Context: Femoroacetabular impingement syndrome (FAIS) is a painfully debilitating hip condition disproportionately affecting active individuals. Mental health disorders are an important determinant of treatment outcomes for individuals with FAIS. Self-efficacy, kinesiophobia, and pain catastrophizing are psychosocial factors that have been linked to inferior outcomes for a variety of orthopedic conditions. However, these psychosocial factors and their relationships with mental health disorders, pain, and function have not been examined in individuals with FAIS. Objective: (1) To examine relationships between self-efficacy, kinesiophobia, pain catastrophizing, pain, and function in patients with FAIS and (2) to determine if these variables differ between patients with and without a self-reported depression and/or anxiety. Design: Cross-sectional. Setting: University health center. Participants: Fifty-one individuals with FAIS (42 females/9 males; age 35.7 [11.6] y; body mass index 27.1 [4.9] kg/m2). Main Outcome Measures: Participants completed the Pain Self-Efficacy Questionnaire, Tampa Scale for Kinesiophobia, Pain Catastrophizing Scale, visual analog scale for hip pain at rest and during activity, and the 12-item International Hip Outcome Tool. Self-reported depression and/or anxiety were recorded. The relationships between psychosocial factors, pain, and function were examined using Spearman rank-order correlations. Independent t tests and Mann–Whitney U tests were used to evaluate the effect of self-reported depression and/or anxiety on psychosocial factors, pain and function. Results: The 12-item International Hip Outcome Tool was correlated with pain during activity (ρ = −.57, P ≤ .001), Tampa Scale for Kinesiophobia (ρ = −.52, P ≤ .001), and Pain Self-Efficacy Questionnaire (ρ = .71, P ≤ .001). The Pain Self-Efficacy Questionnaire was also correlated with pain at rest (ρ = −.43, P = .002) and pain during activity (ρ = −.46, P = .001). Individuals with self-reported depression and/or anxiety (18/51; 35.3%) had worse self-efficacy and pain catastrophizing (P ≤ .01). Conclusion: Self-reported depression and/or anxiety, low self-efficacy, and high kinesiophobia were associated with more hip pain and worse function for patients with FAIS. These findings warrant further examination including psychosocial treatment strategies to improve the likelihood of a successful clinical outcome for this at-risk population.

Context: Postoperative quadriceps strength weakness after knee surgery is a persistent issue patients and health care providers encounter. Objective: To investigate the effect of neuromuscular electrical stimulation (NMES) parameters on quadriceps strength after knee surgery. Data Sources: CINAHL, MEDLINE, SPORTDiscus, and PubMed were systematically searched in December 2018. Study Selection: Studies were excluded if they did not assess quadriceps strength or if they failed to report the NMES parameters or quadriceps strength values. Additionally, studies that applied NMES to numerous muscle groups or simultaneously with other modalities/treatments were excluded. Study quality was assessed with the Physiotherapy Evidence Database (PEDro) scale for randomized controlled trials. Study Design: Systematic review. Level of Evidence: Level 1. Data Extraction: Treatment parameters for each NMES treatment was extracted for comparison. Quadriceps strength means and standard deviations were extracted and utilized to calculate Hedge g effect sizes with 95% CIs. Results: Eight RCTs were included with an average Physiotherapy Evidence Database scale score of 5 ± 2. Hedge g effect sizes ranged from small (−0.37; 95% CI, −1.00 to 0.25) to large (1.13; 95% CI, 0.49 to 1.77). Based on the Strength of Recommendation Taxonomy Quality of Evidence table, the majority of the studies included were low quality RCTs categorized as level 2: limited quality patient-oriented evidence. Conclusion: Because of inconsistent evidence among studies, grade B evidence exists to support the use of NMES to aid in the recovery of quadriceps strength after knee surgery. Based on the parameters utilized by studies demonstrating optimal treatment effects, it is recommended to implement NMES treatment during the first 2 postoperative weeks at a frequency of ≥50 Hz, at maximum tolerable intensity, with a biphasic current, with large electrodes and a duty cycle ratio of 1:2 to 1:3 (2- to 3-second ramp).

To compare ankle, knee and hip isometric peak torque between young and middle-aged adults with CAI, copers and un-injured controls.Cross-sectional.Research Laboratory.One hundred fifty-six young and middle-aged adults with or without CAI volunteered.A handheld dynamometer measured isometric dorsiflexion, plantarflexion, knee extension, hip extension and hip abduction peak force during a 5 s trial. Average peak torque was calculated and normalized to body mass.A significant Age by Injury interaction for dorsiflexion suggest middle-aged un-injured controls (p 0.001) and copers (p 0.001) had lower isometric peak torque compared to their young adult counterparts, but there were no differences between young and middle-aged adults with CAI (p 0.05). Significant Injury main effects suggest the CAI group had decreased plantarflexion (p = 0.004) and hip extension (p = 0.010) strength compared to un-injured controls, but not copers (p 0.05). Significant Age main effects for all primary outcome measures were observed, indicating peak torque decreased with age (p 0.05).Regardless of age, isometric ankle and hip peak torque was lower in participants with CAI compared to un-injured controls, but not copers. These findings provide further evidence towards the impact of CAI in both young and middle-aged adults.

Context Low mental health scores, depression, and anxiety are associated with worse pain and functional outcomes after hip arthroscopy for patients with femoroacetabular impingement syndrome (FAIS). Preoperative screening of psychosocial factors such as self-efficacy, kinesiophobia, and pain catastrophizing may further aid in identifying patients at risk for poorer outcomes. Objective To compare preoperative function and psychosocial factors between patients with and those without elevated postoperative pain 3 months after hip arthroscopy for FAIS. Design Prospective cohort study. Setting University health center. Patients or Other Participants Fifty-one individuals with FAIS (41 women, 10 men; age = 37.6 ± 12.3 years, body mass index = 27.1 ± 4.1 kg/m2). Main Outcome Measure(s) Preoperatively, participants completed the Pain Self-Efficacy Questionnaire (PSEQ), Tampa Scale for Kinesiophobia (TSK), Pain Catastrophizing Scale (PCS), 12-Item International Hip Outcome Tool (iHOT-12), and a 10-point visual analog scale (VAS) for hip pain at rest and during activity. Three months postoperatively, they completed the PSEQ, TSK, PCS, iHOT-12, and VAS. Preoperative scores were compared between those with (VAS ≥ 3) and those without (VAS < 3) elevated postoperative pain using Mann-Whitney U tests, and odds ratios were calculated to determine the odds of having elevated postoperative pain and lower postoperative function. Results Participants with elevated postoperative pain at rest and during activity had worse preoperative psychosocial health (P ≤ .04). Those with elevated postoperative pain at rest had worse preoperative function (P < .001). The odds of having elevated postoperative pain at rest were 45 times and 4.5 times higher for those with low self-efficacy and high pain catastrophizing, respectively (P values ≤ .03). The odds of having elevated postoperative pain during activity were 7.1, 6.2, and 3.5 times higher for those with low self-efficacy, high kinesiophobia, and high pain catastrophizing, respectively (P values ≤ .04). The odds of having lower postoperative iHOT-12 scores were 7.5 and 14.0 times higher for those with high kinesiophobia and pain catastrophizing, respectively (P values ≤ .03). Conclusions Poor preoperative psychosocial health increased the odds of elevated pain and worse function 3 months after hip arthroscopy for FAIS. This is a first step in identifying the psychosocial factors that may contribute to poorer long-term hip arthroscopy outcomes.

