3. Calling attention to opponents of climate action in climate and health messaging

Summary: Previous research suggests that providing information about the health effects of climate change and the health benefits of climate action can increase public engagement with the issue. We sought to extend those findings with an experiment to test the motivational value of calling attention to opponents of climate action. In February, 2022, we conducted a survey experiment with adults from the USA, quota-sampled to represent the USA population (n=2201). Participants were randomly assigned to a no-message control condition, or one of four message conditions identified as authored by concerned health professionals. These messages warned recipients about the negative effects of climate change on health, and either made no mention of an opponent to climate action, or were messages augmented by identifying one of three opponents: (1) fossil fuel chief executive officers and their lobbyists, (2) politicians, or (3) a combination of the two. Portrayal of opponents to climate action increased attitudinal engagement, support for mitigation policies, and intentions to advocate for climate solutions, compared with message conditions not identifying an opponent—with the combined opponent portrayal tending to result in the largest effects; these effects were evident with audiences across political lines, especially political conservatives. Climate and health messages—with or without portrayal of an opponent—also increased trust in the messengers relative to the no-message control. These findings suggest that identifying opponents to climate action can be advantageous to building support for such action, reducing political issue polarisation, and fostering greater trust in health professionals as climate messengers.

Autism spectrum disorder (ASD) is a neuro-behavioral syndrome that develops in childhood and can be comorbid with restrictive and avoidant food intake disorder. This case details a young man who was hospitalized with pancytopenia due to restrictive nutritional intake related to his severe ASD. He was found to have undetectable vitamin B12 levels. His blood counts improved with transfusion, nutritional supplementation, and dental care. This report illustrates the importance of understanding ASD and potential medical complications of related behaviors.

The gene expression analysis of formalin-fixed paraffin-embedded (FFPE) tissues is often hampered by poor RNA quality, which results from the oxidation, cross-linking and other chemical modifications induced by the inclusion in paraffin. Yet, FFPE samples are a valuable source for molecular studies and can provide great insights into disease progression and prognosis. With the advancement of genomic technologies, new methods have been established that offer reliable and accurate gene expression workflows on samples of poor quality. NanoString is a probe-based technology that allows the direct counting of the mRNA transcripts and can be applied to degraded samples. Here, we have tested 2 RNA extraction methods for FFPE samples, and we have performed a titration experiment to evaluate the impact of RNA degradation and RNA input on the gene expression profiles assessed using the NanoString IO360 panel. We have selected FFPE samples of different DV200 values and assessed them on the nCounter platform with 2 different amounts of input RNA. This study concludes that the nCounter is a robust and reliable platform to assess the gene expression of RNA samples with DV200 > 30%; its robustness and ease of use could be of particular benefit to clinical settings.

- Abstract
The limited temporal completeness and taxonomic accuracy of species lists, made available in a traditional manner in scientific publications, has always represented a problem. These lists are invariably limited to a few taxonomic groups and do not represent up-to-date knowledge of all species and classifications. In this context, the Brazilian megadiverse fauna is no exception, and the Catálogo Taxonômico da Fauna do Brasil (CTFB) (http://fauna.jbrj.gov.br/), made public in 2015, represents a database on biodiversity anchored on a list of valid and expertly recognized scientific names of animals in Brazil. The CTFB is updated in near real time by a team of more than 800 specialists. By January 1, 2024, the CTFB compiled 133,691 nominal species, with 125,138 that were considered valid. Most of the valid species were arthropods (82.3%, with more than 102,000 species) and chordates (7.69%, with over 11,000 species). These taxa were followed by a cluster composed of Mollusca (3,567 species), Platyhelminthes (2,292 species), Annelida (1,833 species), and Nematoda (1,447 species). All remaining groups had less than 1,000 species reported in Brazil, with Cnidaria (831 species), Porifera (628 species), Rotifera (606 species), and Bryozoa (520 species) representing those with more than 500 species. Analysis of the CTFB database can facilitate and direct efforts towards the discovery of new species in Brazil, but it is also fundamental in providing the best available list of valid nominal species to users, including those in science, health, conservation efforts, and any initiative involving animals. The importance of the CTFB is evidenced by the elevated number of citations in the scientific literature in diverse areas of biology, law, anthropology, education, forensic science, and veterinary science, among others.

