- Abstract
The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135–15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359–5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138–5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184–5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598–9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090–6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286–5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912–7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138–0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143–0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible.

Los principales objetivos de la investigación fueron detectar en función con la edad, la prevalencia de los genotipos de alto y bajo riesgo oncogénico de virus del papiloma humano (VPH) en muestras cervicales de las mujeres en los catorce cantones de la provincia de Azuay. El proyecto abarcó el diagnóstico histopatológico de las lesiones cervicales intraepiteliales y la relación de los genotipos encontrados, con los factores de riesgo y las vacunas existentes que se utilizan como medida de prevención de cáncer de cuello uterino. Fueron examinadas muestras de frotis cervicales de una población aleatoria de 500 mujeres con la prueba de Papanicolaou (Pap), usando la reacción en cadena de la polimerasa en tiempo real (PCR). El estudio reveló una prevalencia de VPH de 25.6%; 4.8% genotipos oncogénicos de bajo riesgo y el 20.8% genotipos oncogénicos de alto riesgo respectivamente, y sólo en el grupo de edad de 20 a 29 años, una significativa prevalencia mayor de los genotipos de alto riesgo 31 y 66 (p

- Abstract
This project addresses the production of whey-based pickles, for which it is necessary to obtain the raw material, which is raw milk whey, which is monitored through a biofermentation process until a desired pH is obtained; At this point, whey protein is denatured through a thermal and pressurization process. After this thermal process, we proceed to cooling and separation by filtering, in which two semi-products are obtained, on the one hand, the coagulated solid of the proteins (filter cake) and the leachate. For the elaboration of the pickles, only the leached part (liquid) is used, which is the one that contains lactic acid, water and dissolved salts mainly, the pH is maintained even after this process and the lactic acid diluted in this leachate, is the one that aid in the pickling of the products to be made. In the manufacture of pickles, raw materials are used that are commonly used in this type of food preservation, such as: chili peppers, onions, carrots, pork skin, broccoli, cauliflower, among many others. For the elaboration of the pickles, they are cut to a desired size, then they are given a pretreatment that consists of a scald and later they are packaged, the whey is added and an ultra-pasteurization is carried out; leaving it at rest (Asas, Matavaca, Llanos, Verdezoto, 2021).., El presente proyecto aborda la elaboración de encurtidos a base de lactosuero, para ello es necesario la obtención de la materia prima la cual es el suero de leche cruda que a través de un proceso de biofermentación, este se va monitoreando hasta obtener un pH deseado; en este punto se procede a una desnaturalización de la proteína del lactosuero a través de un proceso térmico y de presurización. Después de este proceso térmico, se procede al enfriamiento y separación por medio de filtrado, en el cual se obtienen dos semiproductos por un lado el solido coagulado de las proteínas (torta de filtrado) y el lixiviado. Para la elaboración de los encurtidos se utiliza solamente la parte lixiviada (liquido) que es el que contiene acido láctico, agua y sales disueltas principalmente, el pH se mantiene aun después de este proceso y el acido láctico diluido en este lixiviado, es el que ayuda al encurtido de los productos que se desean elaborar. En la fabricación de los encurtidos se utilizan materias primas que son comúnmente utilizadas en este tipo de conservación de alimentos, como son: chiles, cebollas, zanahorias, cuero de cerdo, brócoli, coliflor entre muchos más. Para la elaboración de los encurtidos se procede al corte de estos a un tamaño deseado, después se les da un pretratamiento que consiste en un escalde y posteriormente se envasan, se le adiciona el lactosuero y se procede a una ultrapasteurización; dejándolo en reposo (Asas, Matavaca, Llanos, Verdezoto, 2021).

