Abstract

In recent years, the rapid evolution of scientific research on DNA has unlocked the codes to genetic conditions and contributed unveiling the causes of diseases once unknown. All this brought about a number of economic issues regarding private and public investment in research, treatment and prevention (e.g. advertising and screening campaigns), which are especially prominent in the case of common (vs rare) genetic diseases. Furthermore, genetic diseases with a high phenotypical impact (HIP), involve more social stigma and are thus particularly ethically connoted. Not only, the advancement of medical (genetic) science usually goes hand in hand with a change in terminology. Most genetic diseases’ denominations are either eponyms, as they were identified and labeled after the name of the physician that first clinically observed and described them (e.g.: Down syndrome, Williams syndrome, Pick syndrome, Marfan syndrome, etc.), or popular names indicating the physical appearance of the patient (e.g.: dwarfism), which sometimes include even disparaging variant words (e.g.: mongol, mongoloid, etc.). Due to directions from WHO and other institutional medical authorities, the scientific community is being invited not to use ambiguous terms in medical intra- and inter-specialised communications, as well in non-specialised encounters, such as the medical-patient interview. This paper aims to offer a brief diachronic description of the terminological change undergone by some common genetic pathologies in the last years both at the specialist and popular levels. This will be conducted by comparing two small corpora from the British Medical Journal and Scientific American to see if and how much terminological directions from medical authorities have been met. The analysis will make use of both quantitative and qualitative methods and the perspective adopted will be that of Critical Discourse Analysis, for the socially relevant issues that the topic addresses.

Abstract

La prevalenza di studenti dislessici in Italia appare inferiore rispetto a quella dei dislessici di lingua Inglese. All’interno di questo articolo, dopo una breve analisi delle cause e delle caratteristiche della dislessia, si cercherà di confrontare, a livello linguistico, la differenza tra i dislessici di lingua Italiana e quelli di lingua Inglese. Particolare rilevanza verrà data agli aspetti linguistici strutturali della lingua Inglese che sembrerebbero essere gli elementi scatenanti la prevalenza di tale disturbo nei paesi di lingua Inglese.

Abstract

PurposeTo assess the effect of atypical pattern of retardation (APR) on retinal nerve fibre layer (RNFL) measurements made by scanning laser polarimetry (SLP) with variable corneal compensation (GDx-VCC) in glaucomatous eyes.MethodsOne eye each of 30 glaucomatous patients (average mean deviation (MD): −6.4±4.8) with APR on GDx-VCC retardation map were selected. In total, 34 glaucomatous, age- and severity-matched eyes (average MD: −7.0±5.3) and 36 age-matched healthy subjects, both with a normal pattern of retardation (NPR) represented control groups. APR on retardation maps was characterized by alternating peripapillary circumferential bands of low and high retardation, or high retardation areas arranged in a spokelike pattern, or high retardation nasal and temporal splotchy areas. Typical scan score (TSS) was extracted for each included eye. GDx-VCC parameters (mean±SD) in the two groups of glaucomatous eyes were compared with healthy eyes' corresponding values (Mann–Whitney U-test). Areas under receiver operating characteristic (AUROC) curves were generated to assess the APR effect on the parameters' diagnostic ability.ResultsAll parameters discriminated adequately between healthy and glaucomatous eyes with NPR (AUROCs ≥0.9 for nine parameters). On the contrary, considering healthy and glaucomatous eyes with APR, four thickness parameters could not separate the two groups and AUROCs ≥0.85 appeared only for Inferior and Superior Ratio, NFI, Max Modulation.ConclusionAPR may void the effect of custom compensation and provide spurious RNFL thickness measurements. Abstract

The purpose of this paper is to analyze the role of figurative speech in Medical English paying special attention to the use of metaphors in medical discourse. After briefly introducing the recent approaches to the study of metaphors based on psycholinguistic and neurolinguistic theories, we propose a qualitative analysis of selected medical texts with three aims: (1) at a lexical level, to find out how commonly doctors use metaphors to name pathologies, and to classify such metaphors in relation to their linguistic type; (2) at a psycholinguistic level, to investigate if metaphors, when used to denominate an illness, use metaphoric mappings; (3) at a communicative level, to establish if doctors’ naming pathologies are doctor- or patient-oriented.

