La obesidad es una enfermedad crónica caracterizada por un aumento de la grasa corporal expresada como una expansión del volumen del tejido adiposo por encima de los niveles considerados normales para la edad, sexo y etnicidad. Con frecuencia, la acumulación de grasa en las personas con obesidad ocurre de forma ectópica en órganos como el músculo y el hígado, lugares, donde es capaz de producir alteraciones metabólicas como la resistencia a la insulina. La organización mundial de la salud, ha catalogado a la obesidad como un problema de salud pública en el ámbito mundial, que ha alcanzado proporciones epidémicas, reconociéndola como la enfermedad metabólica más frecuente en el ser humano. De hecho, su prevalencia se ha triplicado desde 1975 a 2016, de forma que para este último año había más de 1900 millones de personas con sobrepeso y más de 650 millones eran portadores de esta enfermedad. Aunque anteriormente se consideraba un problema de países de altos ingresos, en la actualidad, la obesidad también es muy frecuente en muchos países de ingresos bajos y medianos. Esto ha llevado a que la OMS, diseñara la estrategia mundial para la prevención y control de las enfermedades no transmisibles, que constituye un mapa el establecimiento de iniciativas de vigilancia, prevención y tratamiento de enfermedades como la obesidad, pero que a pesar de su claridad, no ha recibido la atención suficiente por parte de los gobiernos y la colectividad en general con el propósito de lograr un alto impacto en la prevención de la obesidad.

7. Anemia infantil: desarrollo cognitivo y rendimiento académico

The main objective of the present review study is to determine if anemia has any incidence on cognitive development and academic performance in children. The relevance of the study is justified in the need to address anemia as a global health problem that negatively impacts the entire population, especially the development of children. The research is part of a bibliographical review, of descriptive, retrospective and comparative type, which uses the technique of analysis and interpretation of contents, taking as an information source indexed articles consulted in the databases Springer Link, Scielo, Dialnet, Lilacs and Pubmed, complemented by the non-indexed scientific literature of Google Scholar. The results of the various researches in this topic conclude that iron deficiency anemia is directly related to the performance of children in the school due to iron deficiency causes a significant decrease in brain development and cognitive performance. El objetivo principal del presente estudio de revisión es determinar si la anemia tiene alguna incidencia en el desarrollo cognitivo y el rendimiento académico en los niños. La relevancia del estudio se justifica en la necesidad de abordar la anemia como un problema de salud global que afecta negativamente a toda la población, especialmente al desarrollo de los niños. La investigación forma parte de una revisión bibliográfica, de tipo descriptivo, retrospectivo y comparativo, que utiliza la técnica de análisis e interpretación de contenidos, tomando como fuente de información los artículos indexados consultados en las bases de datos Springer Link, Scielo, Dialnet, Lilacs y Pubmed. complementado por la literatura científica no indexada de Google Académico. Los resultados de las diversas investigaciones en este tema concluyen que la anemia por deficiencia de hierro está directamente relacionada con el rendimiento de los niños en la escuela debido a que la deficiencia de hierro provoca una disminución significativa en el desarrollo cerebral y el rendimiento cognitivo.

La artritis reumatoidea (AR) corresponde a un complejo desorden autoinmune que se caracteriza por una marcada inflamación y destrucción progresiva de los tejidos que forman las articulaciones. El tratamiento médico está dirigido a la supresión del proceso inflamatorio mitigando así los síntomas y signos de la enfermedad. La intervención nutricional basada en alimentos fuentes de ácidos grasos poliinsaturados (AGPICL ω-3) como: pescados, salmón, trucha, arenque, sardina, atún, anchoas, pez espada actúan como coadyuvantes en el tratamiento de enfermedades de base inflamatoria. El objetivo de este estudio fue analizar la evidencia disponible sobre los efectos terapéuticos de los ácidos grasos poliinsaturados en el estado inflamatorio crónico y en la sintomatología en pacientes con AR. La revisión consistió en la búsqueda bibliográfica en el 2017, con escritos en español o ingles publicados en las bases de datos Medline, Cochrane, Scopus, Sportdiscus y Amed, entre 2001 y 2017. Se demostró que los ácidos grasos esenciales en la artritis reumatoide evidencian reducción del dolor en articulaciones, y/o inflamadas, de la rigidez matutina. Los efectos producidos por el consumo en la alimentación de ácidos grasos esenciales, resulta ser útil como tratamiento de la inflamación aguda y crónica y la disminución de las dosis de AINES. Rheumatoid arthritis (RA) corresponds to a complex autoimmune disorder characterized by marked inflammation and progressive destruction of the tissues that form the joints. Medical treatment is aimed at suppressing the inflammatory process, thus mitigating the symptoms and signs of the disease. The nutritional intervention based on food sources of polyunsaturated fatty acids (AGPICL ω-3) such as: fish, salmon, trout, herring, sardines, tuna, anchovies, swordfish act as coadjuvants in the treatment of inflammatory-based diseases. The objective of this study was to analyze the available evidence on the therapeutic effects of polyunsaturated fatty acids in the chronic inflammatory state and in the symptomatology in patients with RA. The review consisted of bibliographic search in 2017, with writtings in Spanish or English published in the databases Medline, Cochrane, Scopus, Sportdiscus and Amed, between 2001 and 2017. It was demonstrated that essential fatty acids in rheumatoid arthritis show reduced pain in joints, and/or inflamed, of morning stiffness. The effects produced by the consumption in the feeding of essential fatty acids, it is useful as treatment of acute and chronic inflammation and the decrease of the doses of NSAIDs.

