Dupilumab has been reported to attenuate itch and reduce several serum markers, including blood lactate dehydrogenase (LDH), blood eosinophil count, and serum total immunoglobulin E (IgE).Patients and Methods: The present study investigated retrospectively changes in clinical scores and serum biomarker from 175 adults with moderate-to-severe AD treated with dupilumab. Clinical manifestations were assessed using eczema area and severity index (EASI) and visual analogue scale (VAS) for itch at baseline and subsequently at 16-week intervals up to a duration of 48 weeks. Total IgE, LDH and blood eosinophil count were also collected.Results: The dupilumab treatment significantly improved EASI and VAS scores and decreased serum levels of IgE, LDH, and total eosinophil count. The EASI scores were positively correlated with VAS for itch at all recorded time points, whereas serum biomarkers did not exhibit a strong correlation with EASI scores.Conclusion: These findings highlight the close relationship between the extent and severity of eczema and the intensity of itch experienced by patients and suggest that factors beyond the measured serum biomarkers play a significant role in the clinical manifestations of AD, emphasizing the complex nature of the disease.Keywords: atopic dermatitis, dupilumab, itch, immunoglobulin E, eosinophils, lactate dehydrogenase

Abstract

Allergic contact dermatitis (ACD) is a common inflammatory skin disease caused by delayed hypersensitivity to chemical and biotic contact allergens. ACD significantly affects the patients' quality of life negatively impacting both occupational and non-occupational settings. Patch testing is the gold standard diagnostic in vivo test to precise the ACD etiology and to correctly perform prevention. According to the Italian Medicines Agency (AIFA) legislative decree no. 178 of 29th May 1991, allergens are defined as medicines and therefore they are subject to strict regulation. In 2017, AIFA (decree no. 2130/2017) started a procedure to regulate contact allergens on the Italian market and actually the contact allergens temporarily authorized are reported in AIFA decree no. 98/2022, valid until November 2023. The availability on the market of contact allergens to diagnose ACD and continuous updating on the basis of new epidemiological trends are mandatory, jointly with the continuous update of the baseline and integrative series for patch testing. For this reason, the scientific community represented in Italy by the Skin Allergies Study Group of SIDeMaST (Italian Society of Dermatology and Venereology) and SIDAPA (Italian Society of Allergological, Occupational and Environmental Dermatology) are constantly working, in close relationship with the European scientific communities with large expertise in this important sector of the modern Dermatology. Herein, we report the setting up of regulatory legislation by AIFA and the new Italian Adult Baseline Series for patch testing.

Abstract

Mental health encompasses the ability to cope with important stresses of life and to realize one's abilities in the community, and the COVID-19 pandemic represented a very stressful event for people with mental illnesses. Our aim was to assess mental well-being in people living with different mental disorders, comparing results obtained in 2016 with those observed after the COVID-19 pandemic. Ninety-six participants were assessed using the Mental Health Continuum Short Form and classified as “flourishing,” “moderately mentally healthy,” and “languishing.” Overall, a significant increase in the prevalence of “flourishing” and “moderately mentally healthy” subjects and a reduction of “languishing” subjects (p= 0.003) were observed. However, a significant improvement in well-being (p= 0.005) was observed only in the schizophrenia spectrum disorder group. Moreover, only subjects that never contracted SARS-CoV-2 showed a significant (p= 0.019) increase in positive well-being states. Lockdowns may have led caregivers to spend more time with the participants, also increasing treatment adherence, resulting in an improvement of overall well-being in several participants.

Abstract

The article discusses a case series evaluating the effectiveness of bimekizumab in treating genital psoriasis (GenPs) over 52 weeks. Twelve patients with moderate-to-severe GenPs were treated with bimekizumab, showing significant improvement in their condition. The study highlights bimekizumab's sustained positive trend in managing GenPs, with patients achieving clear/almost clear status and improved quality of life. Abstract

The article discusses cutaneous angiosarcoma, a rare and aggressive skin malignancy that poses diagnostic challenges often mistaken for benign vascular lesions. The study describes the use of line‐field confocal optical coherence tomography (LC‐OCT) for accurate diagnosis of angiosarcoma in an 88-year-old male patient. LC‐OCT helped identify neovascularization and other features of the lesion, leading to prompt surgical intervention and a confirmed diagnosis of angiosarcoma. Abstract

