6. Clinicopathological findings in sea turtles assessed during the Deepwater Horizon oil spill response

Abstract

During the BP Deepwater Horizon (DWH) oil spill in 2010, 319 live oiled sea turtles were rescued and admitted to rehabilitation centers for decontamination and veterinary care. Most turtles were small, surfaceñpelagic juveniles that were collected from oiled habitat distant from shore. Serial hematology, plasma biochemistry, and blood gas analyses were reviewed to characterize abnormalities relative to observed degree of oiling. Clinicopathological abnormalities upon admission indicated acute, nonspecific metabolic and osmoregulatory derangements that were attributable to a combination of events including stress, exertion, physical exhaustion, and dehydration related to oiling, capture, and transport. Specific toxicological effects reported in other taxa were not observed. Initial point-of-care blood data from one rescue center were evaluated using clinical assessment of physiological status for all turtles of all species with available data for pH, pCO2, sodium, and potassium. In addition, a prognostic model that was specifically developed for cold-stunned Kempís ridley sea turtles Lepidochelys kempii was applied to oiled Kempís ridley turtles from one center. Thirty-six percent of oiled turtles were identified as physiologically deranged based on a clinical assessment of their physiological status, and 25% of oiled Kempís ridley sea turtles exceeded the mortality risk threshold of the prognostic model. These results indicate that the physiological derangements in these animals were relatively severe and clinically relevant. Based on observations during the DWH spill, adverse physiological effects in sea turtles may be an important consequence of stress, exertion, physical exhaustion, and dehydration secondary to oiling, capture, and transport.

Abstract

The Deepwater Horizon (DWH) oil spill was unprecedented in extent and duration, and affected marine natural resources, including sea turtles, throughout the northern Gulf of Mexico. Consequently, US federal and state Trustees documented and quantified oil exposure and resulting injuries to sea turtles under the DWH Natural Resource Damage Assessment. At-sea rescue operations focused on surface-pelagic juvenile sea turtles, which were especially at risk to oil exposure within oceanic convergence zones, and provided direct observations of the degree that turtles in this young life stage were exposed to DWH oil. In contrast, locations of larger neritic juvenile and adult turtles were documented during aerial surveys, but because these turtles were not captured, their oiling status could not be directly evaluated. Both the rescue operations and aerial surveys were able to observe only a small fraction of sea turtles within the vast spill footprint. We developed a spatio-temporally explicit approach that used direct observations of oiled surface-pelagic juvenile sea turtles and satellite-derived surface oil distributions to statistically estimate the probabilities of oil exposure for all sea turtles that were present within the area of the DWH spill, but whose oiling status was unknown. Our results enabled an expansion of exposure and injury quantification across the entire DWH spill area and period. This approach was conceptually straightforward and used common geospatial and statistical techniques, making it applicable to other situations in which the full extent of oil exposure for marine natural resources must be estimated from an incomplete sample.

Abstract

The 2010 Deepwater Horizon (DWH) oil spill and associated response activities overlapped with habitats for multiple life stages of federally protected sea turtles in the northern Gulf of Mexico. Although most assessment efforts focused on documenting the presence, abundance, and exposure of sea turtles to DWH oil in marine habitats, oil also washed ashore on sand beaches used by nesting turtles, specifically in NW Florida and Alabama, USA. In addition, extensive and highly mechanized beach cleanup efforts were conducted in NW Florida as well as Alabama, Louisiana, and Mississippi. Thus, the DWH oil spill negatively affected sea turtle nesting directly (e.g. adverse effects of oil exposure) and indirectly (e.g. beach cleanup activities deterring nesting) in several ways. In this study, we used a before-after, control-impact statistical modeling approach to examine long-term (1997-2012) historical records of loggerhead turtle (Caretta caretta) nest densities in 2 segments of the Florida Gulf coastline, one that was heavily affected by the oil spill (NW Florida) and one that was relatively unaffected (SW Florida). Loggerhead nest densities on NW Florida beaches in 2010 were reduced by 43.7% (95% confidence interval: 10-65%) relative to expected nesting rates in the absence of DWH oil and cleanup efforts. When we applied this reduction to all nesting habitat in NW Florida, this equated to a loss of approximately 251 unrealized nests from the 2010 nesting seasonóa loss we attribute to direct (e.g. mortality) and indirect (e.g. deterrence of nesting) effects related to the DWH incident.

