Abstract

BackgroundBaricitinib and dexamethasone have randomised trials supporting their use for the treatment of patients with COVID-19. We assessed the combination of baricitinib plus remdesivir versus dexamethasone plus remdesivir in preventing progression to mechanical ventilation or death in hospitalised patients with COVID-19.MethodsIn this randomised, double-blind, double placebo-controlled trial, patients were enrolled at 67 trial sites in the USA (60 sites), South Korea (two sites), Mexico (two sites), Singapore (two sites), and Japan (one site). Hospitalised adults (≥18 years) with COVID-19 who required supplemental oxygen administered by low-flow (≤15 L/min), high-flow (>15 L/min), or non-invasive mechanical ventilation modalities who met the study eligibility criteria (male or non-pregnant female adults ≥18 years old with laboratory-confirmed SARS-CoV-2 infection) were enrolled in the study. Patients were randomly assigned (1:1) to receive either baricitinib, remdesivir, and placebo, or dexamethasone, remdesivir, and placebo using a permuted block design. Randomisation was stratified by study site and baseline ordinal score at enrolment. All patients received remdesivir (≤10 days) and either baricitinib (or matching oral placebo) for a maximum of 14 days or dexamethasone (or matching intravenous placebo) for a maximum of 10 days. The primary outcome was the difference in mechanical ventilation-free survival by day 29 between the two treatment groups in the modified intention-to-treat population. Safety analyses were done in the as-treated population, comprising all participants who received one dose of the study drug. The trial is registered with ClinicalTrials.gov, NCT04640168.FindingsBetween Dec 1, 2020, and April 13, 2021, 1047 patients were assessed for eligibility. 1010 patients were enrolled and randomly assigned, 516 (51%) to baricitinib plus remdesivir plus placebo and 494 (49%) to dexamethasone plus remdesivir plus placebo. The mean age of the patients was 58·3 years (SD 14·0) and 590 (58%) of 1010 patients were male. 588 (58%) of 1010 patients were White, 188 (19%) were Black, 70 (7%) were Asian, and 18 (2%) were American Indian or Alaska Native. 347 (34%) of 1010 patients were Hispanic or Latino. Mechanical ventilation-free survival by day 29 was similar between the study groups (Kaplan-Meier estimates of 87·0% [95% CI 83·7 to 89·6] in the baricitinib plus remdesivir plus placebo group and 87·6% [84·2 to 90·3] in the dexamethasone plus remdesivir plus placebo group; risk difference 0·6 [95% CI -3·6 to 4·8]; p=0·91). The odds ratio for improved status in the dexamethasone plus remdesivir plus placebo group compared with the baricitinib plus remdesivir plus placebo group was 1·01 (95% CI 0·80 to 1·27). At least one adverse event occurred in 149 (30%) of 503 patients in the baricitinib plus remdesivir plus placebo group and 179 (37%) of 482 patients in the dexamethasone plus remdesivir plus placebo group (risk difference 7·5% [1·6 to 13·3]; p=0·014). 21 (4%) of 503 patients in the baricitinib plus remdesivir plus placebo group had at least one treatment-related adverse event versus 49 (10%) of 482 patients in the dexamethasone plus remdesivir plus placebo group (risk difference 6·0% [2·8 to 9·3]; p=0·00041). Severe or life-threatening grade 3 or 4 adverse events occurred in 143 (28%) of 503 patients in the baricitinib plus remdesivir plus placebo group and 174 (36%) of 482 patients in the dexamethasone plus remdesivir plus placebo group (risk difference 7·7% [1·8 to 13·4]; p=0·012).InterpretationIn hospitalised patients with COVID-19 requiring supplemental oxygen by low-flow, high-flow, or non-invasive ventilation, baricitinib plus remdesivir and dexamethasone plus remdesivir resulted in similar mechanical ventilation-free survival by day 29, but dexamethasone was associated with significantly more adverse events, treatment-related adverse events, and severe or life-threatening adverse events. A more individually tailored choice of immunomodulation now appears possible, where side-effect profile, ease of administration, cost, and patient comorbidities can all be considered.FundingNational Institute of Allergy and Infectious Diseases.

