1. The Benefit of Exercise in Patients With Cancer Who Are Receiving Chemotherapy: A Systematic Review and Network Meta-Analysis

Abstract

Objective. This study aimed to determine which therapeutic exercise-based intervention is most effective in improving cardiorespiratory fitness (CRF) in patients with cancer receiving chemotherapy. Methods. The authors conducted a systematic review with network meta-analysis in MEDLINE (PubMed), Embase, Cumulative Index to Nursing and Allied Health Literature, Scopus, SPORTDiscus, and Web of Science. The authors employed the Physiotherapy Evidence Database and the Revised Cochrane Risk of Bias Tool for Randomized Trials to assess the methodological quality and risk of bias, respectively. Results. A total of 27 studies were included. Data were pooled using a random-effects model. Adding aerobic training (moderate to high intensity), with or without resistance training, to usual care versus usual care was statistically significant, with a small beneficial effect (aerobic training: standardized mean difference = 0.46; 95% CI= 0.17 to 0.75; aerobic and resistance training: standardized mean difference = 0.26; 95% CI = 0.00 to 0.52) for peak oxygen consumption at the postintervention assessment. Conclusion. Therapeutic exercise-based interventions to improve short-term CRF in patients with cancer receiving chemotherapy should include moderate- to high-intensity aerobic exercise, with or without resistance training. Impact. It is important to improve CRF in the oncological population due to its relationship with mortality. The results showed the benefit of exercise to improve cardiorespiratory fitness in the oncology population receiving chemotherapy treatment. Agents to reduce the gonadotoxic effects of chemotherapeutics are still under investigation. in this context, we aimed to investigate the protective effect of sildenafil against chemotherapeutic-induced gonadotoxicity in a rat model. A total of 62 female rats were divided into eight groups as control, sildenafil (1.4 mg/kg, orally), doxorubicin (3 mg/kg, i.p.), cisplatin (5 mg/kg, i.p.), cyclophosphamide (200 mg/kg, i.p.), doxorubicin+sildenafil, cisplatin+sildenafil, and cyclophosphamide+sildenafil (1.4 mg/kg orally sildenafil in addition to the same dose of chemotherapeutics). The groups were compared in terms of follicle count, ovarian size, and anti-mfillerian hormone (AMH) levels. Use of sildenafil with cyclophosphamide was effective only in preserving primary follicle count (p = 0.026) and had no significant change in the secondary follicle count, ovarian size, or AMH level. Adding sildenafil to cisplatin had a significant protective effect on primary follicle count (p = 0.011), secondary follicle count (p = 0.009), and ovarian size (p = 0.001), but this effect could not be demonstrated at AMH level. Sildenafil was not effective on any parameter in the doxorubicin group. Sildenafil may be effective in reducing the gonadotoxicity associated with the use of cisplatin and cyclophosphamide. Key words: cisplatin, cyclophosphamide, doxorubicin, sildenafil, rat. Les agents permettant de reduire les effets gonadotoxiques de la chimiotherapie sont toujours a l'etude. Dans ce contexte, nous avons vise a etudier l'effet protecteur du sildenafil dans un modele de gonadotoxicite provoquee par la chimiotherapie chez le rat. Nous avons reparti 62 rates dans les huit groupes suivants : temoin, sildenafil, doxorubicine, cisplatine, cyclophosphamide, doxorubicine + sildenafil, cisplatine + sildenafil et cyclophosphamide + sildenafil. Nous avons administre par voie intraperitoneale de la doxorubicine a 3 mg/kg dans le groupe doxorubicine, du cisplatine a 5 mg/kg dans le groupe cisplatine, de la cyclophosphamide a 200 mg/kg dans le groupe cyclophosphamide, avec du sildenafil a 1,4 mg/kg en association avec les memes doses d'agents chimiotherapeutiques dans les autres groupes. Nous avons procede a des comparaisons entre les groupes quant au nombre de follicules, a la taille des ovaires et aux taux d'hormone anti-mullerienne (HAM). L'utilisation du sildenafil avec la cyclophosphamide a ete efficace uniquement pour la preservation du nombre de follicules primaires (p = 0.026), sans entrainer de variation marquee quant au nombre de follicules secondaires, a la taille des ovaires et aux taux d'HAM. L'ajout du sildenafil au cisplatine a eu un effet protecteur notable quant aux nombres de follicules primaires (p = 0.011) et secondaires (p = 0.009) de meme qu'a la taille des ovaires (p = 0.001), mais nous n'avons pas pu observer cet effet quant aux taux d'HAM. Le sildenafil n'a eu d'effet sur aucun des parametres dans le groupe doxorubicine. En conclusion, le sildenafil pourrait etre efficace pour la reduction de la gonadotoxicite associee avec l'utilisation du cisplatine et de la cyclophosphamide. Abstract