Abstract

The 2010 Deepwater Horizon (DWH) oil spill posed a severe threat to surface-pelagic sea turtles because the surface convergence zones, which provide vital habitat by aggregating pelagic Sargassum and other floating material, also aggregated floating oil. Following the DWH spill, turtle rescue operations between 17 May and 9 September 2010 documented 937 juvenile sea turtles in the spill area and examined 574 captured turtles. Of the captured turtles, 81% were visibly oiled. Transect searches in convergence zones found Kempís ridleys (51% of individuals), green turtles (37%), loggerheads (7%), hawksbills (2%), and unidentified sea turtles (2%). Line-transect methods estimated the density of all surface-pelagic sea turtles in surface convergence zones to be 3.32 km-2 (95% CI = 2.82ñ3.88), and the density of heavily oiled turtles to be 0.24 km-2 (95% CI = 0.15ñ0.39). Turtle densities and the areal extent of heavy oiling probability were used to estimate total number of turtles exposed to DWH oil. We estimate approximately 402000 surface-pelagic sea turtles were exposed, and of those, 54800 were likely to have been heavily oiled. Our estimates formed the basis of surface-pelagic juvenile sea turtle mortality estimates included in the DWH natural resource damage assessment.

Abstract

Background: Decompensated heart failure (HF) is associated with poor short- and long-term prognosis. Remote invasive monitoring of pulmonary artery pressures (PAP) enables early detection of HF decompensation before symptoms occur and may improve clinical outcomes. We aimed to describe our initial experience with the use of the CardioMEMS™ remote monitoring system in patients with HF, including its safety and effectiveness. Methods and results: Five patients with HF in New York Heart Association class III and at least one hospitalization due to decompensated HF in last 12 months, who underwent invasive remote monitoring of PAP, were included in this prospective registry. The median age was 66.0 years (interquartile range [IQR] 50.5-77.5 years), 80.0% were men and all had HF with reduced ejection fraction. The pulmonary artery (PA) sensor was placed in a left PA branch in all patients and no major procedural complications occurred. In median follow-up of 40 days (IQR 40-61 days), a total of 271 pressure readings were transmitted, patient compliance was 100% and freedom from sensor failure 98.1%. In three patients, PAP remained within the goal during follow-up. Two patients presented an increase in PAP to values above the targets, despite the absence of symptom worsening. These required dietary and diuretic dose adjustment, without the need for outpatient clinic visits, which reduced PAP. No hospitalizations for HF or deaths occurred during follow-up. Conclusion: Hemodynamic-guided HF monitoring was safe and effective and may be a useful adjunctive tool to the standard-of-care management in selected HF patients, particularly in the context of the COVID-19 pandemic, where a reduction in the number of health care visits may be desirable. Introdução: A insuficiência cardíaca (IC) descompensada está associada a mau prognóstico em curto e longo prazo. A monitoração remota invasiva da pressão na artéria pulmonar (PAP) possibilita a deteção precoce da descompensação da IC, previamente ao início dos sintomas, pode melhorar os resultados clínicos. Descrevemos a experiência inicial com o uso do sistema de monitoração remota CardioMEMSTM em doentes com IC, inclusive a sua segurança e eficácia. Métodos e resultados: Foram incluídos prospetivamente cinco doentes com IC em classe III da New York Heart Association, com pelo menos uma hospitalização por IC descompensada nos últimos 12 meses, submetidos a implantação de sistema de monitoração remota invasiva da PAP. A mediana de idade foi de 66 anos (intervalo interquartil [IIQ] 50,5-77,5 anos), 80% eram do sexo masculino e todos apresentavam IC com fração de ejeção reduzida. O sensor de PAP foi colocado num ramo da artéria pulmonar esquerda em todos os doentes, não ocorreram complicações major. Durante o follow-up mediano de 40 dias (IIQ 40-61 dias), foram transmitidas 271 leituras, verificou-se uma adesão dos doentes de 100% e taxa de transmissão bem-sucedida de 98,1%. A PAP manteve-se dentro do objetivo em três doentes durante o follow-up. Apesar de continuarem assintomáticos, dois doentes apresentaram valores das PAP acima do alvo. Foi feito ajuste dietético e da dose de diurético, sem necessidade de visitas clínicas presenciais, objetivou-se uma redução efetiva das PAP. Durante o seguimento, não se registaram hospitalizações por IC ou óbitos. Conclusão: A monitoração da IC guiada pela hemodinâmica demonstrou ser segura e eficaz, pode assumir-se como uma ferramenta útil no manejo de doentes com IC, particularmente no contexto da pandemia Covid-19, quando é desejável uma redução do número de consultas presenciais hospitalares. info:eu-repo/semantics/publishedVersion
