Laser osteotomy may benefits patients with minor damage to the bone and accelerated the healing process. However, the lack of depth control is still needed to be overcome. We proposed to use Optical Coherence Tomography imaging for depth control of the laser ablation process. The OCT image provides geometrical information over the ablation spot and enables the measurement of depth in real-time. Additionally, we demonstrated the use of Kalman filter to predict incoming pulse ablation depth and avoid excessive cut than planned. The result shows that the Kalman filter reduced the error between the planned and real ablated depth.

The Role of Whole Body Bone Scintigraphy in the Diagnosis and Follow-Up of Renal OsteodystrophyBone scintigraphy (BS) is one of the most successfully applied imaging methods in nuclear medicine. Skeletal uptake of Technetium-99m-Diphosphonates depends primarily upon osteoblastic activity. A whole body bone scan image therefore presents a functional display of total skeletal metabolism and such has a valuable role to play in the assesment of patients with metabolic bone disorders. Bone scintigraphy is succesfully applied in the diagnosis and follow-up of renal osteodystrophy (ROD) as well as in the detection of local anomalies such as pseudofractures or Brown tumours. Renal osteodystrophy is present in the majority of the patients with chronic renal failure and has become a considerable clinical problem to confront. Routinely used parameters in the follow-up of the disease are the plasma levels of calcium, phosphorus, alkaline phosphatase, hydroxyproline and parathormone as well as conventional bone radiographic findings. Due to the insufficient common use of BS in ROD, there is a general lack of experience in the interpretation of radionuclide images. In this article, we aimed to clarify the basic principle of bone scanning as well as the general and focal pathological scintigraphic findings in ROD. Kemik sintigrafisi Nükleer tıpda başarıyla uygulanan görüntüleme metodlanndan biridir. Teknesyum-99m ile işaretlenmiş difosfonat moleküllerinin kemikte tutulumu öncelikle osteoblastik aktiviteye bağlıdır. Bu nedenle tüm vücut kemik sintigrafisi total iskelet metabolizmasını görüntülemesi açısından metabolik kemik hastalıklarının değerlendirilmesinde değerli bir konuma sahiptir. Kemik sinügrafisi, üremik kemik hastalığının (ÜKH) tanı ve takibinde olduğu gibi psödofraktür, Brown tümörleri gibi lokal anomalilerin araştırılmasında da başarıyla uygulanmaktadır. Kronik böbrek yetmezlikli hastaların büyük bir kısmında gelişen ÜKH savaşılması gereken önemli bir klinik sorun haline gelmiştir. Hastalığın takibinde rutin olarak kalsiyum, fosfor, alkalen fosfataz, hidroksiprolin ve parathormon serum değerleri ile birlikte konvansiyonel kemik radyografisi bulguları kullanılır. ÜKH'da kemik sintigrafisi yaygın olarak uygulanamadığından radyoizotopik görüntülerin yorumlanmasında genel deneyim eksildiği vardır. Bu derlemede tüm vücut kemik sintigrafisinin temel prensibinin yanı sıra ÜKH'da izlenen genel ve fokal patolojik bulguların klinisyene aktarılması amaçlanmıştır.

Thus, ruling out RAS with non-invasive diagnostic methods is preferable for both die patient and the clinician. The role of Angiotensin Converting Enzym Renography (ACE-R) in the establishment of the diagnosis of RVH is commonly not well known in Turkey due to the limited number of Nuclear Medicine laboratories. In this article, we present the pathophysiological principle, patient selection criteria, methodology, application and interpretation for ACE-R based on the most recent Consensus report on this subject. Bu nedenle RAS olasılığının non-invazif tanı yöntemleri ile ekarte edilmesi hasta ve kli-nisyen açısından öncelik taşır. Günümüzde bu yöntemlerin en başanlı olanları Angiotensin Converting Enzym (ACE) inhibitörlü kinetik böbrek sintigrafisi (ACE-Kbs), Doppler sonografi ve Magnetik rezonans görüntülemedir. Yurdumuzda Nükleer tıp merkezlerinin sayısı henüz yeterli miktarda olmadığından, ACE-Kbs'nin RVH'nun non-invazif tamsmdaki yeri henüz yaygın olarak bilinmemektedir. Bu makalede klinisyene ACE-Kbs nin fizyopatolojik prensibi ve hasta seçim kriterleri aktarılmış olup çalışmanın metodoloji, uygulama ve yorumu bu konuda yayınlanmış olan son konsensüs raporu esas alınarak öz bir biçimde sunulmuştur.

