An important vector for host-to-host infectious disease transmission is given by the transport of tiny pathogen-laden droplets. These are commonly exhaled by individuals while breathing, speaking, coughing, or sneezing. Depending on their size and ambient conditions, they may follow different paths, either settling on surfaces where the pathogen can be further transmitted by contact, or remaining airborne after evaporation where the pathogen can be inhaled. Our understanding of pathogen transmission from the fluid mechanics perspective is still somewhat limited, especially in quantitative terms. In the current work, starting from the fundamental laws of fluid mechanics and diffusion, a detailed analytical model of droplet transport and evaporation in humid air streams is presented and successfully validated against available data in the literature finding remarkable agreement. The model implements closed-form analytical solutions of the equations of transport, evaporation, and energy balance, and an algebraic model to account for the droplet chemical composition. It also features an analytical model of droplet transport within the buoyant exhaled breath cloud based on momentum conservation addressing both jet and puff phases and is able to handle periodic respiratory events. Turbulent dispersion is modeled with a discrete random walk approach. A simple inhalation model is also proposed. Such a model may help in better understanding droplets' fluid dynamic behavior and may be used to assess the risks associated with pathogen transmission under different scenarios for any type of respiratory event. From Poiseuille theory, it is known that incompressible laminar fully-developed flow of a Newtonian fluid in a constant cross-section channel is characterised by steady parabolic velocity profiles after a fully-developed flow condition is attained. In turbulent fully-developed flow the velocity profiles are non-parabolic and become more flat for higher Reynolds numbers. When the incompressible hypothesis does not hold, as in the case of high velocity ideal gas flow, the velocity profile becomes flatter, as if more turbulent, due to the superposition of compressibility and turbulence effects, if applicable. This is typical in microchannel flows, where pressure gradients are high and the gas is rapidly accelerating, eventually up to the sound velocity. As the flow accelerates the effects of compressibility grow stronger and the velocity profile keeps changing shape. The radial velocity component does not zero as in fully-developed flow but reverses after the entrance effects have damped out and grows with the Mach number. A net mass transfer toward the walls is thus generated making the velocity profile more flat. This affects the friction factor which is no longer constant, being proportional to the normal-to-wall velocity gradient, and needs to be evaluated. In the present work, the compressible friction factor is numerically investigated and correlations are proposed based on the velocity profile shape evolution as a function of the Mach number. This, together with other considerations on the velocity profile shape change, is shown to enhance the predictive capability of the Fanno theory for compressible flows.

© 2019 The Authors. Micro-scale fluid systems are becoming common in many applications ranging from electronic cooling to refrigeration systems and more. One-dimensional numerical models represent a simple and fast tool for the design of such devices, yet they struggle to accurately predict the flow characteristics in compressible micro-flows. Under the adiabatic assumption, the elegant theory developed by Fanno allows models for the viscous compressible flow in constant cross-section channels to be easily built. Although reasonably accurate, these models suffer from drawbacks inherent to their being one-dimensional, as such they cannot take into account the local profiles of quantities like the velocity and the temperature. In cascade, this results into incorrect evaluations of other dependent quantities, such as the dynamic pressure and the fluid thermophysical properties. The mismatch turns large when the fluid compressibility becomes important. As the Mach number grows, the velocity profile changes, and so the friction factor, even though a reliable model for predicting this change is still missing. In fact, a constant friction factor throughout the channel is generally assumed, following the incompressible flow theory. Here, a set of correlations is proposed improving the 1D theory accuracy by taking into account the effects of the non-uniform velocity and temperature profiles in a quasi-2D fashion. A detailed analysis of the velocity profiles at different Mach numbers coming from a large set of CFD simulations results in a model for assessing the impact of compressibility on friction and other quantities. The numerical model proposed, being able to properly account for the compressibility effects, offers an improved tool for the design of micro-scale fluid systems. Extending the analysis to include heat transfer is not difficult as the effect of heat flux will be analogous to the effect of pressure drop due to friction.