The purpose of this study was to determine if there are differences in mechanical and physiological demand between live and simulated racing. Fifteen male professional jockeys (31.8±9.5 years, 160.78±7.62 cm, 51.2±1.5 kg) were instrumented with inertial measurement units (IMUs) placed on their helmets to measure mechanical stress and a heart rate (HR) monitor. Head accelerations and HR were measured during a live race (between 5.5 and 6.5 furlongs) and a simulated race (equivalent of 6.5 furlongs or 1.5 min) on a race horse simulator. BACKGROUND: Lower extremity injuries in soccer players are extremely common. Implementation of group injury prevention programs has gained popularity due to time and cost-effectiveness. Unfortunately, players with greater number of risk factors are most likely to sustain an injury, yet less likely to benefit from a group injury prevention program. The purpose of this study was to determine if targeting these high risk players with one-on-one treatment would result in a reduction in the number of risk factors they possess. The authors hypothesized that fifty percent or more of subjects receiving one-on-one intervention would have a reduction of ≥1 risk factor(s). STUDY DESIGN: Quasi-experimental pretest-posttest design. METHODS: Division I men's and women's soccer players were screened for modifiable risk factors using a battery of tests which assessed mobility, fundamental movement pattern performance, motor control, and pain. Players with ≥ 3 risk factors (“high risk”) received one-on-one treatment from a physical therapist via an algorithm twice per week for four weeks. Players with

Context: During thoroughbred races, jockeys are placed in potentially injurious situations, often with inadequate safety equipment. Jockeys frequently sustain head injuries; therefore, it is important that they wear appropriately certified helmets. Objective: The goals of this study are (1) to perform impact attenuation testing according to ASTM F1163-15 on a sample of equestrian helmets commonly used by jockeys in the United States and (2) to quantify headform acceleration and residual crush after repeat impacts at the same location. Participants and Design: Seven helmet models underwent impact attenuation testing according to ASTM F1163-15. A second sample of each helmet model underwent repeat impacts at the crown location for a total of 4 impacts. Setting: Laboratory. Intervention: Each helmet was impacted against a flat and equestrian hazard anvil. Main Outcome Measures: Headform acceleration was recorded during all impact and computed tomography scans were performed preimpact and after impacts 1 and 4 on the crown to quantify liner thickness. Results: Four helmets had 1 impact that exceeded the limit of 300g. During the repeated crown impacts, acceleration remained below 300g for the first and second impacts for all helmets, while only one helmet remained below 300g for all impacts. Foam liner thickness was reduced between 5% and 39% after the first crown impact and between 33% and 70% after the fourth crown impact. Conclusions: All riders should wear a certified helmet and replace it after sustaining a head impact. Following an impact, expanded polystyrene liners compress, and their ability to attenuate head acceleration during subsequent impacts to the same location is reduced. Replacing an impacted helmet may reduce a rider’s head injury risk.

Objective: Our aim was to investigate the effect of body mass index (BMI) levels on quadriceps and hamstring strength and functional outcomes up to 6 months after anterior cruciate ligament reconstruction (ACLR) with hamstring tendon autograft (HTG). Design: Prospective, controlled study. Setting: University clinical laboratory. Patients: Ninety-one participants who had undergone unilateral ACLR with HTG were divided into 2 groups according to their BMI. The participants whose BMI were between 18.5 and 24.9 kg/m(2) were included in group 1 (n = 50, age: 27.2 +/- 6.7 years, BMI: 22.3 +/- 1.6 kg/m(2)) and those whose BMI > 24.9 kg/m(2) were included in group 2 (n = 41, age: 30.2 +/- 6.9 years, BMI: 28.0 +/- 2.4 kg/m(2)). Interventions: Quadriceps and hamstring strength, functional performance including hop, jump, and balance performance, and IKDC score. Main Outcome Measures: Maximum voluntary isometric contraction of the quadriceps and hamstring muscles and the body mass were measured at 1, 3, and 6 months after surgery. Absolute peak torques and normalized peak torques to body weight for both limbs were recorded. Functional outcomes were evaluated at 6 months after surgery. Results: Normalized quadriceps strength improvement was lower in group 2 when compared with group 1 (F-(2,F-178) = 6.23, P = 0.003). Group 2 also demonstrated lower scores in functional performance (P < 0.05). Normalized hamstring and absolute quadriceps and hamstring strength improvement was not affected by higher BMI level (P > 0.05). Conclusions: Higher BMI levels adversely affect quadriceps strength capacity and performance in patients who have undergone ACLR with HTG. Clinicians should consider BMI levels of patients when assessing and targeting muscle recovery because it could negatively affect the success of the ACLR rehabilitation.

Context People with chronic ankle instability (CAI) display a lower regional and global health-related quality of life (HRQoL). Examinations of HRQoL outcomes associated with CAI have addressed younger adults, restricting our understanding of the long-term consequences of CAI. Objective To compare ankle regional and global HRQoL in middle-aged participants with and those without CAI. Design Cross-sectional study. Setting Laboratory. Patients or Other Participants A total of 59 middle-aged volunteers, consisting of 18 with CAI (age = 50.2 ± 9.3 years), 17 who were ankle-sprain copers (age = 54.5 ± 8.7 years), and 24 uninjured controls (age = 56.7 ± 10.0 years). Main Outcome Measure(s) Participants completed the Foot and Ankle Disability Index (FADI) and the Patient-Reported Outcomes Measurement Information System Adult Profile. Regional HRQoL was assessed using the FADI Activities of Daily Living and Sport subscales. Global HRQoL was measured using the 43-item Patient-Reported Outcomes Measurement Information System Adult Profile, which contains 7 short forms—Physical Function, Pain Interference, Fatigue, Depression, Sleep Disturbance, Anxiety, and Ability to Participate in Social Roles and Activities. Separate Kruskal-Wallis tests were used to determine between-groups differences. Results Middle-aged participants with CAI had lower scores on both subscales of the FADI than the coper and control groups (all P values Conclusions Middle-aged participants with CAI displayed worse ankle regional and global HRQoL than their age-matched healthy counterparts and copers. These results demonstrated that CAI can affect HRQoL outcomes in middle-aged adults.