Abstract

Research on teacher motivation has recently given evidence of being closely related to several variables such as pre-service and in-service teacher training, educational reform, teaching practice, student motivation, work environment, psychological fulfilment and general health conditions. Taking into consideration the most recent studies on EFL teacher motivation across different disciplines and cultures, this paper aims at sharing the author’s personal experience with colleagues who have taught in CLIL trainee courses for High School content teachers. The results of the project, carried out in university run-courses from 2013 to 2017, have shown that teaching certain linguistic aspects (such as WE and ELF features, or word-forming processes) makes teachers linguistically aware of the plurality of English in communication and education, stimulates them strategically, increases their intrinsic motivation and influences their teaching effectiveness.

Abstract

Over the last decades, proposals to modernise legislative drafting have been choral and among the specific causes generally mentioned there are sentences of undue length, overuse of archaic expressions, repeated definitions and expressions, partiality of nominalisations, lack of gender neutrality. The aim of this analysis is to explore the legislative techniques adopted by drafters of English-speaking countries over the last decades, who are asked to write legal sentences aiming at gender fair and symmetric representation of men and women. The issue examined from a lexico-grammatical perspective culminates in the proposed questions whether certain techniques used to implement gender-neutral drafting can result in a product that is better than the one had before.

Abstract

In recent years, English has become the lingua franca of medicine and the language teaching profession has seen the emergence of im- plementing English for Medical Purposes (EMP) courses. The needs of EMP learners are very specific and EMP teachers who are language experts and not medical ones need to develop materials that are in line with EMP learners’ specific needs. In response to recognizing EMP students’ needs, extensive corpus-based research has led to specialized medical wordlists (Chen and Ge 2007; Wang, Liang, Ge 2008; Mungra and Canziani 2013; Hsu 2013; Lei and Liu 2016). Such wordlists have given instructors the opportunity to focus on specific vocabulary facil- itating domain specific learning. Among these medical wordlists, the Medical Academic Wordlist for clinical cases (MAWLcc) seems to fur- nish a good base for medical students, since it identifies the specific lexis of patient care. The aim of this paper is to propose an EMP teaching model for lexi- cal verbs extracted from the MAWLcc. These verbs are used in differ- ent communicative contexts for the development of medical students’ academic linguistic competence in their professional context.

Abstract

Medical texts are often thought to pertain to a closed community, but how far the language used by that community overlaps with general academic lexis is unknown. We examined a corpus of clinical case histories using the software RANGE to characterise the lexis of clinical case histories quantitatively and present a wordlist for clinical medicine. Only 58% of the general academic wordlists are found in clinical texts but the 85% overlap with an important academic wordlist, the Coxhead 570, furnishes evidence for the academic nature of the lexis used for clinical cases in Medicine. There was little overlap (38%) between our clinical case wordlist and other wordlists of medical research articles but such overlap might constitute a core of medical lexis.

Abstract

Purpose TO describe Goldmann applanation tonometer (GAT) and rebound tonometer (RT) agreement in measuring intraocular pressure (IOP) in glaucomatous and ocular hypertensive (OH) eyes and to evaluate central corneal thickness (CCT) influence on RT readings. Methods A total of 347 eyes were enrolled and IOP measured between 9 and 11 am. Rebound tonometry was performed first (RT1), followed by 3 consecutive GAT measurements and by a second RT reading (RT2), within a 5-minute span. Mean IOP (±SD) values were compared by means of paired t-test. Agreement between GAT and RT1 (test 1) and RT2 (test 2) was evaluated with Bland-Altman method, whereas a linear function described the relationship between CCT and IOP taken with RT. Results Mean IOP (±SD) taken with RT1, GAT, and RT2 was 18.1±4.3, 15.6±3.3, and 16.3±3.9 mmHg, respectively. Readings were within ±3 mmHg in 63.7% and 86.7% of eyes for test 1 and 2, respectively. A significant (pConclusions When used first, RT significantly overestimated IOP compared with GAT. Differences became clinically negligible when RT was used immediately after GAT. Repeated applanation tonometry may explain this observation. Agreement between instruments was acceptable for low IOP, but worsened with increasing IOP values. RT is significantly influenced by CCT. Goldmann applanation tonometer and RT should not be used interchangeably.

