes, 132 right eyes, and 117 left eyes from subjects aged 14.1 ± 4.0 years on average (range 9 to 38 years), which were selected for secondary analysis processing. A set of custom-built MATLAB codes was built to automate the process from reading Medmont E300 height and distance files to processing and displaying FEA stress and strain outcomes. Measurements from before and after contact lens wear were handled to obtain the corneal surface change in shape and power. Tangential refractive power maps were constructed from which changes in refractive power pre- and post-Ortho-K wear were determined as the refractive power change (RPC). A total of 249 patient-specific FEA with innovative eyelid boundary detection and 3D construction analyses were automatically built and run for every anterior eye and lens combination while the lens was located in its clinically detected position. Maps of four stress components: contact pressure, Mises stress, pressure, and maximum principal stress were created in addition to maximum principal logarithmic strain maps. Stress and strain components were compared to the clinical RPC maps using the two-dimensional (2D) normalised cross-correlation and structural similarity (SSIM) index measure. Results: On the one hand, the maximum principal logarithmic strain recorded the highest moderate 2D cross-correlation area of 8.6 ± 10.3%, and contact pressure recorded the lowest area of 6.6 ± 9%. Mises stress recorded the second highest moderate 2D cross-correlation area with 8.3 ± 10.4%. On the other hand, when the SSIM index was used to compare the areas that were most similar to the clinical RPC, maximum principal stress was the most similar, with an average strong similarity percentage area of 26.5 ± 3.3%, and contact pressure was the least strong similarity area of 10.3 ± 7.3%. Regarding the moderate similarity areas, all components were recorded at around 34.4% similarity area except the contact pressure, which was down to 32.7 ± 5.8%. Conclusions: FEA is an increasingly effective tool in being able to predict the refractive outcome of Ortho-K treatment. Its accuracy depends on identifying which clinical and modelling metrics contribute to the most accurate prediction of RPC with minimal ocular complications. In terms of clinical metrics, age, Intra-ocular pressure (IOP), central corneal thickness (CCT), surface topography, lens decentration and the 3D eyelid effect are all important for effective modelling. In terms of FEA components, maximum principal stress was found to be the best FEA barometer that can be used to predict the performance of Ortho-K lenses. (1) Background: Cataract surgery has evolved significantly with the development of multifocal and extended depth-of-focus intraocular lenses (IOLs), driven by increasing patient desire for spectacle independence. (2) Methods: This retrospective, single-center study conducted on 86 eyes from 59 patients aged 40–80 years compared the performance and patient satisfaction of Symfony® and Synergy® (Johnson & Johnson Vision) IOLs during a follow-up of 1 year postsurgery. Uncorrected and corrected distance, intermediate, and near visual acuities were assessed (UDVA, CDVA, UIVA, CIVA, UNVA, and CNVA, respectively). (3) Results: Although both IOLs demonstrated a commendable distance VA, Synergy® outperformed in near VA (UNVA, p = 0.040; CNVA, p = 0.014), and Symfony® slightly excelled in intermediate VA (UIVA, p = 0.014; CIVA, p = 0.040). The defocus curve of Synergy® maintained a higher VA even at −4 D with a smoother curve and a broad landing zone. Although the optical quality assessments were similar, Symfony® had a nonsignificant edge. Patients indicated higher satisfaction and reduced reliance on spectacles with Synergy® despite more frequent reports of glare and halos. (4) Conclusions: These findings highlight the importance of personalized IOL selection in cataract surgery, which must be customized to apply the distinctive advantages of each IOL to address the unique visual requirements and lifestyle of patients. Background: The spatial position of the lens in patients with cortical age-related cataract (CARC) is unclear. We investigated a basis for the assessment of visual quality after cataract surgery by analysing the ultrasound biomicroscopic characteristics of the biological parameters of the lens in patients with CARC. Methods: In this retrospective study, 119 patients (50 males and 69 females, totalling 238 eyes) with CARC who underwent simple cataract surgery were selected. The lens thickness (LT), axial length (AL), anterior chamber depth (ACD), lens vault (LV), trabecular-iris angle (TIA), iris-lens angle (ILA), iris-lens contact distance (ILCD) were measured by A-scan ultrasound and ultrasound biomicroscopy. The corresponding lens position (LP) and relative lens position (RLP) were calculated. Results: