Worldwide, synthetic compounds are used for both in-office and at-home dental care. They are a valuable resource for both prophylactic and curative treatments for various dental problems, such as tooth decay, periodontal diseases, and many more. They are typically preferred due to their broad range of actions and ability to produce targeted, rapid, and long-lasting effects. Using a 0.12% chlorhexidine mouthwash is capable of reducing the plaque index from 47.69% to 2.37% and the bleeding index from 32.93% to 6.28% after just 2 weeks. Mouthwash with 0.1% OCT is also highly effective, as it significantly lowered the median plaque index and salivary bacterial counts in 152 patients in 5 days compared to a control group (p < 0.0001), while also reducing the gingival index (p < 0.001). When povidone-iodine was used as an irrigant during the surgical removal of mandibular third molars in 105 patients, it resulted in notably lower pain scores after 2 days compared to a control group (4.57 ± 0.60 vs. 5.71 ± 0.45). Sodium hypochlorite is excellent for root canal disinfection, as irrigating with 1% NaOCl completely eliminated the bacteria from canals in 65% patients. A 0.05% CPC mouthwash proved effective for perioperative patient care, significantly decreasing gingival bleeding (p < 0.001) and suppressing Streptococcus levels even one week post-surgery. Lastly, a 6% H2O2 paint-on varnish and 6% H2O2 tray formulations successfully bleached the teeth of 40 patients, maintaining a noticeably whiter appearance up to the 6-month follow-up, with significant color differences from the baseline (p < 0.005). Synthetic compounds have a large research base, which also provides a greater awareness of their mechanism of action and potential adverse effects. For a better understanding of how they work, several methods and assays are performed. These are protocolary techniques through which a compound's efficacy and toxicity are established. Background. Symptomatic patients with COVID-19 typically have a high SARS-CoV-2 viral load in their saliva. Procedures to reduce the viral load in their oral cavity are important for mitigating the viral transmission. Methods. This randomized clinical trial investigated the impact of two mouthwashes (0.075% cetylpyridinium chloride plus 0.28% zinc lactate (CPC+Zn) (n = 32), and 0.075% cetylpyridinium chloride (CPC) (n = 31)) on the viral load of SARS-CoV-2 in saliva when compared to the distilled water negative control (n = 32). BackgroundTo initiate fecal and oral collections in prospective cohort studies for microbial analyses, it is essential to understand how field conditions and geographic differences may impact microbial communities. This study aimed to investigate the impact of fecal and oral sample collection methods and room temperature storage on collection samples for studies of the human microbiota.ResultsWe collected fecal and oral samples from participants in two Iranian cohorts located in rural Yazd (n = 46) and urban Gonbad (n = 38) and investigated room temperature stability over 4 days of fecal (RNAlater and fecal occult blood test [FOBT] cards) and comparability of fecal and oral (OMNIgene ORAL kits and Scope mouthwash) collection methods. We calculated interclass correlation coefficients (ICCs) based on 3 alpha and 4 beta diversity metrics and the relative abundance of 3 phyla. After 4 days at room temperature, fecal stability ICCs and ICCs for Scope mouthwash were generally high for all microbial metrics. Similarly, the fecal comparability ICCs for RNAlater and FOBT cards were high, ranging from 0.63 (95% CI: 0.46, 0.75) for the relative abundance of Firmicutes to 0.93 (95% CI: 0.89, 0.96) for unweighted Unifrac. Comparability ICCs for OMNIgene ORAL and Scope mouthwash were lower than fecal ICCs, ranging from 0.55 (95% CI: 0.36, 0.70) for the Shannon index to 0.79 (95% CI: 0.69, 0.86) for Bray-Curtis. Overall, RNAlater, FOBT cards and Scope mouthwash were stable up to 4 days at room temperature. Samples collected using FOBT cards were generally comparable to RNAlater while the OMNIgene ORAL were less similar to Scope mouthwash.ConclusionsAs microbiome measures for feces samples collected using RNAlater, FOBT cards and oral samples collected using Scope mouthwash were stable over four days at room temperature, these would be most appropriate for microbial analyses in these populations. However, one collection method should be consistently since each method may induce some differences.