Medical students often face problems in using and understanding metaphors when communicating with a patient or reading a scientific paper. These figures of speech constitute an interpretative problem and students need key strategies to facilitate metaphor comprehension and disambiguation of meaning. This article examines how medical students' strategies of metaphor comprehension could be improved by specific teaching on metaphors using a Cognitive Linguistics approach. Medical students' ability to comprehend mirror neuron metaphors was assessed comparing the performance of students who did not receive any instruction about metaphoric extension strategies after a lesson on mirror neurons with the performance of students who received a special lesson both on mirror neurons and on metaphoric extension strategies. Students who received specific teaching on metaphor comprehension strategies performed better (85 %) and were more likely to use analogical reasoning and contextual clues than those who did not receive any specific teaching (55 %), who tended to adopt a violation strategy. Thus, their ability to give an exhaustive reinterpretation of metaphors seems to be affected by their ability to manage metaphoric extension strategies. These findings suggest that introducing a Cognitive Linguistics approach and using metaphoric extension strategies to aid metaphor comprehension may be an active and productive teaching strategy in a medical education programme. Thus, their ability to give an exhaustive reinterpretation of metaphors seems to be affected by their ability to manage metaphoric extension strategies. These findings suggest that introducing a Cognitive Linguistics approach and using metaphoric extension strategies to aid metaphor comprehension may be an active and productive teaching strategy in a medical education programme.

Abstract

Over the last few decades, the increasingly rapid progress of medical science and technology, together with the growing importance of English as the language of international communication, has made the study of medical English a crucial segment in the curriculum of medical undergraduates. In Italy, medical topics are studied not only in medical degree courses but also in those of sports sciences. This paper focuses attention on some medical English-related issues in sports sciences degree courses in Italian universities. More precisely, the work is divided into four main parts. The first part raises the terminological question concerning the denomination of the discipline “lingua inglese”/“lingua straniera” in sports sciences courses, which is discussed in relation to the ministerial objectives about specialized language teaching in university education. The second part draws attention to the important role that medical English plays in Italian sports sciences degree courses, the curriculum of which is outlined on the basis of its main subjects. In this respect, emphasis is given to English in the domain of sports medicine, one of the fields of knowledge which characterises the sports sciences degree course. Moreover, the importance of the acquisition of the lexicon of sports medicine is highlighted, also through a short account of the main eponyms belonging to the specific domain in question. The third part deals with the issue concerning credits, attendance at classes, and syllabuses in sports sciences courses. Finally, the fourth and last part discusses questions common to all medical English courses like students’ initial linguistic competences, comprehension difficulties in dealing with medical English communication, including subtechnical vocabulary acquisition, and the importance of genre-based and content and language integrated learning (CLIL) approaches to medical English teaching. The main purpose of the work is to draw attention to the importance that medical English syllabuses in sports sciences courses be well-designed in terms of effective methodologies and curriculum dominant subject-oriented contents in order to develop students’ linguistic competences within their chosen professional field. The paper also aims at highlighting the importance of uniformizing the time devoted to medical English teaching in sports sciences courses, also on account of the fact that the same learning-related difficulties in facing the language of medicine in English are essentially shared by all Italian undergraduates.

