1. Overcoming Therapeutic Inertia in Type 2 Diabetes: Exploring Machine Learning-Based Scenario Simulation for Improving Short-Term Glycemic Control

Abstract

Background: International guidelines for diabetes care emphasize the urgency of promptly achieving and sustaining adequate glycemic control to reduce the occurrence of micro/macrovascular complications in patients with type 2 diabetes mellitus (T2DM). However, data from the Italian Association of Medical Diabetologists (AMD) Annals reveal that only 47% of T2DM patients reach appropriate glycemic targets, with approximately 30% relying on insulin therapy, either solely or in combination. This artificial intelligence analysis seeks to assess the potential impact of timely insulin initiation in all eligible patients via a “what-if” scenario simulation, leveraging real-world data. Methods: This retrospective cohort study utilized the AMD Annals database, comprising 1,186,247 T2DM patients from 2005 to 2019. Employing the Logic Learning Machine (LLM), we simulated timely insulin use for all eligible patients, estimating its effect on glycemic control after 12 months within a cohort of 85,239 patients. Of these, 20,015 were employed for the machine learning phase and 65,224 for simulation. Results: Within the simulated scenario, the introduction of appropriate insulin therapy led to a noteworthy projected 17% increase in patients meeting the metabolic target after 12 months from therapy initiation within the cohort of 65,224 individuals. The LLM’s projection envisages 32,851 potential patients achieving the target (hemoglobin glycated < 7.5%) after 12 months, compared to 21,453 patients observed in real-world cases. The receiver operating characteristic (ROC) curve analysis for this model demonstrated modest performance, with an area under the curve (AUC) value of 70.4%. Conclusions: This study reaffirms the significance of combatting therapeutic inertia in managing T2DM patients. Early insulinization, when clinically appropriate, markedly enhances patients’ metabolic goals at the 12-month follow-up.

Abstract

Summary: Background: A delay in reaching HbA1c targets in patients with newly-diagnosed type 2 diabetes (T2D) is associated with an increased long-term risk of developing cardiovascular diseases (CVD), a phenomenon referred to as legacy effect. Whether an early introduction of glucose-lowering drugs with proven benefit on CVD can attenuate this phenomenon is unknown. Methods: Using data derived from a large Italian clinical registry, i.e. the AMD Annals, we identified 251,339 subjects with newly-diagnosed T2D and without CVD at baseline. Through Cox regressions adjusted for multiple risk factors, we examined the association between having a mean HbA1c between 7.1 and 8% or >8%, compared with ≤7%, for various periods of early exposure (0–1, 0–2, 0–3 years) and the development of later (mean subsequent follow-up 4.6 ± 2.9 years) CVD, evaluated as a composite of myocardial infarction, stroke, coronary or peripheral revascularization, and coronary or peripheral bypass. We performed this analysis in the overall cohort and then splitting the population in two groups of patients: those that introduced sodium-glucose transport protein 2 inhibitors (SGLT-2i) during the exposure phase and those not treated with these drugs. Findings: Considering the whole cohort, subjects with both a mean HbA1c between 7.1 and 8% and >8%, compared with patients attaining a mean HbA1c ≤ 7%, showed an increased risk of developing the outcome in all the three early exposure periods assessed, with the highest risk observed in patients with mean HbA1c > 8% in the 3 years exposure period (hazard ratio [HR]1.33; 95% confidence interval [CI] 1.063–1.365). The introduction of SGLT-2i during the exposure periods of 0–1 and 0–2 years eliminated the association between poor glycemic control and the outcome (p for interaction 0.006 and 0.003, respectively, vs. patients with the same degree of glycemic control but not treated with these drugs). Interpretation: Among patients with newly diagnosed T2D and free of CVD at baseline, a poor glycemic control in the first three years after diagnosis is associated with an increased subsequent risk of CVD. This association is no longer evident when SGLT-2i are introduced in the first two years, suggesting that these drugs attenuate the phenomenon of legacy effect. An early treatment with these drugs might thus promote a long-lasting benefit in patients not attaining proper glycemic control after T2D diagnosis. Presentamos el caso de una paciente de 70 años, con el único antecedente médico de interés de anafilaxia por Anisakis, que presenta un fracaso renal agudo con biopsia compatible con nefritis tubulointersticial de etiología alérgica tras la ingesta de pescado. A pesar de que no se ha publicado ningún caso previo de nefritis por hipersensibilidad a Anisakis, la clínica, evolución y biopsia fueron compatibles con esta sospecha, sin poderse encontrar otra posible etiología del fracaso renal agudo.