LP was greater in men than in women (P < 0.05), LV was smaller in men than in women (P = 0.002), ILA and ILCD were not statistically significant (P = 0.072 and P = 0.854, respectively). There were significant differences in TIA, ILA, and ILCD in the four quadrants (all P < 0.05), with a trend in the distribution of TIA: superior < inferior < nasal < temporal, ILA: nasal < inferior < temporal < superior, and ILCD: superior < temporal < inferior < nasal. Conclusions: The lens protrudes more obviously in females than in males and the lens tilts to a certain extent with the increase of age and tends to be more upward and temporal in the supine position. Purpose: To investigate the agreement between Pentacam and CASIA2 in the evaluation of corneal densities (CDs) and lens densities (LDs) in myopes. Methods: Fifty-three patients (106 eyes) underwent comprehensive ophthalmologic examinations. CDs and LDs were measured using Pentacam and CASIA2, respectively, based on the grayscale percentage of the obtained images. Agreement between Pentacam and CASIA2 was evaluated using the consistency intraclass correlation coefficient (ICC) and represented using Bland-Altman plots. Results: Compared to Pentacam, CASIA2 showed significantly higher CD and LD values in all measured zones. The ICC of the average CD and LD measured by the Pentacam and CASIA2 were 0.726 and 0.757, respectively. The ICC values of all corneal zones and lenses were above 0.7, except for the measurement of the cornea in the 0–2 mm zone (0.455), suggesting good consistency between the two devices, whose results were of different levels of linear correlation. Bland-Altman plots showed mean percentages of 3.93% for the points falling outside the limits of agreement among the densitometry results. The ICCs in different age groups were similar, but the agreement was poorer in the high myopia group (low and moderate myopia, CD: 0.739, LD: 0.753; high myopia, CD: 0.621, LD: 0.760). Conclusions: CASIA2 demonstrated good consistency with Pentacam in the measurement of CD and LD, except for measurement of CD in the central cornea and in high myopia. Despite difference in the numerical results compared with Pentacam, which made the two devices uninterchangeable, CASIA2 provides a reliable alternative densitometric measurement method. Introduction: The relationship between anterior pelvic tilt and overall sagittal alignment has been well-described previously. However, the relationship between pelvic tilt, frontal, and axial leg alignment remains unclear. The aim of the study was to analyze the relationship between pelvic tilt and frontal and axial leg alignment in healthy subjects.Material and Methods: Thirty healthy subjects (60 legs) without prior surgery underwent standing biplanar long leg radiograph. Pelvic parameters (pelvic tilt, pelvic incidence, sacral slope), hip-knee-ankle angle (HKA), femoral antetorsion and tibial torsion were measured using SterEOS (EOS Imaging) software. EOS was acquired with the feet directing straight anteriorly, which corresponds to a neutral foot progression angle (FPA). The influence of HKA, femoral antetorsion, tibial torsion and gender on pelvic tilt was analyzed in a univariate correlation and multiple regression model.Results: Sixteen female subjects and 14 male subjects with a mean age of 27.1 years ± 10 (range 20-67) were included. HKA, femoral antetorsion, and tibial torsion correlated with anterior pelvic tilt in univariate analysis (all p < 0.05). Anterior pelvic tilt increased 1.1° (95% CI: 0.7 to 1.5) per 1° of knee valgus (p < 0.001) and 0.5° (95% CI: 0.3 to 0.7) per 1° of external tibial torsion (p < 0.001). Overall, linear regression model fit explained 39% of variance in pelvic tilt by the HKA, femoral antetorsion and tibial torsion (R2 = 0.385; p < 0.001).Conclusion: Valgus alignment and increasing tibial torsion demonstrated a weak correlation with an increase in anterior pelvic tilt in healthy subjects when placing their feet anteriorly. The relationship between frontal, axial leg alignment and pelvic tilt needs to be considered in patients with multiple joint disorders at the hip, knee and spine. Alteration of the frontal, or rotational profile after realignment surgery or by implant positioning might influence the pelvic tilt when the FPA is kept constant. [ABSTRACT FROM AUTHOR]

