Breast cancer risk models represent the likelihood of developing breast cancer based on risk factors. They enable personalized interventions to improve screening programs. Radiologists identify mammographic density as a significant risk factor and test new imaging techniques. Pathologists provide data for risk assessment. Clinicians conduct individual risk assessments and adopt prevention strategies for high-risk subjects. Tumor genetic testing guides personalized screening and treatment decisions. Artificial intelligence in mammography integrates imaging, clinical, genetic and pathological data to develop risk models. Emerging imaging technologies, genetic testing and molecular profiling improve risk model accuracy. The complexity of the disease, limited data availability and model inputs are discussed. A multidisciplinary approach is essential for earlier detection and improved outcomes.

We describe the case of a patient with a clinical and molecular diagnosis of Beckwith-Wiedemann Syndrome (BWS) and a clinical, radiologic and histologic diagnosis of colon isolated hypoganglionosis. BWS is a genetic multisystem disorder characterized by generalized and lateralized overgrowth, macroglossia, abdominal wall defects, neonatal hypoglycemia and predisposition to embryonal tumors (Brioude et al., Nat Rev Endocrinol 1998; 14:229-249). Hypoganglionosis of the colon is a condition that clinically resembles Hirschsprung's disease and it is part of a group of numerous and heterogeneous conditions that are defined 'Variants of Hirschsprung's disease' (Friedmacher and Puri, Pediatr Surg Int 2013; 29:855-872). To the best of our knowledge, the association of BWS with Hirschsprung's disease has been observed only in one patient, an infant with hypoglycemia (Shah et al., BMJ Case Rep 2020; 13:e235121). We suppose that dysganglionosis could be rare comorbidity of BWS. We suggest to put particular attention to patients affected by BWS who develop early severe constipation taking into account the possibility to study them at radiologic and histologic levels to show the possible evidence of Hirschsprung's disease variants., (Copyright © 2021 Wolters Kluwer Health, Inc. Ultrasound scores are used to determine whether thyroid nodules should undergo Fine Needle Aspiration (FNA) or simple clinical follow-up. Different scores have been proposed for this task, with the American College of Radiology (ACR) TIRADS system being one of the most widely used. This study evaluates its ability in triaging thyroid nodules deserving FNA on a large prospective monocentric Italian case series of 493 thyroid nodules from 448 subjects. In ACR 1-2, cytology never prompted a surgical indication. In 59% of cases classified as TIR1c-TIR2, the FNA procedure could be ancillary, according to the ACR-TIRADS score. A subset (37.9%) of cases classified as TIR4-5 would not undergo FNA, according to the dimensional thresholds used by the ACR-TIRADS. Applying the ACR score, a total of 46.5% thyroid nodules should be studied with FNA. The ACR system demonstrated a sensitivity and specificity of 58.9% and 59% in the identification of patients with cytology ≥TIR3A, with a particularly high false negative rate for ACR classes ≥3 (44.8%, 43/96), which would dramatically decrease (7.3%, 7/96) if the dimensional criteria were not taken into account. In ACR 3-4-5, a correspondence with the follow-up occurred in 60.3%, 50.2% and 51.9% of cases. The ACR-TIRADS is a useful risk stratification tool for thyroid nodules, although the current dimensional thresholds could lead to an underestimation of malignant lesions. Their update might be considered in future studies to increase the screening performances of the system.

Acute pneumonitis following breast irradiation is a rare and transient phenomenon that can be easily managed by drugs. The aim of this study is to evaluate late sequelae on lung, after postoperative radiotherapy (RT) for breast cancer. We were concerned with investigating late radiological findings when very small lung volumes are involved in the irradiated volume. We studied 28 consecutive patients. They underwent clinical examination and all staging procedures before surgery, evaluation of pulmonary function with spirometry, postoperative chest x-ray and high resolution computed tomography (HRCT) of the lung before RT. Clinical examinations were usually performed every 3 months after RT. A second chest x-ray, HRCT and spirometry were carried out after nearly 7 months from the end of RT. We estimated the irradiated lung volume by measuring the area of the lung surface enclosed by the 50% isodose (LA50) in each profile. We found a significant correlation between LA50 and the score of radiological findings after RT. No correlations were found between other factors (i.e., adjuvant chemotherapy, age, weight, smoking) and lung fibrosis. No woman developed radiation pneumonitis syndrome or respiratory symptoms. Our results indicate that irradiation of the breast and/or chest wall is well tolerated if treatment planning is done accurately. The fibrosis likelihood is strongly correlated to the irradiated lung volume. The use of tangential fields limits radiological changes that can be detected only by HRCT examination and are not associated with clinical symptoms.

