Abstract

Objective Primary hypophysitis might be challenging to diagnose, and there is a lack of evidence regarding optimal treatment strategies due to rarity of the disease. We aim to investigate the clinical features and compare the outcomes of different management strategies of primary hypophysitis in a large group of patients recruited on a nationwide basis. Design A retrospective observational study. Methods The demographic, clinical, and radiologic features and follow-up data were collected in study protocol templates and analyzed. Results One hundred and thirteen patients (78.8% female, median age: 36 years) were included. Lymphocytic (46.7%) and granulomatous hypophysitis (35.6%) were the prevailing subtypes out of 45 patients diagnosed after pathologic investigations. Headache (75.8%) was the most common symptom, and central hypogonadism (49.5%) was the most common hormone insufficiency. Of the patients, 52.2% were clinically observed without interventions, 18.6% were started on glucocorticoid therapy, and 29.2% underwent surgery at presentation. Headache, suprasellar extension, and chiasmal compression were more common among glucocorticoid-treated patients than who were observed. Cox regression analysis revealed higher hormonal and radiologic improvement rates in the glucocorticoid-treated group than observation group (hazard ratio, 4.60; 95% CI, 1.62-12.84 and HR, 3.1; 95% CI, 1.40-6.68, respectively). The main indication for surgery was the inability to exclude a pituitary adenoma in the presence of compression symptoms, with a recurrence rate of 9%. Conclusion The rate of spontaneous improvement might justify observation in mild cases. Glucocorticoids proved superior to observation in terms of hormonal and radiologic improvements. Pituicytomas are rare, solid, well-circumscribed, low grade (grade I), non-neuroendocrine, and noninfiltrative tumors of the neurohypophysis or infundibulum, which appear in the sellar/suprasellar regions. Herein, we present a case with Cushing’s disease (CD) caused by an ACTH-secreting pituitary adenoma in association with an infundibular pituicytoma.

Abstract

Aim: Gestational diabetes mellitus (GDM) is associated with an increased risk of fetal and maternal complications, such astype 2 DM (diabetes mellitus) and cardiovascular disease (CVD). This study aimed to predict the potential for future vascular complications in mothers with GDM by evaluating oxidative stress, endothelial NO synthase (eNOS) expression, and vascular smooth muscle cell (VSMC) proliferation in the umbilical vessels of mothers with GDM. Material and Methods: Subjects were divided into two groups: the normoglycemic control (NGC) group (n = 10) and the GDM group (n = 12). Expression of eNOS and production of reactive oxygen species (ROS) in human umbilical vein endothelial cells (HUVECs) were determined. The mitochondrial mass of HUVECs was evaluated by spectrofluorometry. VSMC proliferation was ascertained in vitro with an EdU cell proliferation assay. Advanced glycation end products (AGEs) accumulation was measured by ELISA and assessed by immunohistochemical staining. Results: VSMC proliferation, eNOS expression, and ROS production in HUVECs were significantly increased, and greater immunohistochemical staining to AGEs was observed in endothelium in GDM. Conclusion: Increased oxidative stress, which elevates eNOS expression and VSMC proliferation in the umbilical vessels of mothers with GDM, may be a sign that mothers have a high potential for developing diabetes or cardiovascular disease in the future. Abstract

PURPOSE: The COVID-19 pandemic brought unprecedented conditions for overall health care systems by restricting resources for non-COVID-19 patients. As the burden of the disease escalates, routine elective surgeries are being cancelled. The aim of this paper was to provide a guideline for management of endocrine surgical disorders during a pandemic.METHODS: We used Delphi method with a nine-scale Likert scale on two rounds of voting involving 64 experienced eminent surgeons and endocrinologists who had the necessary experience to provide insight on endocrine disorder management. All voting was done by email using a standard questionnaire.RESULTS: Overall, 37 recommendations were voted on. In two rounds, all recommendations reached an agreement and were either endorsed or rejected. Endorsed statements include dietary change in primary hyperparathyroidism, Cinacalcet treatment in secondary hyperparathyroidism, alpha-blocker administration for pheochromocytoma, methimazole±beta-blocker combination for Graves' disease, and follow-up for fine-needle aspiration results of thyroid nodules indicated as Bethesda 3-4 cytological results and papillary microcarcinoma.CONCLUSION: This survey summarizes expert opinion for the management of endocrine surgical conditions during unprecedented times when access to surgical treatment is severely disrupted. The statements are not applicable in circumstances in which surgical treatment is possible.

Abstract

Graves' disease (GD) and toxic multinodular goitre (TMNG) are the most common thyroid diseases which mainly lead to thyrotoxicosis, however, the underlying mechanism of distinct clinical presentations remains unclear. Protein extracts from the thyroid tissue specimens of the patients with GD and TMNG were subjected to Difference Gel Electrophoresis (DIGE). Differentially regulated protein spots were determined by image analysis, and the spots displaying statistically significant differences were identified by Matrix-Assisted Laser Desorption Ionization Time of Flight Mass Spectrometer (MALDI-TOF) followed by MASCOT search. Western blot analysis was used to verify changes occurring at the protein levels. The identified proteins were classified based on their functions in metabolic pathways using bioinformatics algorithms. Fifteen proteins showed significant alterations in abundance between the two disease groups. Bioinformatic analysis revealed the differentially regulated proteins were particularly related to catabolism, oxidative stress and especially energy utilization pathways, including glycolysis, proteolysis, ketone body catabolism and other energy metabolism-related pathways. Significance of the study Previously, GD has been the subject of many studies that performed the proteomics approaches in the orbital tissue samples or tear. This is one of the very few studies that investigate the changes in the proteome of thyroid tissue in GD. We demonstrated mainly the upregulation of catabolic activity-related proteins in patients with GD compared to TMNG. Although it remains to be elucidated, some of these proteins can be used as markers for GD or have a role in the pathogenesis of the disease. Abstract

Aim: In this study, we aimed to evaluate the levels of brown adipose tissue markers uncoupling protein-1 (UCP-1), irisin, bone morphogenetic protein-7 (BMP-7) and PR-Domain Zinc Finger Protein-16 (PRDM-16) in Cushing's disease (CH) with hypercortisolism. Methods: The study was conducted prospectively with 48 CH patients and 40 healthy volunteers between 2018 and 2019. Two cc of peripheral blood taken from the participants was centrifuged and stored at -80° C degrees. Cushing's syndrome was excluded by performing 1 mg dexamethasone suppression test in the control group. Blood samples were analyzed by Enzyme-linked Immunosorbent Assay method. Results: The patient group included 11 males (22.9%), 37 female (77.1%); 9 male (22.5%) and 31 females (77.5%) in the control group. Body mass index was 31.29±5.76 kg/m² in the patient group and 33.42±3.11 kg/m² in the control group. PRDM-16, Irisin, BMP-7, UCP-1 levels were not significantly different between the two groups. While there was a positive correlation between serum cortisol and irisin, a negative correlation was observed between urinary free cortisol and UCP-1. Conclusion: These data suggest that long-term exposure to high doses of glucocorticoids in CH patients causes loss of adipose tissue functionality and development of resistance in vivo.
