Concomitantly to the coronavirus disease 2019 (COVID-19), in the child population there was already another pandemic wave in progress: childhood obesity. Numerous studies in adults have been carried out and describe obesity as an independent risk and prognostic factor for the severity of COVID-19. This study aims to systematically review the literature on the relation between weight excess and the severity of COVID-19 in children and adolescents.This systematic review was developed following the PRISMA standards (Preferred Reporting Items for Systematic Review and Meta-Analysis). The literature search was performed in September 2020, in the following databases: MEDLINE (via PubMed), Embase, Scopus, The Cochrane Library (Cochrane Database of Systematic Reviews and Cochrane Central Register of Controlled Trials (CENTRAL)), Web of Science, BVS/LILACS and SciELO. Registration on the PROSPERO platform (International Prospective Registry of Systematic Reviews) can be found under the registration number: CRD42021230686.Of the 667 selected articles, 11 were included according to all agreed criteria, most of them being unicentric and American. In assessing the risk of bias and quality, following the criteria of the Newcastle-Ottawa Scale, eight studies achieved scores higher than 7. Only 5 studies sought to assess obesity and its relation with worse outcomes (such as need for pediatric intensive care unit (PICU), mechanical ventilation (MV), tracheostomy, hospital readmission and mortality), and out of these, only one article demonstrated this comorbidity as a prognostic factor for worse evolution of the COVID picture.Few studies in the literature seek to assess excess weight and its relation with worse outcomes of COVID-19 in children and adolescents. Taking into account that there is already scientific evidence on this subject in adult patients, it is necessary to carry out more research in the pediatric age group.

O objetivo do trabalho e apresentar um e-book elaborado pelo Grupo de Pesquisa em Estilo de Vida e Saude com o objetivo de reunir informacoes relevantes para promover acoes beneficas a saude fisica e mental e maior qualidade de vida neste periodo com foco em estilo de vida saudavel. O material produzido foi intitulado “Estilo de vida: estrategias interprofissionais em saude para lidar com o periodo da quarentena” e e composto de 10 capitulos divididos em 42 paginas. Participaram da analise critica deste produto 10 juizes que avaliaram o conteudo quanto a organizacao, estilo da escrita, aparencia, e apelo do material, conforme instrucoes de avaliacao da adequacao dos materiais do livro Teaching Patients with Low Literacy Skills recomendado pela Harvard T.H. Chan School of Public Health. O e-book proposto neste estudo foi baseado em evidencias cientificas apresentando imagens ilustrativas e links uteis para o leitor com o objetivo de possibilitar interacao em uma linguagem acessivel. A producao do e-book podera servir como ferramenta de promocao de saude e qualidade de vida para ser utilizada pelo publico geral bem como para apoiar acoes dos profissionais da area da saude.

Objetivo: Relatar o processo de construção de uma cartilha psicoeducativa que auxilia pais/cuidadores e crianças a manterem os comportamentos de saúde durante a pandemia. Métodos: O conteúdo foi baseado nas recomendações de instituições de saúde como Organização Mundial da Saúde, Colégio Americano de Medicina do Esporte e Sociedade Brasileira de Pediatria. Após revisão e leitura cuidadosa dos materiais, foi formulado o texto em forma de história em quadrinhos. Além disso, foi realizada uma curadoria de vídeos e materiais online que trouxesse mais informações e incentivassem a adesão a um estilo de vida saudável. O material foi avaliado por 5 juízes correspondentes a profissionais da área de pediatria e pessoas do público-alvo. Resultados: O coeficiente de validade de conteúdo de 0,97 e 1,00 para clareza de linguagem e pertinência respectivamente. A concordância intraclasse de Finn foi igual a 0,96. Conclusão: Com isso, recomenda-se a utilização da presente cartilha para promoção de comportamentos de saúde e psicoeducação. Novos materiais como este, com fundamentação metodológica, assegurando uma robustez científica e um conteúdo baseado em evidências deveriam ser produzidos.

