Its effect on ophthalmology practices in Latin America has not yet been explored. We aimed to assess the perceptions about the pandemic from countries’ ophthalmological national and subspecialty retina societies affiliated to the Pan-American Association of Ophthalmology (PAAO).Patients and Methods: A survey-based study of leaders of national ophthalmological and retinal societies was conducted. The survey was sent by email to 30 societies, from which 20 responded (12 countries, 66.6% response rate). It included closed- and open-ended questions about (1) operational capacity and precautions, (2) telemedicine and virtual care, (3) procedures, and (4) post-pandemic considerations.Results: There was a marked decline in ophthalmology patient visits (80– 95%) and elective surgeries (90%) during 2020 compared to before the pandemic. Precautions like temperature checks, mask usage, and social distancing were widely implemented while personal protective equipment (PPE) availability varied. Telemedicine use was limited due to lack of experience with it. Reopening plans focused on maintaining precautions and gradually resuming activities. Economic and security concerns were raised, and adherence to guidelines was emphasized. Respondents acknowledged the need to adapt to a “new normal”. Long duration drugs, fewer imaging studies, and shorter wait times were preferred; however, availability of long duration drugs was limited.Conclusion: The pandemic impacted ophthalmology in Latin America, with reduced patient visits, procedures, and surgeries. Abstract

Approximately 10% of IDH-mutant gliomas harbour non-canonical IDH mutations (non-p.R132H IDH1 and IDH2 mutations). The aim of this study was to analyse the characteristics of non-canonical IDH-mutant gliomas. We retrospectively analysed the characteristics of 166 patients with non-canonical IDH mutant gliomas and compared them to those of 155 consecutive patients with IDH1 p.R132H mutant gliomas. The median age at diagnosis was 38 years in patients with non-canonical IDH mutant gliomas and 43 years in glioma patients with IDH1 p.R132H-mutant tumours. Abstract

Late-onset Alzheimer Disease (LOAD) is the most common form of dementia and one of the most challenging diseases of modern society 1 . Understanding the preclinical stages of AD that begins in the brain at least 2-3 decades before evidence of episodic memory defects in patients is pivotal for the design of successful approaches to delay or reverse the transition from normal brain function to cognitive impairments. Our working hypothesis is that LOAD genetic risk factors can be sufficient to generate early phenotypical changes before any changes in either Abeta or Tau. To test this hypothesis, we generated an hBIN1 mouse model based on the human BIN1 gene overexpression that we found in post-mortem brain samples from LOAD patients. BIN1 is the second important risk factor for AD, following the APOE gene 2 . We identified co-deregulated gene repertoires common to both 7-week mouse hippocampus sub-regions and post-mortem brain samples from LOAD patients, demonstrating the validity of this hBIN1 model. We evidenced an early phenotype of neurodegeneration starting at 3 months with structural impairment fiber pathways quantified by high resolution (17.2T) (MRI-DTI) and related functional impacts. We found structural changes in entorhinal cortex-dentate gyrus (EC-DG) pathway known to be the earliest brain region impacted in LOAD 3–6 . Similarly, the function of this pathway was impaired both in vitro and in vivo , with the changes in spine density and dendritic simplification of DG neurons, impaired EC-DG long-term potentiation (LTP) and behavioral deficits linked to object recognition episodic memory. As expected for a neurodegenerative model, we evidenced the progression of dysfunction at the morphological, functional and behavioral levels with age. Structural spreading involved impairment of fibers in somatosensory and temporal associative cortexes at month 15. Functional and behavioral spreading was characterized by impact on pattern separation of spatial episodic memory. Moreover, this neurodegeneration occurred without any detectable changes in Aβ42 and tau. Overall, these data show the possibility to identify a repertoire of molecular changes occurring both in patients and in hBIN1 mice and whose further manipulation can be instrumental to rescue or delay episodic memory defects.