Objectives: To describe the laparoscopic management of an atypical bezoar case and present a literature review. Materials and methods: This is the case of a 67-year-old male patient with pyloric stenosis due to intestinal obstruction by a foreign body. Results: The endoscopic finding was an atypical bezoar (Money bezoar) in the prepyloric region with no possible resolution by this route, so laparoscopic treatment was considered. Discussion: Bezoars are defined as any object that was voluntarily or involuntarily swollen and is obstructing some part of the upper gastrointestinal tract, usually the stomach, and cannot be digested using the physiological mechanisms of the body. They are categorized based on their composition. Conclusions: When endoscopic treatment fails to relieve upper gastrointestinal tract obstruction caused by foreign bodies, minimally invasive surgical treatment with laparoscopic surgery is a viable and efficient option. Resumen Objetivos: Presentar el manejo laparoscópico en un caso de bezoar atípico y una revisión de la literatura. Materiales y métodos: Se presenta el caso de un paciente de sexo masculino de 67 años con síndrome pilórico debido a una obstrucción intestinal por cuerpo extraño. Resultados: Se encuentra como hallazgo endoscópico un bezoar atípico (bezoar de dinero) impactado en la región prepilórica sin posibilidad de resolución por este medio, por lo cual se considera el manejo laparoscópico. Discusión: Los bezoares se definen como cualquier objeto el cual tuvo una ingesta voluntaria o involuntaria, que se aloja en alguna parte del tracto gastrointestinal superior, con mayor frecuencia a nivel gástrico, y no se puede digerir por los mecanismos fisiológicos del cuerpo; además, se clasifican según su composición. Conclusiones: En pacientes con obstrucción intestinal alta debido a cuerpos extraños en los cuales el manejo endoscópico falla, el manejo quirúrgico mínimamente invasivo con cirugía laparoscópica es viable y eficaz.

- Abstract
A linear programming model that selects the optimal cropping plan and feeds allocation for diets to minimize the whole dairy farm feed costs was developed. The model was virtually applied on 29 high-yielding Holstein-Friesian herds, confined, total mixed ration dairy farms. The average herd size was 313.2 ± 144.1 lactating cows and the average land size was 152.2 ± 92.5 ha. Farm characteristics such as herd structure, nutritional grouping strategies, feed consumption, cropping plan, intrinsic farm limitations (e.g., silage and hay storage availability, water for irrigation, manure storage) and on farm produced forage costs of production were collected from each farm for the year 2017. Actual feeding strategies, land availability, herd structure, crop production costs and yields, and milk and feed market prices for the year 2017 were used as model inputs. Through optimization, the feeding system was kept equal to the actual farm practice. The linear program formulated diets for each animal group to respect actual herd dry matter intake and fulfill actual consumption of crude protein, rumen-degradable and rumen-undegradable fractions of crude protein, net energy for lactation, neutral detergent fiber, acid detergent fiber, forage neutral detergent fiber, and nonfiber carbohydrate. Production levels and herd composition were considered to remain constant as the nutritional requirement would remain unchanged. The objective function was set to minimize the whole-farm feed costs including cash crop sales as income, and crop production costs and purchased feed costs as expenses. Optimization improved income over feed costs by reducing herd feed costs by 7.8 ± 6.4%, from baseline to optimized scenario, the improved was explained by lower feed costs per kilogram of milk produced due to a higher feed self-sufficiency and higher income from cash crop. - Abstract
Objectives: Tropheryma whipplei has been detected in 3.5% of the blood culture-negative cases of endocarditis in Spain. Experience in the management of T. whipplei endocarditis is limited. Here we report the long-term outcome of the treatment of previously reported patients who were diagnosed with infective endocarditis (IE) caused by T. whipplei from the Spanish Collaboration on Endocarditis-Grupo de Apoyo al Manejo de la Endocarditis Infecciosa en Espana (GAMES) and discuss potential options for antimicrobial therapy for IE caused by T. whipplei. Patients and methods: Seventeen patients with T. whipplei endocarditis were recruited between 2008 and 2014 in 25 Spanish hospitals. Patients were classified according to the therapeutic regimen: ceftriaxone and trimethoprim/sulfamethoxazole, doxycycline + hydroxychloroquine and other treatment options. Results: Follow-up data were obtained from 14 patients. The median follow-up was 46.5months. All patients completed the antibiotic treatment prescribed, with a median duration of 13months. Six patients were treated with ceftriaxone and trimethoprim/sulfamethoxazole (median duration 13months), four with doxycycline + hydroxychloroquine (median duration 13.8months) and four with other treatment options (median duration 22.3months). The follow-up after the end of the treatments was between 5 and 84months (median 24months). Conclusions: All treatment lines were effective and well tolerated. Therapeutic failures were not detected during the treatment. None of the patients died or experienced a relapse during the follow-up. Only six patients received antibiotic treatment in accordance with guidelines. These data suggest that shorter antimicrobial treatments could be effective.

