ObjectivesIn Korea, patients diagnosed with complex regional pain syndrome (CRPS) in the army are typically discharged from the army; however, the course of the disease after discharge is not known. The purpose of this study was to investigate the course of CRPS that occurred in the army and to identify the influences of various clinical and psychosocial factors immediately before discharge on the disease course.MethodsClinical profiles and psychosocial status were collected from the medical records of 16 patients with CRPS type 1 who were discharged during the period between March 2017 and April 2018. The degree of improvement after discharge was assessed by follow-up evaluation through telephone contact. Cox proportional hazards regression was performed to identify clinical and psychosocial predictors for the rate of recovery.ResultsThe median time to recovery after discharge was 39 weeks (95% confidence interval [CI] = 8.8–69.2 weeks). The sole predictor for time to recovery after discharge was the time period from the onset of pain to discharge. Each one-week increase in the duration of pain experienced in the military was associated with a 18.2% (95% CI = 5.3%–29.5%) reduction in the rate of recovery after discharge (P = 0.007).ConclusionsPatients who experienced a short period of pain in the military demonstrated a relatively good prognosis after discharge. This may show how prolonged pain in the army could affect the experience of pain from a social point of view, in that it shows the disease course after a change in the social environment.

Korea Aerospace Research Institute(KARI) has accomplished the planning study for setting up the mid-long term development plan of Goheung Aviation Test Center for the extensive use. In the future the aviation center will have a role of the complex center for the flight test, component test and R&D in Korea. KARI searches the legal requirements and informations for constructing the aviation test airfield in Goheung. Aviation law describes a lot of requirements to construct an airfield and the airfield also needs runway, equipments/facilities, obstacle limitation surfaces/distances, limitation altitudes and air space etc.. In this study the research results for the obstacle limitation Surface are mainly presented relating to the existing runway and the new large-scaled runway in Goheung.

Failure of total hip arthroplasty with central migration of prosthetic components is uncommon. Various pelvis and visceral complications have been reported from intrapelvic migration of the acetabular cup or the cement or from the heat generated by methylmethacrylate polymerization. To our knowledge, we are the first to report intrapelvic migration of a femoral stem causing ipsilateral lower-extremity swelling, pressure sores, and severe bowel symptoms after the removal of the acetabular component.

Most pigmented villonodular synovitis (PVNS) is occurred in knee joint and finger of hand. PVNS is rarely occurred in shoulder joint. In English and French literatures, less than 30 cases were reported. We report 3 PVNS cases with rotator cuff tears, which was treated by arthroscoic extensive synovectomy, debridement and rotator cuff repair. The PVNS with rotator cuff tear in shoulder joint was rarely reported in the Korean literature.

The purpose of this study was to evaluate the reliability and validity of the Junior Temperament and Character Inventory (Junior TCI), a child and adolescent version of the Adult Temperament and Character Inventory (TCI), which is based on Cloninger’s biosocial model of personality. The Junior TCI was translated into Korean and administered to 663 Korean middle school students (male/female, 360/303; age, 13.3 ± 1.0 years) and their parents (mother/father, 444/84). Internal consistency was calculated by Cronbach α. A test-retest study of the Junior TCI was conducted across a 3-month interval with 97 subjects. Factor analyses for the temperament and character dimensions were performed using principal component analysis, rotating factors by Promax. Cronbach α values of the Junior TCI scales ranged from .48 to .80 for the temperament scales and from .64 to .68 for the character scales. Test-retest correlations ranged from .62 to .85 for the temperament scales and from .76 to .79 for the character scales. Principal component factor analyses showed three factors out of four temperament scales (Harm Avoidance 1 to 4; Novelty Seeking 1 to 4; Persistence; and Reward Dependence 1, 3, 4) and three factors out of three character scales (Self-Directedness 1 to 5; Cooperativeness 1 to 5; and Self-Transcendence 1 to 3) that were similar to factor structures of adult versions of the TCI. Explorative factor analysis with a condition of eigenvalue greater than 1 produced six factors, as compared to seven factors extracted in Cloninger’s original report of the adult TCI. In conclusion, this was the first study to tests and report detailed psychometric properties of the Junior TCI. The current study confirms that the Junior TCI has satisfactory psychometric properties for the use in child and adolescent populations and reflects Cloninger’s biosocial model of personality.

We have reviewed 178 intertrochanteric fractures treated by dynamic hip screw (DHS) fixation between March 1995 and December 1999 and followed for a minimum of 1 year. We used Singh's classification of the trabecular bone structure in the proximal femur as a measure of osteoporosis and also classified the fractures according to three different systems (Boyd-Griffin, Evans, AO). The postoperative radiographs were examined for loss of reduction, i.e. varus angulation >100, perforation of the femoral head, more than 20-mm extrusion of a lag screw or metal failure. We found 49 cases which showed radiographic failures. Two were stable fractures and 47 unstable fractures (Evans' classification). Unstable fractures with osteoporosis had a failure rate of more than 50%. In such cases DHS should not be the first choice for treatment.

Of 2,143 admissions following suicide attempts over a seven year period (1980- 1986)in Seoul, Korea, there were 38 cases (1.8%) of suicide by burning. These cases were matched by age and sex with a control group of suicide attempts in general and were investigated for psychosocial differences from other suicide attempters. No significant differences were found in social factors such as education, religion, marital status, seasonal change and occupation. The majority of suicide attempts by burning were in a public place (73.7%) and were eventually fatal despite intensive medical treatment (78.9%). Half (52.7%) suffered from significant mental disorders and marital disharmony.