La investigación se enmarca dentro de una revisión bibliográfica, de tipo descriptivo, retrospectivo y comparativo, que emplea la técnica de análisis e interpretación de contenidos, tomando como fuente de información artículos indexados consultados en las bases de datos Springer Link, Scielo, Dialnet, Lilacs y Pubmed, complementada con la literatura científica no indexada de Google Académico. Los resultados de las diversas investigaciones en este campo concluyen que que la anemia ferropénica está relacionada de manera directa con el rendimiento de los niños en la etapa escolar dado que el déficit de hierro ocasiona una disminución significativa en el desarrollo y desempeño cognitivo del cerebro. Objetivo: Evaluar un programa de recuperación nutricional de niños menores de cinco años. Métodos y Materiales: Estudio descriptivo, transversal, con una muestra de 140 niños menores de cinco años de un programa de recuperación nutricional utilizando el SOFTWARE ANTHRO permitiendo evaluar la clasificación antropométrica al ingreso al programa y su riesgo nutricional, así como características sociodemográficas, condición de peso, talla y tipo de desnutrición. Resultados: la mayor proporción de la muestra evidenció riesgo de desnutrición global, hallándose al inicio del programa un índice de 44.5% para la edad y registrando una disminución significativa de 6.1%; se identificó una talla adecuada para la edad en niños al inicio del programa en un 32%, observando una disminución significativa en un 64%. Conclusiones: Al evaluar el programa de recuperación nutricional en niños y niñas menores de cinco años se identificó que los niños menores de 2 años están en riesgo de desnutrición global, los cuales lograron la recuperación, incrementándose el indicador de talla adecuada para la edad en todos los niños menores de 5 años, logrando superar este flagelo en su mayoría. Estos resultados alertan a que los esfuerzos en salud deben estar dirigidos no solo a solucionar la problemática de desnutrición global en la infancia sino también los que están en riesgo de desnutrición global. Objective: To evaluate a nutritional recovery program for children under five years of age. Methods and Materials: Descriptive, cross-sectional study, with a sample of 140 children under five years of a nutritional recovery program using the ANTHRO SOFTWARE allowing the evaluation of the anthropometric classification upon admission to the program and its nutritional risk, as well as sociodemographic characteristics, weight, height and type of malnutrition. Results: the greater proportion of the sample exhibited global malnutrition risk with an index of 44.5% for age at the beginning of the program and registering a significant decrease to 6.1%; an adequate size for age was identified in children at the beginning of the program in 32%, observing a significant 64% decrease. Conclusions: When evaluating the nutritional recovery program of children under five years old, children under 2 years of age are at risk of global malnutrition, which managed to recover in the program, increasing the appropriate height indicator for the age in all the children under 5 years, managing to overcome this scourge in its majority. These results warn that health efforts should be directed not only to solving the problem of global malnutrition in childhood but also those at risk of global malnutrition.

En esta investigación se determinó el efecto de la suplementación de vitamina A y Zinc como programa de intervención nutricional en 80 días de tratamiento, seleccionados aleatoriamente cuatro grupos de niños entre 7 y 12 años de edad cursantes de la Escuela Bolivariana Catatumbo. Al grupo Nro. 1 se le suministró vitamina A + Sulfato de Zinc; al grupo Nro. 2 sólo vitamina A; al grupo Nro. 3 sólo Sulfato de Zinc, y el grupo Nro. 4, control. Se les realizó antes y después una evaluación clínica, antropométrica, bioquímica y dietética. Fueron colectados 5 ml de sangre periférica para determinar Retinol sérico por cromatografía liquida de alta presión (HPLC), y espectrometría de absorción atómica en llama para Zinc sérico. Encontrándose un incremento de los niveles de Retinol sérico posterior a la suplementación en el grupo Nro. 1, con un promedio antes de la suplementación de 33,45 ± 9,28 µg/dl, y de 35,07 ± 5,92 µg/ dl después de la suplementación, y en el grupo Nro. 2, con valores promedio de 4,73 ± 12,03 µg/dl antes de la suplementación, y de 4,86 ± 12,43 µg/dl posterior a la suplementación se encontraron por debajo del punto de corte (