Context: Return-to-sport criteria after anterior cruciate ligament (ACL) injury are often based on “satisfactory” functional and patient-reported outcomes. However, an individual's decision to return to sport is likely multifactorial; psychological and physical readiness to return may not be synonymous. Objective: To determine the psychosocial factors that influence the decision to return to sport in athletes 1 year post–ACL reconstruction (ACLR). Design: Qualitative study. Setting: Academic medical center. Patients or Other Participants: Twelve participants (6 males, 6 females) were purposefully chosen from a large cohort. Participants were a minimum of 1-year postsurgery and had been active in competitive athletics preinjury. Data Collection and Analysis: Data were collected via semistructured interviews. Qualitative analysis using a descriptive phenomenologic process, horizontalization, was used to derive categories and themes that represented the data. The dynamic-biopsychosocial model was used as a theoretical framework to guide this study. Results: Six predominant themes emerged that described the participants' experiences after ACLR: (1) hesitation and lack of confidence led to self-limiting tendencies, (2) awareness was heightened after ACLR, (3) expectations and assumptions about the recovery process influenced the decision to return to sport after ACLR, (4) coming to terms with ACL injury led to a reprioritization, (5) athletic participation helped reinforce intrinsic personal characteristics, and (6) having a strong support system both in and out of rehabilitation was a key factor in building a patient's confidence. We placed themes into components of the dynamic-biopsychosocial model to better understand how they influenced the return to sport. Conclusions: After ACLR, the decision to return to sport was largely influenced by psychosocial factors. Factors including hesitancy, lack of confidence, and fear of reinjury are directly related to knee function and have the potential to be addressed in the rehabilitation setting. Other factors, such as changes in priorities or expectations, may be independent of physical function but remain relevant to the patient-clinician relationship and should be considered during postoperative rehabilitation.

Context: Patient expectations have been shown to be a major predictor of outcomes. Fulfilled expectations have been linked to increased patient satisfaction and rehabilitation adherence. Expectations may be influenced by a variety of factors, including patient characteristics, preoperative function, or disease characteristics. It is currently unknown what factors may influence patient expectations prior to cartilage repair of the knee, and to what degree. Furthermore, understanding the importance and values of those expectations for recovery using mixed methods has not previously been conducted in this patient population. The purpose of this mixed methods study is to examine and explore the relationships between patient expectations and functional outcome in patients undergoing cartilage repair of the knee. Design: A mixed methods design was used. Methods: Twenty-one patients scheduled to undergo cartilage repair of the knee were included. Participants completed the Hospital for Special Surgery Knee Surgery Expectations Survey and the Knee Injury and Osteoarthritis Outcome Score at their preoperative visit. Knee Injury and Osteoarthritis Outcome Scores were also obtained at 3 and 6 months postsurgery. A selected sample of 6 participants participated in semi-structured interviews 6 months postsurgery. Pearson correlation coefficients were used to determine relationships between expectations and functional outcome. Results: Patients have moderate expectations for recovery, and these expectations were positively associated with preoperative pain, activities of daily living, and quality of life. Expectations also correlated with symptoms 3 months postsurgery, but there were no other significant correlations between preoperative expectations and postoperative function in the short term. Four qualitative themes emerged as participants described how previous recovery experiences shaped their expectations. Conclusions: Formalized patient and caregiver education, prehabilitation, and the use of psychological skills during rehabilitation may help to manage patient expectations and provide more focused and individualized care, thus improving outcomes.

The purposes of this study were to determine the prevalence of anxiety and depression in patients undergoing primary hip arthroscopy for femoroacetabular impingement syndrome (FAIS) and to compare duration of symptoms (DOS), hip morphology and self-reported pain and function between patients with and without anxiety or depression. From our Institutional Review Board approved registry, we identified 127 consecutive participants [94F/33M; age = 35.2 ± 12.1 years; body mass index (BMI) = 26.8 ± 5.6 kg/m2] scheduled for primary hip arthroscopy for FAIS. Anxiety and depression were recorded based on medical chart diagnoses or anti-depressive/anti-anxiety prescriptions. Alpha angle, lateral center edge angle and DOS were recorded, and the Hip Disability and Osteoarthritis Outcome Score (HOOS) was completed. Groups were compared using Mann–Whitney U tests, and categorical variables were compared using Fisher’s exact tests. Overall, 45 of 127 participants (35.4%) (94F/33M; age = 35.2 ± 12.1 years; BMI = 26.8 ± 5.6 kg/m2) had anxiety or depression. DOS and bony morphology did not differ between groups (P = 0.11–0.21). Participants with anxiety or depression were older (P = 0.03) and had worse HOOS QOL (21.4 ± 18.4; 30.7 ± 19.6; P = 0.006; 95% CI 2.4–16.3) and HOOS sport (25.6 ± 17.8, 34 ± 21.1, P = 0.03, 95% CI 1.4–15.4). More than one third of participants presented with anxiety or depression. Self-reported quality of life and function were worse for these participants, but neither symptom chronicity nor bony morphology differed. Concomitant anxiety or depression should be considered when counseling patients with FAIS. Adjunct interventions as well genetic and/or developmental factors that may contribute to the high prevalence should be examined.

Context: The recovery process after autologous chondrocyte implantation (ACI) can be challenging for patients and clinicians alike due to significant functional limitations and a lengthy healing time. Understanding patients' experiences during the recovery process may assist clinicians in providing more individualized care. Objective: To explore and describe patients' experiences during the recovery process after ACI. Design: Qualitative study. Setting: Orthopaedic clinic. Patients or Other Participants: Participants from a single orthopaedic practice who had undergone ACI within the previous 12 months were purposefully selected. Data Collection and Analysis: Volunteers participated in 1-on-1 semistructured interviews to describe their recovery experiences after ACI. Data were analyzed using the process of horizontalization. Results: Seven patients (2 men, 5 women; age = 40.7 ± 7.5 years, time from surgery = 8.7 ± 4.2 months) participated. Four themes and 6 subthemes emerged from the data and suggested that the recovery process is a lengthy and emotional experience. Therapy provides optimism for the future but requires a collaborative effort among the patient, surgeon, rehabilitation provider, and patient's caregiver(s). Furthermore, patients expressed frustration that their expectations for recovery did not match the reality of the process, including greater dependence on caregivers than expected. Conclusions: Patients' expectations should be elicited before surgery and managed throughout the recovery process. Providing preoperative patient and caregiver education and encouraging preoperative rehabilitation can assist in managing expectations. Establishing realistic goals and expectations may improve rehabilitation adherence, encourage optimism for recovery, and improve outcomes in the long term.

The purpose of this study was to compare dynamic balance and weight-bearing dorsiflexion range of motion between young and middle-aged adults with and without chronic ankle instability.Cross-sectional.One hundred and two young adults were classified as either having chronic ankle instability (n = 38), coper (n = 27) or a healthy-control (n = 37). A total of 55 middle-aged adults were identified as having chronic ankle instability (n = 16), coper (n = 15) or a healthy-control (n = 24). Participants completed the weight-bearing lunge test and the anterior, posteromedial and posterolateral reach directions of the star excursion balance test.Middle-aged adults had shorter reach distances for each direction of the star excursion balance test compared to the young adults (p 0.001). Regardless of age, participants with chronic ankle instability performed worse on the star excursion balance test compared to the coper (p 0.05) and healthy-control group (p 0.05). Similarly, participants with chronic ankle instability had less dorsiflexion compared to healthy-controls (p 0.05), but not the coper group (p 0.05).Deficits in dynamic postural control do not continue to decline with advancing age in individuals with chronic ankle instability, however, these impairments appear to continue to persist compared to uninjured controls.

