1. Birth order and reproductive stoppage in families of children with autism spectrum disorder

Abstract

OBJECTIVES: The objective of the study was to evaluate the birth order of children with Autism Spectrum Disorder (ASD) and ways of delivery at birth, together with the phenomenon of reproductive stoppage and the number of siblings in the case families (families of children with ASD in the study). METHODS: One hundred and ninety-six subjects with ASD and 54 healthy children were included into the study. Demographics were collected. Autism Behaviour Checklist (ABC), Aberrant Behaviour Checklist (AbBC), and Childhood Autism Rating Scale (CARS) were administered. Depending on the type of data and on the objective of the assessment, Mann–Whitney U-test, chi-square test, and Spearman tests were used for statistical analysis. A p-value smaller than .05 was considered to be statistically significant. RESULTS: In the study group consisting of children with ASD, the rate of being the first-born child was determined to be more frequent, significantly, compared to the rate in the control group (p = .001). It was also found that 86.7% (n = 170) of the children in the ASD group had at least one sibling compared to the rate of 81.5% (n = 44) for their counterparts in the control group. CONCLUSIONS: Our study compared ASD group to controls in terms of birth order, demonstrating a significant difference for being the first-born child in the ASD group. Birth order can be considered to be one of the several environmental factors that will help in understanding ASD, in which environmental factors can be the cause of phenotypic complexity. For all that, in our study, it was observed that having a child with autism in the Turkish sample did not affect the decision for the next pregnancy.

Abstract

Attention-deficit/hyperactivity disorder (ADHD) is one of the most prevalent psychiatric disorders in children and the pathophysiology remains obscure. Some studies show that lipid imbalances are associated with ADHD etiology. We studied the association of serum total cholesterol, high-density lipoprotein (HDL), low-density lipoprotein (LDL), and triglyceride (TG) levels in ADHD. We examined 88 children aged 8-12 years who were diagnosed with ADHD and 88 healthy children. The exclusion criteria were as follows: obesity, any psychotropic use in the last 3 months, presence of a chronic disease and/or malignancy, history of medically treated lipid metabolism disease in family members, intelligence quotient (IQ) 70, and comorbidities, with the exception of oppositional defiant disorder. The sample was evaluated using a semi-structured clinical assessment interview and Conners' rating scales. Despite controlling for age, sex, and body mass index (BMI) variables, the total cholesterol and LDL levels were significantly higher in the ADHD group than the levels of healthy controls, whereas the TG and HDL cholesterol levels were similar among groups. Conners' rating scales, reflecting symptom severity, and total cholesterol, TG, HDL, and LDL levels of the ADHD group were not correlated. The study results support the difference in serum lipid and lipoprotein profiles of children with ADHD compared with healthy controls. This difference is thought to be related with changes in oxidant/antioxidant balance states in ADHD.

Abstract

DiGeorge Syndrome (DGS) is the most seen microdeletion syndrome and its prevalance varies from 1/4000 to 1/6000. Over embriogenesis, a deletion of an approximately 3 Mb frame from 22q11.2 results in this syndrome and its clinical features varies from congenital heart and gross vessels anomalies, hypocalcemia, respiratory problems, leucomotor or urogenitale system anomalies, hearing loss, to developmental delay. Alongside different clinical features and phenotypes, these patients should be followed up on psychiatric symptoms and disorders such as speech delay, specific learning difficulties (SLD), mental retardation, attention deficit hyperactivity disorder (ADHD), autism, anxiety, depression and even schizophrenia and other pcychotic disorders. This case, diagnosed with partial DGS and followed by immunology, cardiology, endocrinology and neurology departments, admitted to our clinic on account of his difficulties in learning and inattention. The patient was diagnosed with ADHD and SLD, then pharmacotherapy and individual educational programme were planned. The insufficient expression of the catecolamine-O-methyltransferase (COMT) gene located in the 22q11.2 gene could very well play a role in both the etiology of ADHD and SLD. There is still no clear causative relation between these, so there is a need to clarify this with further studies. As seen in our case, the patients with DGS and having some inattention or learning difficulties are needed to be examined by the child psychiatrists. Treatment protocols related to these patients' symptoms would be evaluated with a multidisciplinary attitude.

Abstract

Mucopolysaccharidosis type III (MPS-III, Sanflippo syndrome) is one of the lysosomal storage diseases characterized by four different enzymes playing a role in catabolism of heparan sulfate substance. Type IIIB (Sanflippo B), one of them, is characterized by alpha-N-acetylglucosaminidase (NAGLU) enzyme deficiency and results in accumulation and storage of heparan sulfate in central nervous system, and then emerges itself with developmental deficiency, coarse facial features, delay in speech, mental retardation, hyperactivity, impulsivity, aggressive and disruptive behaviors, difficulty in establishing social relations, aimless hand movements, inadequate eye contact, and sleep disturbances. There are a few studies related to the treatment of neuropsychiatric symptoms of these patients. In this case, atomoxetine treatment added was shown to be effective, safe and tolerable, because risperidone treatment that the patient took was not effective in hyperactivity symptoms. A 12-year-old girl, diagnosed with MPS type IIIB (NAGLU deficiency; OMIM# 252920) when she was 5 years old, was admitted to our clinic with symptoms of hyperactivity, short span of attention, aggressive behavior, and quasi-autistic symptoms such as aimless hand movement, with risperidone use at 1.5 mg/day for approximately 1 year. After clinical and psychometric examinations, she was diagnosed with “mild mental retardation” and “attention deficit hyperactivity disorder”. Atomoxetine was added at a dose of 0.5 mg/kg/day to her risperidone treatment and its dosage was gradually increased up until to 1.4 mg/kg/day. Over the follow-up period, her attention deficit and hyperactivity disorder (ADHD) symptoms were ameliorated, reported by both her teachers and her parents via rating scales of ADHD. Her hyperactivity and irritability levels were noticed as decreased and improved over evaluation process throughout clinical interviews. 7. ANTI-PCA/YO AND ANTI-GAD LEVELS IN AUTISM SPECTRUM DISORDER AND ATTENTION DEFICIT HYPERACTIVITY DISORDER

