Background. Seasonal influenza has a significant impact on public health, generating substantial direct healthcare costs, production losses and fiscal effects. Understanding these consequences is crucial to effective decision-making and the development of preventive strategies. This study aimed to evaluate the economic and the fiscal impact of implementing an incremental strategy for seasonal influenza prevention using the cell-based quadrivalent influenza vaccine (QIVc) among healthcare workers (HCWs) in Italy.Methods: To estimate the economic impact of implementing this strategy, we performed a cost analysis that considered direct healthcare costs, productivity losses and fiscal impact. The analysis considered a 3-year time horizon. A deterministic sensitivity analysis was also conducted.Results: Assuming a vaccination coverage rate of 30% among HCWs, the analysis considered a total of 203 018 vaccinated subjects. On analysing the overall differential impact (including direct costs, indirect costs and fiscal impact), implementing QIVc vaccination as a preventative measure against influenza among HCWs in Italy would yield societal resource savings of euro23 638.78 in the first year, euro47 277.56 in the second year, and euro70 916.35 in the third year, resulting in total resource savings of euro141 832.69.Conclusions:The study demonstrated that implementing the incremental use of QIVc as part of a preventive strategy for seasonal influenza among HCWs in Italy could yield positive economic outcomes, especially in terms of indirect costs and fiscal impact. The resources saved could be utilized to fund further public health interventions. Policy-makers should consider these findings when making decisions regarding influenza prevention strategies targeting HCWs.

Obiettivi - Lo Scompenso Cardiaco (SC) comporta un onere importante per il nostro Ser- vizio Sanitario Nazionale ed è associato ad un elevato carico di malattia legato ai costi, diretti e indiretti, che vengono generati dalla gestione della patologia stessa. La Cardiac Contractility Modulation (CCM) potrebbe rappresentare una nuova possibilità terapeutica per i pazienti con SC. Il presente progetto si propone, pertanto, mediante il coinvolgimen- to di diversi professionisti della salute (cardiologi, elettrofisiologi, professionisti di sanità pubblica) di analizzare e sistematizzare le evidenze attualmente disponibili sulla CCM, al fine di identificare gli elementi chiave per una possibile implementazione di questa tecno- logia innovativa nel contesto di cura italiano. Metodologia - È stata condotta un’analisi della letteratura scientifica sul burden epide- miologico dello SC, sulle caratteristiche del percorso di cura e sull’assorbimento delle risorse per il trattamento dello SC, nonché sulle evidenze scientifiche attualmente dispo- nibili sulla CCM. Risultati - Il burden dello SC riguarda 64,3 milioni di persone in tutto il mondo, diven- tando sempre più comune nei pazienti di età pari o superiore a 65 anni. Circa il 50% dei pazienti con SC muore entro 5 anni dalla diagnosi e si stima che questa sindrome inte- resserà più di 8 milioni di persone nel 2030. Nel corso degli ultimi anni, nuovi trattamenti, farmacologici e no, sono stati sviluppati per la gestione dello SC, come, ad esempio, terapie farmacologie (valsartan, ivabradina ecc.), la terapia di resincronizzazione cardiaca (CRT) e l’ablazione della fibrillazione atriale. Tuttavia, queste opportunità terapeutiche si sono dimostrate efficaci solo nello SC a Frazione di Eiezione (FE) ridotta. The prevalence of overweight and obesity is continuously increasing, both in the adult and pediatric populations, posing a substantial challenge to public health. Understanding the epidemiological burden of metabolic syndrome (MetS) among children, particularly regarding its complications and long-term effects in adulthood, is crucial for identifying effective preventive measures and enhancing the clinical care of obese children. Therefore, by searching two databases, a systematic review was conducted in order to evaluate studies that specifically addressed the epidemiological MetS impact among overweight/obese European children and adolescents. Overall, 15 studies were considered. The epidemiological data concerning the MetS impact were contingent on the diagnostic criteria used and varied across countries, resulting in a prevalence range of 1.44% to 55.8%. Spanish studies were the most numerous (34%), revealing a country prevalence rate ranging from 2.5% to 19.6%. Males (prevalence range: 1.4-55.8%) and subjects with overweight/obesity (prevalence range: 12.9-55.8%) were mainly affected. Obesity emerged as the main risk factor in the MetS development and the consequent onset of cardiovascular complications and diabetes. Knowing the MetS burden and its risk factors could improve their prevention, detection, and treatment, and guide the development of targeted public health interventions to appropriately address the health needs of younger patients.

Le infezioni correlate all’assistenza (ICA) e la resistenza agli antibiotici (Antimicrobial Resistance, AMR) rappresentano una vera e propria emergenza sanitaria, con un impatto rilevante in termini clinici, sociali ed economici [1]. L’utilizzo eccessivo e inappropriato di farmaci antimicrobici è uno dei principali fattori di insorgenza della resistenza agli antibiotici nei patogeni umani, a causa di mutazioni o scambi genetici che ne facilitano la sopravvivenza. Gli effetti della resistenza, ovvero l’incapacità di un antibiotico, somministrato alle dosi terapeutiche, di ridurre la sopravvivenza o inibire la replicazione di un batterio patogeno, comportano gravi rischi di salute pubblica a livello globale, con aumento di mortalità per infezioni e ingenti costi sanitari e sociali. L’antibioticoresistenza è un fenomeno multifattoriale e multisettoriale, contro il quale interventi singoli e sporadici mostrano un impatto limitato [1, 2]. Una delle più importanti conseguenze derivanti da tale fenomeno è rappresentata dalle ICA che, assenti al momento del ricovero, si manifestano in un individuo durante la degenza in ospedale o in un qualsiasi contesto assistenziale, con una sempre più crescente selezione di ceppi di patogeni Multi-Drug Resistant (MDR). A livello europeo, ogni anno, le ICA colpiscono circa 4 milioni di pazienti e sono causa di oltre 37.000 decessi, di cui solo 10.000 in Italia. I costi per la gestione di queste infezioni, inoltre, superano il miliardo di €/anno, con un impatto economico notevole anche in termini di prolungamento dei tempi di degenza e maggior impiego di antibiotici. I dati provenienti dagli ospedali italiani mostrano un’incidenza di ICA del 5 ± 10%, con un tasso di mortalità fino al 20 ± 30% [1-6]. BackgroundCervical Cancer (CC) is a vaccine-preventable disease, and it is treatable if diagnosed early and managed properly. However, it is the fourth most common cancer in women worldwide with about 604,127 cases and 341,831 deaths in 2020. In Italy, it represents the fifth most common cancer in women under 50 years of age with about 2,400 new cases in 2020. The CC elimination is today a global public health goal published by the World Health Organization (WHO) in 2020 and a commitment of the European Union that has included it in Europe's Beating Cancer Plan. Therefore, urgent action is needed, at international and national level, to implement value-based interventions regarding vaccination, screening and timely management of the disease. Our study aims to describe the state of the art of Human Papilloma Virus (HPV) prevention in Italy and to get a consensus on indicators for monitoring the progress toward CC elimination at national level. MethodsThe study envisaged the following activities: research and synthesis of the evidence on strategies and actions for CC elimination at regional Italian level; identification of indicators to monitor such strategies/actions; organization of a multi-stakeholder consensus to reach the agreement on main indicators to be used in Italy. ResultsAs for HPV vaccination coverage, the last Italian available data (December 31st, 2020) showed that it was way below the target (95%) with full cycle vaccination coverage ranging from 6 to 61.7% in female adolescents and from 5.4 to 55.4% in male adolescents (2008 birth cohorts). The coverage rate of CC screening is variable with a range of 61.7-89.6%. Furthermore, coverage rates due to organized screening programs (excluding out-of-pocket screening) shows a range from 20.7 to 71.8%. The efficacy of the first schedule of COVID-19 mRNA vaccines has decreased after the surge of the Delta variant, posing the need to administer a booster dose to enhance the neutralising immune response. This study aims at evaluating the duration of protection given by the booster dose of Pfizer-BioNTech BNT162b2 mRNA vaccine in healthcare workers (HCWs) of a large teaching hospital in Rome and to analyse the factors associated with post-booster vaccination infections. Data about vaccinations of HCWs with the BNT162b2 vaccine and nasal swabs positive for SARS-CoV-2 were extracted from the digital archives of the hospital from 27 September 2021 to 31 May 2022. In total, 5770 HCWs were observed. The cumulative risk of becoming infected by SARS-CoV-2 increased with time (2.5% at 4 weeks, 17% at 12 weeks and 40% at 24 weeks) and was significantly higher for females, younger classes of patients and for those who had developed a hybrid immunity (natural infection plus one dose, namely the primary schedule, added to the booster dose) compared to those who had completed the three doses. This study describes the duration and the determinants of the protection against infections after the booster dose of COVID-19 vaccine, highlighting the need for continuous monitoring of vaccine-induced immunogenicity.