Abstract

International audience; Background & Objectives: Anaplastic oligodendroglioma (AO), IDH-mutant and 1p/19q codeleted (IDHmut+/1p19qcodel) are high grade gliomas. Only few prognostic markers were studied in this specific histomolecular subgroup. The primary aim of this study was to evaluate and compare the prognostic value of two proliferation markers, MCM6 and Ki-67, in a French multicenter series of IDHmut+/1p19qcodel AO (POLA network), using immunohistochemistry. Further transcriptomic approaches were implemented to uncover the molecular pathways associated with the overexpression of these markers.Methods: Two hundred and thirty-one IDHmut+/1p19qcodel AO cases were included from the French national POLA network. MCM6 and Ki-67 labelling index (LI) were evaluated using computer color image analyser. Transcriptomic data were analysed in a subset of 68 microarray samples from the French POLA Network.Results: High MCM6 (≥ 50%) and Ki-67 (≥ 15%) LI correlated with shorter overall survival (P=0.013 and P=0.004, respectively). A high proliferation index, defined by MCM6 ≥ 50% and/or Ki-67 ≥ 15%, was independently correlated to a shorter survival (P=0.027; multivariate Cox model including age, mitotic index, MCM6 and Ki-67). Transcriptomic analyses revealed that while the high mRNA level of both MCM6 and Ki-67 were positively associated with clusters enriched in gene functions like cell cycle progression, DNA replication, mitosis, pro-neural phenotype as well as neurogenesis, they were negatively associated with clusters of other functions like microglial cell activation, immune response, positive regulation of myelination, oligodendrocyte development, beta-amyloid binding, and postsynaptic specialization.Conclusion: In conclusion, both MCM6 and Ki-67 LI were correlated to overall survival. Because multivariate analyses showed that overexpression of MCM6 and/or Ki-67 was independently correlated to shorter survival, these two easy-to-use and costless markers could be used in association in daily practice in order to predict clinical outcome. Transcriptomics showed that IDHmut+/1p19qcodel AO are highly proliferative tumours with upregulated pro-neural phenotype associated genes, and downregulated immune response, glial differentiation, and myelin-related function.

31. Illness beliefs and emotional responses in mildly disabled stroke survivors: A qualitative study

BACKGROUND: As acute stroke services improve, more persons experience mild stroke and need to cope daily with hidden disabilities, which may be influenced by how they perceive stroke, cognitively and emotionally. OBJECTIVE: To investigate cognitive illness beliefs and emotional responses in persons with mild stroke and their possible influences on daily coping. METHODS: Semi-structured interviews were conducted with 24 persons with mild stroke, on average 7.5 months (±0.89) after stroke occurrence. A thematic analysis on verbatim transcripts was guided by the Common-Sense Model of Self-Regulation. RESULTS: All participants experienced difficulties constructing an illness identity at both acute and chronic phase. Behavioral risk factors were less accepted as causes of stroke. Lack (or inappropriate timing) of information from healthcare providers led to limited medication knowledge and low perceived control of stroke recurrence which generated anxiety, fear, and low involvement in coping. Participants who considered stroke a chronic condition experienced more difficulties. Perceived support from relatives and healthcare providers was beneficial for participation in recovery and health behaviour change. CONCLUSION: Despite having mildly disabilities, participants reported difficulties developing illness beliefs conducive to coping, and dealing with their emotional responses. These elements should be considered in tailored programs to improve coping with hidden disabilities post-stroke.

32. Behavior Change Content, Understandability, and Actionability of Chronic Condition Self-Management Apps Available in France: Systematic Search and Evaluation

BACKGROUND: The quality of life of people living with chronic conditions is highly dependent on self-management behaviors. Mobile health (mHealth) apps could facilitate self-management and thus help improve population health. To achieve their potential, apps need to target specific behaviors with appropriate techniques that support change and do so in a way that allows users to understand and act upon the content with which they interact. OBJECTIVE: Our objective was to identify apps targeted toward the self-management of chronic conditions and that are available in France. We aimed to examine what target behaviors and behavior change techniques (BCTs) they include, their level of understandability and actionability, and the associations between these characteristics. METHODS: We extracted data from the Google Play store on apps labelled as Top in the Medicine category. We also extracted data on apps that were found through 12 popular terms (ie, keywords) for the four most common chronic condition groups-cardiovascular diseases, cancers, respiratory diseases, and diabetes-along with apps identified through a literature search. We selected and downloaded native Android apps available in French for the self-management of any chronic condition in one of the four groups and extracted background characteristics (eg, stars and number of ratings), coded the presence of target behaviors and BCTs using the BCT taxonomy, and coded the understandability and actionability of apps using the Patient Education Material Assessment Tool for audiovisual materials (PEMAT-A/V). We performed descriptive statistics and bivariate statistical tests. RESULTS: A total of 44 distinct native apps were available for download in France and in French: 39 (89%) were found via the Google Play store and 5 (11%) were found via literature search. A total of 19 (43%) apps were for diabetes, 10 for cardiovascular diseases (23%), 8 for more than one condition in the four groups (18%), 6 for respiratory diseases (14%), and 1 for cancer (2%). The median number of target behaviors per app was 2 (range 0-7) and of BCTs per app was 3 (range 0-12). The most common BCT was self-monitoring of outcome(s) of behavior (31 apps), while the most common target behavior was tracking symptoms (30 apps). The median level of understandability was 42% and of actionability was 0%. Apps with more target behaviors and more BCTs were also more understandable (?=.31, P=.04 and ?=.35, P=.02, respectively), but were not significantly more actionable (?=.24, P=.12 and ?=.29, P=.054, respectively). CONCLUSIONS: These apps target few behaviors and include few BCTs, limiting their potential for behavior change. While content is moderately understandable, clear instructions on when and how to act are uncommon. Developers need to work closely with health professionals, users, and behavior change experts to improve content and format so apps can better support patients in coping with chronic conditions. Developers may use these criteria for assessing content and format to guide app development and evaluation of app performance. Abstract

