Background: Long-term outcomes of patients treated with helmet noninvasive ventilation (NIV) are unknown: safety concerns regarding the risk of patient self-inflicted lung injury and delayed intubation exist when NIV is applied in hypoxemic patients. We assessed the 6-month outcome of patients who received helmet NIV or high-flow nasal oxygen for COVID-19 hypoxemic respiratory failure. Methods: In this prespecified analysis of a randomized trial of helmet NIV versus high-flow nasal oxygen (HENIVOT), clinical status, physical performance (6-min-walking-test and 30-s chair stand test), respiratory function and quality of life (EuroQoL five dimensions five levels questionnaire, EuroQoL VAS, SF36 and Post-Traumatic Stress Disorder Checklist for the DSM) were evaluated 6months after the enrollment. Results: Among 80 patients who were alive, 71 (89%) completed the follow-up: 35 had received helmet NIV, 36 high-flow oxygen. There was no inter-group difference in any item concerning vital signs (N = 4), physical performance (N = 18), respiratory function (N = 27), quality of life (N = 21) and laboratory tests (N = 15). Arthralgia was significantly lower in the helmet group (16% vs. 55%, p = 0.002). Fifty-two percent of patients in helmet group vs. 63% of patients in high-flow group had diffusing capacity of the lungs for carbon monoxide < 80% of predicted (p = 0.44); 13% vs. 22% had forced vital capacity < 80% of predicted (p = 0.51). Both groups reported similar degree of pain (p = 0.81) and anxiety (p = 0.81) at the EQ-5D-5L test; the EQ-VAS score was similar in the two groups (p = 0.27). Compared to patients who successfully avoided invasive mechanical ventilation (54/71, 76%), intubated patients (17/71, 24%) had significantly worse pulmonary function (median diffusing capacity of the lungs for carbon monoxide 66% [Interquartile range: 47–77] of predicted vs. 80% [71–88], p = 0.005) and decreased quality of life (EQ-VAS: 70 [53–70] vs. 80 [70–83], p = 0.01). Conclusions: In patients with COVID-19 hypoxemic respiratory failure, treatment with helmet NIV or high-flow oxygen yielded similar quality of life and functional outcome at 6months. The need for invasive mechanical ventilation was associated with worse outcomes. These data indicate that helmet NIV, as applied in the HENIVOT trial, can be safely used in hypoxemic patients. - Abstract
Objectives: Computed tomography (CT) provides excellent anatomy assessment of the aortic annulus (AoA) and is utilized for pre-procedural planning of transcatheter aortic valve implantation (TAVI). We sought to investigate if geometrical characteristics of the AoA determined by CT may represent predictors of structural valve degeneration (SVD) in patients undergoing TAVI with balloon-expandable valves. Methods: This is a retrospective study on 124 consecutive patients (mean age: 79 ± 7 years; female: 61%) undergoing balloon-expandable TAVI prospectively enrolled in a registry. AoA maximum diameter (Dmax), minimum diameter (Dmin), and area were assessed using pre-procedural CT. SVD was identified during follow-up with transthoracic echocardiography documenting structural prosthetic valve abnormalities with or without hemodynamic changes. Results: The mean follow-up was 5.9 ± 1.7 years. SVD was found in 48 out of 124 patients (38%). AoA Dmax, Dmin, and area were significantly smaller in patients with SVD compared to patients without SVD (25.6 ± 2.2 mm vs. 27.1 ± 2.8 mm, p = 0.012; 20.5 ± 2.1 mm vs. 21.8 ± 2.1 mm, p = 0.001 and 419 ± 77 mm2 vs. 467 ± 88 mm2, p = 0.002, respectively). At univariable analysis, female sex, BSA, 23-mm prosthetic valve size, Dmax < 27.1 mm, and a Dmin < 19.9 mm were associated with SVD, whereas at multivariable analysis, only Dmin < 19.9 mm (OR = 2.873, 95% CI: 1.191–6.929, p = 0.019) and female sex (OR = 2.659, 95% CI: 1.095–6.458, p = 0.031) were independent predictors of SVD. Conclusions: Female sex and AoA Dmin < 19.9 mm are associated with SVD in patients undergoing TAVI with balloon-expandable valves. When implanting large prostheses in order to avoid paraprosthetic regurgitation, caution should be observed due to the risk of excessive stretching of the AoA Dmin, which may play a role in SVD. We conducted a proof of concept study where Anapnoguard endotracheal tubes and its control unit were used in 15 patients with COVID-19 acute respiratory distress syndrome. Anapnoguard system provides suction, venting, rinsing of subglottic space and controls cuff pressure detecting air leakage through the cuff. Alpha-amylase and pepsin levels, as oropharyngeal and gastric microaspiration markers, were assessed from 85 tracheal aspirates in the first 72 h after connection to the system. Oropharyngeal microaspiration occurred in 47 cases (55%). Episodes of gastric microaspiration were not detected. Patient positioning, either prone or supine, did not affect alpha-amylase and pepsin concentration in tracheal secretions. Ventilator-associated pneumonia (VAP) rate was 40%. The use of the AG system provided effective cuff pressure control and subglottic secretions drainage. Despite this, no reduction in the incidence of VAP has been demonstrated, compared to data reported in the current COVID-19 literature. The value of this new technology is worth of being evaluated for the prevention of ventilator-associated respiratory tract infections.