Background: Pain catastrophizing predicts poor outcomes following orthopedic procedures for patients with chronic conditions; however, limited research has focused on acute injuries. This study aimed to quantify the progression of Pain Catastrophizing Scale (PCS) scores from injury to 6-months post–anterior cruciate ligament reconstruction (ACLR) and determine if preoperative or 6-month PCS scores were related with self-reported pain or function 6 months post-ACLR. The authors hypothesized PCS scores would minimally fluctuate and would be related with worse outcomes 6-months post-ACLR. Methods: All 48 participants (27 male/21 female; aged 22.7 [4.6] y, body mass index 24 [3.3]) included in this secondary analysis of a randomized control trial sustained an ACL injury during sports activity. Participants completed the PCS and Pain Visual Analog Scale (VAS) at 5 time points: within 7 days of injury (INITIAL), day of surgery, 2 weeks postoperative (2W), 6 weeks postoperative (6W), and 6 months postoperative (6M). They completed the Knee Injury and Osteoarthritis Outcome Score (KOOS) at 6M. Wilcoxon signed-rank tests and Spearman rank-order correlations were used for analysis. Results: PCS scores were not fixed (INITIAL: 11.6 [10.8], day of surgery: 2.5 [3.7], 2W: 8.0 [7.8], 6W: 3.7 [6], 6M: 0.8 [2.3]). They fluctuated in response to injury and ACLR similar to Pain VAS scores. Preoperative PCS scores were not related with 6M outcomes; however, 6M PCS scores were significantly related with 6M Pain VAS and KOOS subscales. Conclusions: PCS scores fluctuated in response to injury and ACLR. Preoperative PCS scores were not related with 6M outcomes; however, 6M PCS scores correlated with pain and function at 6M. High pain catastrophizing appears to be a natural response immediately following acute ACL injury and ACLR, but may not be indicative of a poor postoperative result. PCS scores 6-months following ACLR may provide useful information regarding self-reported pain and function.

The impact that visual perturbation has on upright postural stability in an athlete with a concussion has not been established. The present study aimed to characterize the influence that visual perturbation stimuli have on upright balance among athletes with acute concussions.A 2X2X2 repeated measure designed was used.The present study examined the influence visual perturbation has on individuals suffering from an acute concussion. Fourteen participants (7 with a concussion and 7 matched controls) underwent various balance assessments with and without visual perturbation.Overall, athletes with acute concussions demonstrated impairments in balance 24-48 hours following a concussion. However, when assessed using a visual perturbation task, athletes with acute concussions demonstrated improved balance, while control subjects did not show any significant changes during the same visual perturbation task.An athlete's ability to disregard visual perturbation stimuli is imperative for successful participation in sports. Due to the observed alterations in balance when given a visual perturbation task, it is suggested that athletes with acute concussions place more attention on the balance task and may disregard other less meaningful tasks.

Context: Anterior cruciate ligament (ACL) reconstruction (ACLR) is the most commonly used method for helping athletes regain function and return to preinjury activity levels after ACL injury. Outcomes after ACLR have suggested that athletes return to a level of function that would support a return to sport participation; however, in a recent meta-analysis, pooled return rates were only 55%. It is unclear whether this discrepancy is a result of functional impairments.Objective: To compare patient-reported outcomes (PROs), dynamic balance, dynamic functional performance, strength, and muscular endurance in athletes who returned to sport (RTS) and athletes who did not return to sport (NRTS) after ACLR.Design: Case-control study.Setting: University research laboratory.Patients or Other Participants: Two groups of participants with primary unilateral ACLR: 18 RTS individuals (7 males, 11 females; age = 23 ± 11 years, height = 163.58 ± 40.41 cm, mass = 70.00 ± 21.75 kg, time since surgery = 4.02 ± 3.20 years) and 12 NRTS individuals (5 males, 7 females; age = 26 ± 13 years, height = 171.33 ± 48.24 cm, mass = 72.00 ± 21.81 kg, time since surgery = 3.68 ± 2.71 years).Intervention(s): The PROs consisted of the International Knee Documentation Committee Subjective Knee Evaluation Form, Knee Injury and Osteoarthritis Outcome Score, Tegner Activity Scale, and Marx Activity Scale. Functional performance outcome measures were the anterior and posteromedial reach on the Star Excursion Balance Test, a battery of single-legged–hop tests, isokinetic quadriceps and hamstrings strength at 60°/s and 180°/s, and a novel step-down-to-fatigue test. All measures were taken during a single laboratory session.Main Outcome Measure(s): The Limb Symmetry Index was calculated for all functional performance measures. Mann-Whitney U tests were used to compare measures between groups (P < .10).Results: Compared with the RTS group, the NRTS group had lower scores on the International Knee Documentation Committee Subjective Knee Evaluation Form (RTS median = 92.52, range = 66.67–97.70; NRTS median = 82.76, range = 63.22–96.55; P = .03) and Knee Injury and Osteoarthritis Outcome Score Symptoms subscale (RTS median = 88, range = 54–100; NRTS median = 71, range = 54–100; P = .08). No differences were observed for any functional performance measures.Conclusions: The NRTS athletes displayed lower PROs despite demonstrating similar function on a variety of physical performance measures. These results further support existing evidence that physical performance alone may not be the ideal postoperative outcome measure. Measures of patients' symptoms and self-perceived physical function may also greatly influence postoperative activity choices.

Context:There is lack of information related to quadriceps and hamstring strength recovery during the early period of rehabilitation after anterior cruciate ligament reconstruction (ACLR) using hamstring-tendon graft (HTG).Objective:To investigate quadriceps and hamstring isometric strength at 4-, 8-, and 12-wk time points after ACLR and to document the strength changes of these muscles over time.Design:Longitudinal study.Participants:24 patients (age 28.1 ± 8.1 y) who underwent unilateral single-bundle anatomic ACLR with 4-strand semitendinosus and gracilis tendon graft.Main Outcome Measures:The isometric strength of quadriceps and hamstring muscles was measured on an isokinetic dynamometer at a 60° knee-flexion angle 4, 8, and 12 wk after surgery.Results:Quadriceps and hamstring strength significantly increased over time for both the involved limb (quadriceps F2,46 = 58.3, P < .001; hamstring F2,46 = 35.7, P < .001) and uninvolved limb (quadriceps F2,46 = 17.9, P < .001; hamstring F2,46 = 56.9, P = .001). Quadriceps and hamstring indexes significantly changed from 4 wk (QI 57.9, HI 54.4) to 8 wk (QI 78.8, HI 69.9) and from 8 wk to 12 wk (QI 82, HI 75.7) (P < .001); however, there was no difference between indexes at the 12-wk time point (P = .17).Conclusions:The results of this study serve as a reference for clinicians while directing a rehabilitation protocol for HTG ACLR patients to better appreciate expected strength changes of the muscles in the early phase of recovery.

