Abstract. Background:. Hypothermia therapy has been suggested to attenuate myocardial necrosis; however, the clinical implementation as a valid therapeutic strategy has failed, and new approaches are needed to translate into clinical applications. This study aimed to assess the feasibility, safety, and efficacy of a novel selective intracoronary hypothermia (SICH) device in mitigating myocardial reperfusion injury. Methods:. This study comprised two phases. The first phase of the SICH was performed in a normal porcine model for 30 minutes (n = 5) to evaluate its feasibility. The second phase was conducted in a porcine myocardial infarction (MI) model of myocardial ischemia/reperfusion which was performed by balloon occlusion of the left anterior descending coronary artery for 60 minutes and maintained for 42 days. Pigs in the hypothermia group (n = 8) received hypothermia intervention onset reperfusion for 30 minutes and controls (n = 8) received no intervention. All animals were followed for 42 days. Cardiac magnetic resonance analysis (five and 42 days post-MI) and a series of biomarkers/histological studies were performed. Results:. The average time to lower temperatures to a steady state was 4.8 ± 0.8 s. SICH had no impact on blood pressure or heart rate and was safely performed without complications by using a 3.9 F catheter. Interleukin-6 (IL-6), tumor necrosis factor-α, C-reactive protein (CRP), and brain natriuretic peptide (BNP) were lower at 60 min post perfusion in pigs that underwent SICH as compared with the control group. On day 5 post MI/R, edema, intramyocardial hemorrhage, and microvascular obstruction were reduced in the hypothermia group. On day 42 post MI/R, the infarct size, IL-6, CRP, BNP, and matrix metalloproteinase-9 were reduced, and the ejection fraction was improved in pigs that underwent SICH. Conclusions:. The SICH device safely and effectively reduced the infarct size and improved heart function in a pig model of MI/R. These beneficial effects indicate the clinical potential of SICH for treatment of myocardial reperfusion injury.

There is a need for open-source libraries in emission tomography that (i) use modern and popular backend code to encourage community contributions and (ii) offer support for the multitude of reconstruction techniques available in recent literature, such as those that employ artificial intelligence. The purpose of this research was to create and evaluate a GPU-accelerated, open-source, and user-friendly image reconstruction library, designed to serve as a central platform for the development, validation, and deployment of various tomographic reconstruction algorithms. PyTomography was developed using Python and inherits the GPU-accelerated functionality of PyTorch and parallelproj for fast computations. Its flexible and modular design decouples system matrices, likelihoods, and reconstruction algorithms, simplifying the process of integrating new imaging modalities using various python tools. Example use cases demonstrate the software capabilities in parallel hole SPECT and listmode PET imaging. Overall, we have developed and publicly share PyTomography, a highly optimized and user-friendly software for medical image reconstruction, with a class hierarchy that fosters the development of novel imaging applications.

Reverse percolation analyzes the overall connectivity of a network after the addition of nodes or edges at predefined probabilities, which parallels the significance and application potential of traditional percolation theory. This paper explores the intersection between reverse percolation and network growth. It discusses the addition of a set of new nodes and edges simultaneously, offering insight into two distinct scenarios: random attachment, where all potential new edges have equal occupation probability, and preferential attachment, where the occupation probability of a potential new edge is proportional to the degree of the original network node it connects to. Reverse percolation analytic models are developed for these scenarios to compute the spanning cluster fraction and the percolation threshold. The accuracy of these models are evaluated across multiple real-world networks. Furthermore, the possibility of generating scale-free networks through single-step node and edge additions based on the preferential attachment mechanism is investigated. The results reveal that the double-unknown generating-function-based model proposed in this paper ensures universal applicability and accurate prediction across both investigated scenarios. In the context of preferential attachment, the simultaneous addition of nodes and edges may always produce a proportion of networks exhibiting scare-free structures. However, irrespective of the emergence of scale-free properties, the connectivity status of these networks can be effectively predicted using the proposed reverse percolation model.

