OBJECTIVE: To present the preliminary results from treating patients with Legg-Calvé-Perthes Disease (LCPD) by means of hip arthrodiastasis using a monolateral external fixator applied to the hip and to succinctly describe the surgical technique used, in a prospective study. METHODS: Prospective study on 18 patients with LCPD who underwent surgical treatment by means of the hip arthrodiastasis technique using a monolateral external fixator. There were 13 male and five female patients of mean age 8.5 years, ranging from five to 13 years. All the patients presented unilateral hip impairment: nine on the right side and nine on the left. The results were evaluated at maturity using clinical and radiological criteria. RESULTS: All the patients evolved with improvement of joint mobility, and pain relief was achieved in 88.9% of them. Reossification of the femoral epiphysis occurred within the first three months of the treatment. The hips operated at the necrosis stage of the disease did not passed through the fragmentation stage, thus shortening the evolution of the disease. The results were 77.8% satisfactory and 22.2% unsatisfactory. CONCLUSION: Hip arthrodiastasis with a monolateral external fixator during the active phase of LCPD improved the degree of joint mobility. Use of the arthrodiastasis technique at the necrosis stage or at the fragmentation stage (active phase of the disease) presented satisfactory results from treatment of LCPD. MÉTODOS: Estudo prospectivo de 18 pacientes com DLCP submetidos ao tratamento operatório com a técnica de artrodiástase do quadril por meio de fixador externo unilateral. São 13 pacientes do gênero masculino e cinco do feminino com idade média de 8,5 anos com variação de cinco a 13 anos. Todos os pacientes com acometimento unilateral do quadril, nove à direita e nove à esquerda. A avaliação dos resultados foi feita na maturidade e considerou critérios clínicos e radiográficos. RESULTADOS: Todos os pacientes evoluíram com melhoria da mobilidade articular com alívio da dor obtido em 88,9% dos pacientes. A reossificação da epífise femoral ocorreu nos primeiros três meses do tratamento. Os quadris operados na fase de necrose não passaram pela fase de fragmentação e abreviaram o tempo de evolução da doença. Os resultados foram 77,8% satisfatórios e 22,2% insatisfatórios. CONCLUSÕES: A artrodiástase do quadril com fixador externo monolateral na fase ativa da DLCP melhora o grau de mobilidade articular. O emprego da técnica de artrodiástase nas fases de necrose e fragmentação (fase ativa da doença) apresenta resultados satisfatórios no tratamento da DLCP.

OBJETIVO: Apresentar os resultados preliminares da utilização da artrodiastase do quadril em pacientes portadores de artrite reumatoide juvenil e com comprometimento da articulação coxofemoral. MÉTODOS: Estudo prospectivo de 12 pacientes (seis meninos e seis meninas) com idades entre oito e 18 anos (média de 10,5 anos). Foi utilizado um fixador externo monolateral que permite os movimentos de flexão e de extensão no quadril. O fixador externo foi mantido por um período que variou de 78 a 90 dias, com média de 86 dias. O controle radiográfico foi realizado durante o ato operatório e, semanalmente, durante o período de tração e a cada quatro semanas, quando terminado este período. Na avaliação clínica dos resultados, incluímos a graduação da dor e o grau de movimentação articular, com medidas e avaliações pré e pós-operatórias. O período de acompanhamento variou de 12 a 15 anos, com média de 13 anos. RESULTADOS: O valor médio da escala de dor foi de nove (9) antes da operação e de quatro (4) no período pós-operatório. Em dois pacientes não ocorreu melhora da dor. O arco de movimento do quadril aumentou em todos os pacientes, com exceção de dois. Na avaliação radiográfica evidenciamos um aumento no espaço articular de 2mm, em média, e que se manteve no pós-operatório. Não foram observadas complicações com a utilização da técnica. Apenas verificamos soltura dos pinos de Schanz da região do osso ilíaco em dois pacientes. A técnica operatória não ocasionou resultado satisfatório. CONCLUSÃO: O procedimento de artrodiastase está bem indicado para a recuperação da mobilidade em uma articulação coxofemoral comprometida e rígida, como ocorre em pacientes com artrite reumatoide juvenil. OBJECTIVE: To present the preliminary results of the use of hip arthrodiastasis in patients with juvenile rheumatoid arthritis and involvement of the hip joint. METHODS: A prospective study of 12 patients (six boys and six girls) aged between eight and 18 years (mean 10.5 years). We used a monolateral external fixator that allows flexion and extension at the hip. The external fixator was maintained for a period ranging from 78 to 90 days, with a mean of 86 days. Radiographic control was performed during surgery, weekly during the traction period, and every four weeks once this period was completed. The clinical evaluation of results included the degree of the pain and the degree of joint movement, measured and evaluatedpre-and post-operatively. The follow-up period ranged from 12 to 15 years, with a mean of 13 years. RESULTS: The average pain score was nine (9) before surgery and four (4) in the postoperative period. There was no improvement in pain in two patients. The range of motion of the hip increased in all patients except two. Radiographic evaluation evidenced a2 mm increase in joint space, on average,that has remained postoperatively. There were no complications with this technique. Only a loosening of the Schanz screws in the region of the iliac bone was observed in two patients. The surgical technique did not bring satisfactory results. CONCLUSION: The arthrodiastasis procedure is well suited for recovery of mobility in animpairedand rigid hip joint, as occurs in patients with juvenile rheumatoid arthritis.

