It is increasingly recognized that Alzheimer’s disease (AD) exists before dementia is present and that shifts in amyloid beta occur long before clinical symptoms can be detected. Early detection of these molecular changes is a key aspect for the success of interventions aimed at slowing down rates of cognitive decline. Recent evidence indicates that of the two established methods for measuring amyloid, a decrease in cerebrospinal fluid (CSF) amyloid β1−42 (Aβ1−42) may be an earlier indicator of Alzheimer’s disease risk than measures of amyloid obtained from Positron Emission Tomography (PET). However, CSF collection is highly invasive and expensive. In contrast, blood collection is routinely performed, minimally invasive and cheap. In this work, we develop a blood-based signature that can provide a cheap and minimally invasive estimation of an individual’s CSF amyloid status using a machine learning approach. We show that a Random Forest model derived from plasma analytes can accurately predict subjects as having abnormal (low) CSF Aβ1−42 levels indicative of AD risk (0.84 AUC, 0.78 sensitivity, and 0.73 specificity). Refinement of the modeling indicates that only APOEε4 carrier status and four plasma analytes (CGA, Aβ1−42, Eotaxin 3, APOE) are required to achieve a high level of accuracy. Furthermore, we show across an independent validation cohort that individuals with predicted abnormal CSF Aβ1−42 levels transitioned to an AD diagnosis over 120 months significantly faster than those with predicted normal CSF Aβ1−42 levels and that the resulting model also validates reasonably across PET Aβ1−42 status (0.78 AUC). This is the first study to show that a machine learning approach, using plasma protein levels, age and APOEε4 carrier status, is able to predict CSF Aβ1−42 status, the earliest risk indicator for AD, with high accuracy.

- Abstract
Introduction: There is conflicting evidence whether high-density lipoprotein cholesterol (HDL-C) is a risk factor for Alzheimer's disease (AD) and dementia. Genetic variation in the cholesteryl ester transfer protein (CETP) locus is associated with altered HDL-C. We aimed to assess AD risk by genetically predicted HDL-C. Methods: Ten single nucleotide polymorphisms within the CETP locus predicting HDL-C were applied to the International Genomics of Alzheimer's Project (IGAP) exome chip stage 1 results in up 16,097 late onset AD cases and 18,077 cognitively normal elderly controls. We performed instrumental variables analysis using inverse variance weighting, weighted median, and MR-Egger. APOE epsilon 4, the most significant genetic risk factor for Alzheimer disease (AD), may mask effects of other loci. We re-analyzed genome-wide association study (GWAS) data from the International Genomics of Alzheimer's Project (IGAP) Consortium in APOE epsilon 4+ (10 352 cases and 9207 controls) and APOE epsilon 4 - (7184 cases and 26 968 controls) subgroups as well as in the total sample testing for interaction between a single-nucleotide polymorphism (SNP) and APOE e4 status. Suggestive associations (P < 1x10(-4)) in stage 1 were evaluated in an independent sample (stage 2) containing 4203 subjects (APOE epsilon 4+: 1250 cases and 536 controls; APOE epsilon 4 -: 718 cases and 1699 controls). Among APOE epsilon 4 - subjects, novel genome-wide significant (GWS) association was observed with 17 SNPs (all between KANSL1 and LRRC37A on chromosome 17 near MAPT) in a meta-analysis of the stage 1 and stage 2 data sets (best SNP, rs2732703, P = 5.8 x 10(-9)). Conditional analysis revealed that rs2732703 accounted for association signals in the entire 100-kilobase region that includes MAPT. Except for previously identified AD loci showing stronger association in APOE epsilon 4+ subjects (CR1 and CLU) or APOE epsilon 4 - subjects (MS4A6A/MS4A4A/MS4A6E), no other SNPs were significantly associated with AD in a specific APOE genotype subgroup. In addition, the finding in the stage 1 sample that AD risk is significantly influenced by the interaction of APOE with rs1595014 in TMEM106B (P = 1.6 x 10(-7)) is noteworthy, because TMEM106B variants have previously been associated with risk of frontotemporal dementia. Abundant basic science evidence suggests that estrogen has neuroprotective and neuromodulatory effects; however, findings from the Women's Health Initiative (WHI), a large trial using conjugated equine estrogens (CEEs), have raised further concerns over the feasibility of traditional CEEs because of the increased risk of serious adverse effects, including cognitive declines. Soy isoflavones may serve as a critically