Current liver transplantation societies recommend recipients with active coronavirus disease 2019 (COVID-19) be deferred from transplantation for at least 2 wks, have symptom resolution and at least 1 negative severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) test.1 This approach does not address patients who require urgent transplantation and will otherwise die from liver failure. We report a successful orthotopic liver transplant (OLT) in a patient with active COVID-19 infection. This is only the second to be reported worldwide and the first in Canada.

In developed countries, surgeons and anesthesiologists approach the mediastinal mass causing airway compression with prudence and trepidation. Resource-limited settings provide unique challenges in the diagnosis and management of patients with critical airway compression. We report the successful treatment of a patient in Port-au-Prince, Haiti with a posterior mediastinal mass that filled the left chest cavity and caused critical airway stenosis. The pathology revealed a Gardner Fibroma, which is rarely associated with mediastinal airway obstruction.

Background. Increased utilization of donation after circulatory death (DCD) lungs may help alleviate the supply/demand mismatch between available donor organs and lung transplant candidates. Using an established porcine DCD model, we sought to determine the effect of increasing warm ischemia time (WIT) after circulatory arrest on lung function during ex vivo lung perfusion (EVLP). Methods. Porcine donors (n = 15) underwent hypoxic cardiac arrest, followed by 60, 90, or 120 minutes of WIT before procurement and 4 hours of normothermic EVLP. Oxygenation, pulmonary artery pressure, airway pressure, and compliance were measured hourly. Lung injury scores were assessed histologically after 4 hours of EVLP. Results. After EVLP, all 3 groups met all the criteria for transplantation, except for 90-minute WIT lungs, which had a mean pulmonary artery pressure increase greater than 15%. There were no significant differences between groups as assessed by final oxygenation capacity, as well as changes in pulmonary artery pressure, airway pressure, or lung compliance. Histologic lung injury scores as well as lung wet-to-dry weight ratios did not significantly differ between groups. Conclusions. These results suggest that longer WIT alone (up to 120 minutes) does not predict worse lung function at the conclusion of EVLP. Expanding acceptable WIT after circulatory death may eventually allow for increased utilization of DCD lungs in procurement protocols.

