Abstract

PROPÓSITO: el linfoma B primario mediastínico (LBPM) es una entidad clínico-patológica diferenciada reconocida en la clasificación REAL¹. Constituye una entidad agresiva, que afecta fundamentalmente a jóvenes, es potencialmente curable y presenta gran dificultad en su manejo. El tratamiento habitualmente empleado incluye tanto la quimioterapia como la radioterapia. MATERIAL Y MÉTODOS: se presenta el caso de un varón de 14 años, en el que la cirugía y los nuevos avances en el tratamiento han conseguido la curación del paciente. RESULTADOS Y CONCLUSIONES: se revisa el tratamiento del linfoma B primario mediastínico, aportando el papel de la cirugía en los casos refractarios. PURPOSE: Primary mediastinal B-cell lymphoma (PMBL) es recognized as a separate entity by the REAL classification. PMLB is an aggressive tumor of young adults with a significant curability rate. The optimal management of this disorder is not well defined. Chemotherapy and radiotherapy are generally accepted as the best treatments for the management of PMBL. MATERIAL AND METHODS: A 14 year old boy with PMBL treated by surgery and new chemotherapy drugs. RESULTS AND CONCLUSIONS: The patient was cured employing a combination of surgery and new chemotherapy resources. The chemotherapy, radiation therapy and surgery employed for the management of PMBL are reviewed, indicating the role of surgery in resistant cases and recurrent disease.

9. An Experimental Analysis of the High-Cycle Fatigue Fracture of H13 Hot Forging Tool Steels.

Abstract

In this study, the axial fatigue behaviour of hot forging tool steels at room temperature was investigated. Fatigue tests were performed on two steels within the same H13 specification. The fatigue tests were carried out in the high-cycle fatigue domain under normal conditions. These tests were also performed on specimens in contact with a corrosive medium, applying stress values that led to the high-cycle fatigue domain under normal conditions for the sake of comparison. Both materials showed similar fatigue strengths when they were tested under normal conditions. In contrast, corrosion fatigue lives were much lower than in normal tests and differed significantly between the two steels. Crack initiation was triggered by microstructural and surface defects in the normal tests, whereas the formation of corrosion pits caused crack initiation in the corrosion fatigue tests. Moreover, a fracture surface analysis revealed dissimilar crack propagation areas between both steels, which suggested that both steels had different fracture toughness. These results were in line with the differences observed between the carbide and grain sizes of both of the material microstructures.

11. [Audit of clinical records: clinico-microbiologic study of urinary infections in a team of primary care].

Abstract

To carry out an audit of clinical records in our center for the evaluation of the quality of care before the introduction of protocols, several prevalent conditions were selected, and among them urinary tract infections (UTI). Another aim of the study was to evaluate the autochthonous flora responsible for UTI and its resistence to commonly used antimicrobials. A series of acceptable criteria and standards were set as quality controls, and the real index was found below the preselected one in all cases. The most commonly isolated organism was E. coli, followed by Proteus, which were resistent to trimetoprim-sulfamethoxazole in 56% and 71.4% of cases, respectively. Problems of organization and knowledge, and a high resistence rate to common antimicrobials were detected; the following were suggested as measures for improvement: introduction of a protocol, need for continuing education, reduction in the care demand, health education and improvement in the antibiotic policy.