There is a high prevalence of children with complex chronic conditions (CCCs) in pediatric intensive care units (PICUs). However, information on the nutritional status (NS) of this specific population is limited. This study aimed to evaluate the NS of critically ill pediatric patients with CCCs and to relate it to clinical outcomes.A retrospective cohort study of children admitted to a PICU over a 4-year period. We classified NS according to body mass index-for-age (BMI/A) and height-for-age (H/A) z-scores, using the World Health Organization (WHO) growth curves as a reference. We recorded the presence of CCC according to the definition proposed by Feudtner et al. Severity on admission was measured using the Pediatric Index of Mortality 2 (PIM2). We assessed the following outcomes: mortality, multiple organ dysfunction syndrome during PICU stay, and PICU length of stay (LOS).We included 1753 children in the study. Presence of CCC accounted for 49.8% (873) of the sample. Among children with CCCs, 61.7% (539) had appropriate weight, 19.8% (173) were underweight, and 18.4% (161) were overweight. H/A was considered inadequate in 32.2% (281) of patients with CCCs, a higher rate than in those without CCCs (25.3%; 132) (p 0.001). Regarding outcomes, underweight children had more organ dysfunctions and prolonged LOS. The association only remained for prolonged LOS when adjusting for confounders. Although underweight children had a higher PIM2-predicted risk of mortality, there was no significant difference in actual mortality between the three NS groups (p = 0.200).The rates of nutritional inadequacies in patients with CCCs were high. Underweight was independently associated with prolonged LOS in children with CCC.

OBJECTIVES To conduct the Brazilian translation, cross-cultural adaptation, validation, and reliability testing of the EMpowerment of PArents in THe Intensive Care (EMPATHIC-30). DESIGN Prospective study. SETTING PICU of a tertiary-care teaching hospital. PATIENTS Parents (n = 141) completed the translated EMPATHIC-30 questionnaire 72 hours after their child's PICU discharge. INTERVENTIONS None. MEASUREMENTS AND MAIN RESULTS The translation and cultural adaptation were performed in accordance with the principles of good practice for the translation and cultural adaptation process for patient-reported outcomes measures. Sentences were adapted according to the Brazilian syntax. Total content validity coefficient was above the established average (> 0.8). Reliability was evaluated with the coefficients McDonald omega and Cronbach alpha. The lowest Cronbach alpha found was 0.47 (CI 95%, 0.35-0.59) in the organization domain, where the lowest response rate was also concentrated. The values of the other domains were as follows: 0.64 (95% CI, 0.55-0.73) for information, 0.77 (95% CI, 0.71-0.83) for care and treatment, 0.72 (95% CI, 0.66-0.78) for parent participation, and 0.72 (95% CI, 0.65-0.79) for professional attitudes. The total internal consistency independent of the domain was 0.90 (CI 95%, 0.88-0.92). With regard to McDonald Omega, values were identified: 0.68 (95% CI, 0.49-0.88) for information, 0.73 (95% CI, 0.61-0.85) for care and treatment, 0.85 (95% CI, 0.47-0.80) for parent participation, 0.85 (95% CI, 0.76-0.93), and 0.72 (95% CI, 0.58-0.86) for professional attitudes. CONCLUSIONS EMPATHIC-30 has been translated and culturally adapted for the Brazilian population. Validation demonstrated an above-average total content validity coefficient, confirming the instrument content validity. A sufficient reliability was observed in both analyzed coefficients. The results support the use of the Brazilian version of EMPATHIC-30 for the evaluation of parents' satisfaction of children admitted to the PICU.