Introduction - Myopia is a refractive error where parallel rays coming from infinity focus before the retinal plane. Globally the prevalence of myopia has increased, and its progression can be controlled by various methods, including peripheral defocusing lenses, such as the MiSight® 1-day contact lenses. Objectives - Evaluate the progression of myopia in children who wore MiSight® 1-day contact lenses. Methods - This was an observational, descriptive, retrospective, and case series study. Data was collected from clinical registries in an Optician's office, where the contact lenses were fitted. Children with myopia who started treatment with MiSight® 1 day contact lenses to control myopia progression were included. Treatment efficacy was assessed by measuring refractive error (spherical equivalent) before, during, and after initiation of treatment. Results - Five children were included, two males and three females, aged between seven and fifteen years. The duration of treatment ranged from 19 to 61 months. Myopia progression during treatment ranged from 0.00 to -1.75 D. Discussion - All children in the study showed a decrease in myopia progression compared to myopia progression before treatment (0.00 and -2.25 D), as well as to what would be expected without treatment (-1.10 and -2.75 D). Scleral contact lens fitting has provided practitioners with one of the earliest pieces of evidence that the sclera was more often asymmetric than symmetric. Some preliminary observations such as different haptic alignment patterns over the sclera in 360 degrees, the appearance of fogging in the fluid chamber and excessive tear out-in flow through specific meridians, quadrants, or areas of the haptic provide evidence of some scleral asymmetry. The advent of technologies that allowed measurement of the scleral profile led to formal research confirming that only about 6% of scleras are symmetric, while the rest are toric, quad-specific, or irregular. This has an evident impact on how to focus scleral lens fitting. Measuring the scleral profile also provided us with true ocular sagittal height data for cord diameters beyond the cornea. Although high variability was expected in pathological eyes, healthy eyes also showed a wide range of ocular sagittal heights. Due to this variability among healthy eyes, a discussion has emerged on whether the one-size-fits-all approach to soft lens fitting is a proper strategy to fit the whole spectrum of sagittal heights. Inter-instrument variation in anterior corneal shape (ACS) measurement has a consequence for ocular clinical practice. To consider inter-instrument variability in keratometry measurements across the ACS and to explore instrument protocols for determining ACS keratometric analogues (KAs). Mean keratometry/KAs of the right eye were recorded using Javal-Schiøtz keratometer (J-S), Orbscan IIz and Pentacam from 124 subjects (78 females; mean ± SD age: 24.71 ± 6.61 years). Mean radii of curvature were obtained for 1-mm wide annular zones extending up to 6 mm (horizontally) and 4 mm (vertically) from the apex for Orbscan and Pentacam. Zonal mean radius of curvature was calculated by averaging keratometry values for all measured points within the zone. KA (mean ± SD): Horizontal: Orbscan (7.80 ± 0.31 mm) and J-S (7.82 ± 0.29 mm) were not significantly different (p = 0.072). Pentacam (7.86 ± 0.29 mm) was significantly flatter than J-S (p < 0.001) and Orbscan (p < 0.001). Vertical: Orbscan (7.64 ± 0.31 mm) was significantly steeper than J-S (7.67 ± 0.29 mm, p < 0.005) and Pentacam (7.70 ± 0.29 mm, p < 0.001). Pentacam was significantly flatter than J-S (p < 0.001) and significant flatter than Orbscan across the entire profile (1-4 mm zones horizontal and vertical, p < 0.001). LoAs (CI): J-S/Orbscan: ±0.75 mm (0.05–0.18); J-S/Pentacam: ±0.72 mm (0.01–0.12); Pentacam/Orbscan: ±0.16 mm (0.04–0.08). There was a +0.03 mm positive bias for Orbscan compared to J-S, +0.06 mm positive bias for Orbscan compared to Pentacam and −0.03 mm negative bias for Pentacam compared to J-S. Algorithms used by Orbscan and Pentacam to solve the peripheral paraxial ray problem produce significantly different KAs. Instrument-specific KAs cannot be used inter-changeably between instruments. Objectives: The aim of the study was to evaluate the fitting process, clinical performance, and patient satisfaction of hybrid contact lenses (HCL) in patients with keratoconus (KC). Materials and Methods: Sixty-eight KC patients (35 female, 33 male) who were prescribed HCL were included in the study. Corneal topographic parameters, best corrected visual acuity (BCVA) with eyeglasses, the number of HCL trials, prescribed HCL base curve (BC), and visual acuity with HCL were recorded from hospital records. A contact lens satisfaction survey was sent to the patients via email or WhatsApp and the data was statistically analyzed using IBM SPSS Statistics version 22.0. Results: The study included 110 eyes of 68 patients with a mean age of 27.34±8 years (range: 12-48 years). According to the Amsler-Krumeich classification, 35.5% of the eyes were stage 1, 50.9% were stage 2, and 13.5% were stage 3 or 4. Mean K1, K2, and Kmean values were 7.14±0.50 mm (range 5.72-8.30 mm), 6.63±0.49 mm (range 5.07-7.84 mm), and 6.89±0.48 mm (range 5.39-8.06 mm), respectively. The average number of lens trials was 1.59±0.82 (range 1-4). The