Abstract

BackgroundThe oral AKT inhibitor ipatasertib is being investigated in cancers with a high prevalence of PI3K/AKT pathway activation, including triple-negative breast cancer. The LOTUS trial investigated the addition of ipatasertib to paclitaxel as first-line therapy for triple-negative breast cancer.MethodsIn this randomised, placebo-controlled, double-blind, phase 2 trial, women aged 18 years or older with measurable, inoperable, locally advanced or metastatic triple-negative breast cancer previously untreated with systemic therapy were recruited from 44 hospitals in South Korea, the USA, France, Spain, Taiwan, Singapore, Italy, and Belgium. Enrolled patients were randomly assigned (1:1) to receive intravenous paclitaxel 80 mg/m2 (days 1, 8, 15) with either ipatasertib 400 mg or placebo once per day (days 1-21) every 28 days until disease progression or unacceptable toxicity. Randomisation was by stratified permuted blocks (block size of four) using an interactive web-response system with three stratification criteria: previous (neo)adjuvant therapy, chemotherapy-free interval, and tumour PTEN status. The co-primary endpoints were progression-free survival in the intention-to-treat population and progression-free survival in the PTEN-low (by immunohistochemistry) population. This ongoing trial is registered with ClinicalTrials.gov (NCT02162719).FindingsBetween Sept 2, 2014, and Feb 4, 2016, 166 patients were assessed for eligibility and 124 patients were enrolled and randomly assigned to paclitaxel plus ipatasertib (n=62) or paclitaxel plus placebo (n=62). Median follow-up was 10·4 months (IQR 6·5-14·1) in the ipatasertib group and 10·2 months (6·0-13·6) in the placebo group. Median progression-free survival in the intention-to-treat population was 6·2 months (95% CI 3·8-9·0) with ipatasertib versus 4·9 months (3·6-5·4) with placebo (stratified hazard ratio [HR] 0·60, 95% CI 0·37-0·98; p=0·037) and in the 48 patients with PTEN-low tumours, median progression-free survival was 6·2 months (95% CI 3·6-9·1) with ipatasertib versus 3·7 months (1·9-7·3) with placebo (stratified HR 0·59, 95% CI 0·26-1·32, p=0·18). The most common grade 3 or worse adverse events were diarrhoea (14 [23%] of 61 ipatasertib-treated patients vs none of 62 placebo-treated patients), neutrophil count decreased (five [8%] vs four [6%]), and neutropenia (six [10%] vs one [2%]). No colitis, grade 4 diarrhoea, or treatment-related deaths were reported with ipatasertib. One treatment-related death occurred in the placebo group. Serious adverse events were reported in 17 (28%) of 61 patients in the ipatasertib group and nine (15%) of 62 patients in the placebo group.InterpretationProgression-free survival was longer in patients who received ipatasertib than in those who received placebo. To our knowledge, these are the first results supporting AKT-targeted therapy for triple-negative breast cancer. Abstract

Aim ASPEN is a randomized, open-label, Phase III study comparing zanubrutinib and ibrutinib in patients with Waldenström macroglobulinemia (WM). Materials & methods: Patient-reported outcomes were exploratory end points assessed using the EORTC QLQ-C30 and EQ-5D-5L VAS scores. Results: Overall, 201 patients (102 zanubrutinib; 99 ibrutinib) were enrolled. Clinically meaningful differences were observed in diarrhea and nausea/vomiting in both the intent-to-treat population and in patients attaining very good partial response (VGPR) in earlier cycles of treatment, as well as in long-term physical functioning and fatigue in patients achieving VGPR. Conclusion: Treatment with zanubrutinib was associated with greater improvements in health-related quality of life compared with ibrutinib in patients with WM and MYD88 mutations. This article talks about a study called the ASPEN trial, which compares two medicines used for treating a rare blood cancer that doctors call Waldenström macroglobulinemia. The medicines are called zanubrutinib (ZAN) and ibrutinib (IBR). They work in the same way, by blocking a protein called Bruton tyrosine kinase. When patients take medicines for an illness, it is important to learn about their physical, social, emotional and mental well-being (quality of life). In this study, we asked patients to fill out questionnaires about their well-being before starting the study treatment for their blood cancer, and again a few times while taking the medication, to see if there were any changes. What were the results of the study? There were two groups of patients. One group took ZAN and the other took IBR. The patients could not choose which medicine they were going to take. Results from both groups of patients were compared. Patients taking ZAN did not feel worse or better about their diarrhea and sickness, but those taking IBR said these symptoms had become worse. Both medicines improved how patients were feeling. However, improvement in tiredness and physical ability was larger in patients taking ZAN than those on IBR, especially for the patients whose cancer was getting better. What do the results mean? For patients with a rare blood cancer in this study, those taking ZAN had a better quality of life than those taking IBR. Article highlights Waldenström macroglobulinemia (WM) is a rare B-cell lymphoma marked by an excess of monoclonal immunoglobulin-M. Patients with WM experience symptoms including fatigue, muscle weakness, neuropathy and anemia, significantly affecting their health-related quality of life (HRQoL). Bruton tyrosine kinase (BTK) inhibitors, particularly ibrutinib, have shown efficacy in treating WM; however, they are associated with adverse events that can lead to early treatment discontinuation, negatively affecting HRQoL. Zanubrutinib, a next-generation BTK inhibitor, was designed with improved selectivity for BTK to minimize off-target effects and toxicities and thereby potentially improve HRQoL. Materials & methods ASPEN is a randomized, open-label, Phase III study comparing zanubrutinib and ibrutinib in patients with relapsed/refractory or treatment-naive WM. In this study, HRQoL was measured with two validated patient-reported instruments: the European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire – Core 30 and the EQ-5D-5L. Results Clinically meaningful improvements in diarrhea and nausea/vomiting symptoms were observed in patients receiving zanubrutinib compared with ibrutinib. Patients with a very good partial response to zanubrutinib had significantly increased HRQoL, particularly in physical functioning and fatigue symptoms. Both treatment arms had consistently improved EQ-5D-5L scores through Cycle 13. Discussion This is the first head-to-head study comparing HRQoL in patients with WM treated with different BTK inhibitors. The results reported here are consistent with the primary results of the ASPEN study reported by Tam et al. [10]. Abstract