Background Because of its high prevalence and negative impact on quality of life and longevity, overweight in childhood and adolescence is a major public health concern. The objective of the present study was to determine whether excess weight is associated with clinical outcomes in critically ill children and adolescents admitted to the pediatric intensive care unit (PICU). Method This retrospective cohort study was performed with children and adolescents admitted to a PICU over 3 years. Nutrition status was classified based on the body mass index z-score for age, following World Health Organization (WHO) criteria. The following outcomes were assessed: mortality, need for mechanical ventilation, length of admission, and multiple organ dysfunction syndrome. Results Of 1468 patients admitted during the study period, 1407 were included in the study: 956 (68.0%) had adequate weight, 228 (16.2%) were overweight, and 223 (15.8%) were underweight. Associations were detected between most variables and all nutrition categories (underweight, adequate weight, and overweight). In the descriptive analysis, mortality was more prevalent in nutrition status extremes (extremely underweight or overweight). An independent association between nutrition status and mortality was not detected in any category. Conclusion Nutrition status was not independently associated with poor outcomes. However, overweight should be considered a potential risk factor for adverse clinical outcomes in PICU admissions.

BackgroundNew-onset refractory status epilepticus (NORSE) refers to patients without a previous history of seizures who have refractory status epilepticus for at least 72 hours without an identified aetiology. Despite the severe neurological sequelae of NORSE, little is known about this condition in paediatric patients.ObjectiveTo describe the profile of paediatric patients with NORSE, the profile of seizures, possible causes attributed to this condition, treatments offered to patients and the outcomes at discharge from the paediatric intensive care unit (PICU).MethodsThis retrospective, multicentre, descriptive study (case series) was conducted in the PICUs of three tertiary hospitals. We reviewed the medical records of all patients aged 0–16 years admitted to the participating PICUs between December 2013 and December 2017 with refractory status epilepticus, without a previous history of seizures or neurological disease.ResultsFifteen patients (2.4%) had NORSE. The median age of patients was 62.3 (IQR 26.2–75.4) months. All patients experienced prodromes before progressing to refractory status epilepticus. Twelve patients (80%) had fever up to 24 hours before seizures. NORSE was classified as cryptogenic in 66% of patients. Twelve patients were treated with complementary therapies, in addition to anticonvulsants. There was no standardisation in the treatment of patients. The overall mortality rate was 20%.ConclusionsNORSE is associated with high morbidity and mortality, without an identified aetiology in most cases and with a wide range of proposed therapies.

Objective To evaluate the prognostic performance of the Pediatric Index of Mortality 2 (PIM2), ferritin, lactate, C-reactive protein (CRP), and leukocytes, alone and in combination, in pediatric patients with sepsis admitted to the pediatric intensive care unit (PICU). Methods A retrospective study was conducted in a PICU in Brazil. All patients aged 6 months to 18 years admitted with a diagnosis of sepsis were eligible for inclusion. Those with ferritin and C-reactive protein measured within 48 h and lactate and leukocytes within 24 h of admission were included in the prognostic performance analysis. Results Of 350 eligible patients with sepsis, 294 had undergone all measurements required for analysis and were included in the study. PIM2, ferritin, lactate, and CRP had good discriminatory power for mortality, with PIM2 and ferritin being superior to CRP. The cutoff values for PIM2 (> 14%), ferritin (> 135 ng/mL), lactate (> 1.7 mmol/L), and CRP (> 6.7 mg/mL) were associated with mortality. The combination of ferritin, lactate, and CRP had a positive predictive value of 43% for mortality, similar to that of PIM2 alone (38.6%). The combined use of the three biomarkers plus PIM2 increased the positive predictive value to 76% and accuracy to 0.945. Conclusions PIM2, ferritin, lactate, and CRP alone showed good prognostic performance for mortality in pediatric patients older than 6 months with sepsis. When combined, they were able to predict death in three-fourths of the patients with sepsis. Total leukocyte count was not useful as a prognostic marker.