Selection of the most appropriate sonographic scanning approaches for evaluation of hips can improve the method efficacy and decrease the scanning time.To determine the sonographic scanning planes that best assess the proximal femoral vascularity in asymptomatic and pathologic hips of children with Legg-Calvé-Perthes disease (LCPD) and evaluate the frequency (number of hips with evidence of perfusion) and intensity (number of color pixels per region) of color pixels representing superficial cartilaginous and deep transphyseal vascularity in different anatomic regions of pathologic and asymptomatic hips using multipositional power Doppler approaches.Seven scanning approaches (anterior-sagittal, anterior-transverse, coronal, adduction, perineal, 30 degrees and 70 degrees of abduction) were applied in 26 pathologic hips of 26 children with LCPD (age range 3-11 years) and in 25 contralateral asymptomatic hips. The color Doppler signals seen within the proximal femur were analyzed both qualitatively (overall/regional frequency) and quantitatively (intensity).The coronal (P=0.009) and 30 degrees abduction (P=0.047) approaches demonstrated a higher frequency of color pixels in pathologic than in asymptomatic hips. The anterior-sagittal, 30 degrees abduction, adduction and anterior-transverse planes performed best of all approaches (P=0.02) to assess deep transphyseal perfusion. The physis demonstrated a greater intensity of color signals representing intraosseous vascularity than other regions in pathologic hips (P=0.027), as noted with the anterior-sagittal approach. There was a tendency (P=0.06) towards a greater intensity of pixels representing cartilaginous vascularity in pathologic hips' physes with the coronal approach.Specific sonographic scanning planes are recommended for assessment of the proximal femoral vascularity of LCPD hips. The physis is the anatomic region that presents with the greatest intensity of color signals in pathologic hips.

OBJECTIVE: To evaluate the medium-term clinical-functional results (minimum follow-up of six years) from total uncemented hip arthroplasty performed by means of a posterior minimally invasive access, in comparison with the traditional right lateral access.METHODS: In a comparative prospective study, 224 adult patients underwent elective total hip arthroplasty due to a diagnosis of primary or secondary osteoarthrosis. A group of 103 patients with posterior minimally invasive access was compared with a group of 121 patients with the traditional right lateral access. The mean length of follow-up among the patients of this sample was 7.2 years. We evaluated the clinical-functional and radiographic results and occurrences of loosening, along with any complications that occurred, with a minimum follow-up of six years.RESULTS: The clinical-functional analyses before the surgical procedure and six years afterwards were similar in the two groups (p = 0.88 and p = 0.55). One patient in the minimally invasive group underwent revision of the acetabular component and two patients in the control group underwent the same procedure (p = 0.46). The Trendelenburg clinical test, which showed weakness of the hip abductor musculature, was present in five patients operated using the traditional lateral route and absent in all those who underwent the minimally invasive procedure (p = 0.06). There was no difference regarding the radiographic parameters obtained, either in acetabular or in femoral positioning (p = 0.32 and p = 0.58).CONCLUSIONS: The medium-term clinical and radiographic results and the complication rates were similar between the patients who underwent total hip arthroplasty by means of the posterior minimally invasive access and those with the traditional lateral access. OBJETIVO: Avaliar o resultado clínico-funcional em médio prazo (seguimento mínimo de seis anos) da artroplastia total do quadril não cimentada feita por acesso minimamente invasivo (MIS) posterior comparado ao acesso lateral direto (LD) tradicional.MÉTODOS: Em um estudo comparativo prospectivo, 224 pacientes adultos foram submetidos à artroplastia total do quadril em caráter eletivo, por diagnóstico de osteoartrose primária ou secundária, e se comparou o grupo MIS posterior (103 pacientes) com o grupo acesso LD tradicional (121). A média de tempo de seguimento dos pacientes da amostra foi 7,2 anos. Avaliamos os resultados clínico-funcionais e radiográficos e a ocorrência de solturas, assim como complicações ocorridas com um seguimento mínimo de seis anos.RESULTADOS: A análise clínico-funcional antes e seis anos após o procedimento cirúrgico foi semelhante nos dois grupos (p = 0,88 e p = 0,55). Um paciente do grupo MIS foi submetido à revisão do componente acetabular e dois do grupo controle foram submetidos ao mesmo procedimento, p = 0,46. O teste clínico de Trendelenburg, que evidenciou uma fraqueza da musculatura abdutora do quadril, estava presente em cinco pacientes operados pela via lateral tradicional e ausente em todos os submetidos ao MIS (p = 0,06). Não houve diferença quanto aos parâmetros radiográficos obtidos tanto do posicionamento acetabular quanto do femoral (p = 0,32, p = 0,58).CONCLUSÕES: Os resultados em médio prazo, clínicos e radiográficos, e a taxa de complicações foram semelhantes entre os pacientes submetidos à artroplastia total do quadril pelo acesso posterior minimamente invasivo e pela via lateral tradicional.

