Previous studies from our research group have shown that trisomy 8 and the amplification of the 8q24.21 region is very frequent in gastric cancer (GC). Little is known about the role of most genes located in this region. Thus, the aim of this study was to understand the possible impact of transcriptional alterations and copy number variation (CNV) of four genes located in the 8q24.21 region - FAM49B, FAM84B, GSDMC and miR-5194 - in GC.Fifty-one to 85 matched pairs of tumoral and adjacent non-tumoral gastric tissues, from patients with primary GC, were used to analyze gene expression and CNV of the selected genes. We also included 29 H. pylori negative and gastritis negative gastric mucosa tissues from individuals without cancer obtained by endoscopy, as control samples.The expression of FAM49B, GSDMC and miR-5194 was higher in both tumoral and adjacent non-tumoral samples compared to the negative control. The expression of FAM84B showed no significant difference between tumoral samples and negative controls. However, the expression of FAM84B in the adjacent non-tumoral samples was higher compared to negative control and tumoral samples. Moreover, the higher expression of GSDMC was associated with T3 and T4 tumors, with tumors on stage III and IV and with advanced tumors. Higher copy numbers of FAM49B and GSDMC were associated with intestinal tumor type and with moderately or well-differentiated tumors. Higher copy number of FAM84B was associated with moderately or well-differentiated tumors. Furthermore, the expression of all four genes was positively correlated.All four genes are upregulated in GC and may play an important role in these neoplasms. GSDMC expression was associated with more aggressive tumors.

Gastric cancer (GC) is the third leading cause of cancer-related death worldwide. Very few therapeutic options are currently available in this neoplasia. The use of 5-Aza-2'-deoxycytidine (5-AZAdC) was approved for the treatment of myelodysplastic syndromes, and this drug can treat solid tumours at low doses. Epigenetic manipulation of GC cell lines is a useful tool to better understand gene expression regulatory mechanisms for clinical applications. Therefore, we compared the gene expression profile of 5-AZAdC-treated and untreated GC cell lines by a microarray assay. Among the genes identified in this analysis, we selected NRN1 and TNFAIP3 to be evaluated for gene expression by RT-qPCR and DNA methylation by bisulfite DNA next-generation sequencing in 43 and 52 pairs of GC and adjacent non-neoplastic tissue samples, respectively. We identified 83 candidate genes modulated by DNA methylation in GC cell lines. Increased expression of NRN1 and TNFAIP3 was associated with advanced tumours (P 0.05). We showed that increased NRN1 and TNFAIP3 expression seems to be regulated by DNA demethylation in GC samples: inverse correlations between the mRNA and DNA methylation levels in the promoter of NRN1 (P 0.05) and the intron of TNFAIP3 (P 0.05) were detected. Reduced NRN1 promoter methylation was associated with III/IV TNM stage tumours (P = 0.03) and the presence of Helicobacter pylori infection (P = 0.02). The identification of demethylated activated genes in GC may be useful in clinical practice, stratifying patients who are less likely to benefit from 5-AZAdC-based therapies. KEY MESSAGES: Higher expression of NRN1 and TNFAIP3 is associated with advanced gastric cancer (GC). NRN1 promoter hypomethylation contributes to gene upregulation in advanced GC. TNFAIP3 intronic-specific CpG site demethylation contributes to gene upregulation in GC. These findings may be useful to stratify GC patients who are less likely to benefit from DNA demethylating-based therapies.