The search for new green and efficient stabilizers is of great importance for the stabilization of nitrocellulose (NC). This is due to the shortcomings of traditional stabilizers, such as high toxicity. In this study, reduced polyaniline (r-PANI), which has a similar functional structure to diphenylamine (DPA) but is non-toxic, was prepared from PANI based on the action with N2H4 and NH3–H2O, and used for the first time as a potential stabilizer for NC. XPS, FTIR, Raman, and SEM were used to characterize the reduced chemical structure and surface morphology of r-PANI. In addition, the effect of r-PANI on the stabilization of NC was characterized using DSC, VST, isothermal TG, and MMC. Thermal weight loss was reduced by 83% and 68% and gas pressure release by 75% and 49% compared to pure NC and NC&3%DPA, respectively. FTIR and XPS were used to characterize the structural changes of r-PANI before and after reaction with NO2. The 1535 cm−1 and 1341 cm−1 of the FTIR and the 404.98 eV and 406.05 eV of the XPS showed that the –NO2 was generated by the absorption of NO2. Furthermore, the quantum chemical calculation showed that NO2 was directly immobilized on r-PANI by forming –NO2 in the neighboring position of the benzene ring.

Abstract Background Pepper Phytophthora blight is a devastating disease during the growth process of peppers, significantly affecting their yield and quality. Accurate, rapid, and non-destructive early detection of pepper Phytophthora blight is of great importance for pepper production management. This study investigated the possibility of using multispectral imaging combined with machine learning to detect Phytophthora blight in peppers. Peppers were divided into two groups: one group was inoculated with Phytophthora blight, and the other was left untreated as a control. Multispectral images were collected at 0-h samples before inoculation and at 48, 60, 72, and 84 h after inoculation. The supporting software of the multispectral imaging system was used to extract spectral features from 19 wavelengths, and textural features were extracted using a gray-level co-occurrence matrix (GLCM) and a local binary pattern (LBP). The principal component analysis (PCA), successive projection algorithm (SPA), and genetic algorithm (GA) were used for feature selection from the extracted spectral and textural features. Two classification models were established based on effective single spectral features and significant spectral textural fusion features: a partial least squares discriminant analysis (PLS_DA) and one-dimensional convolutional neural network (1D-CNN). A two-dimensional convolutional neural network (2D-CNN) was constructed based on five principal component (PC) coefficients extracted from the spectral data using PCA, weighted, and summed with 19-channel multispectral images to create new PC images. Results The results indicated that the models using PCA for feature selection exhibit relatively stable classification performance. The accuracy of PLS-DA and 1D-CNN based on single spectral features is 82.6% and 83.3%, respectively, at the 48h mark. In contrast, the accuracy of PLS-DA and 1D-CNN based on spectral texture fusion reached 85.9% and 91.3%, respectively, at the same 48h mark. The accuracy of the 2D-CNN based on 5 PC images is 82%. Conclusions The research indicates that Phytophthora blight infection can be detected 48 h after inoculation (36 h before visible symptoms). This study provides an effective method for the early detection of Phytophthora blight in peppers.

