Bacterial contamination of urines, postoperatively, may be due to an endogenous or exogenous cause. In urology, a significant proportion of UTI are of endogenous origin: urethral meatus, urethral stenosis, adenomatous prostatic tissue, vesical tumor, urinary calculi, and endo-ureteral prosthesis. Most cases of bacteriuria associated to catheter use are asymptomatic. To prevent this bacteriuria, the catheter should be used for a strictly limited time, because the rate of bacteriuria is directly proportional to drainage time. Pubic, silicon vesical, or hydrogel coated catheters should be used, whenever possible, in general and gynecological surgery. Antibacterial coated catheters and antiseptic irrigation are not currently recommended. Antibioprophylaxis must be used only for procedures presenting a high or severe infectious risk, according to antibiotherapy protocols determined by medicosurgical teams, according to the bacterial ecology in the ward. These protocols must be posted in the operating room and in the room dedicated to nursing care. The most currently used antibiotics are 2nd and 3rd generation cephalosporins and fluoroquinolones. This antibioprophylaxis must be given as a single dose immediately before or during surgery, in short protocols. It decreases the frequency of per- and postoperative bacteremia, in infectious postoperative syndromes and in immediate postoperative bacteriuria. It is not documented as decreasing secondary bacteriuria, the morbidity and spontaneous disparition of which remain to be assessed. This study may lead to redefining the term nosocomial urinary tract “infection”: asymptomatic bacteriuria is not an infection when it is only a colonization of the urinary tract. Out-of-hospital cardiac arrest occurs most often at home and often in the presence of family members of the patient who witness the event. Cardiopulmonary resuscitation (CPR) training of the next of kin of at-risk patients is thus potentially beneficial. The aim of our study was to document the prevalence of appropriate training among cardiac patients' cohabitants, as well as the motivations or obstacles to seeking training. 153 cohabitants of 127 patients who were hospitalized 1 year prior for confirmed coronary disease in a cardiology department (Paris, France) were interviewed using a structured questionnaire between October 2013 and March 2014. 38 % of interrogated cohabitants had received CPR training, and in two-thirds of the cases, their training was undertaken prior to the onset of the patient's heart disease. The training received was often a single instruction session. Half took place more than 5 years prior to the interview. For two-thirds of interrogated families, the reasons they sought training were related to professional or military duties. Training undertaken solely due to cohabitation with a patient affected by coronary disease represented only 3.5 % of the trained respondents. A lack of information regarding existing training programs and a lack of concrete propositions were given as the main barriers to seeking training. The families of patients who are at-risk for cardiac arrests that were interrogated in our study are inadequately trained in CPR. The creation of dedicated training programs at cardiac rehabilitation services for patients' next of kin or the use of alternative methods such as self-instruction kits could potentially remedy this situation.

We characterized antibiotic resistance and virulence of uropathogenic Escherichia coli (UPEC) strains isolated from urinary tract infections (UTIs) in patients hospitalized in urology departments. A prospective multicentre study was initiated from March 2009 and lasted until February 2010 in French urology units. All patients with asymptomatic bacteriuria (ABU), acute cystitis, acute pyelonephritis or acute prostatitis in whom UPEC was detected were included. Antimicrobial resistance and virulence factors were compared among the different groups. To identify independent associations between virulence markers and the risk of UTI, we used a multivariate logistic regression. We included 210 patients (mean age: 65.8 years; 106 female). Episode of UTI was community acquired in 72.4 %. ABU was diagnosed in 67 cases (31.9 %), cystitis in 52 cases (24.7 %), pyelonephritis in 35 cases (16.7 %) and prostatitis in 56 cases (26.7 %). ABU was more frequent in patients with a urinary catheter (76.1 vs 23.9 %, P<0.001). The resistance rate was 7.6 and 24.8 % for cefotaxime and ciprofloxacin, respectively. UPEC isolated from infections belonged more frequently to phylotypes B2 and D (P =0.07). The papG allele II and papA, papC, papE, kpsMTII and iutA genes were significantly more frequent in infecting strains (P<0.05). In multivariate analysis, strains susceptible to ciprofloxacin were significantly associated with papG allele II (P=0.007), kpsMTK1 (P<0.001) and hlyA (P<0.001) compared with the ciprofloxacin-resistant strains. To the best of our knowledge, this is the first study evaluating the antibiotic resistance and virulence features of UPEC isolated from patients hospitalized in urology departments. High resistance rates were observed, notably for ciprofloxacin, highlighting the importance of a reinforced surveillance in this setting.