Introduction: Quality controls in radiotherapy allow to check the correct running of treatment units and to test our procedures. Portal films taken during the first treatment session are used in quality assurance programs to compare scheduled to administered doses., Material and Methods: To analyze the accuracy of patient positioning in pelvic cancer irradiation, a retrospective study was carried out on 50 treatment schedules carried out at the Radiotherapy Department of S. Anna Hospital (Como, Italy) from June to December, 1996. We checked field mispositioning for correlations with patient or treatment variables, such as patient age, sex, weight, thickness, or unit type. We took one portal film at the first treatment session and checked field positioning by measuring the distance of the isocenter from fixed anatomical structures on the simulation film and on the portal film taken in anteroposterior and lateral projections., Results: Four vectors were defined to evaluate field mispositioning along right-left (ADS), craniocaudal (ACC, LCC) and anteroposterior (AP) directions. The average values of these four vectors were respectively 2.94, 5.23, 5.54 and 3.20 mm. We found a major shift in field centering leftward and toward the patient's feet. To obtain more information about the total isocenter displacement, a vector T was calculated by summing the vectors ADS, ACC and LAP; a further evidence of field mispositioning is given by the vector T mean value (8.66 +/- 4.95 mm). No correlation was found between vector T values and any patient or treatment variable., Discussion: The acquaintance with uncertainties requires adequate statistical tools. A single check at treatment beginning could show a systematic error, but not the random fluctuations which can be recognized only with periodic portal films. To correct a possible systematic error without likely worsening set-up conditions, an adequate threshold value must be chosen for field mispositioning, according to each center's historical data., Conclusion: One portal film at the beginning of treatment is the minimum requirement in a quality assurance program. We feel the need to change our protocol and acquire more than one portal film, because the higher the number of portal films the easier the distinction of systematic from random errors. Using serial portal films, all at the first session, we will be able to introduce quantitative criteria for various action levels.

We illustrate a radiotherapy treatment chart elaborated to fulfil the necessity for clarity in reporting information about radiotherapeutic treatment. The schematic configuration of the chart results from the experience and the cooperation of physicists, physicians and technicians, and an effort has been made to satisfy Levels 2/3 of the ICRU 50 recommendations. The chart has been divided into four sections corresponding to different kinds of information: a cover sheet, a section containing data about the treatment planning geometry and the console parameters adopted, a section showing dosimetric data, and a section showing treatment data. The chart seems to give a good level of accuracy in reporting treatment plan information.

We report on a case of non-fatal myocardial infarction (MI) after electrocution. The diagnosis was made on the basis of electrocardiographic and enzymatic changes and was supported by the results of two-dimensional echocardiogram and radio-nuclide scans, showing segmental hypoperfusion and wall motion abnormalities. The patient was followed for over 8 years, evaluating the evolution of cardiac damage with the above tests. MI associated with electrical injury is very likely overestimated, owing to the low specificity of classical diagnostic criteria, such as ECG changes and CK-MB elevation, and the short monitoring of survivors. We suggest that non-invasive functional methods, exploring myocardial perfusion and ventricular kinesis, and a longer follow-up can reliably assess the prevalence of this complication.

Aims and Background: The role of radiotherapy in the treatment of stage III non-small cell lung cancer is controversial. The aim of this survey was to investigate the use of this modality in current clinical practice in Lombardy, a highly industrialized region of northern Italy., Methods: A questionnaire was sent to all 13 radiotherapy centers in Lombardy, covering statistical, clinical, technical and strategical aspects, and the responses were analyzed., Results: A wide range of attitudes was observed among participating radiation oncologists; the percentage of cases treated with curative intent varied largely between centers (4-100%), as did the proportion of patients given to radiation only rather than combined modality treatment (5-100% vs 0-90%)., Conclusions: An urgent need exists for better cooperation between all clinicians involved in lung cancer treatment, pursuing the goals of a more uniform clinical practice and a more aggressive clinical research attitude.

A case is reported of hemolytic anemia following rifampicin administration and complicated by acute renal failure. Furthermore clotting analyses suggested a slight disseminated intravascular coagulation, very likely activated by hemolysis products. Both hemolysis and renal function impairment subsided spontaneously, after the sole withdrawal of rifampin. Direct antiglobulin test became negative within a few days, while an indirect Coomb's test was demonstrated persistently with the patient's serum using red blood cells sensitized in vitro with the drug. Otherwise from all reports in the literature, the patient developed an acute hemolytic anemia while on daily therapy and as many as twenty years after a previous treatment with rifampicin. Mechanisms of drug-induced immune hemolytic anemia and acute nephropathy are discussed (formation of drug-antibody complexes, which adhere on the red blood cells surface and are able to fix complement and induce intravascular hemolysis; tubular necrosis due to hemoglobinuria or immuno-mediated interstitial nephritis).

