1. Understanding Cancer Prevention Through Geospatial Science : Putting Cancer in Its Place

Abstract

The worldwide cancer burden will double over the next two decades, with the number of new cancer diagnoses increasing in all regions of the world. However, it is estimated that around 40% of all cancers are preventable through the appropriate application of existing epidemiological and public health knowledge, and with further research this proportion will increase considerably. This edited volume explores the role of place in cancer prevention and how geospatial tools can be used to support this globally important goal. Its chapters detail many aspects of the application of geospatial science to cancer control, including quantifying exposure to environmental carcinogens, access to screening services, the importance of partnership building, and more through a variety of geographically diverse case studies. The book also offers background on geospatial modeling tools and methods for adding spatial analysis to cancer surveillance systems. The book is rooted firmly in the notion that geography significantly influences the accumulation of cancer risks (at the individual and community level) that are modifiable through policy, regulatory, and behavioral changes. The landscape of cancer prevention is vast, encompassing the building blocks of population and public health—i.e., epidemiology and causation—through risk factor modifications, behavior and policy change, to the interface of the cellular with society, including epigenetic modifications and gene/environment interactions, cancer genomics and precision medicine/health. It is clear that geography (space, place, setting, context) is central to all these activities. Geospatial methods and data support risk factor identification, can elucidate the interaction between individual behavioral, demographic, and genetic factors with community level contextual factors, and can be used to prioritize interventions more accurately towards at-risk people and at-risk population groups. Geography is also central to the organization of health services and hence, by definition, is fundamental to the organization of preventative services. More broadly, adopting a geographical approach can help move cancer prevention beyond a narrow medical definition of health, to tackle the upstream and structural determinants of cancer.

Abstract

The natural compounds derived from marine organisms often exhibit unique chemical structures and potent biological activities. Cancer-preventive activity is one of the rather new activities that has emerged and been extensively studied over the last decades. This review summarizes the recent updates on the marine chemopreventive compounds covering the relevant literature published in 2013–2021 and following the previous comprehensive review by Stonik and Fedorov (Marine Drugs 2014, 12, 636–671). In the current article, only the molecules having an effect on malignant transformation (or related pathway and molecules), cancer stem cells, or carcinogen-induced in vivo tumor development were considered to be “true” cancer-preventive compounds and were, therefore, reviewed. Additionally, particular attention has been given to the molecular mechanisms of chemoprevention, executed by the reported marine compounds.

Abstract

The role of certain viruses in the etiology of some tumors is today indisputable, but there is a lack, however, of annoverview of the relationship between viruses and cancer with amultidisciplinary approach. For this reason, the Health Sciences Foundation has convened a group of professionals from different areas of knowledge to discuss the relationship between viruses and cancer, and the present document is the result of these deliberations. Although viruses cause only 10-15% of cancers, advances in oncology research are largely due to the work done during the last century on tumor viruses. The clearest cancer-inducing viruses are: HPV, HBV, HCV, EBV and, depending on the geographical area, HHV-8, HTLV-1 and HIV. HPVs, for example, are considered to be the causative agents of cervical carcinomas and, more recently, of a proportion of other cancers. Among the Herpes viruses, the association with the development of neoplasms is well established for EBV and HHV-8. Viruses can also be therapeutic agents in certain neoplasms and, thus, some oncolytic viruses with selective tropism for tumor cells have been approved for clinical use in humans. It is estimated that the prophylaxis or treatment of viral infections could prevent at least 1.5 million cancer deaths per year.