Weight regain (WR) after bariatric surgery (BS) is frequent. The aim of this study was to evaluate whether the occurrence of psychiatric disorders would be associated with short- and long-term WR after BS. Ninety-six patients (77.6% female, age 40.2 ± 10.1 years, BMI of 50 ± 8.2 kg/m2) from the Obesity and Bariatric Surgery Outpatient Clinic of the Universidade Federal Sao Paulo completed the Questionnaire on Eating and Weight Patterns-Revised, the Beck Depression Inventory and an anxiety inventory to assess the occurrence of binge eating, depressive symptoms (DS) and anxious symptoms (AS) before and after short-term and long-term BS. Twenty-four months after BS, the prevalence of binge eating, depression and anxiety decreased from 100 to 13%, 100 to 15% and 43 to 4%, respectively. The mean WR of 35.2 ± 17.3% of weight loss occurred in nine patients after 24 months and was associated with binge eating (p = 0.002) but not with DS or AS. At long-term follow-up (12 ± 1.5 years), 67% had a mean WR of 50.3 ± 24.9%. The prevalence of binge eating, DS and AS were 48%, 46% and 63%, respectively, in this group, and significant associations were observed between WR and binge eating (p = 0.001), DS (p = 0.029) and AS (p = 0.001). Some studies have shown an increase in alcohol use disorders (AUD) after Roux-en-Y gastric bypass surgery (RYGB), but its relationship with binge eating disorder (BED) has not been fully explored. The purpose of this study was to determine the prevalence of AUD and BED after RYGB and also to evaluate if BED is predictive of late postoperative occurrence of AUD or BED. Patients (n = 46) submitted to RYGB, in a tertiary outpatient weight management service at a Federal University of Sao Paulo, Brazil, were tested for BED and AUD using the Questionnaire on Eating and Weight Patterns-Revised (QEWP-R) and AUDIT, respectively. BED was tested before surgery, while both disorders were evaluated with a follow-up period of 12 ± 1.6 years after RYGB. No patients reported AUD before RYBP. After a mean period of 12 years from surgery, ten patients (21.7%) were diagnosed with AUD. Before surgery, BED was present in 24 patients (52.2%) and it was detected in seven out of these 24 patients (29.2%) after RYGB. Thirteen new cases of BED (28.2%) were detected after surgery; total of 20 patients (43.5%) with BED. No association was found between pre- and postsurgery BED (p = 0.148). After RYGB, four out of 24 patients (16.6%) with presurgery BED developed AUD, and no association was found between presurgery BED and postsurgery AUD (p = 0.384). Seven out of ten patients (70%) with AUD after RYGB also developed BED, but no statistical significance was found between these two disorders (p = 0.061). The presence of BED before RYGB did not predict AUD and BED after RYGB. Nevertheless, factors involved in a possible association between BED and AUD after surgery remain to be determined. Postprandial hypertrygliceridemia is a known factor for cardiovascular disease and is often observed in patients with type 2 diabetes mellitus (T2DM) and visceral adiposity. Adiponectin is a hormone with antiatherogenic and anti-inflammatory effects, which decreases in obesity and T2DM subjects. The weight loss induced by diet or bariatric surgery could be restoring adiponectin levels.The aim of the study was to evaluate the impact of weight loss induced by bariatric surgery, which could restore adiponectin and triglycerides (TG) levels in obese and diabetic patients.Ten patients with T2DM (BMI 39.3+2.44) were evaluated before and at 7 and 90 days after Roux-en-Y gastric bypass (RYGB). A meal test was performed and plasma insulin, glucagon-like peptide-1 (GLP-1), glucose, TG, and adiponectin levels were measured at fasting and at 30, 60, 90, and 120 min postprandial.Seven days after surgery, significant reductions in the insulin resistance were observed, while TG and adiponectin levels remained unchanged during the meal test. Ninety days after surgery, TG and glucose levels decreased significantly at fasting, and postprandial, adiponectin, GLP-1, and insulin curves increased significantly after meal ingestion. Both changes in the area under the curve (AUC) of adiponectin correlated with changes in the AUC of TG (R=-0.64, P=0.003) and changes in AUC of adiponectin correlated with changes in total fat mass. No correlation was found between changes in insulin, GLP-1, and TG levels.The adiponectin levels may be involved in the mechanism responsible for high TG levels in obese and diabetic patients. These abnormalities can be reversed by RYGB.