4. Gender-Disparities in Adults with Type 1 Diabetes: More Than a Quality of Care Issue. A Cross-Sectional Observational Study from the AMD Annals Initiative.

Abstract

We evaluated gender-differences in quality of type 1 diabetes (T1DM) care. Starting from electronic medical records of 300 centers, 5 process indicators, 3 favorable and 6 unfavorable intermediate outcomes, 6 treatment intensity/appropriateness measures and an overall quality score were measured. The likelihood of women vs. men (reference class) to be monitored, to reach outcomes, or to be treated has been investigated through multilevel logistic regression analyses; results are expressed as Odd Ratios (ORs) and 95% confidence intervals (95%CIs). The inter-center variability in the achievement of the unfavorable outcomes was also investigated. Overall, 28,802 subjects were analyzed (45.5% women). Women and men had similar age (44.5±16.0 vs. 45.0±17.0 years) and diabetes duration (18.3±13.0 vs. 18.8±13.0 years). No between-gender differences were found in process indicators. As for intermediate outcomes, women showed 33% higher likelihood of having HbA1c ≥8.0% (OR = 1.33; 95%CI: 1.25-1.43), 29% lower risk of blood pressure ≥140/90 mmHg (OR = 0.71; 95%CI: 0.65-0.77) and 27% lower risk of micro/macroalbuminuria (OR = 0.73; 95%CI: 0.65-0.81) than men, while BMI, LDL-c and GFR did not significantly differ; treatment intensity/appropriateness was not systematically different between genders; overall quality score was similar in men and women. Consistently across centers a larger proportion of women than men had HbA1c ≥8.0%, while a smaller proportion had BP ≥140/90 mmHg. No gender-disparities were found in process measures and improvements are required in both genders. The systematic worse metabolic control in women and worse blood pressure in men suggest that pathophysiologic differences rather than the care provided might explain these differences.

Abstract

AIMS: Beta-cell dysfunction is an early event in the natural history of type 2 diabetes. However, its progression is variable and potentially influenced by several clinical factors. We report the baseline data of the BetaDecline study, an Italian prospective multicenter study on clinical predictors of beta-cell dysfunction in type 2 diabetes. MATERIALS AND METHODS: Clinical, lifestyle, and laboratory data, including circulating levels of inflammatory markers and non-esterified fatty acids, were collected in 507 type 2 diabetic outpatients on stable treatment with oral hypoglycemic drugs or diet for more than 1 year. Beta-cell dysfunction was evaluated by calculating the proinsulin/insulin ratio (P/I). RESULTS: At baseline, the subjects in the upper PI/I ratio quartile were more likely to be men and receiving secretagogue drugs; they also showed a borderline longer diabetes duration (P = 0.06) and higher serum levels of glycated hemoglobin (HbA1c), fasting blood glucose, and triglycerides. Abstract

In this work we studied, by means of high-resolution neutron diffraction as a function of temperature, the La1.4Sr0.8Ca0.8Mn2O7 bilayered manganite for two different annealing treatments. Out data allowed us to shown, for the first time, the absence of long-range magnetic order in this optimally doped bilayered manganite where the A-site of the structure is doped with equal proportions of different isovalent cations (Ca and Sr). The system, however, presents defined IM transitions which suggest that the transport properties are not linked to the evolution of long-range order and that two dimensional spin ordering in the layers of the perovskite blocks may be sufficient to 'assist' the hole hopping. Abstract

In this paper we report the results of the synthesis and structural, transport and magnetic characterization of pure La0.5Sr1.5MnO4 (LSMO) and 5% doped samples, i.e. La0.5Sr1.5Mn0.95B0.05O4, where B = Ru, Co and Ni. It is shown that even a light doping is successful in suppressing the charge and orbital ordering found for the pure LSMO. In general, doping favours the carrier motion and, from a magnetic point of view, the set-up a spin-glass state. Moreover, structural parameters show an anisotropy in the lattice constant variation, with the tetragonal distortion increasing as the cell volume reduces, which may suggest a variation in the relative nature of the orbital character of the eg electrons along with the overall cation size., Comment: 22 Pages, 7 Figures