- Abstract
Introduction Remdesivir and Dexamethasone represent the cornerstone of therapy for critically ill patients with acute hypoxemic respiratory failure caused by Coronavirus Disease 2019 (COVID-19). However, clinical efficacy and safety of concomitant administration of Remdesivir and Dexamethasone (Rem-Dexa) in severe COVID-19 patients on high flow oxygen therapy (HFOT) or non-invasive ventilation (NIV) remains unknown. Materials and methods Prospective cohort study that was performed in two medical Intensive Care Units (ICUs) of a tertiary university hospital. The clinical impact of Rem-Dexa administration in hypoxemic patients with COVID-19, who required NIV or HFOT and selected on the simplified acute physiology score II, the sequential organ failure assessment score and the Charlson Comorbidity Index score, was investigated. The primary outcome was 28-day intubation rate; secondary outcomes were end-of-treatment clinical improvement and PaO2/FiO2 ratio, laboratory abnormalities and clinical complications, ICU and hospital length of stay, 28-day and 90-day mortality. Results We included 132 patients and found that 28-day intubation rate was significantly lower among Rem-Dexa group (19.7% vs 48.5%, p<0.01). Although the end-of-treatment clinical improvement was larger among Rem-Dexa group (69.7% vs 51.5%, p = 0.05), the 28-day and 90-day mortalities were similar (4.5% and 10.6% vs. 15.2% and 16.7%; p = 0.08 and p = 0.45, respectively). The logistic regression and Cox-regression models showed that concomitant Rem-Dexa therapy was associated with a reduction of 28-day intubation rate (OR 0.22, CI95% 0.05–0.94, p = 0.04), in absence of laboratory abnormalities and clinical complications (p = ns). Conclusions In COVID-19 critically ill patients receiving HFO or NIV, 28-day intubation rate was lower in patients who received Rem-Dexa and this finding corresponded to lower end-of-treatment clinical improvement. - Abstract
This paper describes a novel methodology where Machine Learning Algorithms (MLAs) have been integrated to assess the landslide risk for slow moving mass movements, processes whose intermittent activity makes challenging any risk analysis worldwide. MLAs has been trained on datasets including Interferometric Synthetic Aperture Radar (InSAR) and additional remote sensing datasets such as aerial stereo photographs and LiDAR and tested in the Termini-Nerano landslides system (southern Apennines, Italy). The availability of such a wealth of materials allows also an unprecedented spatio-temporal reconstruction of the volume and the kinematic of the landslides system through which we could generate and validate the hazard map. Our analysis identifies fifteen slow-moving phenomena, traceable since 1955, whose total area amounts to 4.1 × 105 m2 and volume to ~1.4 × 106 m3. InSAR results prove that seven out of the fifteen slow-moving landslides are currently active and characterized by seasonal velocity patterns. These new insights on the dynamic of the landslides system have been selected as the main independent variables to train three MLAs (Artificial Neural Network, Generalized Boosting Model and Maximum Entropy) and derive the landslide hazard for the area. Finally, official population and buildings census data have been used to assess the landslide risk whose highest values are located in the crown area, south of Termini village, and nearby Nerano. This new methodology provides a different landslide risk scenario compared to the existing official documents for the study area and overall new insights on how to develop landslide risk management strategies worldwide based on a better understanding of slope processes thanks to the latest satellite technologies available.