SUMMARY OBJECTIVE: To verify the association between prone position, increased diuresis, and decreased cumulative fluid balance in critically ill pediatric patients who underwent mechanical ventilation (MV) for pulmonary causes and describe adverse events related to the use of the position. METHODS: This is a retrospective observational study. Patients aged between 1 month and 12 years who underwent MV for pulmonary causes, between January 2013 and December 2015, were selected and divided between those who were put on prone position (PG) and those who were not (CG) during the hospitalization at the Pediatric Intensive Care Unit (PICU). Data were analyzed longitudinally from D1 to D4. RESULTS: A total of 77 patients (PG = 37 and CG = 40) were analyzed. The general characteristics of both groups were similar. In the comparison between the groups, there was no increase in diuresis or decrease in cumulative fluid balance in the prone group. In the longitudinal analysis of D1 to D4, we saw that the PG presented higher diuresis (p = 0.034) and a lower cumulative fluid balance (p = 0.001) in D2. Objective: To compare the nursing workload using the Nursing Activities Score (NAS), Therapeutic Intervention Scoring System-28 (TISS-28) and Nine Equivalents of Nursing Manpower Use Score (NEMS) instruments in children admitted to a Pediatric Intensive Care Unit in a university hospital. Method: A prospective cohort study performed in a Pediatric Intensive Care Unit, with a sample formed of all children hospitalized during the study period. Nursing workload was evaluated using the TISS-28, NEMS and NAS instruments, and further divided into two groups: Group 1 forming a category of basic activities items and Group 2 forming a category of other support and intervention activities. Results: The sample consisted of 490 Pediatric Intensive Care Unit admissions, totaling 4617 observations. NAS presented the best estimate of total working hours. TISS-28 and NEMS showed better agreement and the results showed strong correlations between NAS and TISS-28 and between NEMS and TISS-28. In Group 1 (basic activities), NAS(1) and TISS-28(1) showed moderate correlation, in Group 2 (specialized activities) the three instruments showed strong correlations. Conclusion: NAS stood out in the evaluation of nursing workload and showed good correlation and agreement with the TISS-28. Método: Estudo de coorte prospectivo realizado em Unidade de Terapia Intensiva Pediátrica, com amostra constituída por todas as crianças internadas durante o período do estudo. A carga de trabalho de enfermagem foi avaliada por meio dos instrumentos TISS-28, NEMS e NAS, e posteriormente dividida em dois grupos: Grupo 1 formando uma categoria de itens de atividades básicas; Grupo 2 formando uma categoria de outras atividades de apoio e intervenção. Resultados: A amostra foi composta por 490 internações em Unidade de Terapia Intensiva Pediátrica, totalizando 4617 observações. O NAS apresentou a melhor estimativa do total de horas de trabalho. La carga de trabajo de enfermería fue evaluada a través de los instrumentos TISS-28, NEMS y NAS y, posteriormente dividida en dos grupos: Grupo 1 formando una categoría de ítems de actividades básicas; Grupo 2 formando una categoría de otras actividades de apoyo e intervención. Resultados: La muestra fue compuesta por 490 ingresos en Unidad de Cuidados Intensivos Pediátrica, en un total de 4617 observaciones. El NAS presentó la mejor estimativa del total de horas trabajadas. El TISS-28 y el NEMS presentaron mejor concordancia y los resultados demostraron fuertes correlaciones entre NAS y TISS-28 y entre NEMS y TISS-28. En el grupo 1 (actividades básicas), el NAS(1) y el TISS-28(1) presentaron correlación moderada, en el Grupo 2 (actividades especializadas) los tres instrumentos presentaron correlaciones fuertes. Objective To determine the prevalence of life support limitation (LSL) in patients who died after at least 24 h of a pediatric intensive care unit (PICU) stay, parent participation and to describe how this type of care is delivered. Methods Retrospective cohort study in a tertiary PICU at a university hospital in Brazil. All patients aged 1 month to 18 years who died were eligible for inclusion. The exclusion criteria were those brain death and death within 24 h of admission. Results 53 patients were included in the study. The prevalence of a LSL report was 45.3%. Out of 24 patients with a report of LSL on their medical records only 1 did not have a do-not-resuscitate order. Half of the patients with a report of LSL had life support withdrawn. The length of their PICU stay, age, presence of parents at the time of death, and severity on admission, calculated by the Pediatric Index of Mortality 2, were higher in patients with a report of LSL. Compared with other historical cohorts, there was a clear increase in the prevalence of LSL and, most importantly, a change in how limitations are carried out, with a high prevalence of parental participation and an increase in withdrawal of life support. Conclusions LSLs were associated with older and more severely ill patients, with a high prevalence of family participation in this process. The historical comparison showed an increase in LSL and in the withdrawal of life support.