Abstract

In this work we report a structural, electrical and magnetic characterization of the La1-xNaxMn1-yRuyO3+d (LNMRO) system with x = 0.05, 0.15 and y = 0, 0.05, 0.15, also comprising an investigation of the role of the oxygen content on the related redox properties. The experimental investigation has been realized with the aid of X-ray powder diffraction, electron microprobe analysis, thermogravimetry, electrical resistivity and magnetization measurements, and electron paramagnetic resonance. We demonstrate that the effect of ruthenium doping on the studied LNMRO compounds is not only directly related to the Ru/Mn substitution and to the Ru oxidation state but also indirectly connected to the oxygen content in the sample. Our data show that ruthenium addition can improve electrical and magnetic properties of non-optimally (low) cation doped manganites, causing an increase of the TC value and the insurgence of MR effect, as observed for the x = 0.05 and y = 0.05 sample., Comment: 28 Pages, 7 Figures

Abstract

In this paper we report studies on the correlation between the presence and distribution of cation vacancies in doped manganites (La,M)MnO3+delta (where M = Na, Ca) and their magnetic properties. Results indicate that cation vacancies are distributed differently for the different crystal structures and dopant ion type. In particular it is shown that knowledge of the total vacancies concentration alone is not enough to fully characterize the physical properties of manganites and that their distribution between the A and B sites of the perovskite structure plays a crucial role which should be taken into account in future studies., Comment: 27 pages, 5 figure. Abstract

Purpose Stroke is a leading cause of long-term disability with high mortality rate in the first year after the event. In Campania, mechanical thrombectomy treatment significantly increases in the last 3 years, as well as hospitals delivering acute stroke treatments. The aim of this study is to demonstrate how a full opening of our stroke network improves stroke management and stroked patients’ survival in Campania. Material and methods In Federico II University Hospital of Naples acting as a HUB center of 7 peripheral SPOKE hospitals in regional territory, 68 patients with acute ischemic stroke were evaluated with NIHSS and m-RS clinical scores and neuroradiological ASPECT scores, from January 1 to December 31, 2021. At hospital discharge, NIHSS score and three months after m-RS score were re-assessed to evaluate the therapeutic effects. Results Forty-two of 68 patients (63%) admitted to our hub center had ischemic acute stroke at CT evaluation; 29 patients had ASPECT score > 7 (69%), and 6 a score Discussion The inter-company agreement between Federico II University and several peripheral hospitals allows an absolute and relative increase in endovascular mechanical thrombectomy and intravenous thrombolysis procedures, with a relative prevalence of mechanical thrombectomy. A regional implementation of the stroke multi-disciplinary care system is hardly needed to ensure the optimum treatment for the largest number of patients, improving patient’s outcome.

Abstract

Telemedicine is advocated as a fundamental tool in modern clinical management. However, data on the effects of telemedicine vs face-to-face consultation on clinical outcomes in type 2 diabetes (T2DM) are still uncertain. This paper describes the use of telemedicine during the 2020 COVID-19 emergency and compares volume activity and quality indicators of diabetes care between face-to-face vs telemedicine counseling in the large cohort of T2DM patients from the AMD Annals Initiative.Demographic and clinical characteristics, including laboratory parameters, rate of the screening of long-term complications, current therapies and the Q-score, a validated score that measures the overall quality of care, were compared between 364,898 patients attending face-to-face consultation and 46,424 on telemedicine, during the COVID-19 pandemic.Patients on telemedicine showed lower HbA1c levels (7.1 ± 1.2 % vs 7.3 ± 1.3 %, p 0.0001), and they were less frequently treated with metformin, GLP1-RAs and SGLT2i and more frequently with DPP4i. The telemedicine group showed reduced monitoring of the various parameters considered as process indicators, especially, eye and foot examination. The proportion of patients with a good quality of care (Q score 25) was higher among those receiving face-to-face consultation. Moreover, in the telemedicine group, all major clinical outcomes remained stable when further compared to those collected in the year 2019, when the same patients underwent a regular face-to-face consultation, suggesting that the care provided through telemedicine did not negatively affect the most important parameters.During the COVID-19 pandemic, telemedicine provided an acceptable quality of diabetes care, comparable to that of patients attending face-to-face consultation, although a less frequent screening of complications seems to have occurred in subjects consulted by telemedicine.