- Abstract
Background and purpose: The objective of this study was to assess the neurological manifestations in a series of consecutive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-positive patients, comparing their frequency with a population hospitalized in the same period for flu/respiratory symptoms, finally not related to SARS-CoV-2. Methods: Patients with flu/respiratory symptoms admitted to Fondazione Policlinico Gemelli hospital from 14 March 2020 to 20 April 2020 were retrospectively enrolled. The frequency of neurological manifestations of patients with SARS-CoV-2 infection was compared with a control group. Results: In all, 213 patients were found to be positive for SARS-CoV-2, after reverse transcriptase polymerase chain reaction on nasal or throat swabs, whilst 218 patients were found to be negative and were used as a control group. Regarding central nervous system manifestations, in SARS-CoV-2-positive patients a higher frequency of headache, hyposmia and encephalopathy always related to systemic conditions (fever or hypoxia) was observed. Furthermore, muscular involvement was more frequent in SARS-CoV-2 infection. Conclusions: Patients with COVID-19 commonly have neurological manifestations but only hyposmia and muscle involvement seem more frequent compared with other flu diseases.

The tectonic evolution of the Campania margin during the late Pliocene--Pleistocene is characterized by extensional deformation coeval with the opening of the Marsili oceanic basin. The initial stages of stretching are represented in the analysed area by the development of low-angle normal faults that extended the belt in the same direction as the direction of spreading recognized for the oceanic basin. Detachment faulting was accompanied by the onset of magmatic activity and by major uplifts that resulted in the accumulation of thick conglomerate successions in the half-grabens that developed in the upper plate of the extensional detachment. Deformation continued during the Mid-Pleistocene with the formation of strikeslip and normal faults trending almost parallel to the spreading direction of the Marsili basin. These faults dissected the extensional detachment and promoted counter-clockwise block rotations about vertical axes, which accommodated the ESE-directed stretching of the Marsili basin and the consequent sinistral shear imposed on the southern Apennine chain. The development of a regional detachment fault along the Tyrrhenian margin of Campania explains the complex patterns of subsidence and uplift that characterize this area during the Pleistocene and fits well within the regional geodynamic framework of the southern Tyrrhenian Sea.

- Abstract
The present study focuses on the morphotectonic evolution of the axial portion of the Southern Apennine chain between the lower Calore River valley and the northern Camposauro mountain front (Campania Region). A multidisciplinary approach was used, including geomorphological, field-geology, stratigraphical, morphotectonic, structural, Ar-40/Ar-39 and tephrostratigraphical data. Results indicate that, from the Lower Pleistocene onwards, this sector of the chain was affected by extensional tectonics responsible for the onset of the sedimentation of Quaternary fluvial, alluvial fan and slope deposits. Fault systems are mainly composed of NW-SE, NE-SW and W-E trending strike-slip and normal faults, associated to NW-SE and NE-SW oriented extensions. Fault scarps, stratigraphical and structural data and morphotectonic indicators suggest that these faults affected the wide piedmont area of the northern Camposauro mountain front in the Lower Pleistocene-Upper Pleistocene time span. Faults affected both the oldest Quaternary slope deposits (Laiano Synthem, Lower Pleistocene) and the overlying alluvial fan system deposits constrained between the late Middle Pleistocene and the Holocene. The latter are geomorphologically and chrono-stratigraphically grouped into four generations, I generation: late Middle Pleistocene-early Upper Pleistocene, with tephra layers Ar-40/Ar-39 dated to 158 +/- 6 and 113 +/- 7 ka; II generation: Upper Pleistocene, with tephra layers correlated with the Campanian Ignimbrite (39ka) and with the slightly older Campi Flegrei activity (Ar-40/Ar-39 age 48 +/- 7 ka); III generation: late Upper Pleistocene-Lower Holocene, with tephra layers correlated with the Neapolitan Yellow Tuff (15ka); IV generation: Holocene in age. The Middle Volturno River valley is located in the inner part of the Southern Apennines of Italy, between the SW slope of the Matese Massif and the NE slopes of the Caserta mountains and is underlain by Meso-Cenozoic carbonate rocks and Miocene Flysch deposits. The study sector includes the lower Calore River valley and, below the Calore-Volturno confluence, the valley portion that extends until the Triflisco gorge, from Telese village to the Volturno dam. It is generally E-W, NW-SE and NE-SW elongated and characterized by rectilinear and meandering fluvial patterns. The main infilling of the two valley portions is locally preserved as remnants of fluvial terraces hanging over the local base level up to ca. 30-40 m. It is generally interfingered with and covered by several generations of alluvial fan and travertine deposits. New Ar/Ar datings on tephra layers interbedded in the oldest generations of the alluvial fan deposits and new U/Th datings on travertine deposits, allowed to constrain the main infilling to the late Middle and the early Upper Pleistocene. Both deposits are locally covered by the Campanian Ignimbrite Formation (CI, 39 ky BP) and are interested by high-angle faults generated during extensional tectonic phases that affected this sector of the Apennine chain since the Middle Pleistocene. Furthermore, the geomorphological analyses of aerial photos and topographic maps (1:5000 in scale) highlight the presence of a flight of fluvial terraces younger than the CI deposits that can be grouped into four orders. The stratigraphical data, based on field surveys and boreholes analyses, supported by new tephrostratigraphical constraints and literature data, allow to refer the older orders (I and II) to the late Upper Pleistocene. The III and IV orders, instead, can be referred to the early Holocene and historical times, respectively. These chronological constraints allow to hypothesize that the genesis of the I and II orders seem to be driven by late Quaternary tectonics, while climatic and land use changes played a key role for the formation of the younger orders.