Background: Peristomal skin complications (PSCs) are the most common skin problems seen after ostomy surgery. They have a considerable impact on a patient's quality of life and contribute to a higher cost of care. Methods: A systematic review was conducted, querying three databases. The analysis was performed on international studies focused on the clinical-epidemiological burden of PSCs in adult patients with ileostomy/colostomy. Results: Overall, 23 studies were considered. The main diseases associated with ostomy surgery were rectal, colon and gynecological cancers, inflammatory bowel diseases, diverticulitis, bowel obstruction and intestinal perforation. Erythema, papules, skin erosions, ulcers and vesicles were the most common PSCs for patients with an ostomy (or stoma). A PSCs incidence ranging from 36.3% to 73.4% was described. Skin complications increased length of stay (LOS) and rates of readmission within 120 days of surgery. Conclusions: PSCs data are still limited. A knowledge of their burden is essential to support health personnel and decision-makers in identifying the most appropriate responses to patients' needs. Proper management of these complications plays a fundamental role in improving the patient's quality of life. A multidisciplinary approach, as well as increased patient education and their empowerment, are priority measures to be implemented to foster a value-based healthcare.

BackgroundDiabetic Macular Edema (DME) is the most common cause of vision loss in diabetic patients. Currently, the Vascular Endothelial Growth Factor inhibitors (anti-VEGFs) are used as the first line of DME treatment and corticosteroid implants are usually used as a second-line treatment. These implants are a safe and effective therapeutic option that can improve the quality of life of DME patients by reducing the intravitreal injections number. We determined the economic impact related to DME, also from the social perspective, and the consequences of the increased use of the dexamethasone implant. MethodsThe analysis compares two scenarios: the first based on the current rate of recourse to the therapeutic alternatives available in the Italian healthcare setting (as is) and the second based on the assumption of an increased recourse to dexamethasone implants (to be). The results are expressed both in terms of the resource absorption associated with the two scenarios and in terms of the cost differential yielded by their comparison. ResultsThe increased use of the dexamethasone implant allows considerable savings in terms of healthcare professionals' time, follow-up and productivity lost by patients/caregivers. These savings would reduce healthcare costs for the management of DME patients in Italy by euro2,058,238 in 5 years. ConclusionsTo optimize the healthcare resources allocation, it is necessary to implement treatments that yield not only cost reductions but also a clinical benefit for patients. The dexamethasone implant use is an example of DME management that generates value for patients, health system and society.

SARS-CoV-2 vaccination has been the most effective tool to prevent COVID-19, significantly reducing deaths and hospitalizations worldwide. Vaccination has played a huge role in bringing the COVID-19 pandemic under control, even as the inequitable distribution of vaccines still leaves several countries vulnerable. Therefore, organizing a mass vaccination campaign on a global scale is a priority to contain the virus spread. The aim of this systematic review was to assess whether COVID-19 vaccination campaigns are cost-effective with respect to no vaccination. A systematic literature search was conducted in the WHO COVID-19 Global literature database, PubMed, Web of Science, Embase, and Scopus from 2020 to 2022. Studies assessing the COVID-19 vaccination campaign cost-effectiveness over no vaccination were deemed eligible. The "Drummond's checklist" was adopted for quality assessment. A synthesis of the studies was performed through the "dominance ranking matrix tool". Overall, 10 studies were considered. COVID-19 vaccination was deemed cost-effective in each of them, and vaccination campaigns were found to be sustainable public health approaches to fight the health emergency. Providing economic evaluation data for mass vaccination is needed to support decision makers to make value-based and evidence-based decisions to ensure equitable access to vaccination and reduce the COVID-19 burden worldwide.

In line with how ethics has developed for the last three centuries, public health ethics has been widely dominated by a deontological as well as a utilitarian approach. The latter is a version of consequentialism, which states that maximizing utility is the primary goal of the majority of individuals or group action, while, on the other hand, virtue ethics, or at least the appeal to virtues, has been largely marginalized. The aim of this article is twofold. Firstly, we aim to highlight the political and ethical nature of public health interventions, often interpreted and presented as mere scientific enterprises. Secondly, we try to highlight the need to integrate or at least recognize the value of appeal to virtues in public health measures. The analysis will reference the Italian COVID-19 vaccination program as a case study. Initially, we will explore the political and ethical nature of any public health measure, using the implementation of the COVID-19 vaccination program in Italy as an example. Subsequently, we will illustrate the deontological approach to ethics, the utilitarian one, and the virtues one, focusing on the dynamic of the agent’s perspective. Lastly, we will briefly analyze both the Italian COVID-19 vaccination program and the communication campaign that promoted it.