Background: Recent data from the Danish anterior cruciate ligament (ACL) registry demonstrated increased reoperation rates for hamstring tendon autografts when an anatomic ACL reconstruction is performed. This is consistent with reports of greater time needed for hamstring tendon autografts to mature compared with other autografts. Purpose: To review the literature comparing graft failure rate between patellar and hamstring tendon autografts placed anatomically and to determine if there are differences in return to preinjury activity levels between autografts. Study Design: Systematic review with meta-analysis and meta-regression. Methods: The PubMed, MEDLINE, SPORTDiscus, and CINAHL databases were used to identify studies published from January 1, 2000, through March 7, 2014. To compare postoperative outcomes between patellar tendon and hamstring tendon autografts, summary event rates for graft failure and return to preinjury activity level were calculated. A meta-analysis was performed to calculate a summary odds ratio (OR) for graft failure between autografts using the studies that directly compared the 2 autografts. Meta-regression analyses were performed to assess the influence of postoperative follow-up time on graft failure rate. Results: A total of 28 studies reported graft failures for patellar tendon (6 studies) and hamstring tendon (26 studies) autografts used with anatomic ACL reconstruction; 4 of the 28 were comparison studies. Graft failure rate was not significantly different between patellar tendon (7.0% [95% CI, 4.6%-10.5%]) and hamstring tendon autografts (3.9% [95% CI, 2.7%-5.6%]). The odds of graft failure were slightly higher for hamstring tendon autografts (OR, 1.21 [95% CI, 0.63-2.33]), but this difference was not significant ( P = .57). The rate of patients returning to preinjury activity levels was not significantly different between patellar (n = 1 study; 58.1% [95% CI, 40.4%-73.9%]) and hamstring tendon autografts (n = 5 studies; 75.6% [95% CI, 43.7%-92.5%]). Overall graft failure rate was positively associated with postoperative follow-up time, but this effect was only significant with hamstring tendon autografts ( P < .05). Conclusion: Differences in graft failure rate between patellar tendon and hamstring tendon autografts were not significant. Although follow-up time was only found to have a significant influence on hamstring tendon graft failure rates, this was likely due to the smaller sample of studies assessing patellar tendon graft failures. Differences in return to preinjury activity levels could not be determined due to the lack of studies assessing that outcome. Both patellar and hamstring tendon autografts demonstrate a low risk of failure and moderately high return to activity level after anatomic ACL reconstruction.

Objectives: Autologous chondrocyte implantation (ACI) is a well established treatment for articular cartilage defects of the knee. However, few studies have examined return-to-work rates among ACI patients, and even fewer have investigated the effect of mental health on postoperative outcomes. The purpose of this study was to examine the effect of preoperative mental health on return to work and patient-reported outcomes. Methods: From our IRB-approved prospective outcomes registry, we identified 109 ACI patients (mean age = 34.5 years, mean follow-up = 2.8 years) with pre- and postoperative clinical data. Patients were stratified into those with preoperative VR-12 Mental Component Scores in the bottom quartile (LOW MCS) and in the top 3 quartiles (HIGH MCS) based on normative values. Patients with patellar and/or trochlear lesions were placed in the patellofemoral (PF) group, and all other patients were placed in the tibiofemoral (TF) group. Return to work, IKDC scores, and Lysholm scores were individually compared between the LOW MCS and HIGH MCS groups that had undergone either PF or TF ACI. Fisher’s exact tests were used to compare the return to work rates and frequency of IKDC and Lysholm improvements greater than the minimal clinically important difference. One-way ANOVAs were used to examine magnitude of change of IKDC and Lysholm scores. Results: Preoperatively, 14/55 (25.5%) TF patients and 17/54 (31.5%) PF patients were included in the LOW MCS group. For PF patients, return to work rates were significantly lower for the LOW MCS group (p=.0005). Return to work rates did not differ in the TF group (p>0.99). Changes in IKDC and Lysholm scores did not differ between the LOW MCS and HIGH MCS groups in TF or PF patients (Table 1). Conclusion: Lower preoperative mental component scores were more common in PF ACI patients and were associated with decreased return to work rates in PF ACI patients. The results of this study underscore the need to preoperatively quantify mental health status for this specific patient group in order to set realistic postoperative expectations. Table 1. Objective: To evaluate hip abductor and adductor peak torque outputs and compare their ratios between sexes. Design: A cross-sectional laboratory-controlled study. Setting: Participants visited a laboratory and performed an isokinetic hip abductor and adductor test. All participants performed 2 sets of 5 repetitions of concentric hip abduction and adduction in a standing position at 60 degrees per second. Gravity was determined as a function of joint angle relative to the horizontal plane and was corrected by normalizing the weight of the limb on an individual basis. Participants: A total of 36 collegiate athletes. Independent Variables: Sex (20 females and 16 males). Main Outcome Measures: Bilateral peak hip abductor and adductor torques were measured. The 3 highest peak torque values were averaged for each subject. Results: Independent t tests were used to compare sex differences in hip abductor and adductor peak torques and the abductor:adductor peak torque ratios. Males demonstrated significantly greater hip abductor peak torque compared with females (males 1.29 ± 0.24 Nm/kg, females 1.13 ± 0.20 Nm/kg; P = 0.03). Neither hip adductor peak torque nor their ratios differed between sexes. Conclusions: Sex differences in hip abductor strength were observed. The role of weaker hip abductors in females deserves further attention and may be a factor for higher risk of knee pathologies.

Low back pain (LBP), is a common musculoskeletal problem, affecting 75-85% of adults in their lifetime. Direct costs of LBP in the USA were estimated over 85 billion dollars in 2005 resulting in a significant economic burden for the healthcare system. LBP classification systems and outcome measures are available to guide physical therapy assessments and intervention. However, little is known about which, if any, physical therapists use in clinical practice. The purpose of this study was to identify the use of and barriers to LBP classification systems and outcome measures among physical therapists in one state. A mixed methods study using a cross-sectional cohort design with descriptive qualitative methods was performed. A survey collected both quantitative and qualitative data relevant to classification systems and outcome measures used by physical therapists working with patients with LBP. Physical therapists responded using classification systems designed to direct treatment predominantly. The McKenzie method was the most frequent approach to classify LBP. Barriers to use of classification systems and outcome measures were lack of knowledge, too limiting and time. Classification systems are being used for decision-making in physical therapy practice for patients with LBP. Lack of knowledge and training seems to be the main barrier to the use of classification systems in practice. The Oswestry Disability Index and Numerical Pain Scale were the most commonly used outcome measures. The main barrier to their use was lack of time. Continuing education and reading the literature were identified as important tools to teach evidence-based practice to physical therapists in practice.

Background Neuromuscular alterations have been reported for patients with osteoarthritis of the hip joint; however, the underlying cause associated with altered gluteus medius muscle function has not been examined. This study assessed electromyographic amplitudes of the gluteus medius muscles during function in patients with unilateral end-stage osteoarthritis of the hip joint compared to controls. Methods Patients with unilateral end-stage hip joint osteoarthritis (n = 13) and asymptomatic control participants (n = 17) participated. Average root-mean squared muscle amplitudes represented as a percent of maximum voluntary isometric contraction for both the involved and uninvolved limb gluteus medius muscles were analyzed during step up, step down, and gait. The association between muscle activation and impact forces during stepping tasks was assessed. Findings Patients with hip osteoarthritis exhibited increased gluteus medius muscle electromyographic amplitudes bilaterally during stair ascent, stair descent, and gait compared to controls, regardless of which limb they led. Involved limb muscle activity was inversely related to impact force during step down onto the ipsilateral limb. Interpretation Patients with hip osteoarthritis demonstrated increased gluteus medius muscle activation levels during stepping tasks and gait when compared to controls. The increased activation is most likely a compensatory response to muscle weakness. Therefore, application of strengthening exercises which target the gluteal muscles should assist in neuromuscular control and result in improved strength for patients with hip joint osteoarthritis.