Landslides are a significant component of natural disasters in most countries around the world. Understanding these destructive phenomena through the analysis of possible correlations between microbial communities and the alteration of the soil responsible for landslides is important in order to reduce their negative consequences. To address this issue, bacterial and fungal communities in soils triggering landslides in Termini-Nerano and Massa Lubrense-Nerano (Naples, Italy) were analysed by genetic profiling techniques. Fingerprints were generated by single-strand conformation polymorphisms (SSCP) and random amplified polymorphic DNA (RAPD). The microbial community in both soil types was enriched in species which could contribute to the degradation process occurring during landslides, forming biofilms and leading to the transformation or the formation of minerals. Indeed, some of the identified bacteria were found to favour the transformation of clay minerals. - Abstract
Since the early 70’s the majority of tectonic reconstructions of the western Mediterranean employ the Alboran domain notion as a migrating microcontinent or landmass mainly composed of Paleozoic-Triassic rocks affected by ‘Alpine’ HP-LT metamorphism. For nearly three decades, since the mid-80’s, the Alboran domain was considered as a fragment of the Alpine chain that moved westward, colliding into Iberia and North Africa to produce the Gibraltar arc and Betic-Rif chain. In 2012, a new hypothesis for the evolution of the western Mediterranean was presented in which the Betic-Rif orogenic chain originates from rollback of an initially SE-dipping subduction of the westernmost segments of the Ligurian-Tethys under the Africa margin. This interpretation considers the metamorphic ‘Alboran domain’ rocks as crustal successions of the hyper-extended African and Iberian continental margins, which have undergone a complete subduction-exhumation cycle above a NW- to W-retreating subduction. A key outcome of this hypothesis is that the Alboran domain is not a fragment of the Alpine chain but a consequence of rollback dynamics. In this contribution we try to elucidate the historical reasons behind the classical ‘Alpine’ interpretation of the Betic-Rif, by briefly describing key contributions, which appear linked in a logical sequence that traces the evolution of the Alboran domain concept since its original formulation by Andrieux and coauthors in 1971.

The Ofanto basin is a wide synclinorium, about 30 km in length, located between the axial and external zones of the Southern Apennine chain and characterised by an unusual E-W trend (Fig. 1). The substratum of this Pliocene basin is formed by pelagic units of the Southern Apennines Allochtonous (SAA) (i.e. Sicilide and Lagonegro Units), with overlying Late Miocene siliciclastic deposits of the Castelvetere Flysch. Well data constrain the age of thrusting of the SAA on the Apulian Carbonate Platform as Early Pliocene in this sector of the chain. Coeval to the migration of the SAA was the deposition of Pliocene marine sediments of the Ariano Unit on top of the translating thrust sheets. The E-W trend of the Ofanto synclinorium represents an anomaly in the regional NW-SE structural trend of the Southern Apennines, which has been attributed either to major (lithospheric) strike-slip faults (Ortolani 1974; Schiattarella et al. 2005), or to the effect of deep thrust faulting in the Apulian Platform (Hyppolite et al. 1994). This work integrates the analysis of the synorogenic Ofanto basin together with the interpretation of about 300 km of reflection seismic lines and well data to define: a) the syntectonic characteristics of the Ofanto basin infill and its age based on new biostratigraphic studies; b) the 3-D geometry of the Ofanto basin evolving during the coeval emplacement of the SAA and subsequently deformed by the shortening that affected the underlying Apulian carbonate platform; c) the tectonic evolution and timing of deformation of this segment of the Apennine fold and thrust belt.

