The current treatment objective is to improve the clinical manifestations of the disease in the affected areas, to relieve symptoms and to halt disease progression. Patients with early-stage mycosis fungoides are usually managed with skin-directed therapies, whereas patients with resistant or advanced-stage mycosis fungoides or Sèzary syndrome often require systemic drugs. Over the last decade, new drugs have been developed, increasing the breadth of treatment options for cutaneous T-cell lymphomas patients. Mogamulizumab is a first-in-class defucosylated humanized IgG1 κ monoclonal antibody, which exerts its anti-tumour action by selectively binding to C-C chemokine receptor 4 and increasing antibody-dependent cellular cytotoxicity activity against malignant T-cells. Several clinical trials showed that mogamulizumab is able to effectively control the cutaneous T-cell lymphomas in each site (skin, blood, lymph nodes and viscera), improving patients’ symptoms, function and overall quality of life with a manageable safety profile. In this report, we discuss 12 cases of patients with mycosis fungoides or Sèzary syndrome successfully treated with mogamulizumab in real-life clinical practice in Italy.Keywords: cutaneous T- cell lymphoma, mycosis fungoides, Sèzary syndrome, mogamulizumab

7. Replantation dilemma: lessons learned from managing a dog bite forearm amputation in a sixteen-year-old girl.

Abstract

BACKGROUND: Bite injuries, particularly those involving the hands, present a significant medico-legal challenge, often leading to complications and frequent emergency department visits. Dog and cat bites, especially among children, are major contributors to infections due to the complex anatomy of the hand, which predisposes it to severe infections even from minor bites. Capnocytophaga canimorsus, found in the oral cavity of dogs and cats, is particularly concerning due to its potential to cause severe infections. Prompt and appropriate treatment is essential to mitigate these risks. Managing such injuries poses significant challenges, necessitating clear guidelines for reporting and safety measures. This article highlights the urgent need for additional research, support, and education, particularly focusing on children, along with the development of international guidelines to improve outcomes for patients. CASE REPORT: A case study of a sixteen-year-old girl who had her left forearm amputated due to a rottweiler bite is presented. Despite initial attempts at replantation, complications led to the decision for amputation. CONCLUSIONS: This case underscores the challenges in managing severe dog bite injuries, emphasizing the importance of prompt assessment, thorough debridement, and proper wound management to minimize complications. Additionally, psychological evaluation and treatment are crucial for patients and parents following such traumatic events. From a medical standpoint, this case highlights the importance of monitoring inflammatory markers, appropriate surgical priorities, and the need for psychological support. Prevention of dog bites is crucial, requiring increased awareness among public authorities and dog owners. Abstract

The Erice 2018 Charter was unanimously approved at the conclusion of the 53rd Residential Course of the International School of Epidemiology and Preventive Medicine “Adapted Physical Activity in Sport, Wellness and Fitness; the role of the Departments of Prevention and of the National Health Service in doping prevention and health promotion”, held on 15-19 May 2018 in Erice, Italy, at the “Ettore Majorana” Foundation and Centre for Scientific Culture, and promoted by the Study Group on “Movement Sciences for Health” of the Italian Society of Hygiene, Preventive Medicine and Public Health. The event was part of a larger project supported by the Ministry of Health aimed at preventing doping in the general population involved in sport and physical activities. After an intense discussion the participants focused on ten statements involving the following critical issues: responsibility, priority, message, alphabetization, networks and alliances, school promoting health, player and opportunities, competences, know-how, programming and acting. These statements provide hints to approach doping within a public health frame and summarize the role of the Departments of Prevention and NHS in promoting and coordinating preventive actions with other institutions and stakeholders. Doping represents a complex phenomenon related to cultural, social, economic and legal issues. In addition to regulatory or repressive actions, education to health and legality is proposed as the fundamental strategy to contrast doping by promoting healthy lifestyles, based on scientific knowledge and respect for legality.

33. Sleep/wake rhythm modifications in the Italian population during SARS-CoV2 pandemic: a web-based cross-sectional survey.

Abstract

OBJECTIVE: The study aims to investigate in a representative sample of the Italian population whether the SARS-CoV2 pandemic and the subsequent home isolation had repercussion on the daily sleep/wake cycling and habits. MATERIALS AND METHODS: A web-based cross-sectional survey consisted of various multiple- choice questions concerning demographic characteristics, sleep habits, and sleep-related problems was broadcast through mainstream social-media. Individuals were randomly allowed to participate from April 29th to May 17th, namely 50 days after the lockdown imposition and the day before its abrogation. RESULTS: 58.84% of respondents experienced a change in their sleep habits. 71% of those whose sleep changed showed a delayed sleep pattern. Overall, a two-fold risk of delayed sleep pattern without any change in total sleep time emerged during the investigation period. Females emerged almost 2 times more likely to modify their sleep habits than males. Youths were also more likely to experience modifications than old people, who conversely appeared protected. A significant improvement in daytime sleepiness occurred during the home isolation which additionally correlated with delayed bedtime and less sleep time. CONCLUSIONS: A high rate of change in sleep habits, especially among youths and females, occurred in Italian population during the home isolation to limit the SARS-CoV2 pandemic. Abstract