mean BC of the prescribed HCL was 6.84±0.50 mm (range 5.60-8.00 mm). BCVA with glasses was 0.36±0.2 (range 0.05-0.8), and 0.80±0.14 (range 0.3-1.0) with HCL (p<0.0001). The overall survey score was 3.54 out of 5, the overall satisfaction score was 3.27, the average vision satisfaction score was 3.62, the average satisfaction score for lens insertion and removal was 3.01, and the average satisfaction score for lens comfort was 2.97. Conclusion: Prescribed HCL BC is usually close to the topographic Kmean value and in most of the patients, fitting was successful with the first or second CL trial. The overall satisfaction score was moderate to good and the disadvantages were low comfort compared to soft CL, difficulty with insertion and removal, short lens life, and high cost. Background: The basis of the overcorrecting minus lens is to induce compliance and consequently prevent constant exotropia. Some previous studies advocated early surgical therapy and others suggested over-minus treatment. Our purpose is to evaluate the success rate of the over-minus lens. Methods: This descriptive cross-sectional study was carried out on 106 patients under the age of 7 years with intermittent exotropia (IXT) who attended Amir-Al-Momenin Hospital at Guilan University of Medical Sciences, Iran. The data was gathered by a form including sex, age, level of cycloplegic refraction, the amount of deviation before and after using the over-minus glasses, visual acuity, the amount of the over-minus glasses, duration of treatment, recovery, and follow-up. The success rate was defined as decreasing exotropia to less than ten prism diopters or exophoria. Results: A total of 106 patients with a mean age of 2.25 ± 0.74 years were enrolled in this study. The mean exotropia before and after treatment was 20.96 ± 8.20 and 12.16 ± 11.04 prism diopters, respectively, and there was a statistically significant difference (P < 0.002). The mean refractive spherical and astigmatic errors (cycloplegic refraction) were +1.34 ± 1.07 and -0.32 ± 0.72 diopters, respectively. At the end of the follow-up, exotropia increased in 5.6% of patients, there was no change in 15% of patients with a mean deviation of 25.0 ± 6.06 prism diopters, and 79.24% of patients were treated successfully. Conclusions: According to the results of this study, treatment of IXT by over-correcting lenses can be a safe procedure and effective in preventing exotropia. [ABSTRACT FROM AUTHOR]

Objectives: We aimed to compare the clinical results and topographic data of the new generation hybrid contact lens (HCL) and rigid gaspermeable contact lens (RGPCL) in patients with moderate and advanced keratoconus. Materials and Methods: In this prospective study, HCL users comprised group 1 and RGPCL users comprised group 2. Snellen uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), and lens-corrected visual acuity (LCVA); manifest spherical-cylindrical values; corneal topography measurements (flat keratometry [K1], vertical keratometry [K2], mean K, maximum K [Kmax], central corneal thickness [CCT], and thinnest corneal thickness [TCT]); and cone location were recorded. Results: The study included 83 eyes of 51 patients in group 1 and 61 eyes of 40 patients in group 2. The groups were similar in age and gender (p>0.05). Mean LCVA (logMAR) was significantly lower than BCVA in both groups (p<0.001). The mean visual gain with contact lenses (Snellen chart) was 3.4±1.8 lines in group 1 and 4.0±2.1 lines in group 2. There was no significant difference between the two groups in BCVA, LCVA, or lines gained (p>0.05). There was also no significant difference between the two groups in terms of keratoconus stages, mean Kmax, CCT, TCT, or cone location (p>0.05), while mean UCVA (logMAR) and mean K were higher in group 2 (p<0.05). In both groups, the visual gain with lenses was higher in eyes with central cones, and there was significantly greater visual increase in group 2 (p=0.039). Conclusion: In moderate and advanced keratoconus, HCLs improved vision as much as RGPCLs and both lenses were more effective for central cones. Purpose: This retrospective analysis of electronic medical record (EMR) data investigated the prescribing patterns of soft myopia control contact lens (MCCL) treatments since their introduction in Ireland in 2017. Methods: Anonymised EMR data were sourced from 33 optometry practices in Ireland from 2017 to 2021 to determine the number of practices prescribing MCCLs to myopic children 5–18 years old. In MCCL‐prescribing practices, the proportion of contact lens wearing children fitted with MCCLs and the proportion of progressive (≤−0.25 D/year) myopic children fitted with MCCLs were determined. Logistic regression was used to determine which factors influenced the likelihood of being prescribed a MCCL. Results: Overall, just 10 practices were found