IntroductionInfective endocarditis (IE) still carries high morbidity and mortality and frequently requires surgery. The benefit of mitral valve repair (MVr) in the setting of IE is yet to be proven. The goal of this study was to assess the results of MVr in patients with IE after a minimum follow-up of 1year.MethodsThis study is based on a Spanish nationwide prospective registry that included patients operated on for native mitral valve IE. The collaborating Institutions pooled their pre-, peri-, and postoperative data into the database of the GAMES group [Grupo de Apoyo al Manejo de la EndocarditiS (Group for support and management of infective endocarditis)].ResultsData from 27 hospitals were recorded and 3524 cases of active IE identified between 2008 and 2016. There were 1513 cases of mitral IE, of which 898 involved native valves. Of these, 437 patients underwent surgical treatment, and 369 completed the 1-year follow-up. The valve was repaired in 68 cases (18.4%). Preoperative groups were comparable (EuroSCORE MVr 7.7 vs MVR 8.0; p=ns). Mortality in the repair group was inferior to that in the replacement group (16.2% vs 27.2%, p=0.058). At 1year, mortality remained higher in the replacement group: 3.7% vs 2.9%. Relapse of the infection was slightly more frequent in the repair group (7.1% vs 3.7%; p=ns), although this did not lead to higher rates of reintervention (MVr/MVR: 2.9% vs 4.9%).ConclusionMVr is an attractive option for specific patients with IE and does not seem to negatively impact on relapses.

Background. Outpatient parenteral antibiotic treatment (OPAT) has proven efficacious for treating infective endocarditis (IE). However, the 2001 Infectious Diseases Society of America (IDSA) criteria for OPAT in IE are very restrictive. We aimed to compare the outcomes of OPAT with those of hospital-based antibiotic treatment (HBAT). Methods. Retrospective analysis of data from a multicenter, prospective cohort study of 2000 consecutive IE patients in 25 Spanish hospitals (2008-2012) was performed. Results. A total of 429 patients (21.5%) received OPAT, and only 21.7% fulfilled IDSA criteria. Males accounted for 70.5%, median age was 68 years (interquartile range [IQR], 56-76), and 57% had native-valve IE. The most frequent causal microorganisms were viridans group streptococci (18.6%), Staphylococcus aureus (15.6%), and coagulase-negative staphylococci (14.5%). Median length of antibiotic treatment was 42 days (IQR, 32-54), and 44% of patients underwent cardiac surgery. One-year mortality was 8% (42% for HBAT; P < .001), 1.4% of patients relapsed, and 10.9% were readmitted during the first 3 months after discharge (no significant differences compared with HBAT). Charlson score (odds ratio [OR], 1.21; 95% confidence interval [CI], 1.04-1.42; P = .01) and cardiac surgery (OR, 0.24; 95% CI, .09-.63; P = .04) were associated with 1-year mortality, whereas aortic valve involvement (OR, 0.47; 95% CI, .22-.98; P = .007) was the only predictor of 1-year readmission. Failing to fulfill IDSA criteria was not a risk factor for mortality or readmission. Conclusions. OPAT provided excellent results despite the use of broader criteria than those recommended by IDSA. OPAT criteria should therefore be expanded.