Iodine-131 Therapy of HyperthyroidismThe most common causes of hyperthyroidism are Graves' disease and toxic nodular goitre (TNG). There are 3 recognised modalities of treatment for hyperthyroidism. Antithyroid drug therapy, surgery and Iodine-131(1-131) administration. Antithyroidal drugs are effective in Graves' disease to maintain remission on long term and useful for abating the initial symptoms of hyperthyroidism. Relapses are frequently seen when the medication is discontinued. Surgery,either subtotal or near-total thyroidectomy, has a limited role in the treatment of hyperthyroidism. Approximately for 50 years, 1-131 has been succesfully used for the treatment of hyperthyroidism. The use of radioiodine being a first-line therapy for hyperthyroidism is growing, since the modality is well tolerated, effective, non-expensive and easy to perform with the only significant long term sequelae being the risk of developing radioiodine-induced hypothyroidism. This report outlines the methodology and clinical application of radioiodine treatment in hyperthyroidism. Hipertiroidiye en sık neden olan patolojiler Graves' hastalığı ve toksik nodüler guatr (TNG)dır.Tedavide kabul edilen 3 modalité bulunmaktadır: Antitiroid ilaç kullanımı, cerrahi ve İyot-131 uygulaması. Antitiroid ilaçlar Graves hastalığında uzun sürede re-misyonun sağlanmasında ve hipertiroidinin başlangıç semptomlanmn giderilmesinde faydalıdır. İlaç kesildikten sonra relapslar sık görülür. Subtotal ya da totala yakın cerrahi, hipertiroidi tedavisinde sınırlı bir rol oynar. İyot-131, hipertiroidi tedavisinde yaklaşık 50 yıldır başarıyla uygulanmaktadır ve iyi tolere edilebilen, ucuz, etkin ve uygulaması kolay bir yöntem olması nedeniyle giderek daha sık tercih edilmektedir. İyot-131 tedavisinin uzun sürede tek önemli komplikasyonu, geç gelişen hipotiroididir. Bu derlemede, radyoaktif iyot ile yapılan hipertiroidi tedavisinde metodoloji ve klinik uygulama öz bir biçimde verilmektedir.

PubMed: 15841301 The occurrence of suppressed thyroid stimulating hormone (TSH) levels in the presence of normal free thyroxine (fT4) and free triiodothyronine (fT3) is not rare. Although this entity may be defined as "subclinical hyperthyroidism", many patients presented with the above laboratory findings, suffer from non-specific signs and symptoms that could be related to hyperthyroidism. The aim of this study was to evaluate this entity using the standard method of radionuclide thyroid imaging by 99mTc-pertechnetate (99mTcO4-). Fifty-two patients (18 males; mean age: 45±9 years and 34 females; mean age: 43± 11 years) with suppressed serum TSH levels and normal fT3 and fT4 levels, who were referred to our nuclear medicine department for thyroid scintigraphy were prospectively included in the study. Any thyroidal or non-thyroidal medication, thyroid surgery or non-thyroidal diseases constituted the main exclusion criteria. Thyroid scintigraphy was performed 15 min after the i.v. injection of 111 MBq 99mTcO4- using a pinhole collimator. Scintigraphic findings were visually evaluated. All patients had normal serum levels of fT4 (mean value 1.53±0.14 ng/dl, normal range: 0.89-1.8 ng/dl) and fT3 (mean value 3.9±0.17 ng/dl, normal range: 2.3-4.2 ng/dl). Mean value ot serum TSH levels was 0.09±0.12 ?IU/ml (normal range: 0.35-5.5 ?IU/ml). The above hormones were tested by the chemiluminescent method. Patients showed seven different scintigraphic patterns in their thyroid scintigrams as follows: hyperactive+hypoactive multinodular goiters, 27%; hyperactive multinodular goiters, 23%; hypoactive multinodular goiters, 15%; solitary hypoactive nodular glands, 14%; normal glands, 9%; solitary hyperactive nodular glands, 8%; and diffuse hyperactive glands, 4%. All but two patients (50/52 = 96%) showed mild to moderate hyperplasia of the thyroid gland. It is concluded that most patients with subclinical hyperthyroidism, (96%) show mild to moderate hyperplasia of the thyroid, and many (65%) show multinodularity with at least one hyperactive nodule.

The aim of this study was to evaluate the intratumoral injection technique in the detection of lymphatic drainage of proved or possibly malignant cold thyroid nodules, Materials and Methods: The study group consisted of 13 patients with palpable solitary cold thyroid nodules, None of the patients had cystic nodules on ultrasound examination. After fine-needle aspiration biopsy (at least 3 days later), 15 MBq (0.4 mCi) Tc-99m nanocolloid particles in a small volume (0.2 mi) were injected into the nodule, Dynamic images (60 frame x 1 minute) were acquired during the first hour, followed by static anterior and lateral images at 90 and 120 minutes. Results: Radiopharmaceutical was present in the systemic circulation in two patients, possibly as a result of paratumoral injection. Eleven patients had intratumoral accumulation in early frames. Lymph nodes draining the thyroid nodule were visualized in 10 of 11 patients. In the dynamic acquisition period of 7 of 10 patients, the first draining lymph node could also be identified. Only 5 of 13 patients had malignant nodules, Lymphatic metastases were detected in one patient's specimens, Conclusions: Preoperative visualization of lymphatic pathways may be used when planning for thyroid cancer surgery. Sentinel node detection could yield valuable additional information in these patients.