The reuse of reclaimed water is a cost-effective way to alleviate water resource scarcity, but the residual pathogenic microorganisms inevitably influence the safety of its reuse. The transport behavior of pathogenic microorganisms in receiving porous media varies under different environmental factors and could be harmful to the natural ecology and even human health if not well treated. Biochar is expected to be an effective, environmentally-friendly functional material to inhibit the transport of pathogenic microorganisms, with unreplaceable advantages of low price, simple preparation method, and strong adsorption capacity. In the present paper, we start from identifying the transport behavior of typical pathogenic microorganisms in porous media, including protozoa, bacteria, and viruses, and then analyzing the primary factors affecting the transport of pathogenic microorganisms from the aspects of biology, physics, and chemistry. Furthermore, the effects of types of raw materials, pyrolysis temperature, particle size, and functional modification methods on the remediation performance of biochar for the transport of pathogenic microorganisms are clearly reviewed. Finally, we aim to clarify the transport rules of pathogenic microorganisms in porous media and provide biochar-based technical means for effectively inhibiting the transport of pathogenic microorganisms, thereby improving the ecological and health safety of reclaimed water reuse. In recent years, a surge has been witnessed in cyber-attacks that leverage malicious Excel 4.0 macros (XLM) within documents. Malicious XLM codes often undergo complex obfuscation, posing a substantial challenge for conventional analysis methods and detection systems to discern the actual functionality within a vast array of samples. Consequently, an automated system for deobfuscating XLM and extracting key Indicators of Compromise (IOCs), named XLMRevealer, was developed to counter the diverse obfuscation strategies employed in malicious samples. XLMRevealer was architected upon abstract syntax trees and execution simulation, encompassing 138 comprehensive macro function handlers. Based on that, Word and Token features tailored to XLM code peculiarities were extracted, capturing multi-level, fine-grained features through feature fusion. XLMRevealer incorporated a CNNBiLSTM model to discern familial correlations across dimensions, facilitating family classification. Finally, a dataset comprising 2346 samples from five distinct sources was constructed for both deobfuscation and family classification experiments. Results indicated that XLMRevealer achieved a 71.3% deobfuscation success rate, outperforming XLMMacroDeobfuscator and SYMBEXCEL by 20.8% and 15.8%, respectively. Its efficiency was stable, with an average processing time of only 0.512 seconds. The family classification accuracy for deobfuscated XLM codes stood at 94.88%, surpassing all baseline models and underscoring the efficacy of Word and Token feature integration. Furthermore, to assess the impact of deobfuscation on family classification and account for variability in obfuscation techniques across families, experiments were conducted on both the original and uniformly obfuscated XLM codes. The accuracies were 89.58% and 53.61%, respectively, demonstrating the model's capability to learn obfuscation features and confirming the significant enhancement deobfuscation provides for family classification.

Background: Chronic total occlusion (CTO) is the most challenging subset in percutaneous coronary intervention (PCI), but the optimal selection of patients and indication for such procedures remain a subject of debate. We sought to investigate the role of physiological function in treatment decisions of CTO PCI by measuring fractional flow reserve (FFR) and Dynamic SPECT imaging in this study. Methods: All the FFR of CTO vessel were measured before and immediately after CTO revascularization, and Dynamic SPECT imaging were detected before PCI in patients with an identified CTO. Results: A total of 53 patients with single-vessel CTO lesions were included in this cohort study. The mean FFR value was 0.34 ± 0.09 at baseline. Immediately after successful CTO PCI, the FFR value significantly increased to 0.79 ± 0.11. The regional coronary flow reserve (CFR) of CTO vessels was 1.62 ± 0.64, which was significantly and positively correlated with the baseline FFR value (r = 0.607, p = 0.005). Abstract Background Indobufen is widely used in patients with aspirin intolerance in East Asia. The OPTION trial launched by our cardiac center examined the performance of indobufen based dual antiplatelet therapy (DAPT) after percutaneous coronary intervention (PCI). However, the vast majority of patients with acute coronary syndrome (ACS) and aspirin intolerance were excluded. We aimed to explore this question in a real-world population. Methods Patients enrolled in the ASPIRATION registry were grouped according to the DAPT strategy that they received after PCI. The primary endpoints were major adverse cardiovascular and cerebrovascular events (MACCE) and Bleeding Academic Research Consortium (BARC) type 2, 3, or 5 bleeding. Propensity score matching (PSM) was adopted for confounder adjustment. Results A total of 7135 patients were reviewed. After one-year follow-up, the indobufen group was associated with the same risk of MACCE versus the aspirin group after PSM (6.5% vs. 6.5%, hazard ratio [HR] = 0.99, 95% confidence interval [CI] = 0.65 to 1.52, P = 0.978). Smart manufacturing is an important initiative to promote the transformation and upgrading of industries and the high-quality development of the economy. However, the current situation of digitalized smart transformation in manufacturing enterprises is not optimistic, which is primarily attributed to the ambiguity surrounding the pathways. This study is based on the technology-organization-environment-individual (TOE-I) analytical framework; it selects 20 case studies of advanced manufacturing enterprises; and employs case studies and necessary condition fuzzy set qualitative comparative research methods (NCA and fsQCA) to investigate the pathways through which technology, organization, the environment, and individual microelements synergistically drive smart manufacturing from a configurational perspective. The study reveals that digital technology breakthroughs, digital infrastructure, digital talent, digital sharing, organizational resilience, organizational culture, and the entrepreneurial spirit are the core influencing factors in advancing smart manufacturing for manufacturing enterprises, and four implementation paths driven by smart manufacturing are analyzed. Among them, digital technology breakthroughs and digital infrastructure have a potential substitutive relationship in the “technology + talent” empowerment organizational model. Organizational resilience, organizational culture, and the entrepreneurial spirit are important safeguards for successful advancements in smart manufacturing. In contrast, digital infrastructure plays a more indirect, supporting role. Accordingly, this paper provides theoretical reference and practical guidance.

