Abstract

An elderly woman with a chronic course of disease presented with progressive weakness of lower limbs, heart failure, carbon dioxide retention, periorbital pigmentation, increased hair around the eyebrows, as well as multiple positive autoantibodies. After multidisciplinary team discussion, restrictive ventilatory dysfunction caused by significantly decreased diaphragm movement was taken as a breakthrough to sort out the etiology step by step. Based on clinical manifestations, serological and imaging features, as well as immunohistochemical staining of quadriceps femoris pathological complement, the final diagnosis of inflammatory myopathy associated with anti-mitochondrial antibodies was established. After correcting heart failure, intermittent non-invasive ventilation and rehabilitation training, clinical symptoms and functional status of the patient were significantly improved. The joint participation of the multidisciplinary team to identify the causes and solve problems in diagnosis and treatment decision-making reflects the great significance of multidisciplinary collaboration in rare diseases.

17. Analysis and Application of Structural Mechanical Properties of a New Type of On-line Monitoring Steel Spring Floating Slab track.

Abstract

Steel spring floating slab track is a kind of vibration and noise reduction track structure widely used in urban rail transit. With the increase of track service time, the structure damage, steel spring failure and even fracture of floating slab track may occur. Currently, skylight time is used for detection, which is cumbersome and difficult to operate. Based on this, this paper proposes a new type of steel spring floating slab track structure for online monitoring, which can grasp the deformation data of floating slab track spring in real time. The static and dynamic characteristics of the new structure are analyzed by numerical simulation method, and the application effect of online monitoring is evaluated based on the field measured data. The results show that the mechanical strength of the new online monitoring steel spring floating slab track structure can meet the requirements of normal use, the maximum bending moment of the track plate is 149.7 kN·m, the maximum shear force is 279.5 N, and the maximum shear hinge stress is 186 MPa. The maximum stress of 146.1 MPa of the upper bearing plate appears in the contact position with the jacking equipment, and the maximum stress of the outer sleeve wall is 102.2 MPa. Adopting the new steel spring floating slab track and reasonably setting it in the transition area with other tracks can make the transition of track structure more gentle and reduce the maximum vertical displacement of rail by 0. 85-1.25 mm. The switching sensor and measuring sensor are inserted into the steel spring floating plate vibration isolator, which can measure the compression amount of the steel spring and judge whether the spring is broken or fatigue fault respectively. The new on-line monitoring steel spring floating slab track structure has been laid in Jurong metro line of Nanjing, and the field measurement data shows that the new structure can accurately monitor the track structure displacement in real time. Abstract

Background and purpose: Cerebral small vessel disease (CSVD) is a clinical imaging syndrome with diverse etiology. Total homocysteine (HCY) level might increase the risk of myocardial and cerebral infarction by damaging the vascular endothelium. We aimed to explore the correlation between total HCY and CSVD imaging burden, based on Mendelian randomization methods. Methods: A total of 1,023 participants of the Shunyi study, a population‐based cohort study, were included. Vascular risk factors, total HCY levels and methylenetetrahydrofolate reductase (MTHFR) gene mutations (C677T and A1298C) were examined. CSVD imaging markers, including lacunes, cerebral microbleeds, white matter hyperintensity, enlarged perivascular space and brain parenchymal fraction (BPF) were also assessed. Results: Mutations of C677T were significantly correlated with increased total HCY levels (CC→TT: β = 0.28, p < 0.0001), while mutations of A1298C were correlated with decreased total HCY levels (AA→AC: β = −0.13, p < 0.0001; AA→CC: β = −0.25, p = 0.004). In the Mendelian randomization study, the C677T genotype was significantly associated with lacunes (CC→CT: odds ratio [OR] 2.76, p = 0.008; CC→TT: OR 2.50, p = 0.018), and the A1298C genotype was significantly correlated with BPF (AA→CC: β = 1.32, p = 0.015). Similarly, in multivariate regression analysis, total HCY levels were significantly correlated with lacunes (OR 2.14, p < 0.0001) and negatively correlated with BPF (β = −0.55, p = 0.004). Age, sex and vascular risk factors were adjusted for. Conclusions: Total HCY level was correlated with imaging burden of CSVD, especially with lacunes and brain volume loss. For individuals with risk genetic predisposition, enhanced homocysteine‐lowering strategies might be necessary to reduce the risk and progress of CSVD. [ABSTRACT FROM AUTHOR]

Abstract

The nervous system is one of the most complex biological systems, and nervous system disease (NSD) is a major cause of disability and mortality. Extensive evidence indicates that numerous dysregulated microRNAs (miRNAs) are involved in a broad spectrum of NSDs. A comprehensive review of miRNA-mediated regulatory will facilitate our understanding of miRNA dysregulation mechanisms in NSDs. In this work, we summarized currently available databases on miRNAs and NSDs, star NSD miRNAs, NSD spectrum width, miRNA spectrum width and the distribution of miRNAs in NSD sub-categories by reviewing approximately 1000 studies. In addition, we characterized miRNA–miRNA and NSD–NSD interactions from a network perspective based on miRNA–NSD benchmarking data sets. Furthermore, we summarized the regulatory principles of miRNAs in NSDs, including miRNA synergistic regulation in NSDs, miRNA modules and NSD modules. We also discussed computational challenges for identifying novel miRNAs in NSDs. Elucidating the roles of miRNAs in NSDs from a network perspective would not only improve our understanding of the precise mechanism underlying these complex diseases, but also provide novel insight into the development, diagnosis and treatment of NSDs.