The increased prevalence of overweight and obesity has reached alarming proportions worldwide and has serious health implications, including an association with an increase in metabolic syndrome. Among the methods to control metabolic syndrome, bariatric surgery plays an important role and can provide a significant improvement in the components of metabolic syndrome.The aim of this study was to determine the relationship between the preoperative body mass index (BMI) and the postoperative resolution of metabolic syndrome [using the National Cholesterol Education Program (NCEP ATP III criteria)] in patients undergoing Roux-en-Y gastric bypass (RYGB).Retrospective analysis of a consecutive series of cases, stratified by BMI into three groups (group 1, BMI40 kg/m(2), group 2, BMI 40-49.9 kg/m(2), and group 3, BMI 50 ≥ kg/m(2)) consisting of both sexes between 20 and 60 years of age. The cohort consisted of 149 patients undergoing RYGB. Anthropometric, biochemical, and clinical evaluations were performed preoperatively and then at 30 and 180 days postoperatively.The average age was 40 years, and the patients were predominately female (72%). At the end of the study period, all groups showed a significant reduction in metabolic syndrome compared with preoperative levels. Logistic regression showed a higher percentage of metabolic syndrome in patients in group 3 after 180 days.There was significant resolution of metabolic syndrome in all groups, independently of the preoperative BMI. However, in group 3 (BMI ≥ 50 kg/m(2)), 22% of individuals still presented with metabolic syndrome at 180 days postoperatively (P=0.03).

This study aims to evaluate the thickness of the femoral quadriceps and biceps brachii and brachialis muscles bilaterally and the adjacent subcutaneous fat in patients undergoing gastric bypass Roux-en-Y before and after surgery, using ultrasound as the diagnostic method of choice. We studied 12 patients undergoing this surgical method preoperatively and during the first, third, and sixth postoperative months. During these periods, patients were evaluated by ultrasound to determine the thickness of subcutaneous adipose tissue and muscle of the upper and lower limbs. Postoperatively, these patients showed a reduction in the thickness of the upper and lower extremities muscle and adipose tissue as compared to their preoperative values. There was a significant difference in the loss of muscle thickness in all postoperative months and in the thickness of fatty tissue in the sixth month after surgery, compared to the preoperative muscle and fatty tissue thickness. Ultrasound can be considered as the diagnostic method of choice when assessment of the fat and lean body mass is required in morbidly obese patients before and after bariatric surgery.

Retardo do esvaziamento gástrico de alimentos sólidos ocorre mais intensamente depois de vagotomia gástrica proximal (VGP) que após seromiotomia da curvatura gástrica menor anterior com vagotomia troncular posterior (SMAVTP), podendo estar relacionado à hipóxia da parede gástrica e, principalmente, do marcapasso gástrico. OBJETIVO: Determinar a repercussão da desvascularização cirúrgica da curvatura menor e do fundo gástrico no estômago como um todo e, particularmente, na região do marcapasso. MÉTODOS: Mediu-se a saturação de oxigênio da hemoglobina (SpO2) intra-operatória, por oximetria de pulso, da parede gástrica anterior em 20 portadores de úlcera duodenal crônica, alocados casualmente em dois grupos de 10 pacientes para tratamento cirúrgico por VGP ou SMAVTP. RESULTADOS: As medições, obtidas antes de fundoplicatura parcial, mostraram que o fundo gástrico e a curvatura menor proximal tiveram redução significante da SpO2 quando comparados ao corpo gástrico (p < 0,05); que a SpO2, na região do marcapasso gástrico, não sofreu alteração estatisticamente significante; que a associação entre ligadura dos vasos gástricos curtos e VGP ou SMAVTP reduziu significantemente a SpO2 (p < 0,05); e que a técnica da VGP resultou