PURPOSE: To assess the effectiveness and safety of an intravitreal injection of 1.25 mg bevacizumab (IVB) as a preoperative adjunct to small-gauge pars plana vitrectomy (PPV) compared with PPV alone in eyes with tractional retinal detachment secondary to proliferative diabetic retinopathy. METHODS: This prospective, double-masked, randomized, multicenter, active-controlled clinical trial enrolled 224 eyes of 224 patients between November 2013 and July 2015. All eyes underwent a baseline examination including best-corrected visual acuity, color photos, optical coherence tomography, and fluorescein angiography. Data were collected on intraoperative bleeding, total surgical time, early (< 1 month) postoperative vitreous hemorrhage, and mean change in best-corrected visual acuity at 12 months. P < .05 was considered statistically significant. RESULTS: A total of 214 patients (214 eyes) were randomized in a 1:1 ratio to PPV plus IVB ([study group] 102 eyes) or PPV plus sham ([control] 112 eyes). Iatrogenic retinal breaks were noted intraoperatively in 35 eyes (34.3%) in the study group, and 66 eyes (58.9%) in the control group (P = .001). Grade 2 intraoperative bleeding was noted in 32 (31.3%) eyes in the study group and 58 (51.7 %) eyes in the control group (P = .001). Endodiathermy was necessary in 28 (27.4 %) eyes in the study group, compared with 75 (66.9%) eyes in the control group (P = .0001). Mean surgical time was 71.3 +/- 32.1 minutes in the study group and 83.6 +/- 38.7 minutes in the control group (P = .061). CONCLUSION: Preoperative IVB seems to reduce intraoperative bleeding, improving surgical field visualization, and reducing intraoperative and postoperative complications. (C) 2019 Elsevier Inc. Abstract

The advent of intravitreal anti-vascular endothelial growth factor (anti-VEGF) medications has revolutionized the treatment of diabetic eye diseases. Herein, we report the outcomes of clinical studies carried out by the Pan-American Collaborative Retina Study Group (PACORES), with a specific focus on the efficacy of intravitreal bevacizumab in the management of diabetic macular edema and proliferative diabetic retinopathy. We will also discuss the use of intravitreal bevaci-zumab as a preoperative, adjuvant therapy before vitrectomy for prolif-erative diabetic retinopathy. Abstract

In the context of the general overexploitation of South Pacific reef fisheries, there is a global push to precisely define fishery parameters such as fishing effort, annual landings, and exploitation levels. In Bora-Bora, several surveys were performed to define the fishing grounds and the fishing effort of the reef fishery for the first time. Maps of the fishing grounds under different fishing pressures were created for the Bora-Bora reef fishery. The average fishing effort throughout the lagoon was 40 trips week−1 km−2. Annual landings were estimated following two different methods: school participatory surveys and landing based on the catch per unit effort. These estimations were used to determine that the maximum sustainable yield of the island's fisheries was 6.5 tons km−2 year−1. the reported annual yield in 2022 was 5.35 tons km−2 year−1, under the estimated maximum sustainable yield. The level of exploitation of the fisheries was also estimated based on the catch size composition of the ten most fished species. This revealed that fishing is responsible for more than 50% of fish mortality in six of the ten studied species, and that catch length could be increased to optimize both catches and biomass. Overall, the Bora-Bora reef fishery may be experiencing growth overfishing (i.e., the catching of fish that are too small to maximise biological sustainability and economical yield), which can endanger stock recruitment in the future if no actions are taken.

To better understand these processes and explain how porosity and permeability can be preserved at the great depth of DBRs (deeply buried reservoirs), we developed an experimental device allowing both the simulation of high-pressure/stresses/temperature conditions (80 degrees C, 60MPa of confining pressure, and differential stress up to 40MPa) of DBR and the circulation of different fluids in rock samples. We tested (triaxial multistep creep tests) four core samples of a cemented limestone and analyzed creep deformations, fluids chemistry, and petrographical and petrophysical properties of samples. Different flow conditions (no flow and flow through) and chemical compositions (natural meteoric water with and without phosphate ions) were considered. Our study showed that the precipitation of calcite on free pore walls of micrites blocks the microporosity between micrite crystals, thus rendering the microporosity inaccessible to fluids. Hence, the connected porosity decreased strongly after experimentation. This is due to the PSC (pressure solution creep) which is the main process implied in the porosity reduction of a carbonate rock during deep burial. The preservation of macropores during PSC allows the preservation of permeability. In addition, calcite solubility is positively dependent on mechanical parameters (axial compaction and axial stress), thus suggesting that calcite can precipitate during decompression of deep basinal fluids, resulting in changes in porosity. A comparison of experimental results with theoretical calculations showed that the integration of the PSC process into calculation databases would greatly improve the modeling of DBR.