Methods: Data from the Anglo–Danish–Dutch Study of Intensive Treatment in People with Screen‐Detected Diabetes in Primary Care‐Denmark study was used. A total of 1,170 Danish patients with screen‐detected type 2 diabetes attended a health examination, including assessment of sexual concerns using self‐report questionnaires and of SD using the Female Sexual Function Index (FSFI‐R) and the International Index of Erectile Function (IIEF‐5) instruments. Main Outcome Measures: The main outcome measures used regarding sexual concerns are the following: prevalence of failure to fill sexual needs, of experiencing sexual distress, finding it important to have a good sexual life, and additionally, prevalence of SD. Results: Data regarding sexual activity status during the last 12 months were available among 583 men and 377 women. Seventeen percent of men and 47% of women reported to be sexually inactive, among whom 57% of men and 42% of women reported failure to fill sexual needs; 31% of men and 10% of women that it was important to have a good sexual life, and 32% of men and 11% of women that they were experiencing sexual distress. Around half of men and women were excluded from the SD analysis, mainly because of reporting lack of sexual intercourse during the last 4 weeks. Among those included, 54% of men and 12% of women were found to have SD. Conclusions: Sexual inactivity is highly prevalent among middle‐aged and older men and women with early type 2 diabetes and these patients often have sexual concerns. The high exclusion rates when assessing SD using the FSFI‐R and IIEF‐5 instruments makes it difficult to draw conclusions regarding the prevalence. INTRODUCTION Due to having atypical presentations, women with potential cardiac disease may be approached and treated less aggressively than male patients. Differences in care along gender lines have been described at a university affiliated hospital in West Virginia (WV) in the past three decades. The purpose of this study is to determine if differences along gender lines in the care of patients with chest pain persists. METHODS From July-December of 2015, a retrospective chart review was performed at Cabell Huntington Hospital in WV. Data from 375 patients with a diagnosis of chest pain presenting to the emergency department (ED) was compared to data from 25 years prior. Symptoms, risk factors, medical history, medications, physical examination, laboratory, and electrocardiogram results, treatment in the emergency department, and ultimate diagnoses were studied. RESULTS Women had less reproducible musculoskeletal pain and more use of over-the-counter medicines, hormones, and pulmonary medications before presenting to the ED. Treatment with nitroglycerin and gastrointestinal medications occurred more commonly in women. Regarding evaluations of symptoms, women received more electrocardiograms, cardiac monitoring, and laboratory testing in comparison to 25 years prior. Cardiology consultations and emergency catheterization as well as the use of aspirin, heparin, antibiotics, anxiolytics, and thrombolytic agents were more commonly used in the treatment of women in comparison to 25 years prior. CONCLUSIONS In comparison to 25 years prior, there were documented improvements in the management of women presenting to the ED and experiencing new-onset, non-traumatic chest pain.

Background Morbidity and Mortality Conference (MMC) is a traditional quality assessment tool among surgical residencies to evaluate complications within their care and to integrate and educate best practices. Unfortunately, it is difficult to validate and measure loop closure as a result of MMC. This may result in repeating past mistakes or worse, becoming a meeting of anecdotal experiences. The goal of this study is to present results from a morbidity and mortality assessment tool (MMAT) which provides a means of measuring and tracking factors related to those surgical complications discussed within the MMC. Materials and Methods Three years of MMC presentations were organized into a database and further divided and organized into variables which included case by the class year of the presenting resident, service line, month the case was presented, and potential contributing factors. Contributing factors considered for each case were categorized as: Systems-Based, Direct Patient Care, and Interpersonal Communication. Contributing factors were assigned to cases by a review committee consisting of residents and faculty members. Results The lack of knowledge, technical error, lack of experience, lack of supervision, failure to communicate with team members were present in greater than 10% of the presented cases. There was a “July Effect” in the Trauma service, where there was a statistically significant difference between the percentage of cases presented that involved Failure to Communicate errors when compared with the remainder of the year. Conclusions MMAT allows longitudinal collection of data from each MMC to recognize patterns that facilitate improvements in systems of care and institutional memory.

23. Burn Injury, Characteristics, and Epidemiology in African American Children in North Carolina

Unintentional injuries, including burns, represent the leading cause of morbidity and mortality for children in the United States. This study seeks to identify trends among African American pediatric burn patients compared to other races with regards to burn characteristics, hospital stay, and type of surgical intervention utilized. This is a retrospective review of all admitted pediatric burn patients (< 18 years old) to a busy, tertiary burn center in North Carolina from 2009 through 2019. We used bivariate analysis to compare patients based on reported race, comparing African Americans to all other races. Modified Poisson regression was used to model the probability of undergoing autologous skin grafting based on African American race, adjusted for potential confounders. To estimate socioeconomic disadvantage, we used the Area Deprivation Index (ADI), which uses factors for the theoretical domains of income, education, employment, and housing quality to rank groups of geographical blocks from lowest level of disadvantage (1) to the highest (100). 4,227 children were admitted to UNC between 2009-2019. The median age was 3 years (IQR 1-8) with a male preponderance at 59.8% (n=2,529). The median TBSA was 2% (IQR 1-5). African American (AA) children were disproportionally represented among pediatric burn patients, comprising 33.7% of all patients, compared to an African American state population of 22.2%. This chapter reviews the case of a patient with Parkinson disease (PD) who experienced a sustained reduction in symptoms following perioperative intracranial hemorrhage (ICH). The patient, a 55-year-old man, elected to receive deep brain stimulation (DBS) after presenting with PD with rest tremor, bradykinesia, and rigidity. During the implantation operation, the patient suffered a small ICH in the left subthalamic nucleus (STN). Subsequently, he reported a significant improvement in right-side symptoms with the DBS device still in the off state. The case is discussed in detail in this chapter and briefly compared with similar cases that have previously been reported. Computed tomography and magnetic resonance imaging studies done shortly after the ICH occurred are also included.

