Dental caries is one of the most prevalent diseases around the world and saliva plays an essential role in controlling the evolution of the disease. This study aimed to investigate if salivary pH is correlated with dental caries in a population of Brazilian adolescents. Adolescents, of both sexes, were submitted to clinical examinations for diagnosis of dental caries according to Decay, Missing, and Filling Teeth index (DMFT). The adolescents were allocated into two groups: i. unaffected by caries (DMFT= 0) and ii. affected by caries (DMFT≥ 1). Saliva samples were collected, and the pH was measured using a pH meter. 672 adolescents were examined, mean age of 12 years old (± 0.45), 305 (45.4%) male and 367 (54.6%) female. The DMFT index in the overall population was 1.46; 49% of the population had DMFT=0. The main result point to a negative correlation but significant between dental caries and salivary pH for the total sample (r= -0.119; CI= -0.195 to -0.042; p=0.002) and for males (r= -0.143; CI= -0.254 to -0.028; p=0.012), and a borderline value for females (r= -0.099; CI= -0.202 to 0.006; 0.057). This study showed that there is a correlation between salivary pH and dental caries in examined adolescents.

The currently available nebulization devices have a slow aerosol flow and produce vapor with large microdrops. Improved devices that achieve higher airflow and produce smaller microdrops are needed to improve the clinical care of patients. To address this critical need, we developed a novel system for the molecular vaporization of liquids. This device vaporizes an active pharma-cological substance dissolved in water, alcohol, or a mixture of water and alcohol using two energy sources at the same time: high-frequency ultrasound and thermal induction. Application of energy to a solution contained in the device's tank allows, within tens of seconds, for the vaporization of the solution itself, with the generation of a vapor consisting of microdrops of very small diameter (0.2-0.3 mu m). In this article, we illustrate the technology used, the main verifi-cation tests performed, and the primary fields of application for this device. In particular, the advantages of both the aerosol delivery system and the administration system are highlighted.

Background: Sedentary behaviour (SB) and physical inactivity (PI) are associated with an increased risk of chronic diseases and a significant economic burden. This pilot study aims to estimate the possible cost savings for the Veneto Regional Health Service (Italy) due to a population-based physical activity (PA) intervention. Methods: The PA-related cost-savings were assessed for four chronic diseases in the whole and sedentary populations of the Veneto region. The SB and PA epidemiological data, regarding an additional percutaneous coronary intervention in coronary artery disease, hospitalizations in chronic obstructive pulmonary disease, surgery for colorectal cancer, and femur fracture, were obtained from national and regional administrative sources. A relative risk reduction, due to PA, was obtained from the recent literature. The annual healthcare costs were estimated using the regional diagnosis-related group tariffs. Results: The annual estimated cost-savings for the regional healthcare service related to these four outcomes: an amount between EUR 5,310,179 (if a conservative analysis was performed) and EUR 17,411,317. Conclusion: By a downward estimate, regarding the direct healthcare costs, PA interventions could lead to important cost-savings in the Veneto region. The savings would be greater when considering the cross-sectional impact on other healthcare costs, comorbidities, and indirect costs.