needed alternative therapy to traditional hormone replacement therapy. The few human research projects that have examined the neurocognitive effects of soy isoflavones offer intriguing but as yet preliminary support for isoflavones' beneficial actions on cognition. Careful and systematic study is needed to ascertain the merit of this potential therapeutic. OBJECTIVES: This ongoing investigation is evaluating the effects of soy isoflavones on cognition in older men and postmenopausal women. Data from this healthy aging study will be used to design intervention trials in a patient population (older adults with Alzheimer's disease). HYPOTHESIS: It is hypothesized that isoflavone supplements will ameliorate age-associated cognitive declines for healthy older adults (i.e., decrease rate of decline, stabilize or enhance cognitive abilities) as compared with subjects on placebo. SPECIFIC AIMS: 1) Characterize the cognition-enhancing efficacy of isoflavones. 2) Characterize the close-response relationship between plasma isoflavones levels and cognition. 3) Identify the adverse effects profile of isoflavone treatment for healthy older men and women. 4) Refine intervention strategies for subsequent trials. METHODS: Thirty cognitively healthy older adults will be enrolled in this randomized placebo-controlled double-blind parallel-group design clinical pilot study. Subjects will receive either 100 mg/d of soy isoflavones or a placebo for 6 mo. Cognitive evaluations will be conducted at baseline and at 1, 3, and 6 mo after initiation of study medications and 2 mo after termination of treatment. Blood, collected at each treatment visit, will be assayed for plasma isoflavone levels (genistein, daidzein, and equol) and plasma estradiol. PRELIMINARY RESULTS: Cognitive data, collected after 1 mo of treatment with isoflavones, are currently available for 8 subjects. These data were used to calculate change from baseline. Figures 3 and 4 depict these data. Although still preliminary, these data suggest that isoflavone treatments are associated with improvements in memory and executive function when compared with placebo. SIGNIFICANCE: Given recent findings from the WHI regarding cognitive effects of CEEs plus progesterone, the need for alternatives to traditional estrogen therapies is all the more pressing. The soy isoflavones may offer men and women a novel and safe alternative therapeutic for cognitive changes associated with normal aging.

Introduction: Concerns about the effects of HMG-CoA reductase inhibitors (‘statins’) on health-related quality of life may contribute to their underuse in older adults with and at risk for cardiovascular disease. These concerns also may prevent clinicians from enrolling older patients in clinical trials assessing the efficacy of statins as a preventive therapy for Alzheimer's disease. Objective: To determine the effects of pravastatin and tocopherol (vitamin E), alone and in combination, on health-related quality of life in older adults. Study design: Double-blind, randomised, placebo-controlled, crossover study. Participants: Forty-one community-dwelling men and women aged ≥70 years with low-density lipoprotein-cholesterol (LDL-C) ≥3.62 mmol/L (140 mg/dl) participated. Methods: Subjects received pravastatin for 6 months then pravastatin plus tocopherol for an additional 6 months (group 1), or tocopherol for 6 months then pravastatin plus tocopherol for an additional 6 months (group 2). Dosages were pravastatin 20mg daily and tocopherol 400IU daily. Main outcome measures: The following health-related quality-of-life measures were assessed at baseline, after 6 months and after 1 year: health perception, depression, physical function, cognitive function and sleep behaviour. In addition, data on adverse effects and laboratory abnormalities were obtained. Results: Pravastatin reduced levels of total cholesterol (-21%, p < 0.001) and LDL-C (-29%, p < 0.001). Health-related quality-of-life scores, physical adverse effects, muscle enzyme levels and liver function tests did not change after 12 months of therapy with pravastatin, tocopherol or their combination. Conclusion: Both pravastatin and tocopherol have a good safety profile, are well tolerated and do not adversely affect health-related quality of life in older patients with hypercholesterolaemia. Given the significant beneficial cardiovascular effects of statin therapy in older adults and the potential role of statins in prevention of Alzheimer's disease, concerns about adverse effects on quality of life should not deter use of these medications in this population.