Cetylpyridinium chloride (CPC) is a chemical compound used widely in industry, having been adopted in pharmaceutical and cosmetic formulations as a preservative, antimicrobial, and antiseptic agent. In pharmaceutical development, thermal analysis and Infrared Spectrum (FTIR) are widely used on preformulation studies. Thus, in this work we performed the thermal characterization of CPC through DSC and TG/DTA techniques, and evaluated it by FTIR to understand the thermal events that characterize CPC. In addition, its binary combinations with possible adjuvants: sugar alcohols (sorbitol and xylitol) and flavoring agents (menthol and thymol), were evaluated. Statistical analysis using Pearson's correlation allowed us to define clearly the extent of these interactions. The DSC study showed endothermic peaks at 84.09 °C and 233.84 °C for CPC. TG analysis indicated mass loss of 92.91% at a temperature of 239.57 °C. DTA evaluation showed endothermic peaks at 83.68 °C and 240.12 °C. In the FTIR analysis, bands of characteristic spectra for CPC were presented. The evaluation of binary combinations with the components studied allowed for the identification of interactions of CPC with menthol and thymol, with alterations of melting point and eutectic mixtures formation, even at room temperature, with thymol. The spectral constancy of CPC and mixtures with different components were evaluated through linear correlation, with no significant chemical interactions evidenced. This was due to the Pearson's correlation values being greater than 0.8, which represents an absence of chemical interactions between the components tested. [ABSTRACT FROM AUTHOR]

(1) There are limited clinical trials to support the effectiveness of mouth rinses when used as a preprocedural rinse against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This systematic review aims to evaluate the efficacy of antiseptic mouth rinses as a preprocedural rinse in reducing SARS-CoV-2 oral viral load in-vivo. (2) Methods: A literature search was conducted through November 2022 for the following databases: PubMed, Web of Science, Cochrane Library, and Google Scholar. The evaluated outcomes were quantitative changes in viral load and the statistical significance of that change after using antiseptic mouth rinses. (3) Results: 14 randomized controlled trials (RCT) were selected for risk of bias assessment and data extraction. (4) Conclusion: Within the limits of this systematic review, preprocedural mouth rinses may significantly reduce SARS-CoV-2 in the mouth, thus, reducing the viral particles available for airborne dispersion. Preprocedural mouth rinses may be an effective strategy for reducing airborne SARS-CoV-2 dispersion in the environment. Their use may be a preventive strategy to reduce the spread of COVID-19 in selected medical and healthcare facilities, including dental clinics. Potential preprocedural mouth rinses are identified for use as an integral part of safe practice for healthcare protocols. Purpose This study aimed to assess the effectiveness of ultrasonic denture hygiene interventions in improving denture cleanliness among elderly individuals. Materials and Methods Sixty-six participants who had received upper metal framework removable partial dentures within the past 5 years were randomly allocated into three denture hygiene intervention groups: group 1 (mechanical cleaning with a toothbrush and ultrasonic cleaning with cetylpyridinium chloride), group 2 (mechanical cleaning with a toothbrush and ultrasonic cleaning with distilled water), and control (mechanical cleaning with a toothbrush only). Denture cleanliness was assessed at baseline and 1-month using: i) Denture Cleanliness Index (DCI) scores; ii) plaque coverage percentage; and (iii) microbiological samples for bacterial and yeast detection. Differences between groups were assessed with one-way analysis of variance and Chi-squared tests. Results Mean DCI scores and mean percentages of plaque coverage area were significantly reduced in group 1 and group 2, compared to the control group for both cobalt chromium (CoCr) and acrylic fitting surfaces (p<0.001). No significant differences were found between groups 1 and 2 with regard to the prevalence and viable counts of yeasts or total microbial viable counts. No significant differences in the investigated clinical and microbiological parameters