to prescribe MCCLs of any type. The Coopervision MiSight contact lens was used in 85% of all MCCL fittings with most other fits being off‐label multifocals. The use of MCCLs rose from 3% of contact lens fits in 2017 to 27% in 2021. Children fitted with MCCLs were on average younger (12.2 ± 2.3 years vs. 15.4 ± 2.1 years) but more myopic (−3.46 ± 1.84 D vs. −3.03 ± 1.69 D) than those fitted with standard contact lenses. The most predictive factors for being fitted with MCCLs were year of examination (OR: 2.54, 95% CI: 2.13, 3.03), younger age (OR: 1.52, 95% CI: 1.39, 1.64) and greater myopia (OR: 1.25, 95% CI: 1.11, 1.39). Conclusion: Clinician engagement in myopia management has increased in Ireland since the formal introduction of MCCLs, but more than two‐thirds of practices included are yet to offer this form of myopia management. The proportion of children with progressive myopia that has been prescribed MCCLs has increased, but the majority of children are still managed for vision correction only. There is significant scope for improving the uptake of evidence‐based myopia control treatments and for optimising the age and degree of myopia at which such interventions are initiated. Purpose To compare the measurements of the angle of deviation through spectacles versus contact lenses and with the theoretically calculated angle. Patients and methods A total of 20 patients with comitant strabismus, more than 20 PD with spectacles, associated with bilateral myopia (spherical equivalent >1 D in both eyes) were included. Angle of deviation was measured using prism and cover test with spectacles and with contact lenses fitted according to the cycloplegic refraction and refined by subjective refraction. Agreements between the measurements obtained with contact lenses and those obtained with spectacles as well as the theoretically calculated angle were analyzed. Results The mean age was 15.6±10.9 years. Average spherical equivalent was -9.1±4.2 D. Measurements obtained with contact lenses were significantly lower than those with spectacles (P<0.001) with coefficient of variation of 27.7%, indicating poor agreement of both measurements. Limits of agreement in Bland-Altman plots were more than 9 PD (d±2s, -8.8 to 6 PD). The agreement was lower with larger angles. Measurements obtained with contact lenses were significantly lower than theoretical angles (P=0.002) but coefficient of variation between both measurements was 8.8%, indicating good agreement of measurements. There was little systematic or proportionate bias between the contact lens measurements and the theoretical angle. Conclusions Contact lens measurements were significantly lower than spectacle measurements in myopic patients. The contact lens measurements were in good agreement with the theoretically calculated angle. This overestimation of the angle with spectacles might be responsible for overcorrection of myopic patients with exotropia after surgery. [ABSTRACT FROM AUTHOR]

This article discusses a study that aimed to determine if the irregular astigmatism and optical aberrations in keratoconus (KC) patients could affect the accuracy of optical coherence tomography (OCT) evaluations in the macular region. The study compared macular thickness and volume in KC eyes with and without scleral contact lenses using OCT. The results showed no significant differences in macular thickness or volume with or without contact lenses. To report the potential effect of lens epithelial-mesenchymal transition (EMT) following lens capsular reopening in three patients with long term chronic intraocular inflammation and mildly elevated intraocular pressure. Observational study Although the three patients had different histories of eye surgery and had experienced a long process of diagnosis and treatment, they had the following similarities: 1) They had undergone cataract surgery; 2) All of them had capsular bag opening or reopening and unexplained intraocular inflammation and elevated intraocular pressure for a long time, even up to more than one year; 3) The inflammation was eventually disappeared following complete clearance of the EMT derived material. Our findings highlight the critical role of EMT derived material and capsular bag reopening in long-term post cataract surgery inflammation and pseudophakic ocular hypertension, and complete clearance of EMT derived material with surgical intervention should be considered if necessary. Notwithstanding continuing improvements in contact lens technology and increasing use of daily disposables, discontinuations from soft contact lens wear remain a significant problem with 'drop-out' too often occurring after patients have initially achieved successful routine use. This review examines the possibility that the signs and symptoms of discontinuations might, for example be associated with corneal and/or