OBJECTIVES To evaluate serum ferritin measured within 48 hours of admission as a prognostic marker and examine the association with unfavorable outcomes in a population of pediatric patients with sepsis and high prevalence of iron deficiency anemia in which this biomarker is routinely measured. DESIGN Retrospective cohort study. SETTING PICU of a tertiary care teaching hospital in a middle-income country in South America. PATIENTS All patients 6 months to 18 years old (n = 350) admitted with a diagnosis of sepsis, suspected or proven, were eligible for inclusion. Exclusion criteria were length of PICU stay less than 8 hours and inherited or acquired disorder of iron metabolism that could interfere with serum ferritin levels. INTERVENTIONS None. MEASUREMENTS AND MAIN RESULTS Three-hundred twelve patients had their ferritin levels measured within 48 hours, and only 38 did not. The prevalence of iron deficiency anemia (hemoglobin < 11 g/dL and mean corpuscular volume < 80 fl was 40.3%. The median of the highest serum ferritin level within 48 hours was 150.5 ng/mL (interquartile range, 82.25-362 ng/mL), being associated with mortality (p < 0.001; Exp(B), 5.170; 95% CI, 2.619-10.205). A 10-fold increase in ferritin level was associated with a five-fold increase in mortality. There was a monotonic increase in mortality with increasing ferritin levels (p < 0.05). Regarding the discriminatory power of ferritin for mortality, the area under the receiver operating characteristic curve was 0.787 (95% CI, 0.737-0.83; p < 0.0001). CONCLUSIONS Serum ferritin at lower thresholds predicts mortality in children with sepsis admitted to the ICU in a middle-income country with high prevalence of iron deficiency anemia.

ObjectiveTo evaluate the use of cerebrospinal fluid (CSF) ferritin levels in the diagnosis of purulent meningitis (PM).MethodWe studied 81 children between 28 days and 12 years of age who presented with clinical suspicion of meningitis to the emergency department. CSF ferritin levels were measured and compared between diagnostic groups (PM, aseptic meningitis (AM) and no meningitis).ResultsThe median age was 24 (IQR 8–69) months. There were 32 patients with AM (39%), 23 with PM (28%) and 26 with no meningitis (32%). Median CSF ferritin was 4.2 ng/mL (IQR 3.0–6.5), 52.9 ng/mL (IQR 30.7–103 ng/mL) and 2.4 ng/mL (IQR 2–4), respectively. CSF ferritin was higher in children with PM compared with AM (pConclusionCSF ferritin may be a useful biomarker to discriminate PM in children with clinical symptoms of this disease.