The transformation of water-rich smectite clay minerals into relatively anhydrous illite is a common reaction in sedimentary basins, where it is thought to be driven by temperature increase associated with increasing depth. However, this mineral transformation has also been observed in the gauge of large faults, and since it involves the release of bound water from smectite it is suspected to have a key control on hydrologic and mechanical processes at subduction complexes. In this contribution we report on the distribution of smectite and illite in the shear zone of a regional-scale strike slip fault of the Southern Apennines that deforms Plio-Pleistocene marine mudstones. This shear zone was analysed integrating field based observations with X-Ray diffraction analyses, a peeling technique performed on large oriented samples, grain size analyses and a consolidation test on the undeformed wallrock mudstone. In all observed exposures the sheared mudstones are characterized by a marked darker colouring with respect to the surrounding undeformed material, and compose a 5m wide belt of intensely strained material. Lenses or streaks of relatively undeformed material, discerned by a lighter colour, are enclosed within the deformation zone and have their long axes parallel or moderately oblique to the shear zone boundaries. Both the sheared mudstones and the enclosed light coloured lenses display a pervasive foliation highlighted by the preferred orientation of desiccation cracks forming angles of 135º-180º with the shear zone boundary. The boundaries of the shear zone are extremely sharp and no damage feature was observed outside of the shear zone. X-ray diffraction analyses indicate that the light coloured undeformed mudstones are composed by interstratified smectite/illite, chlorite and kaolinite, as clay minerals (approximately 40%), and quartz, calcite and microcline forming the non-clay fraction. The composition of the dark, sheared mudstone is identical to that of the undeformed wallrock with the only difference that it lacks smectite; therefore indicating a direct link between the darkening in colour and the disappearance of smectite. The heterogeneous distribution of illite and smectite within the shear zone was studied integrating outcrop-scale observations with a peeling technique that allows microscope and SEM observation to be made over areas in excess of 60cm2. Several peels where obtained from different localities along the fault and from different portions of the fault zone, providing examples of a wide range of structures representing also different stages in fabric maturity. In general peels display a compositional banding parallel to the P foliation, formed by domains of smectite/illite alternating with illitic domains in which smectite has disappeared. This compositional banding, occurring with sub-millimetre spacing, is displaced by shears oriented in the Y, P and R orientations. Shears cutting through mildly deformed lenses enclosed in the shear zone, especially P and Y shears, present a filling composed by dark illitic mudstone. Microscope observation of this filling material indicates it is finer grained and its particles are aligned along ‘flow’ trajectories, suggesting a more ductile like behaviour with respect to the light coloured smectite/illite component.

- Abstract
The Sele river plain is located along the western Tyrrhenian margin of the southern Apennine Chain and is defined seawards by a straight sandy coast formed during the Last Interglacial (Tyrrhenian stage, OIS 5e) and the Holocene. It is characterized by the presence of beach-dune ridges which to the rear interfinger with lagoonal and fluvio-palustrine deposits. This belt was accreted progressively and represents the evolution of a barrier-lagoon system shifted alternatively landward and seaward. In this work, we summarize the main results relative to the Holocene evolution of the Sele river coastal plain along the coast in front of the archaeological area of Poseidonia-Paestum, where the knowledge has been improved by two new cores and by many collected archaeo-tephro-stratigraphical data. The area was affected by the Holocene marine transgression that formed cliffs that cut the travertine deposits. During the second part of the Holocene the shoreline shifted seaward and a lagoonal-beach bar system (Fossa Lupata) formed. The archaeological remains of Poseidonia (VI cent. B.C.) and the Agnano Monte Spina tephra layer (4.1 ky BP) confirm the presence of this morpho-sedimentary system during this time interval. After this period, and mostly after the deposition of the A.D. 79 tephra, the shoreline shifted seaward and an additional beach ridge was formed, while the flat area of Fossa Lupata, was rapidly aggraded and dried up.

After the October 31 2002 earthquake that hit the Molise Region, surveys were started for the microzonation of the Campobasso Province. Approximately 150 strategic sites were studied in the period between 2004 and 2007. In order to produce a homogeneous cartography for all sites of the Campobasso Province guidelines were set for the making of geological and thematic maps. These guidelines are based essentially on the criteria established for the CARG project (Quaderni SGN, serie III) and on the contents discussed by the Comitato di Area dell'Appennino meridionale (Southern Apennines Local Committee). The adoption of CARG criteria for the making of technical maps at a local scale allowed fulfilling the technical purpose of the project without renouncing to the geological significance of the studied areas and preserving homogeneity of geological information at regional scale.