Vaccine hesitancy in healthcare workers (HCWs) has been studied for various contagious diseases, but there is still insufficient knowledge about this phenomenon for COVID-19. We developed and validated a knowledge, attitude, and practice survey of 39 questions to assess Italian HCWs' hesitancy toward vaccination in general (general hesitancy), COVID-19 vaccination (COVID-19 hesitancy), and public health injunctive measures (refusal of obligations). The survey was administered through a web platform between July and November 2021. Three multivariable logistic regressions were performed to evaluate the association between the explored dimensions of hesitancy and the potential determinants investigated. Out of 2,132 respondents with complete answers, 17.0% showed to be generally hesitancy toward vaccination, 32.3% were hesitant on COVID-19 vaccination, while 18.8% were categorized as refusing obligations. A significant protective effect against all three dimensions of hesitancy was found for increasing fear of COVID-19, advising COVID-19 vaccination to relatives and patients, having received flu vaccination in the previous year and having higher levels of education. Better self-rated knowledge about COVID-19 vaccines and reading up institutional sources were significantly protective against general and COVID-19 hesitancy, while being a physician rather than another healthcare professional was protective only against COVID-19 hesitancy. Conversely, increasing age and referring to colleagues to expand knowledge about COVID-19 were positively associated with COVID-19 hesitancy. The determinants of general hesitancy, COVID-19 hesitancy and the refusal of obligations are mostly overlapping. Given the great influence they exert on patients and communities, it is pivotal to limit HCWs vaccine hesitancy through appropriate training activities.

Healthcare workers (HCWs), particularly physicians, are a relevant and trusted source of information for patients, especially when health-related choices such as vaccination are concerned. Between July and November 2022, we administered a web-based survey to physicians and dentists living in the Latio region of Italy to explore whether their background might affect their willingness to recommend the COVID-19 vaccination to their patients (RCVtoPat) and their relatives (RCVtoRel). We performed a multivariable logistic regression to study the association between the two outcomes (RCVtoPat and RCVtoRel) and their potential determinants in our sample (n = 1464). We found that being a dentist, an increasing fear of COVID-19, and having been previously vaccinated against flu are positively associated with both RCVtoPat and RCVtoRel, while a better self-rated knowledge of COVID-19 vaccines is associated only with RCVtoRel. No role was found for age, sex, civil status, education level, information sources, previous SARS-CoV-2 infection, and chronic diseases. A subgroup analysis of physicians alone (n = 1305) demonstrated a positive association with RCVtoRel of being specialized in diagnostic/therapeutic services and a negative effect on RCVtoPat of being trained in general practice. We provide useful insights about the factors that should be addressed to ensure HCWs exert a positive influence on their patients and communities.

Background: Seasonal influenza has a considerable public health impact, and vaccination is the key to preventing its consequences. Our aim was to describe how the value of influenza vaccination is addressed in the scientific literature considering a new value framework based on four pillars (personal, allocative, technical, and societal value). Methods: A systematic review was conducted by querying three databases. The analysis was performed on international studies focused on influenza vaccination value, and the four value pillars were taken into consideration for their description. Results: Overall, 40 studies were considered. Most of them focused on influenza vaccination in the general population (27.5%), emphasizing its value for all age groups. Most studies addressed technical value (70.4%), especially in terms of economic models and cost drivers to be considered for the economic evaluations of influenza vaccines, and societal value (63%), whereas few dealt with personal (37%) and allocative values (22.2%). Conclusions: The whole value of influenza vaccination is still not completely recognized. Knowledge and communication of the whole value of influenza vaccination is essential to guide value-based health policies. To achieve this goal, it is necessary to implement initiatives that involve all relevant stakeholders.

Influenza has a significant impact on the health care system and also on production and economic systems. Vaccinated health care workers (HCWs) are more likely to have improved productivity compared to unvaccinated workers. The study aim was to estimate the economic and fiscal impact of an influenza vaccination program for HCWs in Italy. We performed a cost analysis aimed to estimate the indirect costs (productivity losses due to working days lost) and the increase in tax revenues derived from the increase in vaccination coverage among HCWs. Assuming an incremental increase in vaccination coverage of 10% per year over a period of 5 years, total savings could be obtained in terms of a reduction in productivity losses equal to -(sic)4,475,497.16 and an increase in tax revenues of (sic)327,158.84. This revenue could be used to finance other health interventions. Our results are fundamental in view of the sustainability of health systems and of a value-based allocation of health resources. Therefore, a complete social perspective, including the fiscal impact of flu vaccination, should be adopted to assess the economic value of influenza vaccines. Currently, health policies based on the whole value of flu vaccination are needed.

Background: Pediatric influenza causes significant morbidity annually, resulting in an increased economic burden. Therefore, we aimed to summarize existing literature regarding the costs of pediatric influenza in Europe, paying particular attention to the direct and indirect costs considered in the economic evaluations. Knowing health and social costs of childhood influenza is essential to support value-based health decisions to implement effective immunization strategies. Methods: We searched three databases for articles published to September 3, 2021. Eligible studies were those reporting the economic burden of influenza in the pediatric and youth population in European countries written in English language. Results: Overall, 2225 records were screened, and 9 articles were included. Costs estimates are different across countries and in the age groups considered. Direct costs per episode, whose major expense driver are hospitalizations and pediatric examinations, range from about €74 in Italy to €252 in Germany. Important variations are observed based on age, with the youngest group absorbing in some cases double the resources of the older ones such as (in Italy, in France and in Germany). Regarding indirect costs, workdays lost by parents resulted in higher costs for children <2 years and 2–5 years than those >5 years of age and their economic impact was variable reaching €251 per week in Germany. Conclusion: Evidence obtained in our review strengthened the awareness about the economic impact, in terms of direct and indirect costs, of pediatric influenza requiring, as a priority action in Europe, the implementation of influenza vaccination policies in this target population.

Background: Health Technology Assessment (HTA) is a multidisciplinary process that synthesizes, with a systematic, transparent, impartial and robust methodological approach, the main information on the medical, economic, ethical and social implications of the use and dissemination of a health technology. Its aim is to support decision-makers in identifying safe, effective, patient-centered and best-value health policies, in order to promote an equitable, efficient, and high-quality health system. Given the continued application of innovative technologies into clinical practice, healthcare professionals need to be able to adequately evaluate these technologies using evidence-based approaches such as HTA. Therefore, the implementation of training in HTA is crucial. The aim of this study was to investigate existing HTA training initiatives for healthcare professionals provided by international HTA agencies and organizations around the world. Methods: From March to November 2020, the websites of HTA agencies and organizations belonging to the European network for HTA (EUnetHTA) and to the International Network of Agencies for HTA (INAHTA), and the website of the HTA International (HTAi), were explored for identifying the HTA training initiatives directed to healthcare professionals. In addition, we screened the training initiatives proposed at European level by EUnetHTA as part of its Joint Actions and conducted in collaboration with its public-private partners. Specific keywords were searched in English and adapted to French, Portuguese, Spanish, Italian and German. Data extraction of the retrieved training initiatives was conducted from November 2020 to February 2021 and considered the following information: agency, country, website, coordinator, type of initiative, target, topic, main contents, and language. Results: Out of 124 agencies/organizations/EUnetHTA public-private partners screened, only 21 provided training initiatives for healthcare professionals. A total