This mixed method study was designed to investigate self-perceptions before and after experiencing an activity that dynamically and statically challenges proximal stability of the pelvis, spine, and trunk. Twenty-eight, healthy Division II female soccer and volleyball collegiate players (17 soccer, 11 volleyball) completed a self-reported Tegner activity scale, pretest questionnaire and posttest interview. A self-perceived numeric rating of the athletes' proximal stability and performance on a functional movement screen (FMS) were recorded. A guided interview was used to examine the self-perceptions of proximal stability after the FMS testing session. Differences and correlations between the pretest and posttest ratings of proximal stability and FMS scores were analyzed using a 1-sample Kolmogorov-Smirnov test and Spearman's rank order correlation test, respectively. Residual standard error from a 1-way analysis of variance was used to explore the association between variables. Qualitative data were recorded and transcribed. There were significant differences between the pretest (3.4 ± 0.63) and posttest ratings (3.1 ± 0.49) of proximal stability (p = 0.01). The relationship between the pretest proximal stability ratings and the FMS scores was low (r = 0.19, p = 0.33), whereas posttest rating and FMS scores had a moderately high (r = 0.68, p = 0.00) correlation. There was a smaller residual standard error for the posttest ratings (1.7) when compared with the pretest ratings (3.2) with the FMS. Four qualitative themes emerged: (a) wanting to do well, (b) expectations of performance, (c) focused mental mindset, and (d) body control. Self-perceptions of proximal stability in female athletes were influenced by undergoing a test that stressed the proximal stabilizers. Combining assessments of self-perceptions and proximal stability may assist clinicians and athletes in targeting components of training.

Objective: To compare the responsiveness of six common patient-reported outcomes (PROs) following autologous chondrocyte implantation (ACI). Design: A systematic search was conducted to identify reports of PROs following ACI. Study quality was evaluated using the modified Coleman Methodology Score (mCMS). For each outcome score, pre- to postoperative paired Hedge’s g effect sizes were calculated with 95% confidence intervals (CIs). Random effects meta-analyses were performed to provide a summary response for each PRO at time points (TP) I (Results: The mean mCMS for the 42 articles included was 50.9 ± 9.2. For all evaluated instruments, none of the mean effect size CIs encompassed zero. The International Knee Documentation Committee Subjective Knee Form (IKDC) had increasing responsiveness over time with TP-IV, demonstrating greater mean effect size [confidence interval] (1.78 [1.33, 2.24]) than TP-I (0.88 [0.69, 1.07]). The Knee Injury and Osteoarthritis Outcome Score–Sports and recreation subscale (KOOS-Sports) was more responsive at TP-III (1.76 [0.87, 2.64]) and TP-IV (0.98 [0.81, 1.15]) than TP-I (0.61 [0.44, 0.78]). Overall, the Medical Outcomes Study 36-Item Short Form Health Survey Physical Component Scale (0.60 [0.46, 0.74]) was least responsive. Both the Lysholm Scale (1.42 [1.14, 1.72]) and the IKDC (1.37 [1.13, 1.62]) appear more responsive than the KOOS-Sports (0.90 [0.73, 1.07]). All other KOOS subscales had overall effect sizes ranging from 0.90 (0.74, 1.22) (Symptoms) to 1.15 (0.76, 1.54) (Quality of Life). Conclusions: All instruments were responsive to improvements in function following ACI. The Lysholm and IKDC were the most responsive instruments across time. IKDC and KOOS-Sports may be more responsive to long-term outcomes, especially among active individuals.

Weakness of the quadriceps is a common occurrence in patients after knee injury or surgery; this weak-ness is due to a natural mechanism known as arthrogenic muscle inhibition. If inhibition of the quadriceps persists, it can become detrimental to a patient’s function and lead to additional pathologies. A number of therapeutic interven-tions have be used in the rehabilitation of these patients, but few have proven to be successful. Electromyographic biofeedback is one modality that has demonstrated positive outcomes in patients by restoring quadriceps function. However, the reason for the effectiveness of this modality has yet to be fully explained in the area of rehabilitation. Neuroplasticity is a phenomenon that has gained much attention in rehabilitation, and its potential continues to grow. After an injury, the brain has the ability to enhance recovery by strengthening its neural circuitry. Through rehabilita -tion, clinicians can use attentional strategies to foster neuroplasticity and promote the recovery of their patients. In this article we provide reasoning for the effectiveness of electromyographic biofeedback using the evidence of neu-roplasticity. With this information, we hope to provide clinicians a rationale for using this tool in the rehabilitation of patients with persistent quadriceps inhibition.

Context: Fewer athletic injuries and lower anterior cruciate ligament injury incidence rates were noted in studies of neuromuscular-training (NMT) interventions that had high compliance rates. However, several groups have demonstrated that preventive NMT interventions were limited by low compliance rates. Objective: To descriptively analyze coach and athlete compliance with preventive NMT and compare the compliance between study arms as well as among school levels and sports. Design: Randomized, controlled clinical trial. Setting: Middle and high school athletic programs. Participants or Other Participants: A total of 52 teams, comprising 547 female athletes, were randomly assigned to the experimental or control group and followed for 1 athletic season. Intervention(s): The experimental group (n = 30 teams [301 athletes]: 12 basketball teams [125 athletes], 6 soccer teams [74 athletes], and 12 volleyball teams [102 athletes]) participated in an NMT program aimed at reducing traumatic knee injuries through a trunk-stabilization and hip-strengthening program. The control group (n = 22 teams [246 athletes]: 11 basketball teams [116 athletes], 5 soccer teams [68 athletes], and 6 volleyball teams [62 athletes]) performed a resistive rubber-band running program. Main Outcome Measure(s): Compliance with the assigned intervention protocols (3 times per week during the preseason [mean = 3.4 weeks] and 2 times per week in-season [mean = 11.9 weeks] of coaches [coach compliance] and athletes [athlete compliance]) was measured descriptively. Using an independent t test, we compared coach and athlete compliance between the study arms. A 2-way analysis of variance was calculated to compare differences between coach and athlete compliance by school level (middle and high schools) and sport (basketball, soccer, and volleyball). Results: The protocols were completed at a mean rate of 1.3 ± 1.1 times per week during the preseason and 1.2 ± 0.5 times per week in-season. A total of 88.4% of athletes completed 2/3 of the intervention sessions. Coach compliance was greater in the experimental group than in the control group (P = .014). Coach compliance did not differ by sport but was greater at the high school than the middle school (P = .001) level. Athlete compliance did not differ by study arm, sport, or school level. Conclusions: Athletes received instruction in about 50% of each protocol. Nearly 90% of athletes performed more than 2/3 of the assigned NMT interventions. The assigned intervention was performed more often in the experimental arm compared with the control arm. Coaches at the high school level complied with the given protocol more than middle school coaches did. Athletes complied well with the protocol, but coaches did not, especially at the middle school level.