Wound healing is a complex process, which involves three stages: inflammation, proliferation, and remodeling. Inflammation is the first step; thus, immune factors play an important regulatory role in wound healing. In this study, we focused on a chemokine, C-C motif chemokine ligand 3 (CCL3), which is often upregulated for expression during wound healing. We compared cutaneous wound healing at the histological, morphological, and molecular levels in the presence and absence of CCL3. The results showed that the wound healing rate in the wild-type and CCL3 -/- + CCL3 mice was faster than that of CCL3 -/- mice ( P < 0.01), and application of CCL3 to wounds increased the healing rate. In the process of wound healing, the degree of reepithelialization and the rate of collagen deposition in the wound of CCL3 -/- mice were significantly lower than those of wild-type mice ( P < 0.01). The number of macrophages and the expression levels of tumor necrosis factor(TNF)-α and transforming growth factor (TGF)-β1 in the wounds of wild-type mice were much higher than those of the CCL3 -/- mice. Removal of macrophages and CCL3 -/- mice share similar phenotypes. Therefore, we infer that the wound healing requires the participation of macrophages, and CCL3 may play an important regulatory role through recruiting macrophages to the wound sites.

Estimating the percolating cluster fraction is central to many percolation models. For real networks, the total size of clusters with loops can be considered a plausible metric for this fraction. In this paper, we develop a semi-analytic algorithm to estimate clusters with loops for both site and bond percolation via modifying the message passing algorithm. We compared the estimates of the original message passing algorithm and our modified version with simulation results on four real networks. Our findings suggest that our modified algorithm can achieve accuracy for any real network, provided that a sufficient number of possible states following site or bond occupation are selected and analyzed to calculate the final estimate.

The problem of clogging, especially biological clogging, makes the long-term operation of subsurface flow constructed wetlands challenging. This study focused on the two subsurface flow constructed wetlands receiving reclaimed water near the Tuhai River of Shandong Province, aiming to provide a feasible method for the composition analysis and chemical remediation by H2O2 solution of clogging matter. The compositions and distributions of clogging matter in these two wetlands were analyzed, and the effects of H2O2 solutions with different mass fractions on proteins, polysaccharides, nucleic acids, and humic acids in the organic clogging components were studied. The two case studies indicated that the contents of clogging matter in the two subsurface flow constructed wetlands varied greatly due to the operation time and operation mode, and the proportion of inorganic components was higher than that of organic components. The organic components of the clogging matter were mainly proteins and humic acids, with very small amounts of polysaccharides and nucleic acids. In addition, the H2O2 solution with a mass fraction of 10% had a good removal effect on proteins and humic acids in the wetland substrates, which also could increase the permeability coefficient of the substrates without causing a large negative impact on the temperature and pH value of the wetland systems. This article proposed a simple, economical, environmentally friendly method for analyzing and remediating biological clogging of subsurface flow constructed wetlands, which was expected to be widely used in situ remediation of clogging matter.

