Oral doxycycline is more cost-effective for treating most patients with Lyme disease than intravenous ceftriaxone. Researchers created a model to decide which therapy was more cost-effective using several studies that evaluated both. In almost all cases, patients with early Lyme disease or Lyme arthritis would benefit most from oral doxycycline, which is substantially cheaper than intravenous ceftriaxone. The only possible exception would be patients in which the infection had spread to the nervous system. Physicians should try to identify such patients quickly so that intravenous ceftriaxone can be started.

Abstract

Ceftriaxone may be an effective and safe treatment for at least half the episodes of fever that occur in children with sickle cell anemia who are treated as outpatients. Sickle cell anemia is a genetic disorder characterized by sickle-shaped red blood cells. Among 86 sickle cell anemia patients between six months and 12 years old who had a temperature above 38.5 deg C, 44 were treated as outpatients and 42 were treated as inpatients. All the children were initially treated with 50 milligrams of intravenous (IV) ceftriaxone per kilogram of body weight. Outpatients returned to the hospital for a second dosage of ceftriaxone, and inpatients usually received a second dosage of ceftriaxone in the hospital. Eleven outpatients were hospitalized within two weeks of their initial treatment with ceftriaxone, compared with one inpatient. Treatment as an outpatient reduced the cost of medical care by $1,195 per episode of fever.

Abstract

A single dose of ceftriaxone may be as effective as 10 days of treatment with amoxicillin for severe otitis media. Otitis media is a common childhood infection that is characterized by inflammation of the middle ear. Among 233 children between five months and five years old with severe otitis media, 116 were treated with one intramuscular injection of ceftriaxone and an oral suspension of a placebo, or an inactive substance, for 10 days and 117 were treated with an oral suspension of amoxicillin for 10 days and one intramuscular injection of a placebo. Ninety-one percent of the children in each of the treatment groups recovered. Five children treated with ceftriaxone developed complications, compared to seven children treated with amoxicillin. Two-thirds of the parents said they would prefer giving their child a single injection to treat otitis media., This study evaluated the efficacy of a single dose of intramuscular ceftriaxone for acute otitis media in children, using amoxicillin as a control. (There is currently no established single-dose treatment for this condition.) In a prospective, randomized, double-blind, clinical trial, 233 children, aged 5 months to 5 years, with uncomplicated acute otitis media were randomly assigned to receive either a single intramuscular injection of ceftriaxone (50 mg/kg per day divided three times per day) for 10 days in a double-blind fashion. Demographic and clinical characteristics were similar in both groups. Treatment was successful in 107 of 117 given amoxicillin (91%, 95% confidence interval 86% to 97%) and 105 of 116 given ceftriaxone (91%, 95% confidence interval 85% to 96%). Rates of improvement, failure, relapes, and reinfection were similar in both groups, as were the otoscopic and tympanometric evaluations at the 14- and 60-day follow-up visits. It is concluded that a single intramuscular injection of ceftriaxone (50 mg/kg) is as effective as 10 days of oral amoxicillin for the treatment of uncomplicated acute otitis media in children.
