1. A digital instrument simulator to optimize the development of hyperspectral systems: application for intraoperative functional brain mapping

Abstract

Intraoperative optical imaging is a localization technique for the functional areas of the human brain cortex during neurosurgical procedures. These areas can be assessed by monitoring cerebral hemodynamics and metabolism. A robust quantification of these biomarkers is complicated to perform during neurosurgery due to the critical context of the operating room. In actual devices, the inhomogeneities of the optical properties of exposed brain cortex are poorly taken into consideration, which introduce quantification errors of biomarkers of brain functionality. Moreover, the choice of the best spectral configuration is still based on an empirical approach. We propose a digital instrument simulator to optimize the development of hyperspectral systems. This simulator can provide a realistic modelling of the cerebral cortex and the identification of the optimal wavelengths to monitor cerebral hemodynamics (oxygenated and deoxygenated hemoglobin) and metabolism (oxidized state of cytochromes b, c and cytochrome-c-oxidase). The digital instrument allows the modelling of intensity maps collected by a camera sensor as well as images of pathlength to take into account the inhomogeneities of the optical properties. The optimization procedure helps to identify the best wavelength combination of 18 wavelengths that reduce the quantification errors in HbO2, Hb, oxCCO of 61%, 29% and 82% compared to the gold standard of 121 wavelengths between 780 and 900 nm. The optimization procedure does not help to resolve changes in cytochrome b and c in a significant way but help to better resolve oxCCO changes. We proposed a digital instrument simulator to optimize the development of hyperspectral systems for intraoperative brain mapping studies. This digital instrument simulator and this optimization framework could be used to optimize the design of hyperspectral imaging devices.

2. A transportable hyperspectral imaging setup based on fast, high-density spectral scanning for in situ quantitative biochemical mapping of fresh tissue biopsies

Abstract

Histopathological examination of surgical biopsies, such as in glioma and glioblastoma resection, is hindered in current clinical practice by the long times required for the laboratory analysis and pathological screening, typically taking several days or even weeks to be completed. We propose here a transportable, high-density, spectral-scanning based hyperspectral imaging setup, named HyperProbe1, that can provide in situ, fast biochemical analysis and mapping of fresh surgical tissue samples, right after excision, and without the need of fixing or staining. HyperProbe1 is based on spectral scanning via supercontinuum laser illumination filtered with acousto-optic tuneable filters. Such methodology allows the user to select any number and type of wavelength bands in the visible and near-infrared range between 510 and 900 nm (up to 79), and to reconstruct 3D hypercubes composed of high-resolution, widefield images of the surgical samples, where each pixel is associated with a complete spectrum. The system is applied on 11 fresh surgical biopsies of glioma from routine patients, including different grades of tumour classification. Quantitative analysis of the composition of the tissue is performed via fast spectral unmixing to reconstruct mapping of major biomarkers. We also provided a preliminary attempt to infer tumour classification based on differences of composition in the samples, suggesting the possibility to use lipid content and differential cytochrome-c-oxidase concentrations to distinguish between lower and higher grade gliomas. A proof-of-concept of the performances of HyperProbe1 for quantitative, biochemical mapping of surgical biopsies is demonstrated, paving the way for improving current post-surgical, histopathological practice via non-destructive, in situ streamlined screening of fresh tissue samples in a matter of minutes after excision.

Abstract

Diffuse optical modalities such as broadband near-infrared spectroscopy (bNIRS) and hyperspectral imaging (HSI) represent a promising alternative for low-cost, non-invasive, and fast monitoring of functional and structural properties of living tissue. Particularly, the possibility of extracting the molecular composition of the tissue from the optical spectra in real-time deems the spectroscopy techniques as a unique diagnostic tool. However, no established method exists to streamline the inference of the biochemical composition from the optical spectrum for real-time applications such as surgical monitoring. In this paper, we analyse a machine learning technique for fast and accurate inference of changes in the molecular composition of brain tissue. We reconsider and propose modifications to the existing learnable methodology based on the Beer-Lambert law, which analytically connects the spectra with concentrations. We evaluate the method's applicability to linear and non-linear formulations of the Beer-Lambert law. The approach is tested on real data obtained from the bNIRS- and HSI-based optical monitoring of brain tissue. The results demonstrate that the proposed method enables real-time molecular composition inference while maintaining the accuracy of traditional linear and non-linear optimization solvers. Preliminary findings show that Beer-Lambert law-based spectral unmixing allows to contrast brain anatomy semantics such as the vessel tree and tumor area.

