Abstract

Early detection of skin cancer is quite important since some types, such as melanoma, are dangerous and even it can cause the death if are not properly and early treated. Here, deep learning is broadly used to implement non-invasive systems for diagnosing skin cancer based on image analysis. In this work, it is presented a model, which is a derivation of the Visual Geometry Group with 16-layer deep model architecture (VGG16), for implementing a benign keratosis (bkl), melanocytic nevi (nv) and melanoma skin cancer (mel) classifier. Moreover, it is shown that by using a balanced dataset an average classification accuracy of 79.94% for the three types of pigmented skin lesions can be reached. Furthermore, it is presented this accuracy is quite competitive compared when the same skin lesions classifier is implemented by using the VGG16, the ResNet50v2, and the InceptionV3 pre-trained models, since the obtained average accuracies were 78.32%, 79.40% and 81.03%, respectively, under the same conditions. Additionally, it is described that the proposed model is lighter compared with the mentioned pre-trained models since it requires of 1.4 million of trainable parameters. Finally, it is shown that this characteristic contributes to the computational processing time for implementing, training, and evaluating the skin lesions classifier when the proposed model is used. Based on these numerical results, it is shown that the proposed model is competitive in terms of the required number of trainable parameters and the overall processing time in comparison with the required by the mentioned pretrained models without penalizing the pigmented skin lesions classification metrics. 18. Synergistic effect of two formulations of hypochlorous acid in the treatment of 346 chronic ulcers

Abstract

Our objective was to assess the efficacy of two successive applications of hypochlorous acid, first as a liquid and then as a gel because liquid hypochlorous acid is effective but has little residual effect, while the gel form has more residual power, and compare it with that of other products. An experimental non-randomised study was carried out, treating 346 chronic ulcers in 220 patients. The antiseptic treatment has been divided into ‘hypochlorous acid’ (Clortech), ‘hypochlorous acid liquid + gel’ (Clortech + Microdacyn60R-hydrogel) and ‘Others’ (Prontosan or Chlorhexidine or Microdacyn60R-hydrogel). Bivariate and multivariate studies analysed the characteristics of the patients and their ulcers, including size, symptoms, signs, treatments received and their duration, and so on. The ulcers were complicated, of long evolution, and most had a vascular origin. On average, antiseptic treatment lasted 14 weeks. At the time of their discharge, or last treatment in the clinics, 59% of the ulcers had healed completely, 9.5% worsened, and 6.9% had become infected during this period. In the bivariate and multivariate studies, we took as reference the ‘others’ treatments that showed no significant differences in healing time or infection rates compared with liquid hypochlorous acid 100–500 mg/L alone. However, hypochlorous acid liquid + gel showed a synergistic effect, with a higher probability of achieving complete healing (four times) and a lower probability of infection (a fifth), compared to the ‘other’ antiseptics. In conclusion, a synergistic effect was found with the successive application of hypochlorous acid in liquid followed by gel, an effect that increased healing probability and decreased the risk of the ulcer becoming infected

Abstract

Spermatocytic tumour (ST) accounts for 1% of testicular germ cell tumours. It is an indolent neoplasm with good prognosis. In approximately 6% of STs, sarcomatous dedifferentiation may occur, portending an aggressive behaviour and representing a significant diagnostic challenge that can lead to its misdiagnosis. Herein, we report the clinicopathological features of a patient with a sarcomatoid spermatocytic tumor, initially diagnosed as mixed germ cell tumour, who was referred to our institution with lung metastases mainly composed of rhabdomyosarcomatous elements. This case report illustrates the importance of recognizing this entity for adequate management of these patients. [ABSTRACT FROM AUTHOR]