Introduction and aims: the precise role of parenteral nutrition in the management of oncologic patients with intestinal occlusion is not well defined yet. We aimed to identify the effects of parenteral nutrition in these patients regarding prognosis. Material and methods: 55 patients with intestinal occlusion and peritoneal carcinomatosis were included. Parenteral nutrition aimed at 20-35 kcal/Kg/day, and 1.0 g/kg/day of amino-acids. Weight, body mass index, type of tumor, type of chemotherapy, and ECOG among others were recorded and analyzed. Results: 69.1% of the patients had gastrointestinal tumors, 18.2% gynecologic and 12.7% others. Age was 60 ± 13y, baseline ECOG 1.5 ± 0.5 and body mass index 21.6 ± 4.3. Malnutrition was present in 85%. Survival from the start of parenteral nutrition was not significant when considering baseline ECOG (log rank = 0.593, p = 0.743), previous lines of chemotherapy (log rank = 2.117, p = 0.548), baseline BMI (log rank = 2.686, p = 0.261), or type of tumor (log rank = 2.066, p = 0.356). Survival in patients who received home parenteral nutrition after hospital discharge was higher than those who stayed in-hospital (log rank = 7.090, p = 0.008). Survival in patients who started chemotherapy during or after parenteral nutrition was higher than those who did not so (log rank = 17.316, p < 0.001). A total of 3.6% of patients presented catheter related infection without affecting survival (log rank = 0.061, p = 0.804). Conclusions: Parenteral nutrition in patients with advanced cancer and intestinal occlusion is safe, and in those who respond to chemotherapy, further administration of home parenteral nutrition together with chemotherapy may enhance prolonged survival. Introducción y objetivos: el papel preciso de la nutrición parenteral en el manejo de los pacientes oncológicos con obstrucción intestinal no está bien definido todavía. El objetivo del presente trabajo es evaluar los efectos de la nutrición parenteral en este tipo de pacientes en cuanto al pronóstico. Material y métodos: fueron incluidos 55 pacientes con obstrucción intestinal y carcinomatosis peritoneal. La nutrición parenteral proporcionó 20-35 kcal/Kg/día y 1.0 g/kg/día de aminoácidos. El peso, el IMC, el tipo de tumor, el tipo de quimioterapia recibida y el ECOG, entre otras variables, fueron recogidas y analizadas. Resultados: un 69,1% de los pacientes presentaban tumors gastrointestinales, un 18,2% ginecológicos y otros tumores el 12,7% restante. La edad media fue de 60 ± 13 años, con un ECOG basal de 1,5 ± 0,5 y un IMC de 21,6 ± 4,3. La presencia de malnutrición fue de un 85%. La supervivencia desde el inicio de la nutrición parenteral no fue significativamente distinta entre los pacientes al considerar su ECOG basal (log rank = 0,593, p = 0,743), las líneas previas de quimioterapia recibida (log rank = 2,117, p = 0,548), el IMC basal (log rank = 2,686, p = 0,261), o el tipo de tumor (log rank = 2,066, p = 0,356). La supervivencia en los pacientes en que fue posible el alta hospitalaria con nutrición parenteral fue superior (log rank = 7,090, p = 0,008). La supervivencia en los pacientes en que se inició la quimioterapia durante o tras iniciar la nutrición parenteral fue también superior (log rank = 17,316, p < 0,001). Un total de 3,6% de los pacientes presentaron infección relacionada con el catéter sin afectar la supervivencia (log rank = 0,061, p = 0,804). Conclusión: la nutrición parenteral en los pacientes oncológicos con obstrucción intestinal y carcinomatosis peritoneal es segura y, en aquellos que responden a qui-mioterapia, el uso de la nutrición parenteral domiciliaria, junto con en tratamiento antitumoral activo, aumentan la supervivencia.