Study Design Cross-sectional, controlled laboratory study. Background Quantification of muscular activation during different phases of functional activities is important to understand activation deficits in individuals who have undergone anterior cruciate ligament reconstruction (ACLR). Objectives To compare activation levels of the vastus medialis (VM), medial hamstrings (MH), and gluteus medius (GMed) muscles during the different phases of weight-bearing tasks between individuals who had undergone ACLR and healthy controls. Methods Surface electromyography was used to measure the activation levels of the VM, MH, and GMed muscles in 16 participants who had undergone ACLR (average time since surgery, 4 years) and 15 healthy participants during the reach and return phases of the Star Excursion Balance Test (SEBT) and the ascending and descending phases of a step-down task (SDT). Repeated-measures analyses of variance were performed to determine whether muscle activation levels differed between groups during different phases of the tasks. Results There were significant group-by-phase interactions for the GMed during both the SEBT and SDT. Gluteus medius activation was lower for the ACLR group during the return phase of the posteromedial direction of the SEBT compared to the control group (P = .03). During the SDT, GMed activation was higher for the ACLR group during the ascending phase than during the descending phase (P

Background Web-based surveys provide an efficient means to track clinical outcomes over time without the use of clinician time for additional paperwork. Our purpose was to determine the feasibility of utilizing web-based surveys to capture rehabilitation compliance and clinical outcomes among postoperative orthopedic patients. The study hypotheses were that (a) recruitment rate would be high (>90%), (b) patients receiving surveys every two weeks would demonstrate higher response rates than patients that receive surveys every four weeks, and (c) response rates would decrease over time. Methods The study deaign involved a longitudinal cohort. Surgical knee patients were recruited for study participation during their first post-operative visit ( n = 59, 34.9 ± 12.0 years of age). Patients with Internet access, an available email address and willingness to participate were counter-balanced into groups to receive surveys either every two or four weeks for 24 weeks post-surgery. The surveys included questions related to rehabilitation and questions from standard patient-reported outcome measures. Outcome measures included recruitment rate (participants consented/patients approached), eligibility (participants with email/participants consented), willingness (willing participants/participants eligible), and response rate (percentage of surveys completed by willing participants). Results Fifty-nine patients were approached regarding participation. Recruitment rate was 98% ( n = 58). Eligibility was 95% ( n = 55), and willingness was 91% ( n = 50). The average response rate was 42% across both groups. There was no difference in the median response rates between the two-week (50%, range 0–100%) and four-week groups (33%, range 0–100%; p = 0.55). Conclusions Although patients report being willing and able to participate in a web-based survey, response rates failed to exceed 50% in both the two-week and four-week groups. Furthermore, response rates began to decrease after the first three months postoperatively. Therefore, supplementary data collection procedures may be necessary to meet established research quality standards.

Objectives To determine if differences in weight-bearing ankle dorsiflexion range of motion (DFROM) and Star Excursion Balance Test (SEBT) reach distances were present between participants with chronic ankle instability (CAI) and healthy individuals. A secondary objective was to re-examine the relationship between these measures in participants with and without CAI. Design Case–control. Methods Thirty participants with CAI were matched to 30 healthy participants. All participants performed maximum reach in the anterior, posteromedial and posterolateral directions of the SEBT; as well as, the Weight-Bearing Lunge Test (WBLT) to measure DFROM. Dependent variables included maximal DFROM (cm) and normalized SEBT reach distances (%). Independent t -tests were used for group comparisons (a priori p ≤ 0.05). Simple-linear regression examined the relationship between the WBLT and each SEBT direction. Results Significant differences were detected between groups for the WBLT (CAI: 10.73 ± 3.44 cm, healthy: 12.47 ± 2.51 cm; p = 0.03) and anterior reach distance (CAI: 76.05 ± 6.25%, healthy: 80.12 ± 5.88%; p = 0.01). No differences were identified in posteromedial or posterolateral ( p s > 0.70) reach. The WBLT had a significant moderate correlation to anterior reach in both groups ( p s p s > 0.70). Conclusions The results indicate that participants with CAI demonstrated less DFROM and anterior SEBT reach distance compared to health controls. Additionally, both groups demonstrated similar correlations between WBLT and SEBT performance. These findings suggest that participants with CAI have alterations in ankle motion which may negatively influence dynamic postural control; however, the relationship between WBLT and SEBT performance is consistent in those with and without CAI.

We examined the effect of a 2-week anterior-to-posterior ankle joint mobilization intervention on weight-bearing dorsiflexion range of motion (ROM), dynamic balance, and self-reported function in subjects with chronic ankle instability (CAI). In this prospective cohort study, subjects received six Maitland Grade III anterior-to-posterior joint mobilization treatments over 2 weeks. Weight-bearing dorsiflexion ROM, the anterior, posteromedial, and posterolateral reach directions of the Star Excursion Balance Test (SEBT), and self-reported function on the Foot and Ankle Ability Measure (FAAM) were assessed 1 week before the intervention (baseline), prior to the first treatment (pre-intervention), 24-48 h following the final treatment (post-intervention), and 1 week later (1-week follow-up) in 12 adults (6 males and 6 females) with CAI. The results indicate that dorsiflexion ROM, reach distance in all directions of the SEBT, and the FAAM improved (p 0.05). These results indicate that the joint mobilization intervention that targeted posterior talar glide was able to improve measures of function in adults with CAI for at least 1 week.

Summary of Search, “Best Evidence” Appraised, and Key Findings • We searched the literature for studies of level 3 evidence or higher in accordance with the Oxford Centre for Evidence Based Medicine. Level 1 evi-dence is well-designed research with limited inves-tigator bias. Level 2 studies are moderately designed research studies with investigator bias, and level 3 studies include those that are retrospective.• The search of the literature returned 32 possible stud-ies for inclusion; 2 studies met the inclusion criteria and were included. 1,2 • Both studies used a prospective cohort design. There was conflicting evidence to support the hypothesis that self-reported pain differed because of the pres-ence of bone bruise in patients with traumatic knee injury. Clinical Bottom Line There is conflicting evidence to support the idea that self-reported pain differs with the presence of bone bruise in patients with traumatic knee injury. Strength of Recommendation : The grade of recom-mendation for this study is a grade of D: Self-reported pain in patients with traumatic knee injury differs with the presence of bone bruise. According to the Centre of Evidence Based Medicine, a grade of D is reserved for level 5 evidence or, in our case, conflicting evidence of any level.

