Amaç: Bu çalışmada MDS (Movement Disorders Society-Hareket Bozuklukları Cemiyeti)?nin Parkinson Hastalığı-Hafif Kognitif Bozukluk (PH-HKB) tanısı için yayınladığı I.Aşama ve II.Aşama Kriterlerinin hassasiyetinin karşılaştırılması ve PH-HKB alt kategorilerinin belirlenmesi amaçlanmıştır.Yöntem: Çalışmaya İdyopatik Parkinson Hastalığı tanısı alan 86 hasta alındı. Depresyon, uykululuk ve anksiyete testleriyle kognitif bozuklukları başka sebeplerle açıklayabilecek etkenlerle birlikte demans tanısı alanlar dışlandı. Bu şekilde 68 hasta değerlendirmeye alındı. I.Aşama için MOCA, MMSE ve ACE-R tarama testleri uygulandı. II.Aşama için dikkat-çalışma belleği, yönetici işlevler, dil, görsel-mekansal işlevler, bellekten oluşan beş kognitif alan, en az iki test ile değerlendirildi. Tarama testleri arasındaki ilişki için Spearman korelasyon analizi yapıldı. ROC eğrisi hesaplanarak 3 test karşılaştırıldı ve sensitivite-spesifisite belirlendi. Apati, depresyon, anksiyete ve uykululuk probleminin kognitif işlevler üzerindeki etkisini araştırmak için Spearman korelasyon analizi yapıldı.Bulgular: Kalan 68 hastanın %57?sinde normal kognisyon, %43?ünde hafif kognitif bozukluk tespit edildi. I.Aşamada kullanılan 3 tarama testinin PH-HKB tanısı için yeterli sensitivite ve spesifisiteye sahip olmadığı ve performansları arasında da anlamlı fark olmadığı görüldü. II.Aşama kriterleriyle (bir kognitif alanın iki testinde ya da iki farklı kognitif alanın birer testinde bozulma olması) PH-HKB hastalarının %10?u Tek Alan-HKB, %90?ı Çok Alan-HKB olarak sınıflandırıldı. Kriterler, en az bir kognitif alanda 2 testin birden bozuk olması şeklinde değiştirildiğinde PH-HKB oranı %43?den %18?e ve çok alan-HKB ise %90?dan %8?e düşmüştür. Tek Alan-HKB %10?dan %92?ye yükselmiştir. Yeni kriterlerin hastaların çoğunu Çok Alan-HKB olarak sınıflandırdığı görüldü. Sonuç: MDS?nin belirlemiş olduğu I.Aşama kriterleri uygulandığı takdirde PH-HKB hastalarının tamamını yakalama şansı bulunmamaktadır. Bu nedenle II.Aşama kriterleri ile detaylı olarak değerlendirilmesi gerekmektedir. Objectives: The purpose of this study to compare Level I and Level II the new Diagnostic Criteria for Mild Cognitive Impairment in Patients With Parkinson?s Disease published by the Movement Disorders Society (MDS) Task Force and also examine the subtypes of Parkinson disease with mild cognitive impairment (PD-MCI)Method: We enrolled 86 patients who have a diagnosis of idiopathic PD, according to the United Kingdom PD Society Brain Bank criteria. Diagnosis of PD dementia and other PD-associated comorbid conditions (depression, excessive daytime sleepiness, or anxiety) were excluded. We recruited 68 patients from an initial screening of 86. For Level I (abbreviated assessment) we used MOCA, MMSE and ACE-R screening tests. For Level II (comprehensive assessment) Neuropsychological testing that includes at least two tests within each of the five cognitive domains (i.e., attention and working memory, executive, language, memory, and visuospatial. Spearman correlation analysis was performed for the relationship between screening tests. The area under the receiver-operator characteristics curve (AUC) was calculated and compared across the 3 tests. Sensitivity, specificity, and positive and negative predictive values of the MoCA, MMSE, and ACE-R were calculated across all possible cutoff scores. Spearman correlation analysis was performed for Apathy, depression, anxiety and excessive daytime sleepiness to investigate the effect on cognitive functions.Results: According to our primary MDS Task Force Level 2 criteria, 29 individuals (43%) were classified as having PD-MCI and 39 individuals (57%) no PD-MCI. Among the 3 screening tests examined, none of the instruments provided good combined sensitivity and specificity for PD-MCI and we did not find meaningful differences in performances. According to Level II criteria (Impairment on at least two neuropsychological tests, represented by either two impaired tests in one cognitive domain or one impaired test in two different cognitive domains) the vast majority of patients were classified with multi-domain (90%) and 10% of patients were classified with single-domain. When we modified the MDS Task Force criteria to require 2 abnormal tests per domain, the subtype distribution shifted such that the majority of patients were classified with single-domain MCI.Discussion: When applied to the Level I there is no chance to capture all of PD-MCI patients. Therefore need to be evaluate with comprehensive assessment. 115