Background Obesity is a disease characterized by an increase of resting blood pressure and by an increased risk of developing hypertension and cardiovascular events. Exaggerated blood pressure during exercise increases risk of cardiovascular events independently from the baseline blood pressure levels. Purpose to evaluate the blood pressure response and the Workload-indexed blood pressure, during a cardio-pulmonary exercise test (CPET) before and after bariatric surgery (BS). Methods 257 patients with severe obesity performed maximal incremental CPET one month before and six months after BS under the same experimental protocol. The systolic blood pressure was measured at rest (SBPrest), at the submaximal effort (SBPsubmax) at the same exercise intensity (3 METs), at the exercise peak (SBP max) and lastly in the recovery phase (SBPrec). The submaximal and maximal Workload-indexed Sistolic Blood Pressure (W-SBPsubmax and W-SBPmax, respectively) were calculated with the formula: ΔBP/ΔMETs. Diastolic blood pressure was analysed at rest (DBPrest) and during the recovery phase (DBPrec). Results Age was on average 45 ± 10.3 years, BMI before BS was equal to 43.9 ± 6.4 Kg/m2 and 73.5% were females. After BS, there was a significant weight loss (-25.9 ± 6.2%). SBPrest and DBPrest decrease significantly after BS (ΔSBP: -10.2 ± 15.8 and -5.2 ± 11.6 mmHg; p Conclusions After BS, a marked reduction of all blood pressure values was detectable in all phases of CPET. W-SBPsubmax and W-SBPmax, as expression of load independent pressure response, decreased significantly. The reduction in the submaximal blood pressure was significantly but only weakly correlated with changes in body weight, suggesting its substantial independence from weight loss. These findings also support a potential role of CPET in detecting, high risk patients and adequate treatment effectiveness in patients with severe obesity. Abstract Figure. Persistent differentiated papillary thyroid cancer (PTC) with metastasis followed by radical locoregional surgery is an indication for limited reoperation. Despite excellent prognosis the major challenge is controlling locoregional recurrences.To evaluate the efficacy of radioguided excision with combined use of gamma probe and an hand-held gamma camera.From June 2009 to January 2012, we enrolled twenty-two patients with locoregional PTC recurrences, previously undergone to central and/or lateral neck dissection for PTC. The diagnosis of recurrent PTC was based on thyroglobulin (TG) evaluation [basal and after thyroid stimulating hormone (TSH) stimulation], ultrasound (US), iodine-131 (131I) whole body scan (WBS) and fine needle aspiration cytology (FNAC). In the morning of surgery, radiotracer was injected directly into the lesions by US guide. Careful dissection was carried out using gamma probe and hand held gamma camera. Metastatic lymph nodes were identified and excised.In all the patients recruited, 39 pathologic nodes were injected and 61 nodes were removed. Among the removed nodes, 22 (36.1%) were additional nodes (not injected by radiotracer). Of the additional lymph nodes, 7 (31.8%) were metastatic. Mean radioactive count of the lesion (28.633±9.218 counts/s) was higher than tumor bed (385.73±192.23 counts/s) (p0.0001). No complications were observed during radioguided excision, neither on post-operative period.The use of hand-held gamma camera in addition to gamma probe in our preliminary study allows a minimally invasive procedure and safer identifications of the lesions and ensures the completeness of the excision in a difficult surgical field.

To demonstrate the utility of the renal artery embolization (RAE) in the dissection of gross neoplasms and the reduction in blood loss and operative time.We report a case of a gross left renal neoplasm (with the diameter of about 12 cm) in a 45 years old Caucasian female who underwent to renal artery embolization 24 hours before left nephroureterectomy. This procedure has determined a reduction in operative times (about 90 minutes) because of the ligature of the renal vein was facilitated. Intraoperative blood loss was of about 100 ml and the patient didn't need of blood transfusions; the abdominal drain was removed in third postoperative day (daily drained serous fluid was about 20 ml). The patient was discharged 7 days later.RAE facilitates the dissection of gross neoplasms (diameterthan 10 cm), so causing a reduction in intraoperative blood loss and in blood transfusion. The operative times are lower because the ligature of the renal vein is less difficult and the dissection is facilitated for the presence of tissue oedema. The disadvantages are the incomplete hembolyzation, coil migration, hematomes, post-infarction syndrome (nausea, vomit, abdominal pain, leucocytosis, hyperpyrexia, hematoma); other risks include the possibility of pulmonary embolism, intestinal infarction and infections. Its reduced utilization could be due to the lack of randomized prospective studies showing its potential benefits.

Xanthogranulomatous inflammation (XGI) is a disease of unknown origin, most frequently described in the kidney and gallbladder; its localization in the colorectal tract is extremely rare. The extension of the typical inflammatory process to the surrounding tissues may lead to misdiagnosis as cancer. We report the case of a 56-year-old woman presenting to the Emergency Department with pain, increased levels of α1 and α2 proteins and C-reactive protein (17.5 mg/dL; normal value 0-0.5), and a palpable mass, localized in the right lower quadrant of the abdomen. A computed tomography scan showed a large right cecal mass with necrotic areas, local inflammation of retroperitoneal fat, and enlargement of local lymph nodes. Because of the high suspicion of colic abscess as well as malignancy and worsening of the clinical condition, the patient underwent right colectomy after 4 d of antibiotic treatment. Pathology revealed xanthogranulomatous inflammation involving the ileocecal valve. We review the reports of large bowel tract XGI in the international literature.