Abstract

Prophylactic anti-HPV quadrivalent vaccine is marketed, in France, since the end of 2006, and paid off since middle 2007.After having reminded its main characteristics and its mode of action, we study the state of the knowledge of concerned "meurthe-et-mosellan " doctors and patients (general practitioners, gynecologists and paediatricians; women more than 14 year-old) in preventionof the cervical cancer. We bring to light the major role played by family doctors and gynecologists, as well as by the patients' mothers and by the girls themselves, in the access to the vaccination in question. Knowledge concerning this vaccine seems relatively satisfactory. On the other hand,the observed refusais would be motivated by fears concerning the lack of recession, thus the lack of knowledge as for the real efficiency andfor the possible side effects to be foreseen. Progress seems still necessary, in particular as regards the investment of the sanitary authorities in the information campaign. AIso, the organization of a collective cytological screening as well as the communication on it seem indispensable to optimize the here studied preventive initiative.; Le vaccin prophylactique anti-HPV quadrivalent est commercialisé, en France, depuis fin 2006 et remboursé depuis milieu 2007. Après en avoir rappelé les principales caractéristiques et mode d action, nous étudions l'état des connaissances des médecins et patientes meurthe-et-mosellans concernés (généralistes, gynécologues et pédiatres ; patientes de plus de 14 ans) en matière de prévention du cancer du col de l'utérus. L'on met ainsi en évidence le rôle majeur joué par les médecins traitants et gynécologues, ainsi que par les mères des patientes et les jeunes filles elles-mêmes, dans l'accession à la vaccination en question. La connaissance portant sur ce vaccin semble relativement satisfaisante. Par contre, les refus observés seraient motivés par des craintes portant sur le manque de recul donc le manque de connaissances quant à l'efficacité réelle et à long terme, et quant aux éventuels effets secondaires à prévoir. Des progrès semblent encore nécessaires, notamment en ce qui concerne l'investissement des autorités sanitaires dans la campagne d'information. De même, l'organisation d'un dépistage cytologique collectif ainsi que la communication à son sujet semblent indispensables pour optimiser la démarche prophylactique ici étudiée.

7. Καρκίνος μαστού και νοσηλευτικές παρεμβάσεις

Πτυχιακή εργασία -- Σχολή Επαγγελμάτων Υγείας και Πρόνοιας -- Τμήμα Νοσηλευτικής, 2008 -- αα 1805, Ο καρκίνος είναι μια συχνή νόσος, αφού περίπου ένα στα τρία άτομα μπορεί να τον αναπτύσσει σε κάποια στιγμή της ζωής του. Ο καρκίνος του μαστού, που είναι και το θέμα της παρούσας εργασίας , απευθύνεται κυρίως σε γυναίκες στην ηλικία των 40-55 ετών. Η πρόληψη και η έγκαιρη διάγνωση βοηθούν στη θεραπευτική αντιμετώπιση. Ο καρκίνος μαστού στις Ελληνίδες κατέχει την πρώτη θέση σαν αιτία θανάτου από καρκίνο. Ο καρκίνος μαστού ξεκινά από κακοήθη εξαλλαγή ενός κυττάρου που πολλαπλασιαζόμενο δίνει ένα κακοήθη κυτταρικό κλώνο και τελικά επέρχεται η διασπορά. Η μεταστατική διασπορά, που γίνεται αιματογενώς, πολύ πρώιμα μπορεί να προσβάλλει διάφορα όργανα. Μεγάλο ενδιαφέρον παρουσιάζουν οι παράγοντες κινδύνου και οι προδιαθεσικοί παράγοντες για τον καρκίνο του μαστού που παίζουν σημαντικό ρόλο για τη διάγνωση. Η κλινική εξέταση που γίνεται για τη διάγνωση του καρκίνου του μαστού από το γιατρό είναι η λήψη καλού ιστορικού, η επισκόπηση και η ψηλάφηση. Σημαντική όμως είναι η εκμάθηση της αυτοεξέτασης, που ο ίδιος ο ασθενής ψηλαφάται και ανακαλύπτει κάποιο τυχαίο εύρημα. Έτσι βοηθάει στην πρώιμη διάγνωση μιας κακοήθειας. Η θεραπευτική αντιμετώπιση του καρκίνου του μαστού είναι η χειρουργική, η χημειοθεραπεία, η ορμονοθεραπεία και η ακτινοθεραπεία. Στην υλοποίηση όλων των θεραπευτικών μέσων παίζει σπουδαίο ρόλο η συνεργασία του ασθενούς με το νοσηλευτικό προσωπικό. Ο νοσηλευτής οφείλει να ενθαρρύνει τον ασθενή. Αφού γίνει τελικά η διάγνωση και αποφασιστεί αρχικά η χειρουργική θεραπεία, ο νοσηλευτής πρέπει να δείξει ιδιαίτερο ενδιαφέρον στη προεγχειρητική και μεταγχειρητική εξέλιξη του ασθενούς. Ο νοσηλευτής κάνει μια εκτίμηση του σωματικού και ψυχοσυναισθηματικού επιπέδου του ασθενούς που του παρέχουν σημαντικά στοιχεία για τον άμεσο σχεδιασμό των νοσηλευτικών μέτρων. Αφού έχει, αργότερα, αποφασισθεί και το σχήμα της χημειοθεραπείας, ο νοσηλευτής ενημερώνει τον ασθενή για τις αποφάσεις που πάρθηκαν και επίσης τον συμβουλεύει για τις ανεπιθύμητες ενέργειες που μπορεί να προκύψουν