Objectives: To provide a new value-based immunization approach collating the available scientific evidence on the topic. Methods: Four value pillars (personal, allocative, technical, and societal) applied to vaccination field were investigated. A systematic literature review was performed querying three database from December 24th, 2010 to May 27th, 2020. It included studies on vaccine-preventable diseases (VPDs) that mentioned the term value in any part and which were conducted in advanced economies. An in-depth analysis was performed on studies addressing value as key element. Results: Overall, 107 studies were considered. Approximately half of the studies addressed value as a key element but in most of cases (83.3%) only a single pillar was assessed. Furthermore, the majority of papers addressed the technical value by looking only at classical methods for economic assessment of vaccinations whereas very few dealt with societal and allocative pillars. Conclusions: Estimating the vaccinations value is very complex, even though their usefulness is certain. The assessment of the whole value of vaccines and vaccinations is still limited to some domains and should encompass the wider impact on economic growth and societies.

Background. The elderly, commonly defined as subjects aged ≥65 years, are among the at-risk subjects recommended for annual influenza vaccination in European countries. Currently, two new vaccines are available for this population: the MF59-adjuvanted quadrivalent influenza vaccine (aQIV) and the high-dose quadrivalent influenza vaccine (hdQIV). Their multidimensional assessment might maximize the results in terms of achievable health benefits. Therefore, we carried out a Health Technology Assessment (HTA) of the aQIV by adopting a multidisciplinary policy-oriented approach to evaluate clinical, economic, organizational, and ethical implications for the Italian elderly. Methods. A HTA was conducted in 2020 to analyze influenza burden; characteristics, efficacy, and safety of aQIV and other available vaccines for the elderly; cost-effectiveness of aQIV; and related organizational and ethical implications. Comprehensive literature reviews/analyses were performed, and a transmission model was developed in order to address the above issues. Results. In Italy, the influenza burden on the elderly is high and from 77.7% to 96.1% of influenza-related deaths occur in the elderly. All available vaccines are effective and safe; however, aQIV, such as the adjuvanted trivalent influenza vaccine (aTIV), has proved more immunogenic and effective in the elderly. From the third payer’s perspective, but also from the societal one, the use of aQIV in comparison with egg-based standard QIV (eQIV) in the elderly population is cost-effective. The appropriateness of the use of available vaccines as well as citizens’ knowledge and attitudes remain a challenge for a successful vaccination campaign. Conclusions. The results of this project provide decision-makers with important evidence on the aQIV and support with scientific evidence on the appropriate use of vaccines in the elderly.

Introduction. Citizens’ literacy in the field of genomics represents one of the cornerstones of proper implementation of genomics in healthcare services. In order to identify the most effective tools by which to elevate citizens’ literacy in genomics, we conducted a survey among the group of multidisciplinary experts within the Italian Network of Genomics in Public Health (GENISAP). Methods. Two rounds of Delphi surveys were carried out in order to identify the main topics, tools, settings, and healthcare professionals’ backgrounds that might usefully be included in citizens’ training initiatives in genomics. To this end, we distributed a questionnaire with 39 items that are scored on a 5-point scale. Results. By the end of the Delphi process, 43 items were selected (19 for the topics, 6 for the tools, 9 for settings, and 9 for the healthcare professionals’ backgrounds). Genomic tests and counseling were among the main topics included, while in terms of tools, faceto-face discussion with healthcare professionals was prioritized by the experts. Among the most appropriate platforms, the group suggested internet/social media and healthcare settings. The healthcare professional considered to have the most relevant role in terms of citizens’ education was a medical doctor with a specialism in clinical genetics. Discussion. Our study attempted to identify the main characteristics that could guide the design of interventions to promote public literacy regarding the field of genomics . Specifically, we have identified the main topics to be included in an educational program for citizens, the tools and settings to consider when providing educational initiatives on genomics, and the healthcare professionals who need to be involved in these initiatives. The disruptive advances in genomics contributed to achieve higher levels of precision in the diagnosis and treatment of cancer. This scientific advance entails the need for greater literacy for all healthcare professionals. Our study summarizes the training initiatives conducted worldwide in cancer genomics field for healthcare professionals. We conducted a web search of the training initiatives aimed at improving healthcare professionals’ literacy in cancer genomics undertaken worldwide by using two search engines (Google and Bing) in English language and conducted from 2003 to 2021. A total of 85,649 initiatives were identified. After the screening process, 36 items were included. The majority of training programs were organized in the United States (47%) and in the United Kingdom (28%). Most of the initiatives were conducted in the last five years (83%) by universities (30%) and as web-based modalities (80%). In front of the technological advances in genomics, education in cancer genomics remains fundamental. Our results may contribute to provide an update on the development of educational programs to build a skilled and appropriately trained genomics health workforce in the future.

Introduction: Health systems worldwide need to pay attention to both sustainability and quality. The explosion of health technologies represents a challenge for health systems’ sustainability, and evidence-based tools should support resources allocation to guarantee a continuous quality improvement. The value-based approach could disentangle the full benefit of a health technology, and this is of utmost importance in the vaccination field because of several obstacles still existing in reaching optimal vaccination uptake. Areas covered: The paper conveys the evidence on the full value of vaccine(s)/vaccination based on the framework suggested by the Expert Panel on Effective Ways of Investing in Health of the European Commission. Indeed, evidence on the personal, technical, allocative, and societal value of vaccine(s)/vaccination published in the last decade was described as foundation of a following consultation with international experts of the field. The result was the issuing of recommendations for research, decision-making, and public engagement that aimed to drive a value-based decision-making on vaccination. Expert opinion: The development of vaccination programs based on the recognition of the full value of vaccine(s)/vaccination is essential. To achieve this goal, it is necessary to launch intersectoral and multidisciplinary research and implementation initiatives involving all relevant stakeholders.

Human Papillomavirus (HPV) is the most common sexually transmitted infection. Its progression is related to the development of malignant lesions, particularly cervical intraepithelial neoplasias (CINs). CINs correlate with a higher risk of premature births, and their excisional and ablative treatment further increases this risk in pregnant women. These complications are also correlated with higher healthcare costs for their management. In Italy, more than 26,000 new cases of CINs are estimated to occur yearly and their economic burden is significant. Therefore, the management of these conditions is a public health priority. Since HPV vaccination is associated with a reduced risk of relapse in women surgically treated for HPV-related injuries, we estimated the economic impact of extending HPV vaccination to this target population. This strategy would result in a significant reduction in the general costs of managing these women, resulting in an overall saving for the Italian Health Service of € 155,596.38 in 5 years. This lower cost is due not only to the reduced incidence of CINs following vaccination, but also to the lower occurrence of preterm births. Extending HPV vaccination to this target population as part of a care path to be offered to women treated for HPV injuries is therefore desirable.