em SpO2 significantemente menor que a SMAVTP (p < 0,05). CONCLUSÃO: A VGP, quando associada à ligadura dos vasos gástricos curtos, produz alterações isquêmicas agudas da parede gástrica, na região da curvatura menor proximal e do fundo gástrico, mais intensas que a SMAVTP. A região do marcapasso gástrico não sofre hipóxia imediatamente após os procedimentos operatórios.Delayed gastric emptying of solid food is greater after proximal gastric vagotomy (PGV) than after anterior lesser curve seromyotomy with posterior truncal vagotomy (ASPTV) and may be due to the hypoxia in the gastric wall and mainly in the gastric pacemaker. PURPOSE: To verify if operative devascularization of the lesser curve and the gastric fundus could affect the entire stomach and particularly the pacemaker area. METHODS: Measures of intraoperative oxyhemoglobin saturation (SpO2) were taken by pulse oximetry on the anterior gastric wall in 20 patients with chronic duodenal ulcer, randomly allocated in two groups of 10 individuals for surgical treatment, by PGV or ASPTV. RESULTS: Measurements, before partial fundoplication, showed that the area of the proximal lesser curve and the gastric fundus had a significant decrease in the SpO2 when compared to the gastric corpus (p < 0,05); the SpO2 was not reduced significantly by the procedures on the pacemaker area; the ligature of short gastric vessels, in association with PGV or ASPTV, reduced significantly the SpO2 (p < 0,05); and, PGV resulted in a SpO2 significantly lower than in ASPTV (p < 0,05). CONCLUSION: When PGV is associated with the ligature of the short gastric vessels, it causes acute ischemic changes in the proximal lesser gastric curve and in the fundus more intensively than ASPTV. The gastric pacemaker area does not present hypoxia immediately after operative procedures.

This study aimed to investigate urinary abnormalities and responses to an acute oxalate load as an indirect assessment of the intestinal absorption of oxalate in this population.Design, setting, participants, & measurements Twenty-four hour urine specimens were collected from 61 patients a median of 98 months after BS (post-BS) as well as from 30 morbidly obese (MO) participants; dietary information was obtained through 24-hour food recalls. An oral oxalate load test (OLT), consisting of 2-hour urine samples after overnight fasting and 2, 4, and 6 hours after consuming 375 mg of oxalate (spinach juice), was performed on 21 MO and 22 post-BS patients 12 months after BS. The surgical treatment of obesity ameliorates metabolic abnormalities in patients with type 2 diabetes. The objective of this study was to evaluate the early effects of Roux-en-Y gastric bypass (RYGB) on metabolic and hormonal parameters in patients with type 2 diabetes (T2DM).Ten patients with T2DM (BMI, 39.7 ± 1.9) were evaluated before and 7, 30, and 90 days after RYGB. A meal test was performed, and plasma insulin, glucose, glucagon, and glucagon-like-peptide 1 (GLP-1) levels were measured at fasting and postprandially.Seven days after RYGB, a significant reduction was observed in HOMA-IR index from 7.8 ± 5.5 to 2.6 ± 1.7; p0.05 was associated with a nonsignificant reduction in body weight. The insulin and GLP-1 curves began to show a peak at 30 min after food ingestion, while there was a progressive decrease in glucagon and blood glucose levels throughout the meal test. Thirty and 90 days after RYGB, along with progressive weight loss, blood glucose and hormonal changes remained in the same direction and became more expressive with the post-meal insulin curve suggesting recovery of the first phase of insulin secretion and with the increase in insulinogenic index, denoting improvement in β-cell function. Furthermore, a positive correlation was found between changes in GLP-1 and insulin levels measured at 30 min after meal (r = 0.6; p = 0.000).Our data suggest that the RYGB surgery, beyond weight loss, induces early beneficial hormonal changes which favor glycemic control in type 2 diabetes.