Abstract

BackgroundSevere coronavirus disease 2019 (Covid-19) is associated with dysregulated inflammation. The effects of combination treatment with baricitinib, a Janus kinase inhibitor, plus remdesivir are not known.MethodsWe conducted a double-blind, randomized, placebo-controlled trial evaluating baricitinib plus remdesivir in hospitalized adults with Covid-19. All the patients received remdesivir (≤10 days) and either baricitinib (≤14 days) or placebo (control). The primary outcome was the time to recovery. The key secondary outcome was clinical status at day 15.ResultsA total of 1033 patients underwent randomization (with 515 assigned to combination treatment and 518 to control). Patients receiving baricitinib had a median time to recovery of 7 days (95% confidence interval [CI], 6 to 8), as compared with 8 days (95% CI, 7 to 9) with control (rate ratio for recovery, 1.16; 95% CI, 1.01 to 1.32; P = 0.03), and a 30% higher odds of improvement in clinical status at day 15 (odds ratio, 1.3; 95% CI, 1.0 to 1.6). Patients receiving high-flow oxygen or noninvasive ventilation at enrollment had a time to recovery of 10 days with combination treatment and 18 days with control (rate ratio for recovery, 1.51; 95% CI, 1.10 to 2.08). The 28-day mortality was 5.1% in the combination group and 7.8% in the control group (hazard ratio for death, 0.65; 95% CI, 0.39 to 1.09). Serious adverse events were less frequent in the combination group than in the control group (16.0% vs. 21.0%; difference, -5.0 percentage points; 95% CI, -9.8 to -0.3; P = 0.03), as were new infections (5.9% vs. 11.2%; difference, -5.3 percentage points; 95% CI, -8.7 to -1.9; P = 0.003).ConclusionsBaricitinib plus remdesivir was superior to remdesivir alone in reducing recovery time and accelerating improvement in clinical status among patients with Covid-19, notably among those receiving high-flow oxygen or noninvasive ventilation. 5. An analysis of the accounting degree study plans designed on the skills of public accountants.

Abstract

Purpose: In the current context, with several professions undergoing changes, this study aims to analyse the suitability of the accounting degree study plan for the training of public accountants, based on the perception of course coordinators at HEIs. Methodology: Information was collected through questionnaire survey sent to 20 HEIs that offer a degree in accounting. The questionnaire addressed three dimensions: an overview of the accounting degree, a specific analysis of the public accounting discipline and the assessment of the technical skills necessary for the public accounting profession. The data was analysed using descriptive statistics and the opinions of the course coordinators were considered. Results: The study had 11 valid responses out of 20 possible, which represents 55% of the total population. It was possible to identify a consensus on the need to add more hours or disciplines related to public accounting, in addition to the recognition that the profession requires additional specialization in the area. Research limitations: The main limitation of this study is the relatively small sample size, with responses from only 11 HEIs. This may limit the generalizability of the results. Furthermore, the study is based on the subjective perceptions of course coordinators, which may introduce biases. Future research could expand the sample size and include input from other interested parties, such as course teachers, students and employers, to provide a more comprehensive analysis. Originality: This study contributes to the existing literature by providing a focused analysis of accounting degree programs from the perspective of course coordinators. It identifies specific areas where the curriculum can be improved to better prepare students for careers in public accounting. The findings offer practical recommendations for curriculum enhancement, addressing a critical gap in the alignment between academic training and professional requirements in the field of public accounting. Abstract