15. Clinical and epidemiological profiles of intestinal coccidian infection in Mirimire, Falcon state, Venezuela

En investigaciones previas, se han estudiado los patrones clínico-epidemiológicos de las enterococcidiosis en la zona semiárida del estado Falcón, región nor-occidental venezolana. En un intento por comparar estos patrones con los de otras regiones bioclimáticas, se realizó un estudio entre septiembre 2013 y enero de 2014, para caracterizar los perfiles clínicos y epidemiológicos de la infección por coccidios intestinales en 238 habitantes de Mirimire, estado Falcón, Venezuela. Se hizo evaluación clínica y anamnesis para la búsqueda de signos y síntomas que comúnmente se encuentran asociados a las coccidiosis intestinales. El diagnóstico parasitológico se realizó mediante los métodos directo y coloración de Kinyoun. La prevalencia global de parasitosis intestinales fue de 71,84% (171/238), siendo Cryptosporidium spp. (53,36%) y Blastocystis spp. (51,26%) los taxa más frecuentemente detectados. La prevalencia de coccidiosis intestinal fue de 56,30% (134/238), y las de ciclosporiosis y cystoisosporiosis de 40,76% y 5,88%, respectivamente. La diarrea (Odds Ratio [OR] = 2,33) y cólicos (OR = 2,42) fueron las manifestaciones clínicas características significativamente relacionadas con estas infecciones entero-parasitarias (p ≤ 0,05). Convivir en hacinamiento (Odds Ratio = 1,11) y colgar la vestimenta juntas (OR = 1,26), y que se realice el lavado de los bebederos y comederos de animales cada 15-30 días (OR = 1,82) y no se les asee regularmente (OR = 1,74), aparecen como potenciales factores de riesgo significativamente asociados en la dinámica de transmisión y mantenimiento endémico de las coccidiosis intestinales. Se recomienda mejorar las condiciones socio-sanitarias del sector y mantener campañas de promoción y educación para la salud para el control y prevención de las coccidiosis entérica. In previous surveys, clinical and epidemiological patterns of enteric coccidiosis have been studied in a semiarid zone of Falcon state, Venezuelan north-western region. As an attempt to compare these patterns with that of other bioclimatic zones, a descriptive, cross sectional study was conducted during September 2013 and January 2014, to determine the clinical and epidemiological profiles of intestinal coccidian infection in 238 inhabitants in the rural community of Mirimire, Falcon state, Venezuela. Parasitological diagnosis was made by employing direct smear and Kinyoun staining. The overall prevalence of intestinal parasitic diseases was 71.84 % (171/238), being Cryptosporidium spp. (53.36%) and Blastocystis spp. (51.26%) the parasitic taxa most frequently detected. Coccidiosis prevalence was 56.30 % (134/238), and 40.76% and 5.88% for ciclosporiosis and cystoisosporiosis, respectively. Diarrhea (Odds Ratio [OR] = 2.33) and intestinal colics (OR = 2.42) were the clinical features significantly associated with these entero-parasitic infections (p ≤ 0.05). Overcrowded living conditions (Odds Ratio = 1.11) and hanging clothes together (OR = 1.26), as well as the cleaning frequency of animal feeders and drinkers every 15-30 days (OR = 1.82) and their non-regularly bathing (OR = 1.74), seem to be potential factors significantly associated with the transmission dynamic and endemic maintenance of enteric coccidian. Efforts to improve socio-sanitary conditions and to promote hygienic practices and health education were highly recommended in order to control and prevent enteric coccidiosis.

Introduction and aims: the precise role of parenteral nutrition in the management of oncologic patients with intestinal occlusion is not well defined yet. We aimed to identify the effects of parenteral nutrition in these patients regarding prognosis. Material and methods: 55 patients with intestinal occlusion and peritoneal carcinomatosis were included. Parenteral nutrition aimed at 20-35 kcal/Kg/day, and 1.0 g/kg/day of amino-acids. Weight, body mass index, type of tumor, type of chemotherapy, and ECOG among others were recorded and analyzed. Results: 69.1% of the patients had gastrointestinal tumors, 18.2% gynecologic and 12.7% others. Age was 60 ± 13y, baseline ECOG 1.5 ± 0.5 and body mass index 21.6 ± 4.3. Malnutrition was present in 85%. Survival from the start of parenteral nutrition was not significant when considering baseline ECOG (log rank = 0.593, p = 0.743), previous lines of chemotherapy (log rank = 2.117, p = 0.548), baseline BMI (log rank = 2.686, p = 0.261), or type of tumor (log rank = 2.066, p = 0.356). Survival in patients who received home parenteral nutrition after hospital discharge was higher than those who stayed in-hospital (log rank = 7.090, p = 0.008). Survival in patients who started chemotherapy during or after parenteral nutrition was higher than those who did not so (log rank = 17.316, p < 0.001). A total of 3.6% of patients presented catheter related infection without affecting survival (log rank = 0.061, p = 0.804). Conclusions: Parenteral nutrition in patients with advanced cancer and intestinal occlusion is safe, and in those who respond to chemotherapy, further administration of home parenteral nutrition together with chemotherapy may enhance prolonged survival.