Gallstones have been frequently diagnosed after Roux-en-Y gastric bypass (RYGBP). Gallbladder stasis associated with duodenal exclusion may play a role in their pathogenesis.Gallbladder emptying was studied before and on the 30th and 31st postoperative days (POD) after RYGBP in 20 morbidly obese patients. Gallbladder volume after fasting and every 15 min during a 2-h period following administration of a standard liquid meal was determined by sonography. On the 31st POD, the meal was administered through the gastrostomy in order to promote its transit through the duodenum. Fasting volume (FV), maximum ejection fraction (Max EF), and residual volume (RV) were determined. Biliary sludge and calculi were investigated after 1 and 6 months, respectively.FV was 39.4 +/- 20.2 ml, 50.1 +/- 22.7 ml, and 47.9 +/- 23.4 ml, respectively, for the preoperative and two postoperative assessments (P = 0.09). RV was 7.6 +/- 8.7 ml, 25.1 +/- 20.0 ml, and 24.6 +/- 20.9 ml; and Max EF was 80.5 +/- 20.9%, 54.3 +/- 21.4%, and 50.5 +/- 29.0%, respectively, for the pre-, postoral, and postgastrostomy infusion measurements. There was only a significant difference between the preoperative value and the two postoperative values (P0.001). Biliary sludge was detected in 65% of the patients and 46% of them subsequently developed gallstones.Gallbladder emptying became significantly compromised after RYGBP. This impairment was unrelated to duodenal exclusion but it was associated with biliary sludge and stone formation.

Background: Frequent regurgitation is a common complication following Roux-en-Y gastric bypass (RYGBP). This study investigated the risk of becoming a chronic regurgitator, by considering silicone ring size and lower esophageal sphincter (LES) function, and their relationship with weight loss. Methods: 80 morbidly obese patients were randomly selected to undergo surgery using ring length of 62 mm (40 patients, group A) or 77 mm (40 patients, group B), with 6 months' postoperative follow-up. Preoperative esophageal manometry parameters were correlated with occurrence of chronic postoperative regurgitation. Patients were considered to present chronic regurgitation when this occurred on >10 days/month. Results: The groups were homogeneous regarding age, gender, race, weight, BMI (47.8±6.1 vs 50.2±6.4 kg/m2) and obesity-related diseases. There were 15% more chronic regurgitators in group A than in group B. Chronic regurgitators in group A lost more weight than chronic regurgitators in group B (P=0.026) or non-chronic regurgitators in group A (P=0.016). INTRODUÇÃO: Não obstante os bons resultados e reprodutibilidade da derivação gástrica em Y de Roux (DGYR), discutem-se ainda aspectos técnicos que podem influenciar tanto o resultado na perda de peso, como a qualidade de vida pós-operatória (PO). Atribui-se hoje a efeitos restritivos, como volume da bolsa gástrica e sua velocidade de esvaziamento, assim como a fatores metabólicos, o resultado da cirurgia. As influências morfofuncionais não são totalmente compreendidas. OBJETIVO: Avaliar a influência do volume da bolsa gástrica e sua velocidade de esvaziamento sobre a perda de peso e a tolerância alimentar no PO de DGYR. MÉTODO: Pacientes em PO tardio de DGYR; foram avaliados a percentagem de perda de excesso de peso (%PEP), reganho de peso (%PEPreganho), tolerância alimentar por questionário de tolerância (ES), volume da bolsa (V) por tomografia computadorizada tridimensional (TC3D) e velocidade de esvaziamento da bolsa por cintilografia, avaliada por percentual de retenção (%Ret) em 1, 2 e 4 horas. Foram identificados pontos de corte para V e %Ret em 1 hora (%Ret1). A partir destes parâmetros a amostra foi categorizada por V e %Ret1, e os dados correlacionados com resultados em perda de peso e tolerância alimentar. RESULTADOS: Foram estudados 67 pacientes. A idade mediana foi 51 anos, 91,04% sexo feminino, e IMC mediano inicial de 51,44 kg/m2. O tempo de seguimento PO mediano foi de 47 meses. A amostra apresentou 60,27%PEPnadir como mediana da maior perda de peso PO e 16,13%PEPreganho. A mediana de V foi de 28mL; a %Ret em 1, 2, 4 horas foras foram 8%, 2%, 1%, respectivamente. A pontuação mediana do ES foi 21. Os pontos de corte considerados foram V=40mL, e %Ret1 de 12% e 25%. Foram comparados subgrupos por V (V 40mL) e por %Ret1 (%Ret1 =25%). Na análise categorizada por V, houve associação entre V 40mL: %Ret1=44, %Ret2=13,5, p=0,045). Na análise categorizada por %Ret1, observou-se associação entre maior velocidade de esvaziamento em 1 hora e melhor evolução ponderal tardia, representada por menor %PEPreganho (p=0,036), e maior %PEPatual (p=0,033) no grupo com %Ret1 24) e menor %Ret1 (p=0,003). CONCLUSÕES: Houve associação do volume da bolsa gástrica com a velocidade de esvaziamento. O esvaziamento mais rápido correlacionou-se com maior %PEP tardia, menor reganho de peso e melhor tolerância alimentar. Estes dados sugeriram que a presença de bolsa gástrica pequena, com menos de 40 mL, e com rápido esvaziamento, são parâmetros importantes para adequado resultado tardio na DGYR. O uso da TC3D e da cintilografia para avaliação morfofuncional da DGYR podem contribuir para investigação do reganho de peso e intolerância alimentar PO INTRODUCTION: Despite the good results and good reproducibility of the Roux-en-Y gastric bypass (GBP), discussions about technical aspects still remain. Postoperative anatomy can influence the result in weight loss, as the postoperative (PO) quality-of-life. Surgical results are nowadays attributed to restriction, promoted by the small neo-stomach volume and its emptying, as well as metabolic factors. These anatomical and functional influences on surgical results are often poorly evaluated, and yet not fully understood. OBJECTIVE: To evaluate the influence of the gastric pouch volume and its emptying rate on weight loss, and food tolerance, in the late PO of GBP. METHOD: Patients followed-up at HC-FMUSP in late PO of GBP; percentage of excess weight loss (%EWL), weight regained (%EWLregain), food tolerance with specific questionnaire (Suter Score, SS), gastric pouch volumetry (V) by three-dimensional computed tomography (CT3D), and pouch emptying rate by scintigraphy, evaluated by retention percentage (%Ret) in 1, 2, 4 hours, were assessed. Cutoffs were identified for %Ret at 1 hour (%Ret1) and V. From these parameters the sample was categorized by V and %Ret1, looking for associations between V and %Ret and results in weight loss and food tolerance. RESULTS: Eighty patients were enrolled and 67 completed the study. The median age was 51 years, 91.04% were female, with initial median BMI of 51.44 kg/m2. The PO median follow-up time was 47 months. The sample showed median of 60.27% as EWLnadir as PO better weight loss, with 16.13% of EWLregain. The median V was 28mL; %Ret at 1, 2, 4 hours were 8%, 2%, 1%, respectively. The median score of SS was 21. The cutoff points considered were V=40mL, and %Ret1=12% and 25%. Subgroups were compared by V (V 40mL) and Ret1% (%Ret1 = 25%). The categorized analysis by V found associations between V 40mL: %Ret1=44, %Ret2=13.5, p=0.045). In the categorized analysis by %Ret1, it was found association between higher emptying speed in 1 hour and higher late PO weight loss, represented by lower %EWLregain (p=0.036), and higher late %EWL (p=0.033) in the group with %Ret1= 25%. Better food tolerance, assessed by Suter questionnaire (SS > 24), was associated with lower %Ret1 (p=0.003). CONCLUSIONS: There was an association between gastric pouch volume and emptying rate. Smaller gastric pouch have shown faster gastric emptying. The fastest gastric emptying was correlated with higher late %EWL, less weight regain and better PO food tolerance. These data suggest that the construction of small gastric pouch, with less than 40 mL, and with rapid emptying rate, are important parameters for good outcomes in late postoperative in GBP. The use of CT3D and scintigraphy for morphological and functional assessment of GBP can contribute in the evaluation of possible causes of late postoperative weight regain and food intolerance

INTRODUÇÃO: A obesidade é uma doença universal de prevalência crescente e vem adquirindo proporções epidêmicas, sendo um dos principais problemas da saúde pública modernos. Constitui consenso que a abordagem clínica geralmente é ineficaz e a cirurgia bariátrica se apresenta como melhor opção de tratamento. A cirurgia bariátrica é atualmente a única modalidade terapêutica a oferecer perda de peso maciça e mantida ao longo do tempo a pacientes obesos mórbidos. OBJETIVO: Determinar se um programa de fisioterapia respiratória pré-operatória melhora a função pulmonar do paciente e verificar a relação entre quadro (clínico apnéia do sono e/ou espirometria) e circunferências do pescoço, tórax e abdominal. MATERIAIS E MÉTODOS: Foram selecionados 34 pacientes portadores de obesidade mórbida e candidatos