Objective. This trial will analyze the acute and cumulative effects of a tailored program called PRESIONA that combines therapeutic exercise and blood flow restriction to prevent chemotherapy-induced peripheral neuropathy (CIPN) in individuals with early breast cancer undergoing neoadjuvant chemotherapy. Methods. PRESIONA will be a physical therapist-led multimodal exercise program that uses blood flow restriction during lowload aerobic and strength exercises. For the acute study, only 1 session will be performed 1 day before the first taxane cycle, in which 72 women will be assessed before intervention and 24 hours post intervention. For the cumulative study, PRESIONA will consist of 24 to 36 sessions for 12 weeks following an undulatory prescription. At least 80 women will be randomized to the experimental group or control group. Feasibility will be quantified based on the participant recruitment to acceptance ratio; dropout, retention, and adherence rates; participant satisfaction; tolerance; and program security. In the efficacy study, the main outcomes will be CIPN symptoms assessed with a participant-reported questionnaire (EORTC QLQ-CIPN20). In addition, to determine the impact on other participant-reported health and sensorimotor and physical outcomes, the proportion of completed scheduled chemotherapy sessions will be examined at baseline (t0), after anthracycline completion (t1), after intervention (t2), and at the 2-month (t3) and 1-year follow-ups (t4). Conclusion. The proposed innovative approach of this study could have a far-reaching impact on therapeutic options, and the physical therapist role could be essential in the oncology unit to improve quality of life in individuals with cancer and reduce side effects of cancer and its treatments. Impact. Physical therapists in the health care system could be essential to achieve the planned doses of chemotherapy to improve survival and decrease the side effects of individuals with breast cancer. The prevention of CIPN would have an impact on the quality of life in these individuals, and this protocol potentially could provide an action guide that could be implemented in any health care system. Problem: A quality improvement project was conducted to evaluate the use of an integrative wellness program to improve mood and relieve chemotherapy-induced nausea in children and adolescents diagnosed with cancer at a freestanding children's hospital. Background: A wide variety of integrative wellness programs can assist in minimizing symptoms. Yoga and mindfulness create peace and balance by the practice of breathing and meditation, which can promote relaxation and stress reduction. Interventions: An integrative wellness program was implemented for hematology/bone-marrow services in an outpatient clinic setting and inpatient unit within an urban pediatric hospital. The program included yoga, mindfulness, and aromatherapy. Nineteen patients participated in yoga, 18 patients in mindfulness, and 9 patients in aromatherapy over the course of one year. External specialty-trained personnel provided therapies in yoga and mindfulness in both the inpatient and outpatient setting. Additionally, while specialty-trained nurses provided aromatherapy to inpatients. Outcomes measured included nausea and mood pre- and post-implementation using established tools. Results: Results showed an increase in scores in the positive direction for yoga and mindfulness interventions from pre-implementation to post-implementation achieving significance in all four constructs of The Brief Mood Introspection Scale (Mayer & Gaschke, 1988). There was a reported decrease in nausea symptoms after the aromatherapy intervention and an overall satisfaction with the program. Conclusion: Integrative wellness is a positive intervention for this patient population and is beneficial to increase a child/adolescent's mood level.