Aim: To evaluate the effect of the type of surgical indication on mortality in infective endocarditis (IE) patients who are rejected for surgery. Methods and results: From January 2008 to December 2016, 2714 patients with definite left-sided IE were attended in the participating hospitals. One thousand six hundred and fifty-three patients (60.9%) presented surgical indications. Five hundred and thirty-eight patients (32.5%) presented surgical indications but received medical treatment alone. The indications for surgery in these patients were uncontrolled infection (366 patients, 68%), heart failure (168 patients, 31.3%) and prevention of embolism (148 patients, 27.6%). One hundred and thirty patients (24.2%) presented more than one indication. The mortality during hospital admission was 60% (323 patients). The in-hospital mortality of patients whose indication for surgery was heart failure, uncontrolled infection or risk of embolism was 75.6%, 61.4% and 54.7%, respectively (p < 0.001). Surgical indications due to heart failure (OR: 3.24; CI 95%: 1.99-5.9) or uncontrolled infection (OR: 1.83; CI 95%: 1.04-3.18) were independently associated with a fatal outcome during hospital admission. Mortality during the first year was 75.4%. The mortality during the first year in patients whose indication for surgery was heart failure, uncontrolled infection or risk of embolism was 85.9%, 76.7% and 72.7%, respectively (p = 0.016). Surgical indication due to heart failure (OR: 3.03; CI 95%: 1.53-5.98) were independently associated with fatal outcome during the first year. - Abstract
A survey regarding crop enterprise management, forages cost of production, dairy cattle management including reproductive management, housing, heat abatement, body condition scoring, nutrition, grouping strategies, and income over feed cost performance, was carried out from December 2016 to January 2018 on 50 dairy farms by the Department of Animal Science, Food and Nutrition of Università Cattolica del Sacro Cuore (Piacenza, Italy). A total of 41 herds (82%) completed the survey. Average herd size was 327 ± 162 lactating cows with the average land size of 160 ± 94 ha per farm. Herds were located in the provinces of Cremona (17), Brescia (8), Mantova (7), Piacenza (5), Cuneo (4), Bergamo (3), Lodi (3), Torino (2), and Venezia (1). These farms sold 32.8 ± 2.01 kg of milk/day per cow, had an annual culling rate of 34.0 ± 4.00%, a calving interval of 14.16 ± 0.58 months., and a 21-days pregnancy rate of 17.05 ± 2.58%. Implementing effective management strategies to contrast the damage caused by Ostrinia nubilalis, Diabrotica spp. and Myocastor coypus were identified as the main crop enterprise challenges. Main forages cultivated were alfalfa and corn silage second seeding with a total cost of production of (€/ha) 1968 ± 362 and 2,581 ± 221, with an average yield of 9.61 ± 1.24 and 17.22 ± 2.46 ton of DM per hectare, respectively. Results of this study can provide useful benchmark or reference for dairy management practices, crops and dairy performances, forages production costs on very well-managed North Italian dairy farms at the present time.Highlights benchmarks for dairy farms management practices, economic and reproductive performance cost of production of forages in northern Italy.