Abstract Objective Today, the development mode of public hospitals in China is turning from expansion to efficiency, and the management mode is turning from extensive to refined. This study aims to evaluate the efficiency of clinical departments in a Chinese class A tertiary public hospital (Hospital M) to analyze the allocation of hospital resources among these departments providing a reference for the hospital management. Methods The hospitalization data of inpatients from 32 clinical departments of Hospital M in 2021 are extracted from the hospital information system (HIS), and a dataset containing 38,147 inpatients is got using stratified sampling. Considering the non-homogeneity of clinical departments, the 38,147 patients are clustered using the K-means algorithm based on workload-related data labels including inpatient days, intensive care workload index, nursing workload index, and operation workload index, so that the medical resource consumption of inpatients from non-homogeneous clinical departments can be transformed into the homogeneous workload of medical staff. Taking the numbers of doctors, nurses, and beds as input indicators, and the numbers of inpatients assigned to certain clusters as output indicators, an input-oriented BCC model is built named the workload-based DEA model. Meanwhile, a control DEA model with the number of inpatients and medical revenue as output indicators is built, and the outputs of the two models are compared and analyzed. Results Clustering of 38,147 patients into 3 categories is of better interpretability. 14 departments reach DEA efficient in the workload-based DEA model, 10 reach DEA efficient in the control DEA model, and 8 reach DEA efficient in both models. The workload-based DEA model gives a relatively rational judge on the increase of income brought by scale expansion, and evaluates some special departments like Critical Care Medicine Dept., Geriatrics Dept. and Rehabilitation Medicine Dept. more properly, which better adapts to the functional orientation of public hospitals in China. Conclusion The design of evaluating the efficiency of non-homogeneous clinical departments with the workload as output proposed in this study is feasible, and provides a new idea to quantify professional medical human resources, which is of practical significance for public hospitals to optimize the layout of resources, to provide real-time guidance on manpower grouping strategies, and to estimate the expected output reasonably.

The environmental protection industry has become an important support entity for the construction of ecological civilization and economic growth in China. However, there is little research on the competitiveness of environmental protection enterprises (EPEs). The construction of a set of scientific, comprehensive, practical, qualitative, and quantitative evaluation index systems is an important prerequisite for the sustainable and healthy development of the industry. Based on the literature analysis, semi-structured interviews with experts, and the Delphi method, the evaluation indicators for the competitiveness of EPEs were determined. Qualitatively, the evaluation index system of the competitiveness of EPEs contained 5 primary indicators, 12 secondary indicators, and 39 tertiary indicators. The analytic hierarchy process was used to determine the weights of indicators at each level. The primary indicators in order of weighting were organizational management capability, business environment, financial capability, innovation capability, and social responsibility, with corresponding weights of 26.13, 24.82, 21.76, 19.60, and 7.68%, respectively. Eight A-share listed EPEs in the water sector were selected for competitive evaluation. The evaluation index system of EPEs' competitiveness, being scientific-practical, combined, and quantitative, was constructed to provide a reference for the comprehensive evaluation of enterprises and the sustainable and healthy development of the industry. HIGHLIGHTS The evaluation index system was constructed with five dimensions.; Five primary indicators, 12 secondary indicators, and 39 tertiary indicators were determined in the evaluation index system.; Eight A-share-listed EPEs in the water sector were selected for an empirical study of their competitiveness.;