Context:Dynamic shoulder motion can be captured using video capture systems, but reliability has not yet been established.Objective:To compare the reliability of 2 systems in measuring dynamic shoulder kinematics during forward-elevation movements and to determine differences in these kinematics between healthy and injured subjects.Design:Reliability and cohort.Setting:Research laboratory.Participants:11 healthy subjects and 10 post–superior labrum anteroposterior lesion patients (SLAP).Intervention:Contrasting markers were placed at the hip, elbow, and shoulder to represent shoulder elevation and were videotaped in 2 dimensions. Subjects performed 6 repetitions of active elevation (AE) and active assisted elevation of the shoulder, and 3 trials were analyzed using Datapac (comprehensive system) and Dartfish (basic system).Main Outcome Measures:Amplitudes and velocities of the shoulder angle were calculated. Intraclass correlation coefficient (ICC), standard error of measurement (SEM), and levels of agreement (LOA) were used to determine intersystem and intertrial reliability.Results:For AE, the amplitude maximum (ICC = .98–.99, SEM = 2–3°, LOA = −9° to 5°) and average velocity (ICC = .94–.97, SEM = 1°/s, LOA = −4° to 1°/s) indicated excellent intersystem reliability between systems. Intratrial reliability for minimum velocity was moderate for Datapac (ICC = .64, SEM = 4°/s, LOA = 7°/s) and poor for Dartfish (ICC = .52, SEM = 20°/s, LOA = 37°/s). Cohort results demonstrated for AE a greater amplitude for healthy v SLAP (139° ± 11° v 113° ± 13°; P = .001) and interaction for an average velocity increase of 2°/s in healthy and decrease of 2°/s in SLAP patients over the 3 trials (P = .02).Conclusions:Reliability ranges provide the means to assess the clinical meaningfulness of results. The cohort differences are supported when the values exceed the ranges of the SEM; hence the amplitude results are meaningful. For dynamic shoulder elevation measured using video, the assessment of velocity was found to produce moderate to good reliability. The results suggest that with these measures subtle changes in both measures may be possible with further investigations.

Context: Researchers have observed that medial knee collapse is a mechanism of knee injury. Lower extremity alignment, sex, and strength have been cited as contributing to landing mechanics.Objective: To determine the relationship among measurements of asymmetry of unilateral hip rotation (AUHR); mobility of the foot, which we described as relative arch deformity (RAD); hip abduction–external rotation strength; sex; and me-dial collapse of the knee during a single-leg jump landing. We hypothesized that AUHR and RAD would be positively correlated with movements often associated with medial collapse of the knee, including hip adduction and internal rotation excursions and knee abduction and rotation excursions.Design: Descriptive laboratory study.Setting: Research laboratory.Patients or Other Participants: Thirty women and 15 men (age = 21 ± 2 years, height = 171.7 ± 9.5 cm, mass = 68.4 ± 9.5 kg) who had no history of surgery or recent injury and who participated in regular physical activity volunteered.Intervention(s): Participants performed 3 double-leg forward jumps with a single-leg landing. Three-dimensional kinematic data were sampled at 100 Hz using an electromagnetic tracking system. We evaluated AUHR and RAD on the preferred leg and evaluated isometric peak hip abductor–external rotation torque. We assessed AUHR by calculating the difference between internal and external hip rotation in the prone position (AUHR = internal rotation – external rotation). We evaluated RAD using the Arch Height Index Measurement System. Correlations and linear regression analyses were used to assess relationships among AUHR, RAD, sex, peak hip abduction–external rotation torque, and kinematic variables for 3-dimensional motion of the hip and knee.Main Outcome Measure(s): The dependent variables were joint angles at contact and joint excursions between contact and peak knee flexion.Results: We found that AUHR was correlated with hip adduction excursion (R = 0.36, P = .02). Asymmetry of unilateral hip rotation, sex, and peak hip abduction–external rotation torque were predictive of knee abduction excursion (adjusted R2 = 0.47, P < .001). Asymmetry of unilateral hip rotation and sex were predictive of knee external rotation excursion (adjusted R2 = 0.23, P = .001). The RAD was correlated with hip adduction at contact (R2 = 0.10, R = 0.32, P = .04) and knee flexion excursion (R2 = 0.11, R = −0.34, P = .03).Conclusions: Asymmetry of unilateral hip rotation, sex, and hip strength were associated with kinematic components of medial knee collapse.

The postgenomic era and heightened public expectations for tangible improvements in the public health have stimulated a complete transformation of the nation's biomedical research enterprise. The National Institutes of Health (NIH) “Roadmap for Medical Research” has catalyzed this transformation. The NIH roadmap consists of several interrelated initiatives, of which the Clinical and Translational Science Award (CTSA) program is the most relevant for rehabilitation specialists. This article reviews the evolution of this transformation and highlights the unprecedented opportunities the CTSA program provides rehabilitation specialists to play leadership roles in improving the clinical care of their patients.

Context:The National Institutes of Health's Clinical and Translational Science Award initiative is designed to establish and promote academic centers of clinical and translational science (CTS) that are empowered to train and advance multi- and interdisciplinary investigators and research teams to apply new scientific knowledge and techniques to enhance patient care. Among the key components of a full-service center for CTS is an educational platform to support research training in CTS. Educational objectives and resources available to support the career development of the clinical and translational scientists, including clinical research education, mentored research training, and career development support, are described.Objective:The purpose of the article is to provide an overview of the CTS educational model so that rehabilitation specialists can become more aware of potential resources that are available and become more involved in the delivery and initiation of the CTS model in their own workplace. Rehabilitation clinicians and scientists are well positioned to play important leadership roles in advancing the academic mission of CTS. Rigorous academic training in rehabilitation science serves as an effective foundation for supporting the translation of basic scientific discovery into improved health care. Rehabilitation professionals are immersed in patient care, familiar with interdisciplinary health care delivery, and skilled at working with multiple health care professionals.Conclusion:The NIH Clinical and Translational Science Award initiative is an excellent opportunity to advance the academic development of rehabilitation scientists.

Lateral ankle anesthesia has been used as a model to explore effects of ligament deafferentation related to ankle sprain on single limb postural control with conflicting results. Time-to-boundary (TTB) is a postural control measurement technique found to be sensitive in detecting subtle deficits in postural control in those with chronic ankle instability. The objective of this study was to determine the effects of lateral ankle ligament anesthesia on TTB measures of single limb postural control in healthy adults. Twenty-two healthy adults with no history of lower extremity injury within the past 6 months or balance disorders participated in the study. All subjects received a lidocaine injection to the lateral ankle structures on one of two testing days. On both testing days, subjects performed 3 eyes open and 3 eyes closed, 10-s trials of barefoot single limb stance on a forceplate. The dependent variables were the mean of TTB minima(s) and standard deviation of TTB minima(s) in mediolateral (ML) and anteroposterior (AP) directions. Separate condition (anesthesia, control) by vision (eyes open, eyes closed) ANOVAs with repeated measures were used for each TTB variable to determine the effects of anesthesia on postural control. Alpha level was set a priori at p≤0.05. The anesthesia day TTBAP magnitude (p=0.008) and variability (p=0.044) measures were significantly lower than the control day, regardless of vision. Anesthesia of the lateral ankle ligamentous structures significantly reduced the magnitude and variability of TTBAP measures. These findings are similar to deficits found in those with chronic ankle instability.