RCW 86 is a young supernova remnant (SNR) showing a shell-type structure at several wavelengths and is thought to be an efficient cosmic-ray (CR) accelerator. Earlier \textit{Fermi} Large Area Telescope results reported the detection of $\gamma$-ray emission coincident with the position of RCW 86 but its origin (leptonic or hadronic) remained unclear due to the poor statistics. Thanks to 6.5 years of data acquired by the \textit{Fermi}-LAT and the new event reconstruction Pass 8, we report the significant detection of spatially extended emission coming from RCW 86. The spectrum is described by a power-law function with a very hard photon index ($\Gamma = 1.42 \pm 0.1_{\rm stat} \pm 0.06_{\rm syst}$) in the 0.1--500 GeV range and an energy flux above 100 MeV of ($2.91$ $\pm$ $0.8_{\rm stat}$ $\pm$ $0.12_{\rm syst}$) $\times$ $10^{-11}$ erg cm$^{-2}$ s$^{-1}$. Gathering all the available multiwavelength (MWL) data, we perform a broadband modeling of the nonthermal emission of RCW 86 to constrain parameters of the nearby medium and bring new hints about the origin of the $\gamma$-ray emission. For the whole SNR, the modeling favors a leptonic scenario in the framework of a two-zone model with an average magnetic field of 10.2 $\pm$ 0.7 $\mu$G and a limit on the maximum energy injected into protons of 2 $\times$ 10$^{49}$ erg for a density of 1 cm$^{-3}$. In addition, parameter values are derived for the North-East (NE) and South-West (SW) regions of RCW 86, providing the first indication of a higher magnetic field in the SW region., Comment: 13 pages, 10 figures, accepted for publication in ApJ; Corresponding authors : Benjamin Condon, Marianne Lemoine-Goumard, Micaela Caragiulo

- Abstract
Background: The kidney-brain interaction has been a topic of growing interest. Past studies of the effect of kidney function on intracerebral hemorrhage (ICH) outcomes have yielded inconsistent findings. Although the second, main phase of the Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trial (INTERACT2) suggests the effectiveness of early intensive blood pressure (BP) lowering in improving functional recovery after ICH, the balance of potential benefits and harms of this treatment in those with decreased kidney function remains uncertain. Study Design: Secondary analysis of INTERACT2, which randomly assigned patients with ICH with elevated systolic BP (SBP) to intensive (target SBP < 140 mm Hg) or contemporaneous guideline-based (target SBP < 180 mm Hg) BP management. Setting & Participants: 2,823 patients from 144 clinical hospitals in 21 countries. Predictors Admission estimated glomerular filtration rates (eGFRs) of patients were categorized into 3 groups based on the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) creatinine equation: normal or high, mildly decreased, and moderately to severely decreased (>90, 60-90, and <60 mL/min/1.73 m2, respectively). Outcome(s): The effect of admission eGFR on the primary outcome of death or major disability at 90 days (defined as modified Rankin Scale scores of 3-6) was analyzed using a multivariable logistic regression model. Potential effect modification of intensive BP lowering treatment by admission eGFR was assessed by interaction terms. Result(s): Of 2,623 included participants, 912 (35%) and 280 (11%) had mildly and moderately/severely decreased eGFRs, respectively. Patients with moderately/severely decreased eGFRs had the greatest risk for death or major disability at 90 days (adjusted OR, 1.82; 95% CI, 1.28-2.61). In patients with ischemic stroke, endovascular treatment results in a higher rate of recanalization of the affected cerebral artery than systemic intravenous thrombolytic therapy. However, comparison of the clinical efficacy of the two approaches is needed.We randomly assigned 362 patients with acute ischemic stroke, within 4.5 hours after onset, to endovascular therapy (intraarterial thrombolysis with recombinant tissue plasminogen activator [t-PA], mechanical clot disruption or retrieval, or a combination of these approaches) or intravenous t-PA. Treatments were to be given as soon as possible after randomization. The primary outcome was survival free of disability (defined as a modified Rankin score of 0 or 1 on a scale of 0 to 6, with 0 indicating no symptoms, 1 no clinically significant disability despite symptoms, and 6 death) at 3 months.A total of 181 patients were assigned to receive endovascular therapy, and 181 intravenous t-PA. The median time from stroke onset to the start of treatment was 3.75 hours for endovascular therapy and 2.75 hours for intravenous t-PA (P0.001). At 3 months, 55 patients in the endovascular-therapy group (30.4%) and 63 in the intravenous t-PA group (34.8%) were alive without disability (odds ratio adjusted for age, sex, stroke severity, and atrial fibrillation status at baseline, 0.71; 95% confidence interval, 0.44 to 1.14; P=0.16). Fatal or nonfatal symptomatic intracranial hemorrhage within 7 days occurred in 6% of the patients in each group, and there were no significant differences between groups in the rates of other serious adverse events or the case fatality rate.The results of this trial in patients with acute ischemic stroke indicate that endovascular therapy is not superior to standard treatment with intravenous t-PA. Rationale Intravenous (i.v.) thrombolysis with recombinant tissue-Plasminogen Activator (rt-PA) (alteplase) within three hours from symptom onset is the only approved treatment of pharmacological revascularization in acute ischaemic stroke. However, the current license limits the use of rt-PA to patients aged ≤80 years due to the lack of evidence of safety and efficacy of this treatment in the elderly from randomized clinical trials. This article describes the design of the Thrombolysis in Elderly Stroke Patients in Italy (TESPI) trial planned to fill the lack of controlled data on i.v. thrombolysis in this age category of stroke patients. Aims To collect efficacy and safety data on i.v. alteplase (rt-PA) in patients aged more than 80 years, to demonstrate that the treatment of these patients within three hours of symptoms onset of an acute ischaemic stroke with i.v. rt-PA, compared to patients receiving standard treatment (according to the national guidelines), will result in an improved clinical outcome with a favourable benefit/risk ratio. Design TESPI is a prospective, multicenter, national, open-label, controlled (non-treated group as control), randomized, parallel group trial with blinded evaluation of outcome in patients older than 80 years treated with i.v. rt-PA within three hours after ischaemic stroke onset. The randomization procedure assigns patients to the treatment group with IV alteplase (0.9 mg/kg of body weight) or to standard treatment group with a 1 : 1 basis. A three-month follow-up, when applicable, is performed by a blind assessor. Six hundred patients will be enrolled (300 patients per arm) The study period has been planned to be of three years. Study Outcomes The primary efficacy end-point is the disability at day 90, dichotomized as a favourable outcome (modified Rankin Scale 0–2) or unfavourable outcome (modified Rankin Scale 3–6). The main primary safety end-point is symptomatic intracerebral haemorrhage defined as any hemorrhage at the 22–36 h post-treatment scan combined with neurological deterioration leading to an increase of one or more points at the National Institutes of Health Stroke Scale. The TESPI trial, with the protocol number FARM65KNKY, is registered in the European Union Drug Regulating Authorities Clinical Trials database with the number 2007-006177-88 and in the Stroke Trials Registry of the Washington University Internet Stroke Center.