Abstract

Compelling evidences suggest that dysfunction in the cerebellar region of the brain plays has a significiant association with the etiology of autism spectrum disorders (ASD) and attention deficit hyperactivity disorders (ADHD). This study aims to investigate should there is the association between degeneration of the cerebellum and the etiologies of ASD and ADHD. The study recruited participants comprising of 40 children with ASD and 39 children with ADHD. 40 healthy children were recruited as the control group. The participants were analysed for an increase in anti-purkinje cell autoantibodies, called as Yo (anti-PCA/Yo) antibodies and anti-glutamic acid decarboxylase (GAD) antibody levels during cerebellar damage. Each child with ASD was evaluated using the autism behavior checklist (ABC), aberrant behavior checklist (AbBC) and childhood autism rating scale (CARS). The Conners’ Parent and Teacher Rating Scales-Revised Long Form (CPRS and CTRS) screening questionnaires were completed by the parents and teachers of children with ADHD. The anti-PCA/Yo and anti-GAD levels were determined using 10 ml venous samples from the participants. No significant sociodemographical differences were observed between the groups. Significant differences were found in the anti-Yo levels between children with ADHD and healthy subjects (p=0.002). Additionally, a positive correlation was observed between GAD levels and children age. This was the first study that evaluated levels of anti-Yo and anti-GAD levels in ADHD and ASD, and compared it to a control group. It found association between anti-Yo levels and ADHD. Further studies with larger samples and follow-up periods are necessary in order to investigate possible etiological factors among subjects with neuropsychiatric disorders.

8. EVALUATION OF THE PSYCHOLOGICAL PROBLEMS THE CHILDREN DIAGNOSED WITH PRECOCIOUS PUBERTY SUFFER FROM: A CASE CONTROL STUDY

Abstract

Background: One of the most common and difficult to diagnose condition is Precocious Puberty which is prevalent in pediatric patients with endocrinology disorders. The plethora of information available in open sources makes the families concern about their child’s pubertal development. These patients undergo a tremendous psychological turmoil. Hence, the current study was planned with the aims to evaluate children in terms of psychiatric illness who were diagnosed with pubertaprecocs. Methodology: The present study was undertaken in \\\"University of Health Sciences, Ankara Child Health and Diseases Hematology Oncology Training and Research Hospital\\\" Child and Adolescent Psychiatry Department and Child Endocrinology Clinic. There were total 120 samples where in 56 were suffering from Precocious puberty and 64 were control. The patients included those who visited pediatric endocrine polyclinic with complaints of early adolescence and the control group included patients without the disorder with same age and gender. The children were interviewed using DSM-V tool and thereafter assessed on scales like SCARED, CPRS, CTRS, CDQ, Body Perception scale and Piers Harris self-concept scale measurement for children. Results: The two groups were similar in terms of CPRS, CTRS, CDS and Pier-Harris scales distributions (p> 0.05). Body Sensitivity Scale scores of the pubertas praecox group were significantly lower (median 175 vs. 185.5, z = -2.537, p = 0.011). At least one psychiatric disorder was found to be significantly higher in the puberty precox group (32.1% vs. 10.9%), x2 = 8.143, p = 0.004. CPRS-POSITIVE was significantly higher in the pubertas praecox group (67.9% vs. 48.4%), x2 = 4.609, p = 0.032. Similarly, internalization or externalization impairment was found to be higher in pubertal precox cases than control group cases. 9. The assessment of serum omentin levels of children with autism spectrum disorder and attention-deficit/hyperactivity disorder

Abstract

Objectives: We aimed to investigate plasma omentin concentrations in non-obese, drug-free patients with autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) in comparison with healthy volunteers. Methods: Sixty children with ASD, 60 children with ADHD, and 60 control subjects were recruited. Plasma omentin levels were determined by the enzyme-linked immunosorbent assay. Results: Plasma levels of omentin (479.0 ng/ml) were found to be significantly higher in patients with ASD (median = 422.6, min/max; 220–800) than in controls (382.5 ng/ml) (median = 322.0, min/max 184–800). No significant difference was found between ADHD and control groups with respect to plasma omentin levels. There was no significant correlation between omentin levels and age of children, ABCL, AbBC, CARS, CPRS, and CTRS scores. Conclusions: To our knowledge, this is the first study that demonstrated the association between omentin and ASD and ADHD. The present results suggest that plasma omentin levels are increased in non-obese and drug-free patients with ASD when compared with in ADHD and healthy children. The omentin levels in ADHD and ASD need further refinement with larger samples and long-term follow-up periods.