Although the taxonomy of Burkholderia has been extensively scrutinized, significant uncertainty remains regarding the generic boundaries and composition of this large and heterogeneous taxon. Here we used the amino acid and nucleotide sequences of 106 conserved proteins from 92 species to infer robust maximum likelihood phylogenies with which to investigate the generic structure of Burkholderia sensu lato. These data unambiguously supported five distinct lineages, of which four correspond to Burkholderia sensu stricto and the newly introduced genera Paraburkholderia, Caballeronia, and Robbsia. The fifth lineage was represented by P. rhizoxinica. Based on these findings, we propose 13 new combinations for those species previously described as members of Burkholderia but that form part of Caballeronia. These findings also suggest revision of the taxonomic status of P. rhizoxinica as it is does not form part of any of the genera currently recognized in Burkholderia sensu lato. From a phylogenetic point of view, Burkholderia sensu stricto has a sister relationship with the Caballeronia+Paraburkholderia clade. Also, the lineages represented by P. rhizoxinica and R. andropogonis, respectively, emerged prior to the radiation of the Burkholderia sensu stricto+Caballeronia+Paraburkholderia clade. Our findings therefore constitute a solid framework, not only for supporting current and future taxonomic decisions, but also for studying the evolution of this assemblage of medically, industrially and agriculturally important species.

Abstract

4.3. Description of Bradyrhizobium altum sp. nov. Bradyrhizobium altum (al’ tum. L. neut. adj. altum, high, referring to the high-altitude region where this species was isolated from). Cells are motile, Gram-negative rods and is approximately 1.29 × 0.45 μm in size. Colonies are circular, convex, opaque, cream in colour and < 1 mm in diameteron YMA after 7 daysincubation at 28 ◦ C. The pH range for growth on YMA is 5 to 10 and growth can occur between 15 and 37 ◦ Cand thesestrainscangrowinthe presenceof upto 2.0 % (w/v) NaCl. Reduction of nitrates, β- glucosidase urease, tested positive. It is also positive for the production of L-arabinose, Tween 40, Tween 80, adonitol, N-acetyl-D-glucosamine, D-cellobiose, D-arabitol, D-galactose, D- and L-fructose, m-inositol, D-mannitol, maltose, D-mannose, Draffinose, D-melibiose, L-rhamnose, D-sorbitol, sucrose, xylitol, Dtrehalose, turanose, methyl pyruvate, D-galacturonic acid, acetic acid, malonic acid, propionic acid, D-saccharic acid, quinic acid, sebacic acid, L-asparagine, D- and L-alanine, L-leucine, L-proline, L-ornithine and glycerol. Negative reactions were recorded for glucose fermentation, indole production, β- galactosidase activity and L-arginine dihydrolase. It is also negative for the assimilation of N-acetyl-glucosamine, potassium gluconate, D-maltose, adipic acid, capric acid, malic acid, phenylacetic acid, glycogen, α- cyclodextrin, L-arabinose, i-erythritol, Lthreonine, D- and L-serine, glucose-1-phosphate and glucose-6- phosphate. Strain is resistant to penicillin (10 μg/ml), ampicillin (10 μg/ml), gentamycin (10 μg/ml), chloramphenicol (30 μg/ml) and neomycin (10 μg/ml) and is susceptible to kanamycin (30 μg/ml), erythromycin (30 μg/ml), streptomycin (10 and 25 μg/ml) and tetracycline (30 μg/ml). The type strain Pear77 T (=SARCC 754 = LMG 31407), was isolated from root nodules of Pearsonia obovata collected in the Dullstroom area of the Mpumalanga Province, South Africa. This species forms effective nodules on cowpea (Vigna unguiculata) and siratro (Macroptilium atropurpureum). The DNA G + Ccontent of the type strain is 63.3 mol%. The GenBank/EMBL/DDBJ accession numbers for the genome sequence and gene sequences for this type strain are: whole genome (JACMYP000000000) and 16S rRNA (LR877298), atpD (LN650104), glnII (LN650146), dnaK (LN650125), gyrB (LN650209), rpoB (LN650188) and nodA (LN650251).