Objetivo: Analizar en adultos egresados de Clínica Dávila con influenza, en 2009, 2010, 2012 y 2014, soporte ventilatorio, costo de hospitalización, Grupos Relacionados por el Diagnóstico (GRD) y letalidad. Material y Método: Estudio descriptivo retrospectivo usando la ficha médica electrónica. Resultados: Egresaron 115.673 adultos, 338 (0,29%) con diagnóstico de Influenza, edad 56,5 ± 22 años, 59% mujeres, letalidad 4%. Hubo 3 grupos, Grupo 1: sin ningún soporte ventilatorio, 295 pacientes, edad 63 ± 20, estadía 6,6 ± 6,9 días, costo promedio de hospitalización $2.885.261, mediana peso GRD 0,41 (p25 = 0,38 y p75 = 0,62), letalidad 1,01% (3 pacientes). Grupo 2: Ventilación mecánica no invasiva (VMNI), 23 casos, edad 77,1 ± 13, letalidad 22% (5 casos), estadía 16,8 ± 12,4, costo $9.245.242, GRD 0,79 (p25 = 0,62 y p75=1,03). Grupo 3: Intubación y ventilación mecánica invasiva (VMI), 20 pacientes, edad 56,4 ± 15, estadía 36,9 ± 41,4, costo $38.681.099, GRD 5,86 (p25 = 5,82 y p75 = 5,86) y letalidad 30% (6 pacientes). Los GRD grupo VMI versus grupo VMNI y ningún soporte fueron diferentes (p < 0,0001 y p < 0,0001 respectivamente). La letalidad por influenza el 2014 fue de 8,5%, mientras que en los años 2012, 2010 y 2009 fue 1,5%, 3% y 2,5% respectivamente. La mediana de edad el año 2009 fue 37,5 años, menor que la de los otros años (p < 0,0001). Conclusiones: En 2009 los pacientes fueron más jóvenes, la necesidad de soporte ventilatorio provocó un peso GRD, estadía, costo y letalidad mayores que aquellos que no lo requirieron. There were 3 groups, Group 1: without any ventilatory support, 295 patients, age 63 ± 20, stay 6.6 ± 6.9 days, average cost of hospitalization 2,885,261 clp, medium weight DRG 0.41 (p25 = 0.38) andp75 = 0.62), lethality 1.01% (3 patients). Group 2: Non-invasive mechanical ventilation (NIMV), 23 cases, age 77.1 ± 13, lethality 22% (5 cases), stay 16.8 ± 12.4, cost 9,245,242 clp, DRG 0.79 (p25 = 0.62 and p75 = 1.03). Group 3: Intubation and invasive mechanical ventilation (IMV), 20 patients, age 56.4 ± 15, stay 36.9 ± 41.4, cost 38.681.099 clp, DRG 5.86 (p25 = 5.82 and p75 = 5,86) and lethality 30% (6 patients). The DRG group VMI versus group VMNI and no support were different (p < 0.0001 and p < 0.0001 respectively). The lethality for influenza in 2014 was 8.5%, while in 2012, 2010 and 2009 it was 1.5%, 3% and 2.5% respectively. The median age in 2009 was 37.5 yr.o significantly minor, than the other years (p < 0.0001). Conclusions: In 2009 the patients were younger, the need for ventilatory support led to a higher DRG weight, stay, cost and lethality than those who did not require it.

- Abstract
Objectives: To describe the laparoscopic management of an atypical bezoar case and present a literature review. Materials and methods: This is the case of a 67-year-old male patient with pyloric stenosis due to intestinal obstruction by a foreign body. Results: The endoscopic finding was an atypical bezoar (Money bezoar) in the prepyloric region with no possible resolution by this route, so laparoscopic treatment was considered. Discussion: Bezoars are defined as any object that was voluntarily or involuntarily swollen and is obstructing some part of the upper gastrointestinal tract, usually the stomach, and cannot be digested using the physiological mechanisms of the body. They are categorized based on their composition. Conclusions: When endoscopic treatment fails to relieve upper gastrointestinal tract obstruction caused by foreign bodies, minimally invasive surgical treatment with laparoscopic surgery is a viable and efficient option. This study aimed to isolate and identify arbuscular mycorrhizal fungi in melina (Gmelina arborea Roxb) plantations. Two melina plantations of 1 and 3 years old located in the central zone of the humid Ecuadorian tropics, province of Los Ríos, Valencia were sampled. Soil and root samples were collected in the dry season between June and December 2015. The results showed the presence of the arbuscular mycorrhizal fungi genera Glomus and Gigaspora. The genus with the greatest representativeness in quantity of spores per gram of soil was Glomus. The highest percentage of mycorrhizal colonization occurred in the roots of 1-year-old trees. El objetivo de esta investigación fue aislar e identificar hongos formadores de micorriza arbuscular en plantaciones de melina (Gmelina arborea Roxb). Se muestrearon dos plantaciones de melina de 1 y 3 años de edad, ubicadas en la zona central del trópico húmedo ecuatoriano, provincia de Los Ríos, cantón Valencia. Las muestras de suelo y raíces se recolectaron en la época seca entre junio y diciembre de 2015. Los resultados mostraron la presencia de los géneros de hongos micorrízicos arbusculares Glomus y Gigaspora. El género con mayor representatividad en cantidad de esporas por gramo de suelo fue Glomus. El mayor porcentaje de colonización micorrízica se presentó en las raíces de los árboles de 1 año de edad.
