Abstract

Urinary tract infections (UTIs) are among the most frequent bacterial diseases in infants and children. Physician adherence to recommendations is notoriously often poor, but no data are available on UTIs management in the emergency setting. In this multicenter national study, we investigated the policies regarding UTIs management in children aged 2 months to 3 years in Italian emergency units. Between April and June 2021, directors of the emergency units were invited to answer an online survey on the following items: diagnostic approach to children with fever without an apparent source, therapeutic approach to UTIs, the use of kidney and urinary tract ultrasound, and the criteria for hospitalization. A total of 121 (89%) out of 139 of invited units participated in the study. Overall, units manage children with a suspected or confirmed UTI according to available recommendations for most of the items. However, in almost 80% (n = 94) of units, a sterile perineal bag is used to collect urine for culture. When urine is collected by cathether, heterogeneity exists on the threshold of bacterial load considered for UTI diagnosis. Conclusions: Available recommendations on UTIs in children are followed by Italian emergency units for most of the items. However, the methods to collect urine specimens for culture, one of the crucial steps of the diagnostic work-up, often do not align with current recommendations and CFU thresholds considered for diagnosis largely vary among centers. Abstract

© 2015 Wiley Periodicals, Inc.. Biorepositories, or biobanks, provide researchers with access to biological samples and associated data in support of translational research. Efficient operation and ethical stewardship of biobanks involves coordinated efforts among multiple stakeholders including researchers who manage and use the repository, institutional officials charged with its oversight, and patients and volunteers who contribute samples and data. As advancements in translational research increasingly involve more data derived from larger numbers of diverse samples, the size and governance challenges facing biorepositories have grown. We describe an approach to developing efficient and ethical biobank governance that includes all major stakeholders. This model provides a pathway for addressing the technical and ethical challenges that must be resolved to ensure biorepositories continue to support translational research.

Abstract

Ketanserin is a pure and selective antagonist of serotonin S2-receptors in blood vessels, platelets and bronchial tissue. It antagonizes serotonin-induced vasoconstriction, bronchoconstriction and platelet aggregation, and indirectly it blocks platelet release reaction. Ketanserin has little or no effect on healthy subjects. Serotonin-induced or serotonin-potentiated platelet aggregation is inhibited in blood drawn from ketanserin-treated healthy volunteers. Oral or parenteral ketanserin treatment did not cause major changes in beta-thromboglobulin (beta-TG) and platelet factor 4 (PF4) plasma concentrations, when basic values were normal. Increased microaggregate formation was found in alcoholics and heavy drinkers. It was also found that beta-TG and PF4 levels were higher in these patients than in the controls. Ketanserin treatment tended to normalize these protein levels in such patients.

Abstract

Infill-frame interaction is commonly modeled by means of equivalent strut macro-models. The main difficulty in using this phenomenological approach is to properly calibrate a force-displacement curve for the struts, as this depends not only on the geometrical and mechanical properties, but more specifically on the different potential damage mechanisms which may occur for the infill-frame system subject to lateral forces. If fiber-section elements are used as diagonal struts, the force-displacement curve is substituted by a stress-strain law. In both cases, the attribution of the inelastic law of the struts, based on a mechanical approaches, is not valid in general, as mechanical approaches are referred to specified hypotheses on the collapse modalities, which are really difficult to predict a priori. Based on this, the paper proposes a new concrete-type stress-strain relationship to be used with fiber-section elements and calibrated using a semi-empirical approach rather than a mechanical one. The proposed stress-strain model depends on four parameters. Experimental data and additional numerical simulations are used to derive semi-empirical correlations associating geometrical and mechanical properties of the frame-infill system with the aforementioned parameters. Analytical correlations of the best fitting equations are provided as tools for direct identification. Validation tests are finally carried out with experimental results different from those used to build the correlations.

13. Ketanserin, hypertension, and chronic alcoholism: double-blind study in forty patients.

Abstract

In a randomized double-blind trial involving 40 alcoholic hypertensive patients, the antihypertensive activity of ketanserin, a serotonin antagonist with high affinity for S2 serotonergic receptors, was compared with a placebo. Patients in both groups were matched for age, body weight, blood pressure, alcoholic consumption, and length of alcoholism. The administration of ketanserin significantly reduced (p less than 0.001) mean supine blood pressure from 167/106 mmHg (22.3/14.1 kPa) at baseline to 145/87 mmHg (19.3/11.6 kPa) after 90 days of treatment versus a slight non-significant reduction with the placebo. No significant changes in heart rate, body weight, or laboratory parameters occurred. The incidence of side-effects was low in both groups. The results of this study suggest the possible role of serotonin in the pathogenesis of alcohol-related hypertension and the potential treatment of the disease using S2-receptor antagonists such as ketanserin.