Objective: The aim of this study was to verify the association of echocardiogram, ferritin, C-reactive protein, and leukocyte count with unfavorable outcomes in pediatric sepsis. Methods: A prospective cohort study was carried out from March to December 2014, with pediatric critical care patients aged between 28 days and 18 years. Inclusion criteria were diagnosis of sepsis, need for mechanical ventilation for more than 48 h, and vasoactive drugs. Serum levels of C-reactive protein, ferritin, and leukocyte count were collected on the first day (D0), 24 h (D1), and 72 h (D3) after recruitment. Patients underwent transthoracic echocardiography to determine the ejection fraction of the left ventricle on D1 and D3. The outcomes measured were length of hospital stay and in the pediatric intensive care unit, mechanical ventilation duration, free hours of VM, duration of use of inotropic agents, maximum inotropic score, and mortality. Results: Twenty patients completed the study. Patients with elevated ferritin levels on D0 had also fewer ventilator-free hours (p = 0.046) and higher maximum inotropic score (p = 0.009). Patients with cardiac dysfunction by echocardiogram on D1 had longer hospital stay (p = 0.047), pediatric intensive care unit stay (p = 0.020), duration of mechanical ventilation (p = 0.011), maximum inotropic score (p = 0.001), and fewer ventilator-free hours (p = 0.020). Conclusion: Cardiac dysfunction by echocardiography and serum ferritin value was significantly associated with unfavorable outcomes in pediatric patients with sepsis. Resumo: Objetivo: Verificar a associação do ecocardiograma, da ferritina, da Proteína C Reativa (PCR) e da contagem de leucócitos com desfechos desfavoráveis na sepse pediátrica. Métodos: Estudo de coorte prospectivo, no período de março a dezembro de 2014, com pacientes críticos pediátricos de idade entre 28 dias e 18 anos. Critérios de inclusão foram diagnóstico de sepse, necessidade de ventilação mecânica (VM) por mais de 48 horas e uso de drogas vasoativas. Avaliaram-se os níveis séricos PCR, ferritina, contagem de leucócitos, no recrutamento (D0), 24 horas (D1) e 72 horas (D3) após o recrutamento. No D1 e no D3 todos pacientes foram submetidos a ecocardiograma transtorácico para determinação da Fração de Ejeção (FE) do ventrículo esquerdo. Os desfechos avaliados foram tempo de internação hospitalar e na Unidade de Terapia Intensiva pediátrica (UTIP); duração da VM; horas livres de VM; duração do uso de inotrópicos; escore de inotrópicos máximo e mortalidade. Resultados: Vinte pacientes completaram o estudo. Ferritina elevada no D0 associou-se com menor tempo livre de ventilação (p = 0,046) e maior escore de inotrópicos máximo (p = 0,009). A disfunção cardíaca pelo ecocardiograma no D1 relacionou-se com maior tempo de internação hospitalar (p = 0,047), de UTIP (p = 0,020), VM total (p = 0,011), escore de inotrópicos máximo (p = 0,001) e menor tempo livre de VM (p = 0,020). Conclusão: A disfunção cardíaca pelo ecocardiograma e o valor de ferritina sérica associaram-se significativamente com desfechos desfavoráveis nos pacientes pediátricos com sepse. This article aims to review systematically the evidence on nutritional assessment techniques and parameters used to determine the nutritional status of children and adolescents. The literature review and the selection of publications were performed using the Medline, Lilacs, SciELO, Embase, personal files. 17 studies were identified, 7 addressed the anthropometric indices as the main outcome, 7 analyzed the growth and development of children and adolescents through growth curves, and the remainder surveyed body composition. In general, all met the quality criteria, unless 6 of the articles who did not discuss the limitations. The literature review suggests several techniques and parameters that can be applied to determine the nutritional status of children and adolescents from different countries. Growth graphs are essential to assess the health of children, but depend greatly of the growth tables used. Although BMI can be practical, it does not distinguish body fat from lean mass. The best interpretation of anthropometry will depend of valid reference values for age range of the study population. BIA is a quick feasible method, but the measurement has some various nationalities. Resumo O objetivo deste artigo é revisar sistematicamente as evidências sobre as técnicas de avaliação nutricional e parâmetros utilizados para determinar o estado nutricional em crianças e adolescentes. Revisão da literatura com busca nas bases de dados, Medline, Lilacs, SciELO e Embase, além de arquivos pessoais. Identificamos 17 artigos que relatavam dados de diferentes populações, sete estudos abordaram os índices antropométricos, quatro o crescimento e o desenvolvimento de crianças e adolescentes por meio de curvas de crescimento, e o restante a composição corporal. Todos preencheram os critérios de qualidade, com exceção das limitações. A revisão da literatura sugere diversas técnicas e parâmetros que podem ser aplicados para determinar o estado nutricional de crianças e adolescentes de diferentes países. Gráficos de crescimento são essenciais para avaliar a saúde de crianças, mas depende muito das tabelas de crescimento utilizadas. Embora o IMC seja prático, não distingue a gordura corporal de massa magra. Existem várias técnicas para avaliar proporções, tamanho e composição corporal. A melhor interpretação da antropometria dependerá de valores de referência válidos para a faixa etária da população estudada. BIA é um método factível, mas tem algumas limitações para a realização do exame.