were observed between CoCr and acrylic surfaces following the intervention period. Conclusion The ultrasonic cleaner was significantly more effective than mechanical cleaning in the reduction of biofilm coverage on metal framework removable partial dentures over a 1-month intervention period. BackgroundA number of cohort studies have collected Scope mouthwash samples by mail, which are being used for microbiota measurements. We evaluated the stability of Scope mouthwash samples at ambient temperature and determined the comparability of Scope mouthwash with saliva collection using the OMNIgene ORAL Kit.MethodsFifty-three healthy volunteers from Mayo Clinic and 50 cohort members from Bangladesh provided oral samples. One aliquot of the OMNIgene ORAL and Scope mouthwash were frozen immediately and one aliquot of the Scope mouthwash remained at ambient temperature for 4 days and was then frozen. DNA was extracted and the V4 region of the 16S rRNA gene was PCR amplified and sequenced using the HiSeq. Intraclass correlation coefficients (ICC) were calculated.ResultsThe overall stability of the Scope mouthwash samples was relatively high for alpha and beta diversity. For example, the meta-analyzed ICC for the Shannon index was 0.86 (95% confidence interval, 0.76-0.96). Similarly, the ICCs for the relative abundance of the top 25 genera were generally high. The comparability of the two sample types was relatively low when measured using ICCs, but were increased by using a Spearman correlation coefficient (SCC) to compare the rank order of individuals.ConclusionsOverall, the Scope mouthwash samples appear to be stable at ambient temperature, which suggests that oral rinse samples received by the mail can be used for microbial analyses. However, Scope mouthwash samples were distinct compared with OMNIgene ORAL samples.ImpactStudies should try to compare oral microbial metrics within one sample collection type.

A number of baby wipe formulations contain 2-phenoxyethanol (PE) as a preservative and cetylpyridinium chloride (CPC) as a surfactant with antimicrobial activity. Previously, we reported the skin absorption of PE in porcine skin and human skin in vitro. In the present work, the permeation of PE from preparations with CPC and without CPC was investigated in human skin in vivo. The studies were conducted using Confocal Raman Spectroscopy (CRS) and tape stripping (TS) methods. The CRS studies showed that the area under the curve (AUC) of PE for the formulation with and without CPC were not significantly different (p > 0.05). The TS data indicated no significant difference in the amounts of PE recovered from tapes 1-6 for the preparation with and without CPC (p > 0.05). When comparing the in vitro and in vivo data, a correlation was observed between the cumulative amount of PE permeated through human skin in vitro at 24 h and the AUC as measured by CRS (r 2 = 0.97). In addition, the cumulative amount of PE permeated through human skin in vitro at 24 h was found to correlate with the amount of PE recovered from tape 1 to 6 in vivo (r 2 = 0.95). Both CRS and TS techniques demonstrated limitations in assessing the distribution of PE and CPC in the skin in vivo, primarily attributed to the Raman signal intensities of compounds under investigation and the variability in the amount of SC collected by TS. Despite the limitations of CRS and TS, the results from the present study add further insights to the in vitro permeation data. It is currently known that the dentist is one of the health professionals with the highest risk of contagion of COVID-19 due to its direct contact with the oral cavity. High exposure to aerosols generated by rotating instruments in COVID-19 patients increases contact with the SARS-CoV-2 viral load in routine procedures. It has been described that mouthwashes prior to dental care could be effective solutions to reduce contagion despite their little clinical evidence. Mouthwashes with cetylpyridinium chloride (CPC), hydrogen peroxide (H2O2), povidone-iodine (PVP-I) and chlorhexidine gluconate (CHX) show great potential to reduce the viral load of SARS-CoV-2 in the aerosols generated from saliva during the dental visit. Therefore, the objective of this article was to review the current scientific information on the relationship of the use of mouthwashes with the decrease in the viral load of SARS-CoV-2.