- Abstract
Rheumatoid arthritis (RA) corresponds to a complex autoimmune disorder characterized by marked inflammation and progressive destruction of the tissues that form the joints. Medical treatment is aimed at suppressing the inflammatory process, thus mitigating the symptoms and signs of the disease. The nutritional intervention based on food sources of polyunsaturated fatty acids (AGPICL ω-3) such as: fish, salmon, trout, herring, sardines, tuna, anchovies, swordfish act as coadjuvants in the treatment of inflammatory-based diseases. The objective of this study was to analyze the available evidence on the therapeutic effects of polyunsaturated fatty acids in the chronic inflammatory state and in the symptomatology in patients with RA. The review consisted of bibliographic search in 2017, with writtings in Spanish or English published in the databases Medline, Cochrane, Scopus, Sportdiscus and Amed, between 2001 and 2017. It was demonstrated that essential fatty acids in rheumatoid arthritis show reduced pain in joints, and/or inflamed, of morning stiffness. The effects produced by the consumption in the feeding of essential fatty acids, it is useful as treatment of acute and chronic inflammation and the decrease of the doses of NSAIDs. La retroperitoneoscopia urológica pediátrica presenta tres etapas diferenciadas que han condicionado su desarrollo, aunque no impedido: 1) limitadas indicaciones en la edad pediátrica, debido a que la cirugía pediátrica convencional se caracteriza por ser poco invasiva; 2) adecuación del desarrollo tecnológico realizado en el adulto a la edad pediátrica; 3) superar las controversias entre cirujanos pediátricos y no laparoscopistas. Tras superar ésta etapa, la retroperitoneoscopia se ha impuesto como herramienta imprescindible para el tratamiento de las diversas patologías de la celda renal, siendo de las indicaciones ablativos la nefrectomía su "gold estándar", y de las reconstructivas la pieloplastia, por abordaje retroperitoneoscopico completo o asistido. Urologic pediatric retroperitoneoscopy has had three different stages that have conditioned, although not hindered, its development: 1) limited number of indications in the pediatric age, because pediatric surgery itself is not much invasive; 2) adaptation of the technological development from adult to children; 3) overcoming the controversies between laparoscopic and non laparoscopic pediatric surgeons. After overcoming these stages, retroperitoneoscopy has become an indispensable tool for the treatment of various diseases of the kidney, being nephrectomy the gold standard among the indications for ablation, and pyeloplasty among reconstructive, through a complete or assisted retroperitoneoscopic approach.

- Abstract
Background: Home parenteral nutrition (HPN) has become a common therapy, with tunneled central venous catheters (CVCs) being preferred for its administration. Peripherally inserted central catheters (PICCs) are not currently recommended for long-term HPN, although evidence to support this statement is scarce. The authors aimed to evaluate the outcomes of HPN, focusing on CVC-related complications., Materials and Methods: All patients attended at the authors' center for HPN from 2007-2011 were prospectively included. HPN composition aimed at 20-35 kcal/kg/d, 3-6 g/kg/d of glucose, 1.0 g/kg/d of amino acids, and <1 g/kg/d of lipids. HPN was infused in an intermittent schedule, mostly at night. Catheter-related bloodstream infections (CRBSIs) were confirmed with positive semi-quantitative or quantitative culture of the catheter or simultaneous differential blood cultures drawn through the CVC and peripheral vein., Results: Seventy-two patients received HPN, with 79 implanted CVCs (48 PICCs, 10 Hickman, and 21 ports). Mean catheter-days were 129.1 for PICCs, 98.5 for Hickman, and 67.7 for ports (P = .685). When analyzing CRBSIs, ports had 44, Hickman had 20, and PICC had 0 episodes per 1000 catheter-days (P = .078). Only PICCs showed less incidence of CRBSIs vs ports (P = .043). Multivariate logistic regression, correcting by catheter-days, patients' age and sex, underlying disease, and type of catheter, showed that only catheter-days (P = .031) was a predictor for CRBSIs (P = .007, Nagelkerke R= = 0.246)., Conclusion: PICCs are similar in terms of catheter-related complications to other CVCs for the administration of HPN, especially for oncology patients with HPN lasting <6 months.