a cirurgia bariátrica, com sinais clínicos de apnéia do sono (ronco, sono agitado, dor de cabeça matinal e sono diurno excessivo) e/ou espirometria com padrão restritivo, obstrutivo, inespecífico ou misto. Os pacientes foram selecionados de acordo com sua disponibilidade para aderir ao preparo pré-operatório e que aceitaram participar do estudo assinando o termo de consentimento livre e esclarecido. Os pacientes eram de ambos os sexos, idade entre 20 a 75 anos. Todos os pacientes foram submetidos a duas avaliações fisioterapêuticas: 15 dias antes da cirurgia e a outra na véspera, sendo analisadas as seguintes variáveis: circunferências do pescoço, tórax, abdome e quadril, IMC, FR, SpO2, capacidade vital lenta, volume minuto , PImax, PEmax , espirometria, índice de percepção de esforço de Borg e sinais clínicos de apnéia do sono. Foram constituídos dois grupos, grupo controle G1 e grupo experimental G2. A diferenciar os grupos de estudo, um programa de fisioterapia respiratória pré-operatória foi oferecido aos doentes G2, mas não aos doentes G1. RESULTADOS: Dentre as variáveis analisadas, mostraram diferença estatisticamente significante principalmente as relações: VEF1/CVF 1 e VEF1/CVF 2 do G1 e G2 (p-valor =0,030 e 0,005); PImáx (p-valor=0,032) e FR (p-valor=0,001), IPE de Borg (p=0,001), SpO2 (p=0,001) do G2. Os pacientes com sinais clínicos de apnéia têm circunferência de tórax e abdominal maiores em relação aos pacientes sem sinais clínicos de apnéia (p=0,012 e p=0,021). CONCLUSÃO : Um programa de fisioterapia respiratória é capaz de melhorar volumes, capacidades pulmonares e força da musculatura respiratória em pacientes obesos mórbidos em preparo pré-operatório para cirurgia bariátrica. BACKGROUND: Obesity is an increasingly prevalent universal disease acquiring epidemic proportions and is one of the main problems of modern public healthcare. There is consensus that the clinical approach is usually ineffective in more serious cases of obesity and bariatric surgery constitutes a better treatment option. Bariatric surgery is currently the best therapeutic modality for massive weight loss maintained over time and consequently offers a cure or control of associated diseases, which is its main goal. AIM: The aim of the present study was to determine the effect of an intensive preoperative chest physiotherapy program on morbidly obese patients scheduled to undergo bariatric surgery with the intention of improving lung function and verify the relationship between clinical (sleep apnea and spirometry) and circumferences of the neck, chest and abdominal. MATERIALS AND METHODS: Thirty-four morbidly obese candidates for bariatric surgery with clinical symptoms of sleep apnea (snoring, restless sleep, morning headaches and excessive daytime sleep) and/or a restrictive, obstructive, nonspecific or mixed spirometry pattern were selected. The selection of patients for surgical treatment was supervised by the Department of Obesity Surgery of the University Hospital of the University of São Paulo. Patients were selected based on their availability regarding the preoperative therapy, willingness to participate in the study and having signed a term of informed consent. Male and female participants between 20 and 75 years of age were included. All were submitted to preoperative chest physiotherapy program and two evaluations: 15 days prior to surgery and a second 1 day prior to surgery. The following variables were analyzed: circumference of the neck, chest, abdomen and hip; body mass index (BMI); respiratory rate (RR); arterial oxygen saturation (SpO2); slow vital capacity (SVC); minute volume (Vol-min); maximal inspiratory pressure (MIP); spirometry; Borg Rate of Perceived Exertion (RPE); and clinical signs of sleep apnea. Two groups: control group (G1) and experimental group (G2). The G2 was submitted a program of preoperative respiratory therapy. RESULTS: There were favorable significant differences between evaluations for the following variables: FEV1/FVC 1 and VEF1/CVF 2 of G1 and G2 (p-valor =0.030 and 0.005) MIP (p = 0.032); RR (p = 0.001); Borg RPE (p = 0.001); SpO2 (p = 0.001) of G2. CONCLUSION: A chest physiotherapy program is capable of improving lung volume, lung capacity and respiratory muscle strength in morbidly obese patients in preoperative preparation for bariatric surgery. There is correlation between thorax and abdominal circumference and clinical symptoms of obstructive sleep apnea.