New Caledonia is an important biodiversity hotspot, where numerous plant and animal groups show high levels of species diversity and endemicity, while facing multiple threats to their habitats. Leaf beetles in the subfamily Eumolpinae illustrate this pattern, with species estimates higher than the number of described taxa and distribution ranges that are often consistent with microendemicity. In this study, we increase the knowledge of this group by focusing on the Taophila subgenus Lapita, known from three species but here expanded to eleven with eight new species: T. atlantis sp. nov., T. hermes sp. nov., T. kronos sp. nov., T. oceanica sp. nov., T. olympica sp. nov., T. ouranos sp. nov., T. riberai sp. nov., and T. tridentata sp. nov. Additionally, we infer the evolutionary history of the group using mtDNA markers (COI and rrnS). This phylogeny and the species distribution help hypothesize a model of evolution for this lineage in the context of historical climatic and geological changes of New Caledonia. 25. Pain Reduction in Rheumatoid Arthritis Patients Who Use Opioids: A Post Hoc Analysis of Phase 3 Trials of Baricitinib.

Abstract

Objective: Pain reduction with baricitinib was assessed in patients with rheumatoid arthritis (RA) who either used opioids or did not use opioids during three randomized, double‐blind phase 3 trials. Methods: Analysis populations were as follows: i) baricitinib 4 mg once daily versus placebo groups integrated from RA‐BEAM (NCT01710358) for patients with inadequate response (IR) to methotrexate, RA‐BUILD (NCT01721057) with IR to conventional disease‐modifying antirheumatic drugs, and RA‐BEACON (NCT01721044) with IR to at least one tumor necrosis factor inhibitors; ii) baricitinib 2 mg versus placebo from RA‐BUILD and RA‐BEACON; and iii) adalimumab 40 mg every other week versus placebo from RA‐BEAM. Pain was measured by the Patient Assessment of Pain Visual Analog Scale. Analysis of covariance modeling assessed differences in pain reduction between treatments at each time point through Week 24, with an interaction term to test heterogeneous treatment effects across opioid users and nonusers. Results: Baricitinib 4 mg had greater pain reduction versus placebo in opioid users and nonusers (P < 0.05) at all time points starting from Week 1; the pain reduction was similar between opioid users and nonusers. Baricitinib 2 mg had greater pain reduction versus placebo in opioid users and nonusers starting at Week 4. A significant difference in pain reduction was not observed for adalimumab versus placebo in the opioid users but was observed in nonusers at all time points. Conclusion: Pain reduction was observed and was similar between opioid users and nonusers with baricitinib 2 mg and 4 mg but not adalimumab in this post hoc analysis. [ABSTRACT FROM AUTHOR]

32. Untangling extant biodiversity in Hydroporini

Abstract

Presentation given at the XXXVIII Annual Meeting of the Willi Hennig Society, in University of California, Berkeley (US), on May 26th-31st, 2019., Aquatic beetles can be characterized based on the habitats where they live, with a main division between species living in standing water or species living in running waters. Both habitat types present differences based on their physical and chemical characteristics, leading to different ecological dynamics at geological scale, in essentially long-term habitat stability. Small standing water environments, where aquatic beetles are usually found, are usually filled up by sediments in decades, whilst running water environments are stable and they are connected with the drainage network, even when they sometimes dry up seasonally. Some studies had shown that one of the consequences of living in one kind or the other is a difference in range size linked to their dispersal capabilities, with standing water species having broader geographical ranges than running water species. It has been hypothesizing that dispersal abilities may affect gene flow and turnover, but there is a lack of knowledge of how these differences affect long-term diversification. Hydroporini is a speciose tribe containing ca. 680 species living in both habitats, making them and ideal group to study diversification. We built a phylogeny of Hydroporini combining 5 genes obtained with sanger sequences plus 69 mitochondrial genomes. In total, 473 terminals representing species were used. Ecological data were obtained by bibliographic revision plus own collected information in the field. Phylogeny and habitat type evolution were reconstructed using Bayesian inference. Diversification rates were calculated using Bayesian Analysis of Macroevolutionary Mixtures (BAMM) and method of moments (MM). Preliminary results suggest that the most recent common ancestor of Hydroporini was reconstructed with a preference for running water, with multiple shifts to standing waters. We also found several significant diversification shifts across the phylogeny, and a clear pattern of gradual accelerated speciation.