The system is applied to 11 fresh surgical biopsies of glioma from routine patients, including different grades of tumor classification. Quantitative analysis of the composition of the tissue is performed via fast spectral unmixing to reconstruct the mapping of major biomarkers, such as oxy-(HbO2) and deoxyhemoglobin (HHb), as well as cytochrome-c-oxidase (CCO). Abstract

Simple Summary: In the era of targeted therapies, achieving a maximal safe resection still remains a critical prognosis factor in diffuse low-grade gliomas and represents the goal to reach for neurosurgeons who manage these tumors. Whereas fluorescence-guided surgery, using 5-aminolevulinic acid or even fluorescein sodium significantly increases the extent of resection of high-grade gliomas, its relevance for the resection of low-grade gliomas is very limited. However, new intraoperative techniques such as spectroscopic detection of 5-aminolevulinic acid-induced fluorescence or confocal laser endomicroscopy have been developed in order to increase the sensitivity of fluorescence detection in low-grade gliomas or to generate optic biopsies, respectively. Therefore, the goal of this review was to sum up the limitations of macroscopic fluorescence detection in low-grade gliomas and to highlight how new technologies likely to be implemented in the surgical routine in the near future, could overcome these limitations. Diffuse low-grade gliomas are infiltrative tumors whose margins are not distinguishable from the adjacent healthy brain parenchyma. The aim was to precisely examine the results provided by the intraoperative use of macroscopic fluorescence in diffuse low-grade gliomas and to describe the new fluorescence-based techniques capable of guiding the resection of low-grade gliomas. Only about 20% and 50% of low-grade gliomas are macroscopically fluorescent after 5-amino-levulinic acid (5-ALA) or fluorescein sodium intake, respectively. However, 5-ALA is helpful for detecting anaplastic foci, and thus choosing the best biopsy targets in diffuse gliomas. Spectroscopic detection of 5-ALA-induced fluorescence can detect very low and non-macroscopically visible concentrations of protoporphyrin IX, a 5-ALA metabolite, and, consequently, has excellent performances for the detection of low-grade gliomas. Moreover, these tumors have a specific spectroscopic signature with two fluorescence emission peaks, which is useful for distinguishing them not only from healthy brain but also from high-grade gliomas. Confocal laser endomicroscopy can generate intraoperative optic biopsies, but its sensitivity remains limited. In the future, the coupled measurement of autofluorescence and induced fluorescence, and the introduction of fluorescence detection technologies providing a wider field of view could result in the development of operator-friendly tools implementable in the operative routine. Abstract