Il vaccino antinfluenzale Fluenz Tetra® (vaccino vivo attenuato quadrivalente, spray nasale, in sospensione) sarà disponibile in Italia per la prevenzione e il controllo dell’influenza per la stagione influenzale 2021-2022. Tale vaccino è indicato per la profilassi dell’influenza in bambini e adolescenti di età compresa tra i 24 mesi e i 18 anni [1]. I decision makers dovranno, quindi, presto valutarne l’utilizzo. Questa decisione potrebbe essere supportata dalla disponibilità di uno specifico rapporto di Health Technology Assessment (HTA). Infatti, l’HTA rappresenta oggi l’approccio valutativo più utile per i decision makers per prendere le più appropriate decisioni in ambito sanitario in base alle prove scientifiche disponibili. Anche il Piano Nazionale Prevenzione Vaccinale 2017-2019 (PNPV 2017-2019) conferma il ruolo rilevante dell’HTA ai fini della valutazione delle potenzialità e dei limiti di un vaccino e auspica un sempre maggiore utilizzo di questa metodologia per favorire le decisioni basate sulle evidenze, in un’ottica di corretto utilizzo delle limitate risorse disponibili [2]. In ambito vaccinale l’HTA si esplica nella valutazione dell’epidemiologia della specifica infezione e delle malattie correlate nella popolazione target, del disease burden della patologia, delle attuali misure preventive e terapeutiche disponibili contro l'infezione e le malattie esaminate, dell’efficacia e sicurezza del vaccino oggetto di studio e degli eventuali altri vaccini disponibili, delle valutazioni economiche, e infine degli aspetti etici, legali, sociali e organizzativi correlati all’introduzione di tale vaccino [3]. Pertanto, tutte le evidenze scientifiche disponibili su questi aspetti dovrebbero essere raccolte e esaminate criticamente all’interno di un rapporto di HTA al fine di eseguire una valutazione completa. Background: Vaccination coverages need to be constantly maintained and improved with the implementation of vaccination strategies. This paper describes the development of an evidence-based tool to guide their planning and evaluation. Methods: A scoping review was performed in MEDLINE and institutional websites to search for similar available tools. A first version of the tool was developed considering review results and a four-step method used for the control and continuous improvement of processes and products, namely the Deming cycle. A panel of eight experts was then involved in a Delphi study for the finalization of the tool that was eventually discussed in a face-to-face meeting. Results: The scoping review found only one document and the first version of the tool was composed of 30 items. After the Delphi first round, 11 additional items were suggested and 5 original items amended. After the Delphi second round 41 items were eventually included. During the face-to-face meeting, 7 items were recognized as requisites for setting vaccination strategies, whereas 17 as relevant ones. Conclusions: Current public health challenges impose the need for evidence-based tools to organize effective vaccination strategies. Our tool is a first proposal which aims to reflect this focus.

Introduction: Population awareness and empowerment in omics sciences represent a fundamental driver to increase the adoption of evidence-based approaches in personalized medicine. In this context, a pivotal role is played by citizens’ literacy, and educational initiatives carried out in this context are key assets to drive future effective interventions. With the present study, we summarized the educational initiatives conducted worldwide aimed at increasing citizens’ literacy in omics sciences. Materials and Methods: We conducted a web search of the educational initiatives aimed at improving citizens’ literacy in omics sciences undertaken worldwide, by using three search engines (Google, Bing, and Yahoo Search), in English and in Italian languages. Results: We identified five initiatives in Europe, 22 in non-European countries, and 13 in Italy. Overall, the majority (69%) were web-based initiatives, while 31% required in-person attendance. The online initiatives included web pages for reading, online lessons/courses, web portals, videos/short movies, animations, and apps for mobile devices. The residential initiatives, on the other hand, included exhibitions, seminars, courses, symposia, information stands in public places, guided visits to research laboratories, and interactive laboratories. All the initiatives were highly heterogeneous in terms of methodologies and the topics addressed. Discussion and Conclusion: Overall, we identified a variety of initiatives aimed at improving citizens’ literacy in omics sciences, with the largest majority carried out in the United States and being web-based. Our results showed heterogeneity among the initiatives as to the dealt topics and the adopted methods. Further research is needed, however, to quantitatively assess the effectiveness of educational initiatives to improve citizens’ literacy in omics sciences.

Background: The rapid spread of personalized medicine requires professionals to manage the “omics revolution.” Therefore, the genetics/genomics literacy of healthcare professionals should be in line with the continuous advances in this field, in order to implement its potential implications for diagnosis, control and treatment of diseases. The present study investigates the effectiveness of a distance learning course on genetics and genomics targeted at medical doctors. Methods: In the context of a project funded by the Italian Ministry of Health, we developed a distance learning course, entitled Genetics and Genomics practice. The course focused on genetic/genomics testing, pharmacogenetics and oncogenomics and was developed according to andragogical training methods (Problem-based Learning and Case-based Learning). We used a pre-test vs. post-test study design to assess knowledge improvement on a set of 10 Multiple Choice Questions (MCQs). We analyzed the proportion of correct answers for each question pre and post-test and the mean score difference stratified by gender, age, professional status and medical discipline. Moreover, the test was submitted to the participants 8 months after the conclusion of the course (follow-up), in order to assess the retained knowledge. Results: The course was completed by 1,637 Italian physicians, most of which were primary care physicians (20.8%), public health professionals (11.5%) and specialist pediatricians (10.6%). The proportion of correct answers increased in the post-test for all the MCQs. The overall mean score significantly increased, from 59.46 in the pre-test to 71.42 in the post-test (p < 0.0001). The comparison in test performance between follow-up and pre-test demonstrated an overall knowledge improvement. Conclusion: Genomics literacy among healthcare professionals is essential to ensure optimal translation to healthcare delivery of research. The development of high-throughput omics technologies represents an unmissable opportunity for evidence-based prevention of adverse effects on human health. However, the applicability and access to multi-omics tests are limited. In Italy, this is due to the rapid increase of knowledge and the high levels of skill and economic investment initially necessary. The fields of human genetics and public health have highlighted the relevance of an implementation strategy at a national level in Italy, including integration in sanitary regulations and governance instruments. In this review, the emerging field of public health genomics is discussed, including the polygenic scores approach, epigenetic modulation, nutrigenomics, and microbiomes implications. Moreover, the Italian state of implementation is presented. The omics sciences have important implications for the prevention of both communicable and noncommunicable diseases, especially because they can be used to assess the health status during the whole course of life. An effective population health gain is possible if omics tools are implemented for each person after a preliminary assessment of effectiveness in the medium to long term.