Bimekizumab è un anticorpo monoclonale umanizzato, recentemente approvato per il trattamento della psoriasi a placche moderato-grave, inibitore selettivo delle isoforme dell’interleuchina-17A e F. Le esperienze real-life riguardanti l’utilizzo del farmaco sono limitate ed il profilo del paziente con riposta più elevata PASI100 cioè super responder SR non è stato analizzato. Presentiamo questo studio multicentrico, retrospettivo volto a disegnare il profilo del paziente che beneficia maggiormente del trattamento con bimekizumab, considerando come SR coloro che raggiungono PASI 100 alla settimana-4 e 16. Sono stati studiati pazienti adulti affetti da psoriasi moderato-grave, trattati con bimekizumab per almeno 16 settimane presso 21 centri dermatologici italiani, secondo regole di appropriatezza AIFA e in accordo con la scheda tecnica del farmaco. Endpoints di efficacia erano PASI75, PASI90 e PASI100 alla settimana 4 e 16, ed in particolare la percentuale di SR ai due tempi. I pazienti che non raggiungevano tale target venivano definiti come non-super responders NSRs. Sono stati studiati 137 pazienti con età media 52,47±15,56 anni, BMI medio 27,43±5,91 e PASI medio al basale di 16,00±9,29. Alla settimana 4 il 72% dei pazienti raggiungeva PASI75, il 50% PASI90, mentre il 43% otteneva PASI100, definendo la popolazione SR alla settimana 4. Alla settimana 16, il 93% dei pazienti raggiungeva PASI75, il 77% PASI90, il 70% dei pazienti risultava essere SR mentre solo il 7% non raggiungeva PASI 75. Sono emerse alcune differenze caratterizzanti pazienti con un più rapido raggiungimento dello stato di SRs alla settimana 4 rispetto alla settimana 16: età ≥46<65, coesistenza di <2 comorbidità e stato naïve a precedenti trattamenti biologici. Considerando alcune caratteristiche clinico-demografiche è stata condotta un’analisi univariata per valutare le differenze tra SR e NSRs. Abstract

Background: Tumor Necrosis Factor Receptor Associated Periodic Syndrome (TRAPS) is a hereditary autoinflammatory syndrome characterized by recurrent episodes of fever and localized inflammation. Clinical presentation can be very variable in terms of duration of fever attacks, periodicity, and accompanying manifestations. One of the most characteristic symptoms is the occurrence of migrating skin rash with myalgia that is sustained by monocytic inflammation.Observations: We herein present the case of a family suffering from TRAPS who had been misdiagnosed for a long period of time and whose main symptom was migrating angioedema. Skin biopsy from one of the patients documented a monocytic panniculitis. All the living patients responded dramatically to anakinra treatment.Conclusions: The classic symptom of migratory angioedema with myalgia in TRAPS can be produced by monocytic panniculitis.This manifestation is so characteristic of TRAPS that its occurrence, even in the absence of other manifestations, should prompt genetic analysis. Our patient’s condition responded promptly to anakinra treatment.

38. Influence of regression, its extent and tumor-infiltrating lymphocytes on sentinel node status, relapse, and survival in a 10-year retrospective study of melanoma patients

Abstract

This case-control study seeks to investigate the influence of histological findings, specifically regression, its extent and tumor-infiltrating lymphocyte (TILs), on result of sentinel lymph node (SLN) biopsy, 5-year melanoma-specific survival (MSS), and relapse-free survival (RFS). We included all patients with cutaneous melanoma who underwent SLN biopsy at the Melanoma Center of the University of Brescia, following the Italian Association of Medical Oncology National guidelines from January 2008 to August 2018. Regression and its extent (<75 or ≥75%) and the presence of TILs were reevaluated by a trained dermatopathologist, adhering to the 2017 College of American Pathologists Cancer Protocol for Skin Melanoma. These patients were followed up for 5 years. Our study uncovered significant associations between regression and male sex (P < 0.05), melanoma location on the trunk, upper limbs, and back (P = 0.001), ulceration (P < 0.05), lower Breslow thickness (P = 0.001), and the presence of lymphocytic infiltration (both brisk and nonbrisk) (P < 0.001). Regression and its extent, however, did not appear to affect SLN positivity (P = 0.315). Similarly, our data did not reveal a correlation between TILs and result of SLN biopsy (P = 0.256). When analyzing MSS and RFS in relation to the presence or absence of regression and TILs, no statistically significant differences were observed, thus precluding the need for logistic regression and Kaplan–Meier curve analysis. This study’s findings underscore that regression and TILs do not appear to exert an influence on sentinel lymph node status, MSS, or RFS in our cohort of patients.