March 1988 through April 1992, three hundred and ninety-six patients affected with bronchogenic carcinoma were treated at the Department of Radiation Oncology of the S. Anna Hospital, Como, Italy. A hundred and ten patients presenting stage-III non-small-cell lung carcinoma were evaluable. All evaluable patients underwent radiation therapy alone, with either palliative or curative purposes. Two main periods can be distinguished: in period A, before June 30th, 1990, treatment planning included conventional techniques, with no simulators; the patients were treated with opposing anteroposterior fields only. In period B, after July 1st, 1990, either the simulator alone was used or a simulator, a CT unit and a treatment planning computer system were combined; anteroposterior opposing fields or multiportal technique were used. Median overall survival was 10 months. Independent of treatment goals, the irradiated volume was markedly different in the patients treated in period A than in those treated in period B. A marked and statistically significant increase in survival was observed in group B. Survival also increased in patients treated with doses > 40 Gy, but only if treatment planning had used adequate technology and accuracy. To conclude, better survival can be achieved only by improving treatment accuracy and quality.

One hundred patients, admitted to the Emergency Unit for paroxysmal supraventricular tachycardia (SVT) with 1:1 AV conduction, atrial fibrillation (af) and flutter (AF) of recent onset (less than 72 hours) were treated with intravenous propafenone (P). The drug was administered at the dose of 70 mg over 5 min, repeated after 10 min if sinus rhythm (SR) was not restored and eventually followed by continuous infusion (0.35-0.50 mg/min) until conversion to SR or during the next 48 hours. Exclusion criteria were ventricular rate < 100/min, R-R intervals > 1 s, clinical signs of heart failure or asthma. Termination of SVT within 30 min was obtained in 94% of the patients, while reversion to SR occurred in 79% with af and in 55% with AF. For af and AF conversion was achieved within 30 min in 49% of overall responders (R), between 30 min and 6 hours in 27% and between 6 hours and 48 hours in 24%. The efficacy of P was significantly influenced by the duration of arrhythmia and left atrial size, measured by 2D-echocardiography. On the contrary, no difference was observed between R and non-R in mean age and in the percentage of primary or relapsing arrhythmias. Adverse effects were encountered in 7 patients: in 1 case worsened arrhythmia and in 6 patients, with long-lasting arrhythmias, congestive heart failure. Neither conduction disturbance nor extra-cardiac complications occurred. In conclusion, P provides effective and safe treatment for paroxysmal atrial tachyarrhythmias, so that it can be considered among the drugs of first choice even in non-intensive care units.

208 children, male and female, aged 6 to 12 years, grouped in 6-month steps, were tested for three spatial skills: two basic abilities, recognition of two lines of differing slope and of one line of differing slope in a frame and with no frame, and one more complex skill, route-finding by means of maps. The results support the hypotheses of neuropsychological development according to which the hemisphere strategies used in the course of cognitive development for the handling of spatial tasks differ by sex and may change before and after the age of ten.

Forty-eight children with unilateral cerebral lesions, incurred before or after the first year of life, were given the Wechsler Intelligence Scale for Children, with an average delays of 4 and 8 1/2 yr for the late and early injuries, respectively. Thirty-one of their siblings were chosen as controls. Results are as follows: early and late right-hemisphere lesions significantly lower only Performance IQ. Early left lesions impair equally Verbal and Performance IQ, while late left-hemisphere lesions do not significantly affect Verbal or Performance IQ relative to controls.

The records of 168 patients treated by definitive radiation therapy for non-oat-cell-carcinoma of the lung in the Radiotherapy Department of the "Ospedale Regionale Multizonale" of Varese, Italy, from may 1979 to december 1984, were analyzed. Cumulative probability of survival from the beginning of radiotherapy were 48 +/- 4% at 12 months and 8 +/- 3% at 60 months. It appears to be no significant difference in survival according to TNM classes, UICC stages, histology and grading. Significant differences in survival were found grouping patients by performance status (I.K. less than 70 v.s. greater than or equal to 70), total absorbed dose (dose less than 56 Gy v.s. greater than or equal to 56 Gy), total isoeffect levels (CRE less than 17 reu v.s. greater than or equal to 17 reu), and response to treatment. The latter could be the most important prognostic factor. Median survival for CR, PR and NC categories were 48.5, 12 and 10 months respectively. We suggest that RT may be of value in improving quality of life and survival of patients affected by non operable non-oat-cell-carcinoma of the lung.