Introdução: O adenocarcinoma gástrico é uma doença de elevada incidência e alta mortalidade. A gastrectomia com linfadenectomia é tratamento potencialmente curativo, promovendo controle loco-regional da doença e fornecendo material para análise histopatológica. Para o adequado estadiamento dos pacientes é recomendado que pelo menos 16 linfonodos sejam examinados pela patologia, entretanto, espera-se maior sobrevida quando >= 30 linfonodos são avaliados, mesmo em pacientes com tumores precoces. A justificativa para este achado é o sub-estadiamento de pacientes com poucos linfonodos examinados. Linfonodos pequenos são particularmente difíceis de serem encontrados, mas podem conter metástases e impactar negativamente na sobrevida. Visando facilitar sua identificação, soluções clareadoras de gordura foram propostas, entretanto não há evidência clara de seu benefício clínico. Objetivos: Comparar as soluções de Carnoy e de formalina neutra tamponada em relação ao número absoluto de linfonodos encontrados na peça cirúrgica de pacientes submetidos a gastrectomia. Averiguar se linfonodos retirados cirurgicamente são perdidos com a fixação em formalina e, caso isso ocorra, se este fato é relevante para o estadiamento. Observar se o protocolo de pesquisa influenciou o número de linfonodos encontrados. Métodos: Cinquenta produtos de gastrectomia subtotal com linfadenectomia D2 por adenocarcinoma gástrico foram randomizados para fixação em Carnoy ou formalina com posterior dissecção da peça em busca de linfonodos. Após a dissecção do grupo Formalina, a gordura residual a ser desprezada foi imersa em Carnoy e reavaliada posteriormente. Os dados de 25 gastrectomias D2 operadas previamente ao estudo também foram avaliados. Resultados: A média de linfonodos encontrados nos grupos Carnoy e Formalina foi de 50,4 e 34,8; respectivamente (p < 0,001). Na gordura residual foram encontrados linfonodos em todos os casos (média 16,9 linfonodos), elevando a média do grupo Formalina para 51,7 (valor similar ao do grupo Carnoy, p=0,809). Com exceção de 1 linfonodo de 7mm, todos os demais encontrados na gordura residual mediram =30 lymph nodes are examined, even for early gastric cancer. The understaging of patients with less examined lymph nodes justifies this findings. Small lymph nodes are particularly difficult to identify and fat clearing solutions have been proposed to improve this, but there is no evidence of their clinical benefit. Objectives: Compare Carnoy\'s solution (CS) and formalin in terms of the total number of examined lymph nodes following gastrectomy. Verify if surgically retrieved lymph nodes are lost with the formalin fixation and if this fact is clinically significant. Observe if a research protocol influences the number of examined lymph nodes. Methods: Fifty specimens of gastrectomy with D2 lymphadenectomy were randomized for fixation in CS or formalin with posterior dissection in search for lymph nodes. In the Formalin group, the residual fat to be discarded was immersed in CS and dissected again. Data from 25 D2 gastrectomies performed previously the present study were retrospectively analyzed. Results: The medium number of examined lymph nodes was 50.4 and 34.8 for CS and formalin, respectively (p < 0.001). Lost lymph nodes were found in all cases in the Residual Fat group (medium 16.9), this increased the Formalin group average to 51.7 (which is similar to the CS group, p=0.809). With one exception (7mm), all other examined lymph nodes in the Residual Fat group measured