Abstract

One of the main neutron sources for the astrophysical s process is the reaction ^{13}C(α,n)^{16}O, taking place in thermally pulsing asymptotic giant branch stars at temperatures around 90 MK. To model the nucleosynthesis during this process the reaction cross section needs to be known in the 150-230 keV energy window (Gamow peak). At these sub-Coulomb energies, cross section direct measurements are severely affected by the low event rate, making us rely on input from indirect methods and extrapolations from higher-energy direct data. This leads to an uncertainty in the cross section at the relevant energies too high to reliably constrain the nuclear physics input to s-process calculations. We present the results from a new deep-underground measurement of ^{13}C(α,n)^{16}O, covering the energy range 230-300 keV, with drastically reduced uncertainties over previous measurements and for the first time providing data directly inside the s-process Gamow peak. Selected stellar models have been computed to estimate the impact of our revised reaction rate. For stars of nearly solar composition, we find sizeable variations of some isotopes, whose production is influenced by the activation of close-by branching points that are sensitive to the neutron density, in particular, the two radioactive nuclei ^{60}Fe and ^{205}Pb, as well as ^{152}Gd.

Abstract

Background: Shell hydrogen burning during the asymptotic giant branch (AGB) phase through the oxygen isotopes has been indicated as a key process that is needed to understand the observed O 18 / O 16 relative abundance in presolar grains and in stellar atmospheres. This ratio is strongly influenced by the relative strengths of the reactions O 18 ( p , α ) N 15 and O 18 ( p , γ ) F 19 in low-mass AGB stars. While the former channel has been the focus of a large number of measurements, the ( p , γ ) reaction path has only recently received some attention and its stellar reaction rate over a wide temperature range rests on only one measurement. Purpose: Our aim is the direct measurement of states in F 19 as populated through the reaction O 18 ( p , γ ) F 19 to better determine their influence on the astrophysical reaction rate, and more generally to improve the understanding of the nuclear structure of F 19 .Method: Branchings and resonance strengths were measured in the proton energy range E p lab = 150 – 400 keV , using a high-purity germanium detector inside a massive lead shield. The measurement took place in the ultra-low-background environment of the Laboratory for Underground Nuclear Astrophysics (LUNA) experiment at the Gran Sasso National Laboratory, leading to a highly increased sensitivity. Results: The uncertainty of the γ branchings and strengths was improved for all four resonances in the studied energy range; many new transitions were observed in the case of the 334 keV resonance, and individual γ decays of the 215 keV resonance were measured for the first time. In addition a number of transitions to intermediate states that decay through α emission were identified. The strengths of the observed resonances are generally in agreement with literature values. Conclusions: Our measurements substantially confirm previous determinations of the relevant resonance strengths. Therefore the O 18 ( p , γ ) F 19 reaction rate does not change with respect to the reaction rate reported in the compilations commonly adopted in the extant computations of red-giant branch and AGB stellar models. Nevertheless, our measurements definitely exclude a nonstandard scenario for the fluorine nucleosynthesis and a nuclear physics solution for the O 18 depletion observed in Group 2 oxygen-rich stardust grains.