Abstract

The popularity of tattoos has led to an increase in associated skin reactions, including complications such as infection, allergic reactions and rare conditions such as tattoo‐induced cutaneous lymphoid hyperplasia (CLH). CLH is a benign lymphoproliferative reaction with clinical features resembling malignant cutaneous lymphomas. Non‐invasive diagnostic tools like reflectance confocal microscopy (RCM) and the new line‐field confocal optical coherence tomography (LC‐OCT) are being studied in dermatology better to understand the morphological patterns of many dermatological diseases. Between September 2021 and May 2023, patients with suspicious lesions for tattoo‐related CLH were analysed using RCM and LC‐OCT before confirming the diagnosis of CLH through skin biopsy and histopathological examination. The study included five cases of CLH. It focused on the analysis of high‐quality LC‐OCT images/videos and RCM images to investigate the features of CLH in tattooed individuals. Most (80%) cases exhibited a mixed T and B lymphocyte infiltration subtype, while 20% showed a predominant T infiltration subtype. RCM and LC‐OCT revealed characteristic features, including architectural disarray, fibrosis, lymphoid infiltrates, and pigment deposits in the epidermis and dermis. Non‐invasive tools such as RCM and LC‐OCT are valuable in diagnosing tattoo‐related CLH. While skin biopsy remains the current standard for diagnosis, RCM and LC‐OCT can serve as helpful adjuncts in identifying the most representative area for biopsy. They may potentially become alternative diagnostic options in the future, offering benefits in terms of cost, diagnostic efficiency, aesthetics and patient satisfaction as the prevalence of tattoo‐related adverse reactions continues to rise. 40. Sexual quality of life in patients with pemphigus: A case–control study.

Abstract

Background: Pemphigus, an autoimmune blistering disease that affects the skin and mucous membranes, significantly impairs the quality of life (QoL) of affected individuals. While there are a variety of QoL measurement tools available for assessing this disease, there is a lack of studies that specifically evaluate the sexual QoL of patients with pemphigus. Objectives: This case–control study aims to investigate the impact of the disease on sexual activity as well as its overall effect on QoL. Materials and Methods: Fifty pemphigus patients, who were referred to the Dermatology Department at the University Hospital of Brescia in the period March 2019–September 2021, completed several QoL surveys, including the 36‐item Short Form Health survey (SF‐36), the 12‐Item General Health Questionnaire (GHQ‐12), the Autoimmune Bullous Disease Quality of Life (ABQOL) and either the International Index of Erectile Function (IIEF) or the Female Sexual Function Index (FSFI). The severity of the disease was assessed using the Pemphigus Disease Area Index (PDAI). Differences in QoL surveys between the case and control groups were analysed using either the t‐test or the Wilcoxon–Mann–Whitney test. The correlation between QoL surveys in pemphigus patients and disease severity were analysed using Spearman's coefficient (r). Results: The results revealed a marked impairment in overall QoL among patients with pemphigus compared to the healthy control subjects. Significant differences were observed in various domains of QoL, including physical health, mental well‐being, social functioning and, notably, sexual health. Furthermore, disease severity as evaluated by the PDAI showed correlations with specific aspects of health status, and disease‐specific QoL demonstrated associations with nearly all domains of health status. No significant correlations were found between sexual activity, mucosal involvement or steroid therapy and PDAI scores or disease‐specific QoL measures. Conclusion: These findings emphasize the significant impact of pemphigus on patients' well‐being, with particular attention to the impaired sexual activity. [ABSTRACT FROM AUTHOR]

Abstract

Background: Although long-term management of psoriasis is paramount, this approach is challenging in clinical practice. In the recent PSO-LONG trial, a fixed-dose combination of betamethasone dipropionate (BD) and calcipotriol (Cal) foam applied twice a week on non-consecutive days for 52 weeks (proactive treatment) reduced the risk of relapse. However, the role of Cal/BD foam in the long-term management of psoriasis needs further clarifications. The ProActive Management (PAM) program, a nationwide Italian project, aims at reaching a consensus on the role of proactive management of psoriasis. Methods: A steering committee generated some statements through the nominal group technique (NGT). The statements were voted by an expert panel in an adapted Delphi voting process. Results: Eighteen statements were proposed, and the majority of them (14/18) reached a consensus during the Delphi voting. The need to provide long-term proactive topical treatment to reduce the risk of relapse for the treatment of challenging diseases sites or in patients where phototherapy or systemic therapies are contraindicated/ineffective was widely recognized. A consensus was reached about the possibility to associate the proactive treatment with systemic and biological therapies, without the need for dose intensification, thus favoring a prolonged remission. Moreover, the proactive treatment was recognized as more effective than weekend therapy in increasing time free from relapses. Approaches to improve adherence, on the other hand, need further investigation. Conclusions: The inclusion in guidelines of a proactive strategy among the effective treatment options will be a fundamental step in the evolution of a mild-moderate psoriasis therapeutic approach.

Abstract

This article discusses the use of line-field confocal optical coherence tomography (LC-OCT) as a new tool for assessing cutaneous mycosis fungoides (MF), the most common variant of cutaneous T-cell lymphoma. The early diagnosis of MF is challenging and often results in a therapeutic delay. LC-OCT allows for the visualization of various features of MF, such as epidermal architectural disarray, atypical lymphocytes, and dermal changes. The study demonstrates that LC-OCT has a high correlation with histology and can provide valuable information for the diagnosis and follow-up of MF. [Extracted from the article]