Background and Methods Multiple sclerosis often occurs in young women, and the effect of pregnancy on the disease is poorly understood. We studied 254 women with multiple sclerosis during 269 pregnancies in 12 European countries. The women were followed during their pregnancies and for up to 12 months after delivery to determine the rate of relapse per trimester and the score on the Kurtzke Expanded Disability Status Scale (range, 0 to 10, with higher scores indicating more severe disability). The relapse rate in each trimester was compared with the rate during the year before the pregnancy. The effects of epidural analgesia and breast-feeding on the frequency of relapse during the first three months post partum and the disability score at 12 months post partum were also determined. 42. Hyperemesis gravidarum complicated by Wernicke encephalopathy: Background, case report, and review of the literature

- Abstract
Wernicke encephalopathy (WE) is a rare but known complication of severe hyperemesis gravidarum caused by thiamine deficiency. This article presents an unusual case that occurred at our institution and reviews the 48 previously published cases of WE in pregnancy. Considering all the 49 cases, the mean (±standard deviation) patients' age was 26.7 ± 4.9 years, the mean gestational age when WE manifested was 14.3 ± 3.4 weeks, and the mean duration of vomiting and feeding difficulties was 7.7 ± 2.8 weeks. Wernicke's classic triad (confusion, ocular abnormalities, and ataxia) manifested in only 46.9% (23 of 49) of the patients. Confusion affected 63.3% (31 of 49) of the patients, ocular signs 95.9% (47 of 49) and symptoms 57.1% (28 of 49), and ataxia 81.6% (40 of 49). Deterioration of consciousness affected 53.1 % (26 of 49) of the subjects and memory impairment 61.2% (30 of 49). Complete remission of the disease occurred in only 14 of 49 cases. Symptom resolution required months and permanent impairments were common. The overall pregnancy loss rate, directly (spontaneous fetal loss) and indirectly (planned abortion) attributable to WE, was 47.9% (23 of 49). The diagnosis of WE is clinical and can be rapidly confirmed by magnetic resonance imaging. We emphasize the importance of thiamine supplementation to women with prolonged vomiting in pregnancy, especially before intravenous or parenteral nutrition. We also underline the necessity to promptly replace vitamin B1 when neurologic symptoms and/or signs develop in a patient with hyperemesis gravidarum.