Abstract

The smallest beetles and the smallest non-parasitic insects belong to the staphylinoid family Ptiliidae. Their adult body length can be as small as 0.325 mm and is generally smaller than 1 mm. Here we address the phylogenetic relationships within the family using formal analyses of adult morphological characters and molecular data, and also a combination of both for the first time. Strongly supported clades are Ptiliidae + Hydraenidae, Ptiliidae, Ptiliidae excl. Nossidium, Motschulskium and Sindosium, Nanosellini, and a clade comprising Acrotrichis, Smicrus, Nephanes and Baeocrara. A group comprising Actidium, Oligella and Micridium + Ptilium is also likely monophyletic. Limulodes + Cephaloplectus, strongly supported as a clade (Cephaloplectus included only in morphological analysis), are deeply nested within Ptiliidae in the morphology-only non-weighted and in the molecular analyses, but placed as sister to the remaining Ptiliidae (excl. Nossidium, Motschulskium and Sindosium) after successive reweighting. We propose that Ptiliidae should be taxonomically divided into two subfamilies: the most recently established Nossidiinae and Ptiliinae, the latter currently composed of seven tribes: Acrotrichini, Cephaloplectini, Discheramocephalini, Nanosellini, Ptenidiini, Ptiliini and Ptinellini, although the monophyly and taxonomic status is still uncertain in some cases. Important morphological innovations evolved in the stem group of Hydraenidae and Ptiliidae, including partly internalized mouthparts suitable for saprophagy and sporophagy, a labral-mandibular locking device, a specific elytral locking mechanism with elongated alacristae, wings with fringes of hairs, and a very unusual spermathecal pump. A complex feature of Ptiliidae linked with miniaturization is the transformation of the alae into “feather wings”, with a highly efficient flight mechanism unique in beetles. Nanosellini include the smallest ptiliid species and display features correlated with extremely small body size, such as simplification of the endoskeleton of the head and thorax, far-reaching reduction of the wing venation, and a reduced number of antennomeres.

Abstract

Objective To evaluate the long-term efficacy of once-daily baricitinib 4 mg in patients with active RA who were either naïve to DMARDs or who had inadequate response (IR) to MTX. Methods Analyses of data from two completed 52-week, phase III studies, RA-BEGIN (DMARD-naïve) and RA-BEAM (MTX-IR), and one ongoing long-term extension (LTE) study (RA-BEYOND) were performed (148 total weeks). At week 52, DMARD-naïve patients treated with MTX monotherapy or baricitinib 4 mg+MTX in RA-BEGIN were switched to open-label baricitinib 4 mg monotherapy; MTX-IR patients treated with adalimumab (+MTX) in RA-BEAM were switched to open-label baricitinib 4 mg (+MTX) in the LTE. Patients who received placebo (+MTX) were switched to baricitinib 4 mg (+MTX) at week 24. Low disease activity (LDA) [Simple Disease Activity Index (SDAI) ≤11], clinical remission (SDAI ≤ 3.3), and physical functioning [Health Assessment Questionnaire Disability Index (HAQ-DI) ≤ 0.5] were assessed. Data were assessed using a non-responder imputation. Results At week 148, SDAI LDA was achieved in up to 61% of DMARD-naïve patients and 59% of MTX-IR patients initially treated with baricitinib, and SDAI remission was achieved in up to 34% of DMARD-naïve patients and 24% of MTX-IR patients; HAQ-DI ≤ 0.5 was reached in up to 48% of DMARD-naïve patients and 38% of MTX-IR patients initially treated with baricitinib. Over 148 weeks, 3.6% and 10.7% of MTX-IR patients discontinued across treatment groups due to lack of efficacy or due to adverse events, respectively; discontinuation rates were similar in the DMARD-naïve population. They are the ecologically most diverse subfamily, with terrestrial species as well as species in almost all types of aquatic habitats, including hypersaline waters. Ochthebiinae include the tribes Ochtheosini (four species in three genera) and Ochthebiini. We provide here the first comprehensive phylogeny of the tribe Ochthebiini, based on 188 species and four subspecies from most genera, subgenera and species groups. We obtained sequence data for a combination of mitochondrial and nuclear gene fragments including the 5’ and 3’ ends of the cytochrome c oxidase subunit 1, the 5’ end of 16S RNA plus the Leucine tRNA transfer plus 5’ end of NADH dehydrogenase subunit I, and internal fragments of the large and small ribosomal units. The analyses with maximum likelihood and Bayesian probabilities consistently recovered a generally well supported phylogeny, with most currently accepted taxa and species groups as monophyletic. We provide a new classification of the tribe based on our phylogenetic results, with three genera: Meropathus Enderlein, Ochthebius Leach and Protochthebius Perkins. We could not obtain sequences of any species of Protochthebius, which is therefore maintained as a distinct genus of Ochthebiini. We elevated to species rank two subspecies of Ochthebius: O. fallaciosus Ganglbauer stat.n. (former subspecies of O. viridis Peyron) and O. deletus Rey stat.rest. Abstract