Objectives: To systematically review the evidence about the impact of nutritional status in critically-ill pediatric patients on the following outcomes during hospitalization in pediatric intensive care units: length of hospital stay, need for mechanical ventilation, and mortality. Data source: The search was carried out in the following databases: Lilacs (Latin American and Caribbean Health Sciences), MEDLINE (National Library of Medicine United States) and Embase (Elsevier Database). No filters were selected. Results: A total of seven relevant articles about the subject were included. The publication period was between 1982 and 2012. All articles assessed the nutritional status of patients on admission at pediatric intensive care units and correlated it to at least one assessed outcome. A methodological quality questionnaire created by the authors was applied, which was based on some references and the researchers’ experience. All included studies met the quality criteria, but only four met all the items. Conclusion: The studies included in this review suggest that nutritional depletion is associated with worse outcomes in pediatric intensive care units. However, studies are scarce and those existing show no methodological homogeneity, especially regarding nutritional status assessment and classification methods. Contemporary and well-designed studies are needed in order to properly assess the association between children's nutritional status and its impact on outcomes of these patients. Resumo: Objetivos: Revisar, de forma sistemática, as evidências acerca do impacto do estado nutricional de pacientes pediátricos críticos sobre os seguintes desfechos durante a internação em Unidades de Terapia Intensiva Pediátrica: Tempo de internação, necessidade de ventilação mecânica e mortalidade. Fonte de dados: A busca foi realizada nas seguintes bases de dados: Lilacs (Literatura Latino-Americana e do Caribe em Ciências da Saúde), Medline(National Library of Medicine Estados Unidos), e Embase (Elsevier Base de Dados). Não houve seleção de filtros. Resultados: Foram incluídos sete artigos relevantes sobre o tema. O período de publicação variou entre 1982 e 2012. Todos avaliam o estado nutricional de pacientes no momento da internação em Terapia Intensiva Pediátrica e relacionam com pelo menos um desfecho estudado. Foi aplicado um questionário de qualidade metodológica criado pelos autores. Ele foi baseado em algumas referências e experiência dos pesquisadores. Todos os estudos incluídos alcançaram os critérios de qualidade, porém apenas quatro, atenderam à todos os itens. Conclusão: Os estudos incluídos nesta revisão sugerem que a depleção do estado nutricional está relacionado com piores desfechos em Terapia Intensiva Pediátrica. Porém os estudos são escassos, e os existentes, não tem uniformidade metodológica, em especial nos métodos de avaliação e classificação do estado nutricional. São necessários estudos atuais e bem delineados com o objetivo de avaliar adequadamente esta relação entre o estado nutricional de crianças e sua repercussão nos desfechos destes pacientes. RESUMO Objetivo: Comparar a prevalência de desnutrição em dois momentos de uma unidade de terapia intensiva pediátrica. Métodos: Estudo transversal retrospectivo, com pacientes internados na unidade de terapia intensiva pediátrica de um hospital universitário em dois períodos de 1 ano com intervalo de 4 anos. A avaliação nutricional foi realizada a partir do peso e da estatura aferidos no momento da internação. O índice de massa corporal para idade foi o parâmetro escolhido para avaliação do estado nutricional classificado de acordo com a Organização Mundial de Saúde, conforme faixa etária. Resultados: A amostra total de pacientes foi de 881 (406 da amostra contemporânea e 475 da amostra histórica). Houve redução significativa da desnutrição na amostra