lid wiper neuropathy and/or aqueous, mucin and lipid deficiencies and/or mental health disorders. In addition, this review examines how soft contact lens wear places high demands on tear functions. Limitations in clinically detecting evidence of some adverse responses to soft contact lens wear may explain how discontinuations can sometimes be described as unexplained. Similarly, limitations in clinically determining the significance of different forms of tear dysfunction and their exacerbation by soft contact lens wear may be a barrier to explaining why symptoms of dryness are most commonly associated with discontinuations. The wider experience obtained with cosmetic contact lens fittings that is discussed in this review may be relevant to problems encountered in therapeutic contact lens applications. In this study, the spectral and optical properties of organic spectacle lenses used as visual aids were analyzed using Jasco V-730 UV/VIS spectrophotometer device in the ultraviolet and visible light wavelength ranges. The results obtained showed that the light transmittance in the wavelength (550nm) range to which the human eye is most sensitive in the lens samples examined was over 80% and that Polycarbonate (A3) lenses had a maximum value of 97.87%. The largest cut-off edge wavelength value was found to be 390nm for the A3 lens. At 550 nm for all lenses, the absorption spectrum was below 0.07. In terms of visual quality, it is expected that the visible light transmittance is high and the ultraviolet light transmission is minimal. Our eyes are exposed to ultraviolet rays almost every day, and these rays cause damage to the ocular tissues. The degree of damage caused by the amount of ultraviolet light absorption increases. Among the organic lenses with ultraviolet protection coating, the protection of the A3 lens was relatively higher. Furthermore, optical band gap energies were found to range from 3.145 – 4.155 eV and 2.994 – 3.936 eV for direct and indirect transitions, respectively. Background: Keratoconus (KC) is a progressive condition that usually affects young persons between their first and fourth decades. Myopia and irregular astigmatism are the common presentations which are proceeded by corneal steepening and thinning. Keratoconus is a progressive ectasia of the cornea that presents bilaterally, although often asymmetrical. Aim: To explore the experiences of KC patients attending public eye care facilities in Capricorn District of Limpopo province. Setting: Capricorn District, Limpopo province, South Africa. Methods: Descriptive qualitative phenomenology approach was used in the study to explore the lived experiences of patients diagnosed with KC, attending public eye care facilities. Purposive sampling was used to select 16 patients who were diagnosed with KC and referred for contact lens fitting. Data were collected through face-to-face, one-on-one interviews. Results: Patients reported to have developed gradual vision loss which worsened when they grew older. There was lack of knowledge of KC amongst patients and this was exacerbated by limited health literacy and vocabulary barriers. Patients had difficulty performing daily activities where some had dropped out of school while others worked at a slower pace and reduced working distances. Conclusion: Patients with KC received inadequate information on their condition due to limited health literacy and vocabulary barriers. Programmes to promote practitioner and patient education are required to improve the perceived service level provided to KC patients. To compare the efficacy and safety between traditional lens fitting and computer-aided fitting methods for orthokeratology (OrthoK) in the Chinese population. A multi-center, examiner-masked, randomized controlled study was conducted with a one-year follow-up period, enrolling 280 participants with spherical equivalent (SE) ranging from −0.5D to −4.0D. Participants were assigned to either the computer-aided orthokeratology fitting group (trial group) or the traditional lens fitting group (control group) using stratified randomization based on age (8 to 13 years, 13 to 18 years, and ≥ 18 years) to ensure a minimum of 30 cases in each sub-age group. Ocular examinations included visual acuity, objective and subjective refraction, corneal endothelial cell density, corneal topography, intraocular pressure, axial length, and ocular health assessment. Successful lens-correction was defined as the residual refraction with the OK lens, which should not exceed ± 0.5D, and/or an uncorrected visual acuity of no worse than 0.1 logMAR. Statistical analysis involves t-tests, analysis of variance, and Chi-squared tests. 