Ochthebiinae, with c. 650 species distributed worldwide, are the second most speciose subfamily of the aquatic beetle family Hydraenidae. They are ecologically the most diverse hydraenid subfamily, with terrestrial species as well as species in almost all types of aquatic habitats, including hypersaline waters. Ochthebiinae include the tribes Ochtheosini (four species in three genera) and Ochthebiini. We provide here the first comprehensive phylogeny of the tribe Ochthebiini, based on 186 species and four subspecies from most genera, subgenera and species groups. We obtained sequence data for a combination of mitochondrial and nuclear gene fragments including the 5′ and 3′ ends of the cytochrome c oxidase subunit 1, the 5′ end of 16S RNA plus the leucine tRNA transfer plus 5′ end of NADH dehydrogenase subunit I, and internal fragments of the large and small ribosomal units. The analyses with maximum likelihood (ML) and Bayesian probabilities consistently recovered a generally well supported phylogeny, with most currently accepted taxa and species groups as monophyletic. We provide a new classification of the tribe based on our phylogenetic results, with six genera: Meropathus Enderlein, Ochthebius Leach, Protochthebius Perkins, Prototympanogaster Perkins, Tympallopatrum Perkins and Tympanogaster Janssens. 46. Systematics of the New Caledonian endemic genus Taophila Heller (Coleoptera: Chrysomelidae, Eumolpinae) combining morphological, molecular and ecological data, with description of two new species

Abstract

The genus Taophila Heller, 1916 was considered a monotypic taxon until the recent addition of 11 new species. We examined the recent revision of this genus to evaluate these new taxonomic considerations in the light of molecular data for a sample of 85 individuals in 8 out of 12 Taophila species, and to provide characters for a phylogenetic systematic analysis of the genus. These data include two mitochondrial DNA markers (cox1 and rrnS) and one nuclear DNA marker (wingless) to infer the species phylogeny, and one gene from the chloroplastic DNA (psbA-trnH) of putatively ingested plant tissue for diet inference. Molecular data support the monophyly of the species studied, except for T.subsericea Heller, 1916, which possibly represents an ancient genetic polymorphism. The molecular phylogeny and a reassessment of morphological characters are used to propose several taxonomic changes that will achieve stable systematics in Taophila, including: (i) description of two new species, T.aphrodita sp.n. and T.gaea sp.n., both related to T.mars Samuelson, 2010; (ii) removal of T.cancellata Samuelson, 2010 from Taophila and its new combination with Dematochroma Baly, 1864; (iii) proposal of two new subgenera in Taophila to reflect deep phylogenetic divergence and profound morphological discrepancies with the anatomy of the generic type and relatives, Jolivetiana subgen.n. (type: Taophila mantillerii Jolivet, Verma & Mille, 2007) and Lapita subgen.n. (type: Taophila mars Samuelson, 2010). Abstract