Abstract

3078 Background: Gene fusions caused by chromosomal rearrangements comprise a key category of oncogenic driver mutations. However, given the diverse array of potentially novel loci where each proto-oncogene can translocate, many assays including DNA-based CGP have technical limitations that disallow the detection of all relevant fusion partners potentially leading to false negatives. Hybrid Capture RNA sequencing renders a more comprehensive evaluation of genes and allows detection of novel and known fusion partners. Here we assessed the impact of utilizing in-house CGP testing with a paired RNA-DNA hybrid assay in the identification of pathogenic fusions and their potential clinical actionability for patients with solid tumors across a large US health system. Methods: Patients in the Providence health system diagnosed with advanced solid tumor malignancies over a two-year period (2019-2021) received reflex CGP testing at the time of diagnosis utilizing an internally validated workflow. DNA/RNA sequencing results as well as histology and staging information were curated from deidentified electronic medical records and in-house databases, and tumor types were mapped to OncoTree tissue categories. Potential clinical actionability was assessed based on OncoKB therapeutic levels 1-3 and clinical trial eligibility matched to the biomarker inclusion criteria for ASCO TAPUR, NCI-MATCH and MyPathway studies (both without time limits and at time of testing). Results: The median patient age at diagnosis was 67 years, 52% of patients were female, and the majority (80%) were white. Across all tested advanced solid tumors, 6.7% (217/3218) were found to harbor a pathogenic fusion. The tumor types most enriched in this set of pathogenic fusions were prostate (30%), lung (27%), CUP (10%) and breast (9%). 29% (n = 64) of the identified pathogenic fusions were identified as actionable based on OncoKB criteria (levels 1-3), and 31% (n = 69) matched to one or more arms in the ASCO TAPUR, NCI-MATCH or MyPathway basket clinical trials. The most frequent actionable fusion driver genes identified were ALK (12%), FGFR 1-3 (12%), RET (7%) NTRK 1-3 (3%), and ROS1 (2%) and a subset of these key drivers were fused with novel gene pairs. A subset of fusions co-occurred with other targetable biomarkers, with the most common comprising tumor mutational burden high (TMB-H) (13%), PIK3CA (7%) and high microsatellite instability (MSI-H) (2%). Conclusions: In-house CGP testing utilizing an RNA-DNA based assay identified actionable fusion targets across tumor types, with many novel fusion partners that may be undetectable by prior generation sequencing assays. While many of these actionable targets are rare individually, the expanding totality of actionable gene alterations supports the growing utility of CGP for identifying patients who are candidates for approved targeted therapies and clinical trials.

Abstract

e15035 Background: Precision therapies and immunotherapies have revolutionized cancer care, with novel genomic biomarker-associated therapies being introduced into clinical practice rapidly. We assessed the utility of comprehensive genomic profiling (CGP)-based testing for identifying biomarkers associated with approved therapies, and therapies in precision medicine basket clinical trials (CT) across a large cohort of advanced cancer patients in the Providence health system. Methods: Advanced cancer patients were tested utilizing the Providence CGP workflow between 2019-2021. Clinical actionability was assessed for CGP and compared with results from an in silico 50-gene panel based on a previously utilized lab-developed test at Providence. Clinical actionability was assessed based on OncoKB and alterations were assigned as: FDA recognized (Level 1), standard of care (Level 2), FDA approved/investigational drug in another indication or having compelling clinical evidence (Level 3). CT matching was assessed based on enrollment criteria for ASCO-TAPUR, NCI-MATCH and My Pathway CTs at time of testing. Pooled electronic medical record and genomic data were curated and standardized. Results: Of the 3,218 advanced cancer patients tested with CGP, 52% were female, 80% were white, and median age was 67 years. Across 31 tumor types, the most commonly tested were lung (26%), bowel/colon (16%), and breast (9%). Overall, 48% of patients tested with CGP harbored at least one actionable biomarker (OncoKB Levels1/2/3). Clinical actionability was significantly higher in the CGP cohort compared to the in silico cohort based on presence of at least one Level 1 biomarker (45% vs. 19%, p < 0.001). CGP cohort had higher proportion of patients with multiple/co-occurring Level 1 biomarkers compared to in silico cohort (20% vs. 9%, p < 0.001). Of the most prevalent tumor types, 57% lung, 94% bowel/colon, and 37% breast had Level 1 alterations with CGP testing. Notably, 49% of CGP cohort vs. 23% in silico cohort (p < 0.001) harbored a biomarker matching to one or more arms of the three basket CTs. Conclusions: CGP and small panel testing can both identify patients eligible for approved therapies and/or basket CTs with CGP having significantly higher clinical actionability and CT eligibility. We expect value of CGP to increase as biomarker actionability transforms clinical practice.