8. Κακοήθη νοσήματα στην παιδική ηλικία και νοσηλευτική παρέμβαση

Πτυχιακή εργασία--Σχολή Επαγγελμάτων Υγείας και Πρόνοιας--Τμήμα Νοσηλευτικής, 2011, αα 1700, Σκοπός της εργασίας αυτής είναι η αναγνώριση των κακοήθων νεοπλασμάτων που εμφανίζονται σε βρέφη και παιδιά και η παράθεση ενός νοσηλευτικού πλάνου που βασίζεται στην ολιστική αντιμετώπιση του καρκίνου. Τα κακοήθη νεοπλάσματα σε παιδιά αποτελούν τη δεύτερη αιτία θανάτου μετά τα ατυχήματα, με ποσοστό επιβίωσης 1 : 900, με πιο κοινή μορφή τη λευχαιμία που προσβάλλει τον μυελό των οστών και το λεμφικό σύστημα. Την αμέσως επόμενη μορφή αποτελούν τα λεμφώματα με συχνότερες τη νόσο Hodgkin και μη Hodgkin. Λιγότερο συχνές αποτελούν οι όγκοι του ΚΝΣ, οι όγκοι των οστών και τέλος οι όγκοι στα όργανα. Τα σημεία και τα συμπτώματα ενός καρκίνου είναι συχνά αφανής και η έναρξη της νόσου βαθμιαία και ύπουλη. Η προσεκτική και ενδελεχής εξέταση συμβάλλει στην έγκαιρη διάγνωση. Πολλές από τις παρενέργειες της θεραπείας είναι αναπόφευκτες, αλλά ο έγκαιρος εντοπισμός και η αντιμετώπισή τους μπορεί να ελαχιστοποιήσει τον πόνο και τη δυσφορία του παιδιού. Η νοσηλευτική φροντίδα του παιδιού που πάσχει από νεοπλασματική πάθηση είναι σύνθετη και περιλαμβάνει την αναζήτηση, εκτίμηση, αξιολόγηση και αντιμετώπιση των προβλημάτων, τόσο των σωματικών όσο και των ψυχολογικών. Τα παιδιά που πάσχουν από καρκίνο χρειάζονται σημαντική ηθική υποστήριξη, λόγω των παρενεργειών της θεραπείας (ιδιαίτερα της πτώσης των μαλλιών), που είναι πολύ επώδυνες εμπειρίες και επηρεάζουν την αυτοπεποίθηση και την αυτοεκτίμηση του παιδιού. Οι οικογένειες των καρκινοπαθών παιδιών χρειάζονται, ειδικά προσαρμοσμένη στις ανάγκες τους, ηθική συμπαράσταση, ιδιαίτερα αν η πρόγνωση της νόσου είναι κακή ή απαιτείται μακροχρόνια θεραπεία. Δεδομένου ότι βαθμιαία αυξανόμενο μέρος της φροντίδας και της θεραπείας του παιδιού γίνεται στο σπίτι, ο ρόλος του νοσηλευτή στη ενημέρωση των γονέων είναι πολύ σημαντικός.