Contemporary taxonomic work on New Caledonian Eumolpinae (Chrysomelidae) has revealed their high species richness in this Western Pacific biodiversity hotspot. To estimate total species richness in this community, we used rapid DNA-based biodiversity assessment tools, exploring mtDNA diversity and phylogenetic structure in a sample of 840 specimens across the main island. Concordance of morphospecies delimitation with units delimited by phenetic and phylogenetic algorithms revealed some 98-110 species in our sample, twice as many as currently described. Sample-based rarefaction curves and species estimators using these species counts doubled this figure (up to 210 species), a realistic estimate considering taxonomic coverage, local endemism, and characteristics of sampling design, amongst others. New Caledonia, compared with larger tropical islands, stands out as a hotspot for Eumolpinae biodiversity. Molecular dating using either chrysomelid specific rates or tree calibration using palaeogeographical data dated the root of the ingroup tree (not necessarily a monophyletic radiation) at 38.5Mya, implying colonizations after the Cretaceous breakage of Gondwana. Our data are compatible with the slowdown in diversification rates through time and are also consistent with recent faunal origins, possibly reflecting niche occupancy after an initial rapid diversification. 50. Rapid biodiversity assessment in biodiversity hotspots: diversity and diversification of New Caledonian leaf beetles

Trabajo presentado en el 2nd BioSyst.EU (BioSyst.EU 2013 Global systematics!), celebrado en Viena del 18 al 22 de febrero de 2013., Biodiversity hotspots attracted a lot of attention in recent years precisely to characterize their particularly rich, unique and relatively endangered biota. In this effort, DNA-based biodiversity surveys generally proved to be cost-effective in terms of species characterization, but also species discovery and, some- times, to study the underlying diversification processes leading to the diversity we observe today. New Caledonia, the fragmented piece of Gondwana in the western Pacific, is one such hotspot profiting from this approach for several groups of organisms. In our case, we worked with leaf beetles in the subfamily Eumolpinae, which contemporary taxonomic work – 48 newly described species in only five years – reveals as far more diverse than expected. To estimate how many species of New Caledonian Eumolpinae still await discovery, we used rapid DNA-based biodiversity assessment tools, exploring mtDNA (cox1 and rrnS) diversity and phylogenetic structure in a sample of 840 specimens across the main island in the archipelago. A notable concordance of species delimited by phenetic (Automated Barcode Gap Discovery) and phylogenetic (Generalized mixed Yule-Coalescent models) algorithms with those informed by exter- nal and genitalic differences (morphospecies) revealed some 98–110 species in our sample, twice as many as were described so far and all of them almost certainly endemic. The study of sample-based rarefac- tion curves and species estimators using these species counts doubled this figure (up to 210 species), a plausible outcome based on information on taxonomic coverage, local endemism and characteristics of sampling design, among others. New Caledonia, compared with larger tropical islands, stands out as a hotspot for Eumolpinae biodiversity. MtDNA phylogenies were also exploited to illuminate on the ori- gins of this diversity. Molecular clocks using leaf-beetle specific rates or internal tree calibration using the Pleistocene uplifting of nearby islands, dated the root of the neocaledonian Eumolpinae tree (not necessarily a monophyletic radiation) at 38.5 Ma, implying colonisations well after the Late Cretaceous breakage of Gondwana. The diversification mode was consistent with a reduction in diversification rates through time, also consistent with recent faunal origins, possibly reflecting niche occupancy after a rapid initial diversification. Environmental factors ( e.g. soil characteristics) seemingly played a role in this diversification process.
