Abstract

Las roturas inveteradas o crónicas del tendón distal del bíceps braquial son infrecuentes. Su tratamiento supone un reto importante por el grado de retracción y deficiente calidad del tendón y músculo lesionados, que en ocasiones obligan al empleo de injertos para tratar de restaurar la anatomía y mejorar los resultados funcionales. La tasa de complicaciones en las reparaciones de roturas crónicas es mayor con respecto a las reparaciones de lesiones agudas. Presentamos el caso de una reparación quirúrgica de una rotura crónica de 3 meses de evolución mediante la utilización de aloinjerto liofilizado de Aquiles. Chronic or inveterate disruptions of the distal biceps brachii are uncommon. Treatment remains a major challenge by shrinkage and poor quality of the injured tendon and muscle, which sometimes require the use of grafts to try to restore the anatomy and improve the functional outcomes. The rate of complications in surgical treatment of chronic tears is more than the repairs of acute injuries. We report the case of a surgical reconstruction of a chronic tear three months of evolution using lyophilized Achilles allograft.

Abstract

La rotura bilateral y simultánea del tendón de Aquiles es un hecho excepcional. La producción de esta lesión en una persona joven, sin antecedentes patológicos ni de administración de ciertos medicamentos es aún más excepcional. El tratamiento quirúrgico y un adecuado proceso de seguimiento y rehabilitación contribuyen a una buena recuperación y pronóstico en los individuos jóvenes y activos con roturas del tendón de Aquiles. Se presenta un caso clínico de una mujer de 21 años tratada quirúrgicamente con buen resultado. The bilateral and simultaneous rupture of Achilles tendon is an excepcional situation. The production of this injury in a young person, without history pathological or administration of certain drugs is even more exceptional. The surgical treatment and a proper process of following-up and rehabilitation help to a good recovery and prognosis in young and active people with Achilles tendon ruptures.

7. [Single-row versus double-row arthroscopic repair in the treatment of rotator cuff tears. A systematic review.]

Abstract

Purpose: The purpose of this study is to perform a systematic review of the scientific literature to compare single and double row techniques for rotator cuff tears in terms of functional, pain improvement and structural healing of the tendon., Methods: In order to carry out the research, a systematic selection of scientific articles has been made by consulting PubMed and The Cochrane Library databases. The articles included in the present study compare the single-row and double-row techniques, with publication date between 2016 and 2021., Results: Diverse results are observed in the UCLA, SST, Constant and ASES assessment scales and in the range of mobility, therefore, the superiority of one technique in terms of functional results is not established. The double row technique may show superiority in the strength variable, although there is a methodological variability in its measurement. The double-row technique may be related to more intense postoperative pain, while the single-row technique to residual shoulder pain. However, the variables strength and pain show non-homogeneous or coincident data. The superiority of the double row repair in the structural healing of the tendon and a lower re-tear rate than the single row technique is established with statistically significant results, being the most homogeneous and coincident studies., (Universidad Nacional de Córdoba)

Abstract

Background: The rupture of the pectoralis major muscle is rare, occurring typically in young male athletes. When the break is complete and the patient active, surgical treatment is the choice., Clinical Case: 48-year-old man with severe pain in left shoulder after lifting weights. He presents impotence to adduction and internal rotation of the shoulder with thinning of the axillary fold. MRI confirms the diagnosis of complete rupture of the pectoralis major. Surgical treatment was decided, performing a deltopectoral approach and reinserting the pectoralis major tendon with three cortical buttons., Conclusion: The rupture of the pectoralis major is a typical lesion of active young patients with an increasing incidence. Although there have been no significant differences between the different repair options, in our experience the use of cortical buttons provides very satisfactory results., (Universidad Nacional de Córdoba)
