Abstract

Introduction: Although advances in treatment modalities have improved the survival of head and neck (H&N) cancer patients over recent years, survivors’ quality of life (QoL) could be impaired for a number of reasons. The investigation of QoL determinants can inform the design of supportive interventions for this population. Objectives: To examine the QoL of H&N cancer survivors at 1 year after treatment and to identify potential determinants affecting their QoL. Methods: A systematic search of literature was done in December 2011 in five databases: Pubmed, Medline, Scopus, Sciencedirect and CINAHL, using combined search terms ‘head and neck cancer’, ‘quality of life’, ‘health-related quality of life’ and ‘systematic review’. The methodological qualities of selected studies were assessed by two reviewers using predefined criteria. The study characteristics and results were abstracted and summarized. Results: Thirty-seven studies met all inclusion criteria with methodological quality from moderate to high. The global QoL of H&N cancer survivors returned to baseline at 1 year after treatment. Significant improvement showed in emotional functioning while physical functioning, xerostomia, sticky/insufficient saliva, and fatigue were consistently worse at 12 months compared with baseline. Age, cancer sites and stages, social support, smoking, presence of feeding tube are significant QoL determinants at 12 months. Conclusions: Although the global QoL of H&N cancer survivors recover by 12 months after treatment, problems with physical functioning, fatigue, xerostomia and sticky saliva persist. Regular assessment should be carried out to monitor these problems. Further research is required to develop appropriate and effective interventions for this population.

Abstract

Background: The importance of quality-of-life (QoL) research has been recognised over the past two decades in patients with head and neck (H&N) cancer. The aims of this systematic review are to evaluate the QoL status of H&N cancer survivors one year after treatment and to identify the determinants affecting their QoL. Methods: Pubmed, Medline, Scopus, Sciencedirect and CINAHL (2000–2011) were searched for relevant studies, and two of the present authors assessed their methodological quality. The characteristics and main findings of the studies were extracted and reported. Results: Thirty-seven studies met the inclusion criteria, and the methodological quality of the majority was moderate to high. While patients of the group in question recover their global QoL by 12 months after treatment, a number of outstanding issues persist – deterioration in physical functioning, fatigue, xerostomia and sticky saliva. Age, cancer site, stage of disease, social support, smoking, feeding tube placement and alcohol consumption are the significant determinants of QoL at 12 months, while gender has little or no influence. Conclusions: Regular assessments should be carried out to monitor physical functioning,degree of fatigue, xerostomia and sticky saliva. Further research is required to develop appropriate and effective interventions to deal with these issues, and thus to promote the patients’ QoL.

Abstract

Objective: Flow abnormalities which occur in AF patient due to irregular heart rate which resultant turbulent flow both in left atrium and systemically may lead to endothelial dysfunction. The purpose is to detect endothelial dysfunction in non rheumatic AF patient. And test the hypothesis that endothelial dysfunction is reversible upon restoration of normal sinus rhythm and correction of the blood flow dynamics. Method: Endothelium-dependent (flow-mediated dilation) vasodilator function of brachial artery was measured using high resolution ultrasound in 30 patients with persistent non rheumatic AF who were scheduled for elective electrical cardioversion and in 10 control subjects. In patients who remained in sinus rhythm after cardioversion, these measurements were repeated after one month and two months. Result: Compared with control subjects, patients showed lower FMD during AF (6.66Â ± 1.62% vs 14.29Â ± 2.93%, p < 0.001). In patients who remained in sinus rhythm, FMD increased at both one month (6.66Â ± 1.62% vs 10.71Â ± 2.81%, p < 0.001) and two months (6.66Â ± 1.62% vs 14.28Â ± 3.48%, p < 0.001). Conclusion: There is endothelial dysfunction associated with non rheumatic persistent AF patients which is reversible upon restoration of normal sinus rhythm and correction of blood flow dynamics.

Abstract

Objective: Elevated high sensitivity C reactive protein (hs-CRP) has been identified as a strong predictor of prognosis acute coronary syndrome. The prognostic significance of hs-sensitivity CRP level in percutaneous coronary intervention (PCI) is unclear. This study is to assess the hs-CRP value and its prognostic significance in short and long term follows up after PCI. Method: We prospectively studied 41 patients with chronic stable angina (28 patients) and unstable angina (13 patients) who underwent elective coronary stenting. All patients had normal troponin level before the procedure. Blood samples for hs-CRP were obtained before the procedure, 24 hours after the procedure and followed up at 1 month and after 2years. Result: Mean hs-CRP before and post the procedure in all patients underwent PCI was 2.38Â ± 2.21µg/ml and 7.43Â ± 10.6 µg/ml respectively. There was significant difference between pre procedural hs-CRP and 24 hours post procedural (P = 0.007). At follow up period (1 month), no major adverse cardiac events (MACE) have occurred. At follow up period (2 years), MACE has occurred in 13 patients. There was a weak correlation between the level of the pre-procedural hs-CRP and occurrence of MACE (r = 0.22, P = 0.2) and no correlation between post procedural hs-CRP and occurrence of MACE. Conclusion: Mechanical disruption of atherosclerotic plaque during elective coronary stent implantation causes a systemic inflammatory response. Measuring of hs-CRP either pre-procedural or post procedural in low risk patients is not useful for predicting of either early or late cardiovascular events.