By employing halloysite (Hal) as the carrier, a synergistic cetylpyridinium (CP+)/Cu2+/Hal (PCH) system was constructed via a vacuum loading method. Hal's lumen serves as a storage "container" for CP+ and Cu2+, achieving their optimized thermal stability and "burst + sustained" release. Antibacterial agents' loading makes PCH tend to hydrophobicity and exhibit synergistic antibacterial efficacy. With the released antibacterial agents and adsorbed PCH particles, 100 % of Staphylococcus aureus and Escherichia coli are inactivated by PCH in 120 min at concentrations of 20 mg/L and 150 mg/L, respectively. The effect of Hal's tubular structure on PCH's performance shows that the shortening of Hal's nanotube and the expansion of lumen diameter with the preservation of intact tubular structure can intensify the loading-release and antibacterial process of CP+ and Cu2+. However, it requires balancing the demands of rapid bactericidal and durable antibacterial activity. The obtained PCH is well dispersed and compatible with thermoplastic polyurethane, enhancing its mechanical properties. The composite film containing 10 wt% PCH inactivates 100 % Staphylococcus aureus and 98.5 % Escherichia coli in 4 h, while exhibiting excellent elongation at break (274.3 %) and tensile strength (40.7 MPa). Seeking non-antibiotic solutions against bacterial infections has become a global priority. Therefore, this work employs sepiolite as the carrier to load cetylpyridinium (CP+) and Cu2+, constructing a CP+/Cu2+/sepiolite (PCS) system with excellent antibacterial and application properties. Sepiolite, serving as a micro-container for storing antibacterial agents, enhances their thermal stability and realizes their "burst + sustained" release. Antibacterial agents improve the organic compatibility of sepiolite and provide a synergistic antibacterial effect. With the released antibacterial agents and contact inactivation effect, PCS at 10 mg/L and 75 mg/L achieve 100 % inactivation of Staphylococcus aureus and Escherichia coli within 120 min, respectively. Furthermore, the sepiolite carrier with an intact fibrous structure facilitates the loading-release and antibacterial activity of agents in PCS. The obtained PCS disperses uniformly in thermoplastic polyurethane and binds tightly with it, improving its mechanical properties. Film of ST-10wt%PCS with 10 wt% PCS inactivates 100 % of Staphylococcus aureus and 93.0 % of Escherichia coli in 4 h, while exhibiting superior elongation at break (263 %) and tensile strength (35 MPa) compared to the pure film (242 % and 30 MPa). BackgroundQuaternary ammonium salts (QUATS), such as cetylpyridinium chloride (CPC) and benzalkonium chloride (BAK), are frequently used in antiseptic formulations, including toothpastes, mouthwashes, lozenges, throat and nasal sprays, and as biocides. Although in a recent ruling, the U.S. Food and Drug Administration (FDA) banned CPC from certain products and requested more data on BAK's efficacy and safety profile, QUATS, in general, and CPC and BAK, in particular, continue to be used in personal health care, food, and pharmaceutical and cleaning industries.ObjectivesWe aimed to assess CPC's effects on mitochondrial toxicity and endocrine disruption in vitro.MethodMitochondrial O2 consumption and adenosine triphosphate (ATP) synthesis rates of osteosarcoma cybrid cells were measured before and after CPC and BAK treatment. Antiestrogenic effects of the compounds were measured by a luciferase-based assay using recombinant human breast carcinoma cells (VM7Luc4E2, ERalpha-positive).ResultsCPC inhibited both mitochondrial O2 consumption [half maximal inhibitory concentration (IC50): 3.8μM] and ATP synthesis (IC50: 0.9μM), and additional findings supported inhibition of mitochondrial complex 1 as the underlying mechanism for these effects. In addition, CPC showed concentration-dependent antiestrogenic activity half maximal effective concentration [(EC50): 4.5μM)]. BAK, another antimicrobial QUATS that is structurally similar to CPC, and the pesticide rotenone, a known complex 1 inhibitor, also showed mitochondrial inhibitory and antiestrogenic effects. In all three cases, there was overlap of the antiestrogenic activity with the mitochondrial inhibitory activity.ConclusionsMitochondrial inhibition in vitro occurred at a CPC concentration that may be relevant to human exposures. The antiestrogenic activity of CPC, BAK, rotenone, and triclosan may be related to their mitochondrial inhibitory activity. The saliva of patients with COVID-19 has a high SARS-CoV-2 viral load. The risk of spreading the virus is high, and procedures for viral load reduction in the oral cavity are important. Little research to date has been performed on the effect of mouthwashes