Abstract

Host genetics is a key determinant of COVID-19 outcomes. Previously, the COVID-19 Host Genetics Initiative genome-wide association study used common variants to identify multiple loci associated with COVID-19 outcomes. However, variants with the largest impact on COVID-19 outcomes are expected to be rare in the population. Hence, studying rare variants may provide additional insights into disease susceptibility and pathogenesis, thereby informing therapeutics development. Here, we combined whole-exome and whole-genome sequencing from 21 cohorts across 12 countries and performed rare variant exome-wide burden analyses for COVID-19 outcomes. In an analysis of 5,085 severe disease cases and 571,737 controls, we observed that carrying a rare deleterious variant in the SARS-CoV-2 sensor toll-like receptor TLR7 (on chromosome X) was associated with a 5.3-fold increase in severe disease (95% CI: 2.75-10.05, p = 5.41x10-7). This association was consistent across sexes. These results further support TLR7 as a genetic determinant of severe disease and suggest that larger studies on rare variants influencing COVID-19 outcomes could provide additional insights.

16. Rare top-quark decays $t \to cg(g)$ in the aligned two-Higgs-doublet model

Abstract

We update the Standard Model (SM) predictions for the branching ratios of the rare top-quark decays $t \to cg(g)$, and evaluate the maximum rates that can be reached in the aligned as well as in the four conventional two-Higgs-doublet models (2HDMs) with $\mathcal{Z}_2$ symmetries. Taking into account the relevant constraints on the model parameters resulting from a global fit obtained at the $95.5\%$ confidence level, we find that the branching ratios of $t \to cg$ and $t \to cgg$ decays can reach up to $3.36\times 10^{-9}$ and $2.95\times 10^{-9}$ respectively, being therefore of the same order, in the aligned 2HDM (A2HDM). This is obviously different from the SM case, where the predicted branching ratio of the three-body decay $t \to cgg$ is about two orders of magnitude larger than that of the two-body decay $t \to cg$. On the other hand, compared with the SM predictions, no significant enhancements are observed in the four conventional 2HDMs with $\mathcal{Z}_2$ symmetries for the branching ratios of these two decays. Abstract

BackgroundIn COVACTA, a randomised, placebo-controlled trial in patients hospitalised with coronavirus disease-19 (COVID-19), tocilizumab did not improve 28-day mortality, but shortened hospital and intensive care unit stay. Longer-term effects of tocilizumab in patients with COVID-19 are unknown. Therefore, the efficacy and safety of tocilizumab in COVID-19 beyond day 28 and its impact on Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) clearance and antibody response in COVACTA were investigated.MethodsAdults in Europe and North America hospitalised with COVID-19 (N = 452) between April 3, 2020 and May 28, 2020 were randomly assigned (2:1) to double-blind intravenous tocilizumab or placebo and assessed for efficacy and safety through day 60. Assessments included mortality, time to hospital discharge, SARS-CoV-2 viral load in nasopharyngeal swab and serum samples, and neutralising anti-SARS-CoV-2 antibodies in serum. ClinicalTrials.gov registration: NCT04320615.FindingsBy day 60, 24·5% (72/294) of patients in the tocilizumab arm and 25·0% (36/144) in the placebo arm died (weighted difference -0·5% [95% CI -9·1 to 8·0]), and 67·0% (197/294) in the tocilizumab arm and 63·9% (92/144) in the placebo arm were discharged from the hospital. Serious infections occurred in 24·1% (71/295) of patients in the tocilizumab arm and 29·4% (42/143) in the placebo arm. Median time to negative reverse transcriptase-quantitative polymerase chain reaction result in nasopharyngeal/oropharyngeal samples was 15·0 days (95% CI 14·0 to 21·0) in the tocilizumab arm and 21·0 days (95% CI 14·0 to 28·0) in the placebo arm. All tested patients had positive test results for neutralising anti-SARS-CoV-2 antibodies at day 60.InterpretationThere was no mortality benefit with tocilizumab through day 60. Tocilizumab did not impair viral clearance or host immune response, and no new safety signals were observed. Future investigations may explore potential biomarkers to optimize patient selection for tocilizumab treatment and combination therapy with other treatments.FundingF. Abstract

Osteoarthritis (OA) is a painful joint disease that is common among the middle-aged and elderly populations, with an increasing prevalence. Therapeutic options for OA are limited, and the pathogenic mechanism of OA remains unclear. The roles of cytokines and signaling pathways in the development of OA is a current research hot spot. Interleukin (IL)-17 is a pleiotropic inflammatory cytokine produced mainly by T helper 17 cells that has established roles in host defense, tissue repair, lymphoid tissue metabolism, tumor progression, and pathological processes of immune diseases, and studies in recent years have identified an important role for IL-17 in the progression of OA. This narrative review focuses on the mechanisms by which IL-17 contributes to articular cartilage degeneration and synovial inflammation in OA and discusses how IL-17 and the IL-17 signaling pathway affect the pathological process of OA. Additionally, therapeutic targets that have been proposed in recent years based on IL-17 and its pathway in OA are summarized as well as recent advances in the study of IL-17 pathway inhibitors and the potential challenges of their use for OA treatment.