Abstract

INTRODUCTION Information, research and new ideas in medicine are spread mainly by the written word. Therefore, reading and writing are fundamental skills in collecting information and building a wide knowledge base not only in the student population but even as practising physicians and specialists. Knowing lexical items is an important building block in developing both of these language skills. There are several different measures of lexicographic competence, such as the General Service List or GSL (West, 1953), Academic Word List or AWL (Laufer &Nation,1999; Coxhead, 2000) and Medical Academic Word List or MAWL (Wang, Liang & Ge, 2008). The purpose of this study is to measure lexical competence for effective reading and writing and we thus addressed two main questions: how many words do our students actually use, given the Latin-based origin of many medical terms and how does lexis use vary with acquisition of content knowledge in medicine. METHODS: We collected examples of student writing summarizing a clinical case and ran these two cohorts of student writing from different years of medicine in Palermo and Rome against GSL and MAWL wordlists using software by Heatley, Nation & Coxhead (2002). Data were collected as number of tokens used and word families present. RESULTS: We will present data concerning our most salient findings about whether any difference between data from Rome was significantly different from Palermo in terms of proficiency levels and production, what portion of GSL and MAWL was actually used by our students and how word use varies with content knowledge. DISCUSSION: We will discuss our findings in the context of our corpus size, the relationship to L1 and whether the generally used word lists have any correspondence to an actual academic context. Abstract

Aim To investigate the rate and topological profile of minor physical anomalies (MPAs) (prenatal errors of morphogenesis) in a group of children with Autism Spectrum Disorder (ASD), in order to better set a temporal framing of embryological factors involved in the neurodevelopmental etiology. Method A new modified Waldrop scale and a mixed approach of computerized photogrammetry and classic anthroposcopy was used to detect the presence or absence of 41 MPAs in 24 children (mean age: 7 years; sex ratio: 22M:2F) with ASD and 24 healthy comparison subjects (mean age: 7 years; sex ratio: 19M:5F) selected with DSM IV and CARS. Results We found that children with ASD presenting MPAs (n = 23; 96%) had significantly higher rates of MPAs in four body areas (head, ears, mouth, hands); interestingly three of 41 MPAs best discriminated ASD groups from comparison subjects: abnormal head circumference, abnormal cephalic index, abnormal palate. Moreover, our results suggest that most MPAs occur predominantly after the first trimester of pregnancy. Conclusions These results support a prenatal neurodevelopmental model of the autism spectrum disorder.

Abstract

To assess the effect of atypical pattern of retardation (APR) on retinal nerve fibre layer (RNFL) measurements made by scanning laser polarimetry (SLP) with variable corneal compensation (GDx-VCC) in glaucomatous eyes.One eye each of 30 glaucomatous patients (average mean deviation (MD): -6.4+/-4.8) with APR on GDx-VCC retardation map were selected. In total, 34 glaucomatous, age- and severity-matched eyes (average MD: -7.0+/-5.3) and 36 age-matched healthy subjects, both with a normal pattern of retardation (NPR) represented control groups. APR on retardation maps was characterized by alternating peripapillary circumferential bands of low and high retardation, or high retardation areas arranged in a spokelike pattern, or high retardation nasal and temporal splotchy areas. Typical scan score (TSS) was extracted for each included eye. GDx-VCC parameters (mean+/-SD) in the two groups of glaucomatous eyes were compared with healthy eyes' corresponding values (Mann-Whitney U-test). Areas under receiver operating characteristic (AUROC) curves were generated to assess the APR effect on the parameters' diagnostic ability.All parameters discriminated adequately between healthy and glaucomatous eyes with NPR (AUROCsor =0.9 for nine parameters). On the contrary, considering healthy and glaucomatous eyes with APR, four thickness parameters could not separate the two groups and AUROCsor =0.85 appeared only for Inferior and Superior Ratio, NFI, Max Modulation.APR may void the effect of custom compensation and provide spurious RNFL thickness measurements. When a printout of glaucomatous eyes with APR is evaluated, it is proper to rely on ratios, modulation parameters, and NFI, since the diagnostic ability of thickness parameters is significantly reduced.

At the 24-month examination, this gain was achieved by 11 patients (65%) of the SGLT group and 11 (58%) of the TGLT group. Moreover, at the 24-month examination 59% and 26% gained 3 lines in the SGLT and TGLT groups, respectively., Conclusions: Resolution of macular edema and VA improvement are similar to those obtained with conventional TGLT, but SGLT is not associated with biomicroscopic and angiographic signs. A multicenter randomized clinical trial would be needed to ascertain the real efficacy and the most appropriate settings of SGLT for macular edema secondary to BRVO.