contemporânea (p = 0,03). Os pacientes desnutridos da amostra histórica tiveram associação significativa com mortalidade e tempo de internação, enquanto que os desnutridos da amostra contemporânea não apresentaram piores desfechos. Conclusão: Houve redução significativa da desnutrição entre os pacientes da mesma unidade de terapia intensiva pediátrica quando comparamos dois momentos. Nossos achados de modificação de perfil nutricional em pacientes gravemente enfermos corroboram dados de estado nutricional de crianças e adolescentes a nível mundial. Objective: To estimate the workload and size the nursing team using the scales TISS-28 and NEMS in a pediatric intensive care unit. Methods: An observational prospective study with a quantitative approach was conducted at the pediatric intensive care unit of a university hospital from Jan 1st, 2009 to Dec 31st, 2009. All children who remained hospitalized for more than 8 hours were included, with length of stay of 4 hours in case of death. Clinical data were collected and the Paediatric Index of Mortality 2 and the scores TISS-28 and NEMS were determined. The TISS-28 and NEMS were converted into working hours of the nursing team and sizing complied with the parameters of the Brazilian Federal Nursing Council. Pearson's correlation and the Bland-Altman model were used to verify the association and agreement between the instruments. Results: A total of 459 children were included, totaling 3,409 observations. The average values for the TISS-28 and NEMS were 20.8±8 and 25.2±8.7 points, respectively. The nursing workload was 11 hours by TISS-28 and 13.3 hours by NEMS. The estimated number of professionals by TISS-28 and NEMS was 29.6 and 35.8 professionals, respectively. The TISS-28 and NEMS showed adequate correlation and agreement. Conclusion: Time spent in nursing activities and team sizing reflected by the NEMS were significantly greater when compared to the TISS-28. RESUMO Objetivo: Estimar a carga de trabalho e dimensionar a equipe de enfermagem utilizando as escalas TISS-28 e NEMS em uma unidade de terapia intensiva pediátrica. Métodos: Estudo prospectivo observacional com abordagem quantitativa, realizado na unidade de terapia intensiva pediátrica de um hospital universitário, no período de 1o de janeiro de 2009 a 31 de dezembro de 2009. Foram incluídas todas as crianças que permaneceram internadas por mais de 8 horas, com duração de internação de 4 horas em caso de óbito. Foram coletados os dados clínicos, e determinados o Paediatric Index of Mortality 2 e as escalas TISS-28 e NEMS. O TISS-28 e o NEMS foram convertidos em horas de trabalho da equipe de enfermagem, e o dimensionamento seguiu os parâmetros do Conselho Federal de Enfermagem. A correlação de Pearson e o modelo de Bland-Altman foram utilizados para verificar a associação e a concordância entre os instrumentos. Resultados: Foram incluídas 459 crianças, totalizando 3.409 observações. As médias do TISS-28 e do NEMS foram 20,8±8 e 25,2±8,7 pontos, respectivamente. A carga de trabalho de enfermagem foi de 11 horas pelo TISS-28 e 13,3 horas pelo NEMS. A estimativa do número de profissionais pelo TISS-28 e NEMS foi de 29,6 e 35,8 profissionais, respectivamente. O TISS-28 e o NEMS apresentaram correlação e concordância adequadas. Conclusão: O tempo despendido nas atividades de enfermagem e o dimensionamento da equipe refletido pelo NEMS foram significativamente maiores quando comparados ao TISS-28.

Um lactente de 37 dias de vida, do sexo masculino, previamente higido e sem intercorrencias durante periodo perinatal, foi admitido em unidade de terapia intensiva pediatrica com insuficiencia ventilatoria aguda devido a bronquiolite viral pelo virus sincicial respiratorio. Foi submetido a intubacao traqueal por deterioracao clinica, sendo realizada sequencia rapida de intubacao com fentanil, cetamina e succinilcolina.