Observational prospective population-based incidence study. The main objective of this study was to assess the incidence of traumatic spinal cord injuries (TSCIs) and incidence rates, in order to provide estimates by age, gender, characteristics and cause. This study was conducted at acute-care spinal cord injury (SCI) hospitals and SCI centers from 11 Italian regions, between 1 October 2013 and 30 September 2014. Data of all consecutive patients with acute TSCI who met the inclusion criteria were obtained through case reporting by clinicians. The data were collected into a web database. Incidence rates and incidence rate ratios were calculated and stratified by age, gender, cause, level and completeness. From 50% of the entire population of Italy, 445 new cases of TSCI were included. The crude incidence rate of TSCI was 14.7 cases per million per year (95% CI: 13.4–16.4); the overall male to female ratio was 4:1 and the mean age was 54. Complete information was available in 85% of the sample and revealed tetraplegia in 58% and incomplete lesion in 67% of cases. The leading cause of TSCI was falls (40.9%) followed by road traffic accidents (33.5%). The leading cause was falls for patients over 55 and road traffic accidents for patients under 55. The changing trend of TSCI epidemiology concerns the increase in the average age of TSCI people and the increase of both cervical and incomplete lesions. The etiology shows the primacy of falls over road traffic accidents and suggests the need for a change in prevention policies.

Abstract

Pain and secretion of purulent materials are symptoms that are often associated with the pilonidal sinus. Generally, these symptoms are neglected by patients for a long time. Patients seek medical attention too late, fearing a prolonged period of pain and inability after surgery. The optimal therapy for pilonidal sinus should have the following characteristics: high healing rate, low recurrence rate, minimal postoperative pain and low cost. The aim of this study is to explore the effectiveness and safety of a new minimally invasive treatment: the scarless outpatient ablation of pilonidal sinus (SOAP). A total of 31 consecutive patients were enrolled; 27 of 31 patients were declared healed after the surgery (87·09%). After 1 year, only 4 of 27 patients (14·81%) reported symptoms related to recurrence of the disease. The average duration of the operation was 7·47 minutes. Patients reported the disappearance of painful symptoms after approximately 2·62 days and had been away from work for 0·53 days. No complications were recorded during the period of study. The advantages of the procedure examined in this study are its simplicity and rapidity of execution, its outpatient setting, its low cost and lack of complications. In our opinion, a randomised controlled trial should be conducted to validate the results related to this technique.

Abstract

Dune characterization in coastal zones is important for planning coastal defenses, groundwater management, and tourism development. Dune protection should be one of the main goals of any Integrated Coastal Zone Management Plan (ICZM). Coastal dunes efficiently oppose saltwater intrusion: their topographic elevation and their excellent infiltration capacity provide freshwater recharge and a hydraulic head that protect from saline intrusion according to the Ghyben-Herzberg principle (Fetter, 2001). The lateral continuity of the dunes system is also important for creating a continuous freshwater lens along the coast. As part of the Interreg IIIc Beachmed – “PosiDune project”, a monitoring campaign for groundwater levels and quality is being conducted on some test sites along stretches of the Ravenna coast: P.to Corsini, Marina Romea, Marina di Ravenna, Bevano river mouth. Dunes are still preserved in these areas and they act as natural defenses against storms, erosion and wave attack (Caruso et al., 2004; Caruso et al. 2006). A monitoring network has been established in each test site by placing piezometers both on the back and the crest of the dune and by carrying out a VES (Vertical Electri Sounding) geophysical campaign. The two methods (giving the chance to compare direct – piezometers - and indirect – geoelectric - data) has allowed to characterize the thickness of the fresh groundwater lens and the depth of the freshwater-saltwater interface. These actions have been integrated using: 1. a G.I.S. for data management; 2. a topographic and bathymetric reconstruction of the area using a LIDAR (Light detection and Ranging) survey; 3. a systematic seasonal update of the topography base and geomorphology using DGPS and theodolite surveys; 4. a survey of the geomorphologic evolution of the area through the periodical (monthly) acquisition of aerial images (flight height at 300m). The techniques employed are aimed at a precise charactetrization of the morphologic and hydrogeologic framework of the study areas. In addition, the maintenance of the monitoring system in future years will allow a better understanding of the effects of vegetation, dune restoration, and climate change to coastal hydrology and to ecosystem development (Gabbianelli & Sangiorgi, 2004; Ranjan et al. 2006). A clear comprehension of the role of dunes, achieved with this project, will be helpful for the management of the sites studied, especially considering their naturalistic values and peculiarities (they all belong to the Ramsar Zones list, SIC/ZPS sites list, Regional Po Delta Park – Emilia Romagna Region areas)

La spinta liberista impressa dai cambiamenti degli anni ’90 ha coinvolto sotto diversi aspetti la società italiana, modificando anche lo sport e i modelli di gestione delle strutture sportive, sorti nelle città italiane per ospitare le partite della World Cup. Il capitolo considera i Mondiali “ITALIA’90” una sorta di spartiacque anche per i processi politici che prepararono e gestirono l’evento, e che definirono le caratteristiche degli stadi. Dai rivolgimenti istituzionali successivi – in particolare “Tangentopoli” – derivarono cambiamenti importanti nella gestione delle strutture sportive in Italia e delle politiche per lo sviluppo delle città. Il capitolo individua il modello emergente di stadio che si affermò a partire dalla fine degli anni ’90 e offre un bilancio critico delle trasformazioni avvenute negli ultimi due decenni e del passaggio al nuovo modello di politica urbana.

Abstract

During the last few decades coastal dunes, for centuries a distinctive feature of Ravenna’s coastline, have been seriously affected by tourist development and human activities. Considering the importance of these structures as natural defences against beach erosion from sea storms, the main purpose of this work is to characterize the dunes and to reconstruct their recent evolution from both a botanical and a geo-environmental point of view. Another purpose is to evaluate the future natural evolution of dunes in this type of coast.