Abstract

The presence of non-hexagonal rings in the honeycomb carbon arrangement of graphene produces rippled graphene layers with valuable chemical and physical properties. In principle, a bottom-up approach to introducing distortion from planarity of a graphene sheet can be achieved by careful insertion of curved polyaromatic hydrocarbons during the growth of the lattice. Corannulene, the archetype of such non-planar polyaromatic hydrocarbons, can act as an ideal wrinkling motif in 2D carbon nanostructures. Herein we report an electrochemical bottom-up method to obtain egg-box shaped nanographene structures through a polycondensation of corannulene that produces a new conducting layered material. Characterization of this new polymeric material by electrochemistry, spectroscopy, electron microscopy (SEM and TEM), scanning probe microscopy, and laser desorption-ionization time of flight mass spectrometry provides strong evidence that the anodic polymerization of corannulene, combined with electrochemically induced oxidative cyclodehydrogenations (Scholl reactions), leads to polycorannulene with a wavy graphene-like structure., A bottom-up synthesis of wavy graphene structures obtained through an anodic polymerization process, combined with an electrochemically triggered oxidative cyclodehydrogenation, of the bowl-shaped polyaromatic hydrocarbon corannulene.

Presentamos el caso de una paciente de 70 años, con el único antecedente médico de interés de anafilaxia por Anisakis, que presenta un fracaso renal agudo con biopsia compatible con nefritis tubulointersticial de etiología alérgica tras la ingesta de pescado. A pesar de que no se ha publicado ningún caso previo de nefritis por hipersensibilidad a Anisakis, la clínica, evolución y biopsia fueron compatibles con esta sospecha, sin poderse encontrar otra posible etiología del fracaso renal agudo.

49. Distal displacement of maxillary sinus anterior wall versus conventional sinus lift with lateral access: a 3-year retrospective Computerized Tomography study

Abstract

Background To compare a sinus augmentation procedure with a distal displacement of the anterior wall to a standard sinus lifting and grafting with a lateral window approach. Methods In the displacement group, a sinus surgical fracture results in the distal displacement of the anterior wall by means of an electromagnetic device. In the filling group, a sinus lifting with lateral access and grafting with particulate bone was performed. Bone volume beneath the maxillary sinus was investigated with Computerized Tomography after data superimposition. Clinical and radiological outcomes over 3 years had been evaluated. Results Forty-three dental implants were enrolled. The surgery of both groups significantly increased the bone volume in the VOIs (p-value≤0.0017) respectively for displacement group from 1.17±0.34cc to 1.53±0.39cc with a final bone volume gain of +0.36±0.17cc and for filling group from 1.24±0.41cc to 1.94±0.68cc with a bone augmentation of 0.71±0.31cc. Two implants early failed in the filling group, attesting the 3-year survival rate to 92.6%(CI95%: 82.7%-100%). Marginal bone loss at the distal aspect was 1.66±0.72mm and 1.25±0.78mm for displacement and filling group, respectively (p-value=0.0497). Conclusions The study showed an effective bone gain around dental implants at 3-year of survey both for sinus augmented by backward displacement of the anterior wall (+34%) and by sinus lifting with a lateral window approach (+57%).

Abstract

Direct measurements of reaction cross-sections at astrophysical energies often require the use of solid targets able to withstand high ion beam currents for extended periods of time. Thus, monitoring target thickness, isotopic composition, and target stoichiometry during data taking is critical to account for possible target modifications and to reduce uncertainties in the final cross-section results. A common technique used for these purposes is the Nuclear Resonant Reaction Analysis (NRRA), which however requires that a narrow resonance be available inside the dynamic range of the accelerator used. In cases when this is not possible, as for example the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ reaction recently studied at low energies at the Laboratory for Underground Nuclear Astrophysics (LUNA) in Italy, alternative approaches must be found. Here, we present a new application of the shape analysis of primary $$\gamma $$ rays emitted by the $$^{13}\hbox {C}(\hbox {p},\gamma )^{14}\hbox {N}$$ radiative capture reaction. This approach was used to monitor $$^{13}\hbox {C}$$ target degradation in situ during the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ data taking campaign. The results obtained are in agreement with evaluations subsequently performed at Atomki (Hungary) using the NRRA method. A preliminary application for the extraction of the $$^{13}\hbox {C}(\alpha ,\hbox {n})^{16}\hbox {O}$$ reaction cross-section at one beam energy is also reported.