As children age, motor performance develops. Motor performance abilities enable children to process information in handling specific task efficiently. Although children develop motor skills in a variety of physical activities, it could be, however, easily achieved when they engage in voluntary activities in conformity with their interests. Traditional games offer the opportunity for children to play and officiate the rules without any constraint. The current study intends to explore the effectiveness of the traditional games intervention program in the improvement of form one school-age children's motor skills related performance components. A total number of 40 form one Malaysian student [male (n=20) and female (n=20)] with age range of 12-13 selected randomly participated in the study. The quasi-experiment method was applied in the study and experiment group (n=40) went through traditional games intervention which consisted of performing selected traditional games for 60 minutes, three times weekly for eight weeks. The pre-test (before treatment), mid-test (week fourth) and posttest (week eight) data were collected and analysed using MANOVA repeated measure. The results indicate a significant improvement of motor performance through traditional games intervention [F (8, 29) = 1704.16, p < .05]. Follow-up tests also show that the traditional games intervention is a factor [F(12,105) =1.99, p < .05] to agility [F(3,36) = .50, p >.05], reaction time [F(3,36) = .51, p >.05], speed [F(3,36) = 3.64, p <.05] and balance [F(3,36) = .02, p > .05]. There is sufficient evidence to conclude that Malaysian based traditional games are effective in improving motor abilities of school-age children. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Viruses are the leading etiology of acute respiratory infections (ARI) in children. However, there is limited knowledge on drivers of severe acute respiratory infection (SARI) cases involving viruses. We aimed to identify factors associated with severity and prolonged hospitalization of viral SARI among children < 5 years in Burkina Faso., Methods: Data were collected from four SARI sentinel surveillance sites during October 2016 through April 2019. A SARI case was a child < 5 years with an acute respiratory infection with history of fever or measured fever ≥ 38 °C and cough with onset within the last ten days, requiring hospitalization. Very severe ARI cases required intensive care or had at least one danger sign. Oropharyngeal/nasopharyngeal specimens were collected and analyzed by multiplex real-time reverse-transcription polymerase chain reaction (rRT-PCR) using FTD-33 Kit. For this analysis, we included only SARI cases with rRT-PCR positive test results for at least one respiratory virus. We used simple and multilevel logistic regression models to assess factors associated with very severe viral ARI and viral SARI with prolonged hospitalization., Results: Overall, 1159 viral SARI cases were included in the analysis after excluding exclusively bacterial SARI cases (n = 273)very severe viral ARI cases were common among children living in urban areas (AdjOR = 1.3; 95% CI: 1.1-1.6), those < 3 months old (AdjOR = 1.5; 95% CI: 1.1-2.3), and those coinfected with Klebsiella pneumoniae (AdjOR = 1.9; 95% CI: 1.2-2.2). Malnutrition (AdjOR = 2.2; 95% CI: 1.1-4.2), hospitalization during the rainy season (AdjOR = 1.71; 95% CI: 1.2-2.5), and infection with human CoronavirusOC43 (AdjOR = 3; 95% CI: 1.2-8) were significantly associated with prolonged length of hospital stay (> 7 days)., Conclusion: Younger age, malnutrition, codetection of Klebsiella pneumoniae, and illness during the rainy season were associated with very severe cases and prolonged hospitalization of SARI involving viruses in children under five years. Background: Phenotypic variability is a consistent finding in neurogenetics and therefore applicable to hereditary spastic paraparesis. Identifying reasons for this variability is a challenge. We hypothesized that, in addition to genetic modifiers, extrinsic factors influence variability., Objectives: Our aim was to describe the clinical variability in hereditary spastic paraparesis from the person's perspective. Our goals were to identify individual and environmental factors that influence muscle tone disorders and derive interventions which could improve spasticity., Methods: This study was based on self-assessments with questions on nominal and ordinal scales completed by participants with hereditary spastic paraparesis. A questionnaire was completed either in-person in the clinic or electronically via lay organization websites., Results: Among the 325 responders, most had SPG4/SPAST (n = 182, 56%) with a mean age at onset of 31.7 (SD 16.7) years and a mean disease duration of 23 (SD 13.6) years at the time of participation. The 2 factors identified as improving spasticity for > 50% of the responders were physiotherapy (193/325, 59%), and superficial warming (172/308, 55%). Half of the responders (n = 164, 50%) performed physical activity at least once a month and up to once a week. Participants who reported physiotherapy as effective were significantly more satisfied with ≥ 3 sessions per week. Psychologically stressful situations (246/319, 77%) and cold temperatures (202/319, 63%) exacerbated spasticity for most participants., Conclusion: Participants perceived that physiotherapy reduced spasticity and that the impact of physiotherapy on spasticity was much greater than other medical interventions. Therefore, people should be encouraged to practice physical activity at least 3 times per week. 49. Timing of seasonal influenza epidemics for 25 countries in Africa during 2010-19: a retrospective analysis.