The last 2019/20 northern hemisphere influenza season overlapped with the first wave of coronavirus disease 2019 (COVID-19) pandemic. Italy was the first western country where severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spread to a significant extent. In this representative cross-sectional survey, we aimed to describe some opinions and attitudes of the Italian general population towards both influenza vaccination and the COVID-19 pandemic, and to identify potential modifiers of the decision-making process regarding the uptake of the 2020/21 influenza vaccine. A total of 2543 responses were analyzed. Although most (74.8%) participants valued influenza vaccination positively and declared that it should be mandatory, some misconceptions around influenza persist. The general practitioner was the main source of trusted information on influenza vaccines, while social networks were judged to be the least reliable. Younger and less affluent individuals, subjects not vaccinated in the previous season, and those living in smaller communities showed lower odds of receiving the 2020/21 season influenza vaccination. However, the COVID-19 pandemic may have positively influenced the propensity of being vaccinated against 2020/21 seasonal influenza. In order to increase influenza vaccination coverage rates multidisciplinary targeted interventions are needed. The role of general practitioners remains crucial in increasing influenza vaccine awareness and acceptance by effective counselling.

Influenza carries an enormous burden each year. Annual influenza vaccination is the best means of reducing this burden. To be clinically effective, influenza vaccines must be immunogenic, and several immunological assays to test their immunogenicity have been developed. This study aimed to describe the patterns of use of the various immunological assays available to measure the influenza vaccine-induced adaptive immune response and to determine its correlates of protection. A total of 76.5% of the studies included in our analysis measured only the humoral immune response. Among these, the hemagglutination-inhibition assay was by far the most widely used. Other, less common, humoral immune response assays were: virus neutralization (21.7%), enzyme-linked immunosorbent (10.1%), single radial hemolysis (4.6%), and assays able to quantify anti-neuraminidase antibodies (1.7%). By contrast, cell-mediated immunity was quantified in only 23.5% of studies. Several variables were significantly associated with the use of single assays. Specifically, some influenza vaccine types (e.g., adjuvanted, live attenuated and cell culture-derived or recombinant), study phase and study sponsorship pattern were usually found to be statistically significant predictors. We discuss the principal findings and make some suggestions from the point of view of the various stakeholders.

The World Health Organization (WHO), the United States (US) Centers for Disease Control and Prevention (CDC), the European Center for Disease Control (ECDC), and the immunization guidelines of many countries issue vaccination recommendations for adults and the elderly. However, the uptake of vaccination in these groups is generally low due to several reasons. The present study aimed to identify strategies implemented in Italy in unconventional settings to promote vaccination against influenza, pneumococcal, and herpes zoster virus (HZV) infections among these subjects, i.e., the at-risk adult population and the elderly. We conducted a literature review and a survey of experts. The literature search yielded seven strategies; all of these concerned influenza vaccination, while three also addressed pneumococcal and HZV vaccination. The survey of experts identified 15 strategies; 10 regarded influenza vaccination, while four regarded pneumococcal vaccination and one regarded HZV vaccination. Most of the strategies were implemented in hospital clinics and rest homes. Regarding influenza and pneumococcal vaccinations, the target population mainly comprised at-risk adults, while the elderly represented the main target population for HZV vaccination. Our results show that, in Italy, there are initiatives aimed at promoting vaccination in unconventional settings, but further efforts are required to assess their effectiveness and to further extend them.

Background: The huge development of omics sciences is changing the classical medical approach and making new technologies available. In this context, education of citizens is essential to allow appropriate decisions about their own health. Hence, we aimed to summarize existing literature regarding citizens' knowledge, attitudes, and educational needs on omics sciences. Methods: We performed a systematic literature review (SLR) using Pubmed, ISI Web of Science, and Embase databases. The eligibility criteria for inclusion in this review required that the studies investigated knowledge, attitudes, or educational needs regarding omics sciences among the general population. Results: We included 54 studies, published between 2006 and 2020. Most of the included studies (72%) investigated citizens' knowledge, half of them (56%) attitudes, and 20% educational needs in the field of omics sciences, while 52% investigated attitudes and perceptions about genetic and/or omics tests. Most studies (64%) reported a limited knowledge level among citizens, even though most (59%) reported participants understood the benefits of the use of omics sciences into medicine. As for omics tests, a controversial opinion toward their use into practice was reported among citizens. Most of the studies (82%) investigating citizens' educational needs highlighted a clear gap to be filled. Conclusions: Our SLR summarizes current knowledge on citizens' literacy, attitudes, and educational needs on omics science, underlining the need for strengthening public engagement on this topic. Further research is needed, however, to identify appropriate methods and models to achieve such an improvement.

Background: achieving Human Papilloma Virus (HPV) - related diseases control is an important challenge in public health. In Italy HPV vaccination uptake does not rise a sufficient level. The aim of this project is to identify strategies to promote HPV vaccination in Italy.Methods: an integrated approach consisting of a systematic review and a two-step panel consultation was used to identify strategies to increase vaccination uptake among adolescents, population target of the national vaccination program, and to promote vaccination in additional targets. Overall, ten experts in the fields of Gynecology, Public Health, General Practice and Pediatrics were involved along with Patients representatives. Recommendations were elaborated according to a set of criteria drawn from the Evidence to Decision (EtD) framework.Results: the systematic review led to the identification of three categories of strategies: reminds, education and multicomponent approaches respectively. A strong recommendation was formulated to use reminds tailored to vaccine recipients or their parents, and a moderate recommendation to use reminds directed to health professionals. A moderate recommendation was developed on the implementation of multicomponent interventions. A strong recommendation was yielded with respect to the promotion of HPV vaccination among women already treated for HPV-related diseases, fertile women not previously vaccinated and 25 year-old women undergoing cervical cancer screening. Lastly, a strong recommendation was formulated for catch-up initiatives targeted to women and men turning 18 years of age.Conclusion: this project led to the identification of several valuable strategies to improve HPV vaccination and strengthen HPV-related diseases control at national level.

Document recommendation systems for locating relevant literature have mostly relied on methods developed a decade ago. This is largely due to the lack of a large offline gold-standard benchmark of relevant documents that cover a variety of research fields such that newly developed literature search techniques can be compared, improved and translated into practice. To overcome this bottleneck, we have established the RElevant LIterature SearcH consortium consisting of more than 1500 scientists from 84 countries, who have collectively annotated the relevance of over 180 000 PubMed-listed articles with regard to their respective seed (input) article/s. The majority of annotations were contributed by highly experienced, original authors of the seed articles. The collected data cover 76% of all unique PubMed Medical Subject Headings descriptors. No systematic biases were observed across different experience levels, research fields or time spent on annotations. More importantly, annotations of the same document pairs contributed by different scientists were highly concordant. We further show that the three representative baseline methods used to generate recommended articles for evaluation (Okapi Best Matching 25, Term Frequency-Inverse Document Frequency and PubMed Related Articles) had similar overall performances. Additionally, we found that these methods each tend to produce distinct collections of recommended articles, suggesting that a hybrid method may be required to completely capture all relevant articles. The established database server located at https://relishdb.ict.griffith.edu.au is freely available for the downloading of annotation data and the blind testing of new methods. We expect that this benchmark will be useful for stimulating the development of new powerful techniques for title and title/abstract-based search engines for relevant articles in biomedical science.