215 subjects were included in the statistical analysis (109 in the trial group and 106 in the control group). In both groups, compared to baseline data, the uncorrected visual acuity (UCVA) improved significantly, with SE reduced and central corneal curvature flattened greatly after wearing OrthoK lens (P < 0.05 for all). Compared to the control group, the trial group exhibited a higher successful rate in correcting UCVA (93.6 % vs. 84.0 %, P = 0.03) and slightly better correction in refraction (77.1 % vs. 66.0 %, P = 0.07) at 1-month follow-up. However, no significant differences were observed in the axial length elongation, corneal changes, or the incidence of adverse events between the two groups. These findings indicate the higher efficiency and slightly better performance in correcting myopia and improving UCVA of computer-aided lens fitting approach compared to the traditional one, but similar outcomes in controlling axial elongation. With advances in contact lens fitting for keratoconus management and the implementation of UVA-Riboflavin Crosslinking (CXL), the landscape of keratoplasty indications is undergoing changes. Procedures and indications in a single tertiary care center over the past two decades are reviewed in this retrospective analysis. Methods: Retrospective analysis utilized anonymized electronic data from the LIONS cornea bank Baden-Württemberg, Eye Center Freiburg, spanning from beginning of 2004 to end of 2023. The primary focus was on surgical procedures and indications for keratoplasty.The study encompassed a comprehensive analysis of 7130 corneal transplants. Penetrating keratoplasty (PK) exhibited a significant decline from 95% (n = 206, 2004) to 11% (n = 46, 2023), while DMEK increased from 48% (n = 157, 2012) to 82% (n = 347, 2023). Fuchs endothelial dystrophy (FED) emerged as the dominant indication, witnessing a significant increase from 24% (2004, n = 39) to 72% (2023, n = 288). Conversely, keratoconus (KC) showed a reciprocal change from 25% (2004, n = 40) to 4% (2023, n = 17). PK demonstrated a noteworthy indication shift in descending order from Bullous Keratopathy (BK), FED, and KC in 2004 to KC, graft failure, corneal scars, and ulcers in 2023. With advances in contact lens fitting for keratoconus management and the implementation of UVA-Riboflavin Crosslinking (CXL), the landscape of keratoplasty indications is undergoing changes. Procedures and indications in a single tertiary care center over the past two decades are reviewed in this retrospective analysis. Methods: Retrospective analysis utilized anonymized electronic data from the LIONS cornea bank Baden-Württemberg, Eye Center Freiburg, spanning from beginning of 2004 to end of 2023. The primary focus was on surgical procedures and indications for keratoplasty.The study encompassed a comprehensive analysis of 7130 corneal transplants. Penetrating keratoplasty (PK) exhibited a significant decline from 95% (n = 206, 2004) to 11% (n = 46, 2023), while DMEK increased from 48% (n = 157, 2012) to 82% (n = 347, 2023). Fuchs endothelial dystrophy (FED) emerged as the dominant indication, witnessing a significant increase from 24% (2004, n = 39) to 72% (2023, n = 288). Conversely, keratoconus (KC) showed a reciprocal change from 25% (2004, n = 40) to 4% (2023, n = 17). PK demonstrated a noteworthy indication shift in descending order from Bullous Keratopathy (BK), FED, and KC in 2004 to KC, graft failure, corneal scars, and ulcers in 2023. Repeat keratoplasty following DMEK remained rare, but a discernible upward trend was observed for PK.This retrospective analysis highlights significant changes in both keratoplasty indications and techniques over the past two decades. DMEK has emerged as a successful intervention for treating endothelial diseases, while PK retains its qualitative indispensability for a wide range of indications. Modern corneal banks are urged to maintain a robust supply of grafts for all surgical techniques in anticipation of potential increased demand in the future, particularly for repeat keratoplasties.What is known• Over the past two decades, lamellar keratoplasty procedures such as DSAEK and DMEK have increasingly replaced penetrating keratoplasty (PK) in clinical practice due to their improved outcomes and reduced complications for certain indications.New Findings• Our study reveals a significant shift in keratoplasty indications, with Fuchs endothelial dystrophy (FED) emerging as the predominant indication, increasing from 24% in 2004 to 72% in 2023, while keratoconus (KC) declined from 25 to 4% during the same period.• Penetrating keratoplasty (PK) has shown a marked decline in use, dropping from 95% of keratoplasties in 2004 to 11% in 2023, whereas DMEK has grown to represent 82% of procedures in 2023.• Despite the rise of DMEK, PK remains vital for a broad spectrum of indications, highlighting the necessity for corneal banks to maintain a versatile supply of grafts to meet diverse clinical needs, particularly in cases of repeat keratoplasties.