Background: Using country-specific surveillance data to describe influenza epidemic activity could inform decisions on the timing of influenza vaccination. We analysed surveillance data from African countries to characterise the timing of seasonal influenza epidemics to inform national vaccination strategies., Methods: We used publicly available sentinel data from African countries reporting to the WHO Global Influenza Surveillance and Response FluNet platform that had 3-10 years of data collected during 2010-19. We calculated a 3-week moving proportion of samples positive for influenza virus and assessed epidemic timing using an aggregate average method. The start and end of each epidemic were defined as the first week when the proportion of positive samples exceeded or went below the annual mean, respectively, for at least 3 consecutive weeks. We categorised countries into five epidemic patterns: northern hemisphere-dominant, with epidemics occurring in October-March; southern hemisphere-dominant, with epidemics occurring in April-September; primarily northern hemisphere with some epidemic activity in southern hemisphere months; primarily southern hemisphere with some epidemic activity in northern hemisphere months; and year-round influenza transmission without a discernible northern hemisphere or southern hemisphere predominance (no clear pattern)., Findings: Of the 34 countries reporting data to FluNet, 25 had at least 3 years of data, representing 46% of the countries in Africa and 89% of Africa's population. Study countries reported RT-PCR respiratory virus results for a total of 503 609 specimens (median 12 971 [IQR 9607-20 960] per country-year), of which 74 001 (15%; median 2078 [IQR 1087-3008] per country-year) were positive for influenza viruses. 248 epidemics occurred across 236 country-years of data (median 10 [range 7-10] per country). Six (24%) countries had a northern hemisphere pattern (Algeria, Burkina Faso, Egypt, Morocco, Niger, and Tunisia). Eight (32%) had a primarily northern hemisphere pattern with some southern hemisphere epidemics (Cameroon, Ethiopia, Mali, Mozambique, Nigeria, Senegal, Tanzania, and Togo). Three (12%) had a primarily southern hemisphere pattern with some northern hemisphere epidemics (Ghana, Kenya, and Uganda). Three (12%) had a southern hemisphere pattern (Central African Republic, South Africa, and Zambia).