Seasonal influenza is the leading infectious disease in terms of its health and socioeconomic impact. Annual immunization is the most efficient way to reduce this burden. Several correlates of influenza vaccine-induced protection are commonly used, owing to their ready availability and cheapness. Influenza vaccine-induced immunogenicity is a function of host-, virus-and vaccine-related factors. Host-related factors constitute the most heterogeneous group. The objective of this study was to analyze the available systematic evidence on the host factors able to modify influenza vaccine-induced immunogenicity. An umbrella review approach was undertaken. A total of 28 systematic reviews/meta-analyses were analyzed—these covered the following domains: intravenous drug use, psychological stress, acute and chronic physical exercise, genetic polymorphisms, use of pre-/pro-/symbiotics, previous Bacillus Calmette–Guérin vaccination, diabetes mellitus, vitamin D supplementation/deficiency, latent cytomegalovirus infection and various forms of immunosuppression. In order to present effect sizes on the same scale, all possible meta-analyses were re-performed and cumulative evidence synthesis ranking was carried out. The meta-analysis was conducted separately on each health condition category and virus (sub)type. A total of 97 pooled estimates were used in order to construct an evidence-based stakeholder-friendly map. The principal public health implications are discussed.

A Surgical Suite (SS) is a complex system in which different healthcare professionals work. Inefficient management could lead to waste of money and time and reduce quality of care. The aim of the study was to carry out an organizational assessment of a SS in northern Italy, in order to identify weaknesses and inadequacies related to its performance and promote strategies to increase efficiency. The study was conducted by process mapping of the working context, qualitative and quantitative analysis of patient documents and an evaluation of the critical issues using the root cause analysis (RCA) tool. The Plan Do Check Act (PDCA) method was used to implement the necessary changes. A detailed description of the staff involved, medical devices available, organization and timing of the SS was performed. Inefficiencies in the unit were caused mainly by insufficient medical devices and underusage of the radiological software Picture Archiving and Communication System (PACS). Root causes of inefficiencies were identified and classified into four areas: organization/structure, personnel, technologies and methods. In particular, critical issues were identified in: the planning processes, the heterogeneity of technical and Information technology skills and educational background of nursing staff, the presence of several computerized information systems and lack of a connection interface between the different software, the lack of internal procedures and paths and lack of continuing professional education opportunities. Two multidisciplinary working tables were launched by the hospital management in order to identify improvement strategies. The evaluation allowed us to define the root causes of SS inefficiency in this hospital, leading to a reorganization with a view to continuous improvement. An innovative aspect of the present study was the use of RCA to perform an organizational assessment in healthcare, rather than as a reactive risk management tool.

INTRODUZIONE L'audit clinico è uno strumento di miglioramento della qualità assistenziale basato su sistematica revisione, azioni di miglioramento e monitoraggio nel tempo. Scopo dello studio è stato valutare l’implementazione e l'efficacia dell’audit clinico nelle emergenze sanitarie. METODI È stata effettuata una revisione di letteratura, dal 2004 al 2015, tramite algoritmo di ricerca ad hoc e consultazione di banche dati (PubMed e CINAHL). Gli articoli sono stati selezionati da due revisori indipendentemente in base a titolo, abstract e full text. Sono stati inclusi audit clinici svolti nell’area delle emergenze, a livello di assistenza primaria e secondaria. Gli audit focalizzati su formazione, percezione degli operatori sanitari o conformità a procedure/protocolli, dati economici o esclusiva analisi della mortalità sono stati esclusi dalla revisione. Da ogni articolo sono stati estratti i seguenti dati: primo autore e anno di pubblicazione, disegno di studio, Paese e durata, popolazione, obiettivi generali e specifici, indicatori utilizzati e principali risultati. RISULTATI I 62 articoli selezionati (Tab. 1) tra i 6164 emersi dalla ricerca sono stati effettuati prevalentemente in Europa (51,61%) e Oceania (27,42%). Il 64,51% degli audit è stato condotto da un team multidisciplinare e il 51,61% si è focalizzato sui processi, il 33,87% sui risultati e il 14,52% su entrambi; l’88,71% è stato condotto in ospedale e l’11,29% sul territorio, con vari campi di applicazione (ad es. cardiologia e neuroscienze); il 15% (Fig. 1) erano audit completi (valutazione e confronto con standard, azioni di miglioramento e monitoraggio nel tempo).CONCLUSIONI L’audit clinico è un valido strumento per valutare la pratica clinica (processi ed esiti) e migliorare la qualità assistenziale nelle emergenze, soprattutto a livello ospedaliero. INTRODUZIONE Le malattie genetiche sono associate ad alta morbilità, mortalità ed elevato impatto socio-sanitario ed economico. Negli ultimi 40 anni, pochi studi hanno indagato la frequenza e l’impatto delle malattie genetiche negli ospedali pediatrici. Questo progetto si propone di valutare, per la prima volta in Italia, il burden delle malattie genetiche e a larga componente genetica, presso un Centro nazionale e internazionale di eccellenza per le cure pediatriche - l’Ospedale Pediatrico Bambino Gesù (OPBG) - al fine di comprenderne le implicazioni per il SSN. METODI E’ stato condotto uno studio retrospettivo dei dati di attività ospedaliera, su base annuale (2014) e comprensivo di ricoveri ordinari (RO) e day hospital (DH). RISULTATI. Oltre il 33% dei RO e l’1,2% dei DH si correla a malattie genetiche o a larga componente genetica. I maggiori volumi di attività ospedaliera si correlano con malformazioni congenite (42,2%), malattie da mutazioni somatiche ossia tumori (18,4%) e malattie multifattoriali (15,5%) (Figura 1). La percentuale dei pazienti extraregione è pari al 43,7%, di cui il 21,7% si sposta dalla propria regione a causa di malformazioni congenite, il 7,6% per malattie mendeliane e il 6% per patologie oncologiche. CONCLUSIONI I nostri dati sottolineano che oggi le malattie geneticamente determinate o a larga componente genetica rappresentano uno dei principali bisogni di salute della popolazione. Conoscere il “peso” complessivo delle malattie genetiche è di estremo interesse per la sanità pubblica, in quanto il percorso assistenziale di questi pazienti è molto oneroso per il SSN. In questa fase storica in cui la sostenibilità dei sistemi sanitari appare fortemente compromessa, diventa importante non solo conoscere la numerosità della casistica dei pazienti pediatrici affetti da patologie genetiche, ma anche comprenderne l’impatto sul SSN e garantire ai pazienti una migliore programmazione dei servizi assistenziali di cui hanno bisogno.