on the salivary SARS-CoV-2 viral load. This pilot randomized single-center clinical trial investigated whether three types of mouthwash with solutions containing either 0.075% cetylpyridinium chloride plus 0.28% zinc lactate (CPC + Zn), 1.5% hydrogen peroxide (HP), or 0.12% chlorhexidine gluconate (CHX) reduce the SARS-CoV-2 viral load in saliva at different time points. Sixty SARS-CoV-2-positive patients were recruited and randomly partitioned into a placebo (oral rinsing with distilled water) group and other groups according to the type of mouthwash. Saliva samples were collected from the participants before rinsing (T0), immediately after rinsing (T1), 30 min after rinsing (T2), and 60 min after rinsing (T3). The salivary SARS-CoV-2 viral load was measured by qRT-PCR assays. Rinsing with HP and CPC + Zn resulted in better reductions in viral load, with 15.8 ± 0.08- and 20.4 ± 3.7-fold reductions at T1, respectively. Although the CPC + Zn group maintained a 2.6 ± 0.1-fold reduction at T3, this trend was not observed for HP. HP mouthwash resulted in a significant reduction in the SARS-CoV-2 viral load up to 30 min after rinsing (6.5 ± 3.4). The CHX mouthwash significantly reduced the viral load at T1, T2, and T3 (2.1 ± 1.5-, 6.2 ± 3.8-, and 4.2 ± 2.4-fold reductions, respectively). In conclusion, mouthwash with CPC + Zinc and CHX resulted in significant reductions of the SARS-CoV-2 viral load in saliva up to 60 mins after rinsing, while HP mouthwash resulted in a significant reduction up to 30 mins after rinsing. Despite this transitory effect, these results encourage further studies and suggest that these products could be considered as risk-mitigation strategies for patients infected with SARS-CoV-2.

The possibility of analytical determination of the uranyl cation UO22+ (taken in the form of the complex uranyl benzoate anion) by using the potentiometric titration technique has been theoretically investigated via DFT modeling and OPLS-AA force field molecular dynamics. The results of the quantum chemical calculations indicate that, among the benchmarked candidate carboxylate anions, the benzoate anion is seemingly characterized by the optimum combination of the binding properties for complexation with the uranyl cation: it shows a relatively high binding to the uranyl cation and, simultaneously, a high binding of the formed uranyl benzoate anion to the cetylpyridinium cation used as a titrant. Moreover, among the considered uranyl-carboxylate anions, the uranyl benzoate anion has been found to be the most lipophilic, which characterizes this form as the most suitable for uranyl determination. Background: Oral health preservation is of major importance in patients undergoing periodontal surgery. Tooth brushing is a competency that is challenging to perform. Therefore, materials with biochemical properties that can control the tooth plaque biofilm are used. Objective: The purpose of the study was to determine the clinical outcome of alcohol-free Chlorhexidine (AFCHX) 0.12% oral rinse in comparison with 0.05% Cetylpyridinium chloride (CPC) non-alcoholic mouthwash in outpatients with periodontitis. Methods: Patients with periodontal diseases attending the Department of Periodontology, Dental School of the Islamic Azad University of Medical Sciences, Tehran, Iran received a double-blind randomized controlled trial. In this study, a total of 38 patients with gingivitis and pocket depth =4 mm were randomly allocated into two equal study groups and followed up within 90 days: Group A (performed mouth rising twice a day with the liquid including 10 ml of the 0.05% CPC without ethanol), Group B (performed mouth rising twice daily with 10 ml of the alcohol-free 0.012% chlorhexidine solution). Furthermore, periodontal parameters and Stain Index (SI) were obtained at baseline and for both groups one month later. ANOVA was used to analyze the data. Statistical significance was defined as p-value <0.05. Results: Full-mouth plaque index (PI) was non-significantly different in the two groups, and the decline was 35% for the CPC group and 44% for the individual AFCHX group. Moreover, reductions in BOP were statistically significant for the CHX group after the 12th week (p=0.0001). Additionally, the PD drop within 30 days was statistically significant (1.92±0.30) AFCHX group and (2.30±0.36) CPC group (p=0.002). Meanwhile, CPC and AFCHX plan of teeth were also compared, and greater rates of staining were observed in AFCHX areas of teeth at days 7 to four weeks. Conclusion: The findings indicated that the application of a therapeutic CPC mouth rinse, twice daily aids control plaque as well as gingivitis, and periodontitis surpassing traditional plaque control, although not as much as AFCHX mouthwash.