Abstract

4.2. Description of Bradyrhizobium oropedii sp. nov. Bradyrhizobium oropedii (o.ro.pe’ di.i. Gr. neut. n. oropedion mountain plain; N.L. gen. n. oropedii, of the central Southern African plateau, from where the species was isolated). Cells are motile, Gram-negative, rod shaped and approximately 1.87 × 0.48 μm in size. Growth occurs on YMA at 28 ◦ C after 7 days and colonies are translucent to opaque, circular, convex and cream in colour. Grows at pH values ranging from 5 to 10 and at temperatures ranging from 15 to 35 ◦ C. Isolates are unable to grow in the presence of more than 0.5% NaCl (w/v). Strains tested positive for oxidase, catalase, urease and β- glucosidase activity as well as the production of Tween 40 and Tween 80. Tested positive for the following carbon sources: Dfructose, L-fructose, D-gluconic acid, D-mannitol, α and β- hydroxy butyric acid, p-hydroxy phenylacetic acid, L-alaninamide, L-lactic acid, L-leucine, D-saccharic acid, and urocanic acid. Negative reactions were recorded for indole production and the assimilation of capric acid, adipic acid, malic acid, glycogen, α- D-lactose, N-acetyl-D-galactosamine, α- cyclodextrin, Adonitol, D-cellobiose, D-raffinose, i-erythritol, maltose, lactulose, D-melibiose, sucrose, D-trehalose, turanose, xylitol, L-threonine, uridine, inosine, thymine, glucose-1-phosphate and glucose-6-phosphate, and m-inositol. Resistant to ampicillin (10 μg/ml), chloramphenicol (30 μg/ml), tetracycline (30 μg/ml), penicillin (10 μg/ ml), and susceptible to streptomycin (25 μg/ml). Abstract

South Africa is well-known for the diversity of its legumes and their nitrogen-fixing bacterial symbionts. However, in contrast to their plant partners, remarkably few of these microbes (collectively referred to as rhizobia) from South Africa have been characterised and formally described. This is because the rules of the International Code of Nomenclature of Prokaryotes (ICNP) are at odds with South Africa's National Environmental Management: Biodiversity Act and its associated regulations. The ICNP requires that a culture of the proposed type strain for a novel bacterial species be deposited in two international culture collections and be made available upon request without restrictions, which is not possible under South Africa's current national regulations. Here, we describe seven new Mesorhizobium species obtained from root nodules of Vachellia karroo , an iconic tree legume distributed across various biomes in southern Africa. For this purpose, 18 rhizobial isolates were delineated into putative species using genealogical concordance, after which their plausibility was explored with phenotypic characters and average genome relatedness. For naming these new species, we employed the rules of the recently published Code of Nomenclature of Prokaryotes described from Sequence Data (SeqCode), which utilizes genome sequences as nomenclatural types. The work presented in this study thus provides an illustrative example of how the SeqCode allows for a standardised approach for naming cultivated organisms for which the deposition of a type strain in international culture collections is currently problematic. Abstract

Background: Coronavirus disease 2019 (COVID-19) is highly contagious and has claimed more than one million lives, besides causing hardship and disruptions. The Fleischner Society has recommended chest X-ray (CXR) in detecting cases at high risk of disease progression, for triaging suspected patients with moderate-to-severe illness, and for eliminating false negatives in areas with high pre-test probability or limited resources. Although CXR is less sensitive than real-- time reverse transcription-polymerase chain reaction (RT-PCR) in detecting mild COVID-19, it is nevertheless useful because of equipment portability, low cost and practicality in serial assessments of disease progression among hospitalized patients. Objective: This study aims to review the typical and relatively atypical CXR manifestations of COVID-19 pneumonia in a tertiary care hospital. Methods: The CXRs of 136 COVID-19 patients confirmed through real-time RT-PCR from March to May 2020 were reviewed. A literature search was performed using PubMed. Results: A total of 54 patients had abnormal CXR whilst the others were normal. Typical CXR findings included pulmonary consolidation or ground-glass opacities in a multifocal, bilateral peripheral, or lower zone distribution, whereas atypical CXR features comprised cavitation and pleural effusion. Conclusion: Typical findings of COVID-19 infection in chest computed tomography studies can also be seen in CXR. The presence of atypical features associated with worse disease outcome. Recognition of these features on CXR will improve the accuracy and speed of diagnosing COVID-19 patients.