28. Probing new physics in class-I $B$-meson decays into heavy-light final states

Abstract

With updated experimental data and improved theoretical calculations, several significant deviations are being observed between the Standard Model predictions and the experimental measurements of the branching ratios of $\bar{B}_{(s)}^0\to D_{(s)}^{(*)+} L^-$ decays, where $L$ is a light meson from the set $\{\pi,\rho,K^{(\ast)}\}$. Especially for the two channels $\bar{B}^0\to D^{+}K^-$ and $\bar{B}_{s}^0\to D_{s}^{+}\pi^-$, both of which are free of the weak annihilation contribution, the deviations observed can even reach 4-5$\sigma$. Here we exploit possible new-physics effects in these class-I non-leptonic $B$-meson decays within the framework of QCD factorization. Firstly, we perform a model-independent analysis of the effects from twenty linearly independent four-quark operators that can contribute, either directly or through operator mixing, to the quark-level $b\to c\bar{u} d(s)$ transitions. It is found that, under the combined constraints from the current experimental data, the deviations observed could be well explained at the $1\sigma$ level by the new-physics four-quark operators with $\gamma^{\mu}(1-\gamma_5)\otimes\gamma_{\mu} (1-\gamma_5)$ structure, and also at the $2\sigma$ level by the operators with $(1+\gamma_5)\otimes(1-\gamma_5)$ and $(1+\gamma_5)\otimes(1+\gamma_5)$ structures. However, the new-physics four-quark operators with other Dirac structures fail to provide a consistent interpretation, even at the $2\sigma$ level. Then, as two specific examples of model-dependent considerations, we discuss the case where the new-physics four-quark operators are generated by either a colorless charged gauge boson or a colorless charged scalar, with their masses fixed both at the $1$~TeV. Abstract

The susceptibility-based positive contrast MR technique was applied to estimate arbitrary magnetic susceptibility distributions of the metallic devices using a kernel deconvolution algorithm with a regularized L-1 minimization.Previously, the first-order primal-dual (PD) algorithm could provide a faster reconstruction time to solve the L-1 minimization, compared with other methods. Here, we propose to accelerate the PD algorithm of the positive contrast image using the multi-core multi-thread feature of graphics processor units (GPUs). The some experimental results showed that the GPU-based PD algorithm could achieve comparable accuracy of the metallic interventional devices in positive contrast imaging with less computational time. Abstract

A cross-sectional study was conducted to estimate the age-stratified normal levels and age-related changes in the risk predictors of benign prostatic hyperplasia (BPH) progression. A total of 4706 male participants aged 40 years or older in Zhengzhou (China) were enrolled. The values of the International Prostate Symptom Score (IPSS), prostate-specific antigen (PSA), prostate volume (PV), and postvoid residual urine volume (PVR) significantly increased with age. Nonlinear relationships between age and IPSS scores ≥8 (P for nonlinearity = 0.046), PSA level ≥1.6 ng ml–1, PV ≥31 ml, or PVR ≥39 ml (all P for nonlinearity 1), regardless of the predictors of the IPSS score, PSA level, PV, or PVR; and the OR values increased gradually. Therefore, after the age of 61 years, the risk predictors related to BPH progression were positively correlated with age.

Abstract

Mindfulness training among patients with major depressive disorder (MDD) reduces symptoms, prevents relapse, improves prognosis, and is more efficient for those with a high level of trait mindfulness. Upon hospital admission, 126 MDD patients completed the Beck Depression Inventory (BDI), World Health Organization Quality of Life Brief, Five-Factor Mindfulness Questionnaire (FFMQ), and the Rumination Response Scale (RRS). The 65 patients that scored less than the median of all subjects on the FFMQ were placed into the low mindfulness level (LML) group. The other 61 patients were placed in the high mindfulness level (HML) group. All facet scores were statistically different between the mental health assessment scores of the HML and LML groups except for RRS brooding and FFMQ nonjudgement. Trait mindfulness level exhibited a negative and bidirectional association with MDD severity primarily through the facets of description and aware actions. Trait mindfulness was also related positively with age primarily through the facets of nonreactivity and nonjudgement. Being married is positively associated with trait mindfulness levels primarily through the facet of observation and by an associated increase in perceived quality of life. Mindfulness training prior to MDD diagnosis also associates positively with trait mindfulness level. Hospitalized MDD patients should have their trait mindfulness levels characterized to predict treatment efficiency, help establish a prognosis, and identify mindfulness-related therapeutic targets.