The authors evaluated the usefulness of an ultrasound contrast agent (SHU 508A) to help identify different scintigraphic phases of revascularization of the femoral head in children with Legg-Calve-Perthes (LCP) disease. Eighteen unenhanced and contrast-enhanced power Doppler images and scintigrams of the pathologic hip in 18 children with LCP disease were compared. The scintigraphic stages of Conway's classification for LCP disease (stage A, recanalization; stage B, neovascularization) were compared with the degree of vascularity and mean peak enhancement ratios obtained from analysis of Doppler sonograms. Qualitatively, the patient's age (≤ or >7 years old) at the time of examination had a significant effect on the degree of vascularity visualized on postcontrast ultrasound images according to the scintigraphic stages. Quantitatively, there were no differences for overall mean peak enhancement ratios between stages A and B. Power Doppler ultrasound increased visualization of Doppler signals significantly but did not help in differentiating scintigraphic phases.

A total of 26 children (26 affected, 25 control hips) with Legg-Calve-Perthes’ disease (LCP) met the cooperation requirements for inclusion in the study using contrast-enhanced power Doppler sonography for assessment of changes in the revascularization flow. Sagittal sonograms were obtained before and at the peak effect of an intravenously injected microbubble-based contrast agent. Overall mean peak enhancement ratios were greater for children with later onset of disease (>5.8 years old) than for those with earlier onset (≤5.8) ( p = 0.02). The use of ultrasound (US) contrast agents improved depiction of proximal femoral vascularity in all regions of the femoral head: epiphysis ( n = 3, pre; n = 8, postcontrast), physis ( n = 12, pre; n = 21, postcontrast) and metaphysis ( n = 6, pre; n = 8, postcontrast) and resulted in a marked increase in the number of mean pixel intensity values within the physis ( p = 0.02). In conclusion, contrast-enhanced power Doppler sonography is an effective method for demonstration of changes from the revascularization process in LCP, particularly within the physis. O tratamento das infecções pós-estabilização intramedular das fraturas dos membros inferiores apresenta uma grande variedade de opções, desde a limpeza cirúrgica com manutenção da haste até a retirada da haste e colocação de fixador externo. O espaçador diafisário ainda é uma técnica pouco utilizada para o tratamento desse tipo de infecção, existindo poucos relatos na literatura sobre sua aplicação. No IOT HCFMUSP, esta técnica vem sendo empregada de maneira crescente e, no presente trabalho, temos o objetivo de descrever o protocolo de tratamento utilizado em nossa instituição, bem como a apresentação de nossa casuística inicial. O protocolo consiste na antibioticoterapia endovenosa, retirada da haste intra-medular, desbridamento cirúrgico do canal medular e colocação do espaçador diafisário. Revisamos retrospectivamente o prontuário de 11 pacientes com 13 fraturas, sendo cinco femorais e oito tibiais, submetidos à técnica apresentada. O tempo de seguimento variou de 6 a 36 meses, média de 14,27 meses, com resultados satisfatórios ocorridos em dez das treze fraturas estudadas, representando uma taxa de eficácia de 76,93%. Concluímos que o método representa uma boa alternativa para o tratamento destes casos, necessitando ainda novos trabalhos comparativos para a avaliação de suas vantagens e para difundir o uso do método.