Spectral optical imaging refers to the formation of images using several wavelengths of the light spectrum. This technique can be applied to biological tissues to quantify intrinsic markers. In this thesis, two biomedical applications have been investigated: intraoperative functional neuroimaging and the detection of pathological tissues in endoscopic explorations (proof of concept for the endoscopic exploration in a murin model). For both applications, the objective is to estimate the oxygenation spatio-temporal changes in biological tissues. These measurements are guided by physiology and are based on the development of experimental devices, the acquisition and processing of data in "real time" and the development of detection models. Functional neuroimaging is an imaging technique based on the detection of cortical areas associated with cognitive processes. This identification is essential to avoid any cognitive impairments after neurosurgery. Functional MRI (fMRI) is the preoperative gold standard for the identification of functional areas but is rarely used intraoperatively due to the brain shift phenomenon. The intraoperative gold standard is the electrical brain stimulation. However this technique suffers from limitations: the measurements are punctual and could trigger epilepsy seizures. In order to respond to these issues, our contributions in the field of spectral optical imaging are : - The development of an experimental setup for real-time data acquisition and processing ; - The development of a method for optimizing an experimental setup based on the simulation of the hemodynamic and metabolic responses in the brain. - The development of robust models for the intraoperative identification activated cortical areas validated a clinical study carried out on 3 patients In medical imaging, endoscopy is an optical method of visualizing tissue in a cavity. Most endoscopes used in clinical or preclinical studies acquire color images obtained with a continuous wave white light illumination. Recently, multispectral endoscopes, such as NBI endoscopes, have led to an improvement in image contrast for the characterization and even detection of pathological tissues. These devices have the ability to collect tissue surface information more related to local blood volume than to its oxygenation. Moreover these devices do not allow binary identification of tissue lesions, indeed, the identification is dependent to the visual perception of the endoscope users. Our contribution consisted in developing a prototype multispectral endoscope in the visible and near infrared range for real-time data acquisition and processing. Data processing are based on a simple model for the extraction of semi-quantitative parameters., L'imagerie optique spectrale désigne la formation d'images pour plusieurs domaines du spectre lumineux. Cette technique peut être appliquée aux tissus biologiques dans le but de quantifier des marqueurs intrinsèques. Le contexte biomédical de cette thèse est la neuroimagerie fonctionnelle interventionnelle d'une part et la détection de pathologies tissulaires lors d'explorations endoscopiques d'autre part (preuve de concept pour l'exploration endoscopique du côlon du modèle murin). Pour les deux organes visés, les applications ont pour objectif d'estimer les variations spatio-temporelles de l'oxygénation tissulaire. Ces mesures sont guidées par la physiologie et reposent sur l'élaboration de dispositifs expérimentaux, l'acquisition et le traitement de données en temps réel et le développement de modèles de détection. La neuroimagerie fonctionnelle est une technique d'imagerie basée sur la détection des zones du cortex qui sont le siège de processus cognitifs. Cette détection est primordiale afin d'éviter tout déficit cognitif à la suite d'une opération de neurochirurgie. Si l'IRM fonctionnelle (IRMf ) est à cette fin la technique de référence pré-opératoire, elle souffre de biais de localisation pendant l'intervention chirurgicale ("brain shift"). Pendant la neurochirurgie la technique de référence est la stimulation électrique. Cette technique comporte cependant les inconvénients de fournir des informations ponctuelles et de présenter un risque de déclenchement de crises d'épilepsie. Afin de surmonter les insuffisances des techniques de références actuelles, nos contributions dans le cadre de l'imagerie optique spectrale sont : - Le développement d'un dispositif expérimental pour l'acquisition et le traitement temps réel de données ; - Le développement d'une méthode d'optimisation d'un dispositif expérimental basé sur la simulation de la réponse hémodynamique et métabolique cérébrale ; - Le développement de modèles robustes pour la détection per-opératoire de la fonctionnalité cérébrale validé dans un cadre clinique sur trois patients L'endoscopie désigne en imagerie médicale une méthode optique de visualisation des tissus dans une cavité. La plupart des endoscopes utilisés dans un contexte clinique ou préclinique délivrent des ux d'images "en couleurs" obtenues avec une illumination continue en lumière blanche. Récemment, les endoscopes multispectraux (NBI notamment) ont permis une amélioration du contraste des images pour la caractérisation, voire la détection de pathologies tissulaires. Ces dispositifs d'inspection donnent accès à une information sur la surface tissulaire, davantage liée au volume sanguin local qu'à son oxygénation, et ne permettant pas in fine une identification binaire des lésions tissulaires (la détection des anomalies tissulaires est dépendante de l'appréciation visuelle du praticien). Notre contribution a consisté à développer un prototype d'endoscope multispectral dans le visible et le proche infrarouge, pour l'acquisition et le traitement en temps réel des données. Le traitement des données est basé sur un modèle simple d'extraction de paramètres semi-quantitatifs.
