Abstract

From January 1, 2018, through October 9, 2019, 82 HIV diagnoses occurred among people who inject drugs (PWID) in Cabell County, West Virginia. Increasing the use of HIV preexposure prophylaxis (PrEP) among PWID was one of the goals of a joint federal, state, and local response to this HIV outbreak. Through partnerships with the local health department, a federally qualified health center, and an academic medical system, we integrated PrEP into medication-assisted treatment, syringe services program, and primary health care settings. During the initial PrEP implementation period (April 18–May 17, 2019), 110 health care providers and administrators received PrEP training, the number of clinics offering PrEP increased from 2 to 15, and PrEP referrals were integrated with partner services, outreach, and testing activities. The number of people on PrEP increased from 15 in the 6 months before PrEP expansion to 127 in the 6 months after PrEP implementation. Lessons learned included the importance of implementing PrEP within existing health care services, integrating PrEP with other HIV prevention response activities, adapting training and material to fit the local context, and customizing care to meet the needs of PWID. The delivery of PrEP to PWID is challenging but complements other HIV prevention interventions. The expansion of PrEP in response to this HIV outbreak in Cabell County provides a framework for expanding PrEP in other outbreak and non-outbreak settings. [ABSTRACT FROM AUTHOR]

Abstract

Expeditious linkage and consistent engagement in medical care is important for people with HIV's (PWH) health. One theory on fostering linkage and engagement involves HIV status disclosure to mobilize social support. To assess disclosure and social support's association with linkage and engagement, we conducted a qualitative study sampling black and Latino men who have sex with men (MSM of color) in the U.S. Participants' narratives presented mixed results. For instance, several participants who reported delaying, inconsistent access, or detachment from care also reported disclosing for support purposes, yet sporadic engagement suggests that their disclosure or any subsequent social support have not assisted. The findings contribute to the literature that questions disclosure and social support's influence on care engagement, especially when decontextualized from circumstances and intentions. Our findings suggest the mechanics of disclosure and social support require planned implementation if intending to affect outcomes, especially among MSM of color. 41. 'Still Here, Still Queer' and We Ain't Going Nowhere: A Qualitative Study of Community During a Second-wave of Activity

Abstract

Are we witnessing the emergence of queer community? To answer this question, I interviewed self-identified queer people living in Atlanta, Georgia. During one-on-one and relational interviews, 31 participants reflected on how they understand and live queer, as well as socialize with other queers. An intention of this study is to advance theory; as such, this analysis inspected tenets asserted by ?first wave? theoreticians and activists of the 1980s and 1990s. To test theory, I attend to queer as fluid, non-normative and diverse. The participants viewed their queerness in sexuality, gender, and political terms congruent with a first-wave framework. On the whole, participants supported the emergence of queer community, yet offered a cautionary tale as to whether collective queer will be able to achieve its political goals. ?Still here, still queer? extends theory in the direction of shared identity and code for conduct, essential dynamics of community.

45. Social Connectivity and Pleasure: The Rational Choice of Methamphetamine Use During Sex Among Gay and Bisexual Men.

Abstract

Sex helps bind gay and bisexual men fostering distinct meanings when combined with methamphetamine (meth). Few studies illuminate on gay and bisexual men's meth use during sex as a rational choice process dominated by social connectivity and pleasure. The process is complex and unpacking it exposes why these men engage in these behaviors. This study explored meth use during sex through semi-structured interviews revealing gay and bisexual men are knowledgeably choosing to use meth during sex given they reflect on the complexity of the situation. Existing research supports the findings, yet more often frames the choice as irrational due to behaviors' association with disease. I assert social connectivity and pleasure drives the behaviors that are explained by a rational choice framework. This framework involves the broader context in which risks are being assessed alongside benefits as well as the compelling properties of the drug. This assertion moves us beyond a narrow focus of dichotomous "good/bad" outcomes toward an understanding of lived experiences. Stories of connecting with others through pleasurable sexual interactions where meth is present also involve problematic outcomes and loss of a sense of control. 47. Physician Professional Prestige and Perceptions of Increased Governmental Oversight of Prescription Drug Practices.

Abstract

Increasing government oversight on medical providers is decreasing professional autonomy and authority. Concern and action by government first evolved around healthcare cost increases and practice abuses. More recent actions attempt to address epidemic level prescription drug abuse. The current study surveyed licensed medical providers on their perceptions regarding practice monitoring, prescription drug abuse among their patients, and increased criminal justice oversight intending to curb epidemic drug abuse rates. The findings suggest with increased professional prestige, measured by medical specialization median level incomes, comes distinct perceptions regarding who should have access to patient data, thus reducing professional autonomy; higher professional prestige predicts how likely the provider is to acknowledge prescription drug abuse and support criminal justice oversight, and both are a negative relationship. The implications goes to the ever shifting "engines" of medicalization toward a more complex healthcare system in which providers are becoming less authoritative and autonomous.