I percorsi clinico assistenziali (PCA) sono strumento fondamentale nella gestione di realtà ospedaliere complesse e di patologie che necessitano di elevata integrazione. Essi consentono una migliore gestione delle risorse, una riduzione dei tempi di attesa e una presa in carico globale del paziente. L’ospedale “Fondazione A Gemelli” ha sviluppato negli ultimi anni diversi PCA tra questi recentemente è stato approvato quello dedicato ai pazienti con lesione solida cerebrale. Il PCA dedicato al paziente con lesione solida cerebrale è stato sviluppato sotto la guida dell’unità operativa del “Governo clinico” con la partecipazione di tutte le figure professionali coinvolte nelle cure. Sulla base delle evidenze scientifiche e della realtà ospedaliera di riferimento è stata creata una flow-chart in cui sono riportate nell’asse verticale Ie fasi di cura e nell’asse orizzontale i luoghi di cura. Tre sotto-gruppi si sono occupati rispettivamente di definire il contesto epidemiologico dei pazienti che accedono presso la nostra struttura per lesione cerebrale; redigere la flow-chart; definire un insieme di indicatori di qualità per il monitoraggio delle cure erogate. Il lavoro finale è stato approvato da tutti i professionisti coinvolti. I gruppi di lavoro hanno prodotto un report finale comprensivo di flow-chart e di indicatori di qualità riguardanti 5 aree valutative sottoposti alla approvazione di tutti gli stakeholder coinvolti. I principali risultati sono stati lo snellimento del percorso del paziente e l’introduzione delle riunioni multi-disciplinari (Tumor Board) come strumento di decisione clinica. Con l'applicazione del PCA sono inoltre migliorate la gestione e l’integrazione dell'ospedale con il territorio. Il coinvolgimento di tutti i professionisti che operano nella cura della patologia è un elemento chiave.

Background Genetic diseases are an important cause of infant mortality and morbidity and have a high impact on social-health system. About 50-70% of pediatric patients suffer from genetic disorders or diseases with a significant genetic component. At the time, the key role of genetics in understanding the biological basis of the disease has been well defined but its real impact remains to be defined. The aim of this study is to assess the impact of genetic disorders through the ‘‘genetic encoding’’ of the International Classification of Diseases-9th Revision (ICD9), currently used in Italy, in order to understand the implications for the healthcare system. Methods This study included the following activities: ‘‘Genetic’’ classification of the diseases according to scientific literature; ‘‘Genetic encoding’’ of disorders present in ICD9 using as resource the Online Mendelian Inheritance in Man (OMIM); Selection of ICD9 codes relevant for the study. Results The analysis of literature resulted in a ‘‘genetic’’ classification of 8 classes of diseases: Chromosomal anomalies, Monogenic disorders, Congenital Malformations, Mitochondrial diseases, Genomics diseases, Somatic cell genetic diseases, Multifactorial Disorders, Probably genetic disorders. According to the ‘‘genetic encoding’’ of the ICD9 about 29% of ICD-codes is ‘‘genetic’’. The most represented classes are: Multifactorial Disorders (37,9%), Somatic cell genetic diseases (30,6%), Congenital Malformations (14%) and Monogenic disorders (10,6%). Conclusions Genetic diseases are quite common and costs for society and for the health system are very high. The ICD9 has some limitations: for example it does not include all genetic diseases currently known with consequent possible coding errors by clinicians and incorrect correlation with Diagnosis Related Groups. Background Clinical audit is a healthcare quality improvement process based on systematic review of care, implementation of change and further monitoring over time. The aim of this study was to investigate the level of application and effectiveness of clinical audit in the particular field of healthcare emergency. Methods A systematic literature search was carried out, from 2004 to 2015, by a specific search algorithm through PubMed and CINAHL databases. Resulting studies were selected by two reviewers independently according to title, abstract and full text. Inclusion criteria were: clinical audits in emergency area at both primary and secondary care level. Audits focused on training, healthcare professionals’ perception or compliance to procedures/protocols, health economics or death analysis only were excluded from the review. From each article the followinginformation were extracted: first author and year of publication, study design, country and duration, study population, setting, main and specific objectives, indicators used and main results. Results Out of the 6164 retrieved studies 62 were finally selected, which were mainly carried out in Europe (51,61%) and Oceania (27,42%); 64,51% of audits were performed by a multidisciplinary team and 51,61% were focused on process, 33,87% on outcomes and 14,52% on both aspects; 88,71% were conducted at hospital and 11,29% at primary care level with various fields (e.g. cardiology, neuroscience) of application. Out of the 62 studies about 15% were complete audits (assessment and comparison with standard phase followed by improvement actions and monitoring over time). Conclusions Even in emergency clinical audit can be a valuable tool to assess clinical practice both in terms of process and outcome and improve healthcare quality, mainly at hospital level. Indeed in hospital medical records and data flows are available, hospital team are more cohesive than territorial ones and hospital is more organized than primary care

Issue Evidence-based decision-making has become the leading framework for hospital management. Clinical pathways are evidence-based tools supporting healthcare delivery by linking best scientific evidence and clinical practice. Description of the problem ‘‘Fondazione A. Gemelli‘‘ hospital developed several clinical pathways to pursue a continuous healthcare quality improvement. A new one for patients with cerebral lesion has been recently approved to reduce waiting times, hospital length of stay and to offer a leaner care. All professionals involved in the care of these patients leaded by the Clinical Governance Directorate cooperated for the project. A flow-chart pathway has been designed on the basis of evidence and current practice in a diagram showing on the vertical axis the stages of care and on the horizontal axis the places of care. Health professionals were divided into three teams carrying out: analysis of the epidemiological context of patients with cerebral lesion attending the hospital, design of the flow-chart pathway and definition of quality indicators useful for healthcare monitoring. After joint meetings and debates of the working groups, the whole clinical pathway was approved by all of the involved professionals.Results A report with flowchart, description of the pathway activities and 15 indicators covering 5 quality dimensions was drawn up approved and shared by the stakeholders. Tumor Boards were introduced in hospital practice and multidisciplinary approach in medical management of patients was strengthened. Case management and integration between primary and secondary care were also implemented. Healthcare efficiency, patients’ satisfaction and outcomes were improved. Lessons Clinical Pathway is a valid instrument for addressing health needs and resources and to improve efficiency in terms of timing and resources. Commitment of all professionals involved in the pathway of care is indispensable.

According to the National Institute for Health and Care Excellence, "Clinical audit is a quality improvement process that seeks to improve patient care and outcomes through systematic review of care against explicit criteria and the implementation of change...Where indicated, changes are implemented at an individual, team, or service level and further monitoring is used to confirm improvement in healthcare delivery". According to the scientific literature, clinical audit is more commonly used in secondary care with respect to primary care, probably because of the high variability that characterizes the latter. The aim of this study was to investigate, through a systematic literature review, the use and effectiveness of clinical audit in a particular field such as that of healthcare emergency.