Mitochondria-associated ferroptosis exacerbates cardiac microvascular dysfunction in diabetic cardiomyopathy (DCM). Nicorandil, an ATP-sensitive K+ channel opener, protects against endothelial dysfunction, mitochondrial dysfunction, and DCM; however, its effects on ferroptosis and mitophagy remain unexplored. The present study aimed to assess the beneficial effects of nicorandil against endothelial ferroptosis in DCM and the underlying mechanisms. Cardiac microvascular perfusion was assessed using a lectin perfusion assay, while mitophagy was assessed via mt-Keima transfection and transmission electron microscopy. Ferroptosis was examined using mRNA sequencing, fluorescence staining, and western blotting. The mitochondrial localization of Parkin, ACSL4, and AMPK was determined via immunofluorescence staining. Following long-term diabetes, nicorandil treatment improved cardiac function and remodeling by alleviating cardiac microvascular injuries, as evidenced by the improved microvascular perfusion and structural integrity. mRNA-sequencing and biochemical analyses showed that ferroptosis occurred and Pink1/Parkin-dependent mitophagy was suppressed in cardiac microvascular endothelial cells after diabetes. Nicorandil treatment suppressed mitochondria-associated ferroptosis by promoting the Pink1/Parkin-dependent mitophagy. Moreover, nicorandil treatment increased the phosphorylation level of AMPKα1 and promoted its mitochondrial translocation, which further inhibited the mitochondrial translocation of ACSL4 via mitophagy and ultimately suppressed mitochondria-associated ferroptosis. Importantly, overexpression of mitochondria-localized AMPKα1 (mitoAα1) shared similar benefits with nicorandil on mitophagy, ferroptosis and cardiovascular protection against diabetic injury. In conclusion, the present study demonstrated the therapeutic effects of nicorandil against cardiac microvascular ferroptosis in DCM and revealed that the mitochondria-localized AMPK-Parkin-ACSL4 signaling pathway mediates mitochondria-associated ferroptosis and the development of cardiac microvascular dysfunction.

Este asumió unas responsabilidades imprescindibles para que dos potencias tan distintas como España y China, generaran las primeras relaciones diplomáticas fructíferas en los años setenta del siglo XVI, produciéndose lo que hoy llamaríamos un fenómeno de globalización.

Abstract Background Dual antiplatelet therapy (DAPT) with aspirin and a P2Y12 inhibitor is the standard of care after coronary stenting, including coronary stenting involving bioresorbable scaffolds (BRSs). Current clinical guidelines recommend at least 12 months of DAPT after BRS implantation. However, the correlation between prolonged DAPT and net clinical benefits remains unknown. Methods The SPARTA trial is designed to be a prospective, randomized, parallel-group, clinical trial. It aims to compare the benefits and risks of DAPT applied for either 12 or 36 months after XINSORB BRS implantation. The primary endpoints are the incidence of the composite endpoint of major adverse cardiac events (MACEs), including all-cause death, any myocardial infarction (MI), and all revascularizations, as well as Bleeding Academic Research Consortium Definition (BARC) type 3 or 5 bleeding events. The secondary endpoints of the study include the device-oriented composite endpoint of target lesion failure (defined as cardiac death, target vessel-related MI, or ischemia-driven target lesion revascularization), target vessel failure (defined as cardiac death, MI, or ischemia-driven target vessel revascularization), scaffold thrombosis, and minor bleeding events. This trial will enroll 2106 subjects treated with the XINSORB BRS only. All subjects will receive DAPT after the index procedure for 12 (± 1) months. Subjects without MACEs or major bleeding will be randomized to receive either 24 additional months of DAPT or aspirin alone. Discussion This trial is designed to investigate the impact of extending the duration of DAPT up to 3 years after XINSORB BRS implantation by investigating the balance of risks and benefits in a broad population of treated patients. In this work, we propose an 8-channel arrayed waveguide grating (AWG) designed for synchronized de-multiplexing of both polarization and wavelength. The AWG device is fabricated on a 220 nm-thick top-silicon Silicon-On-Insulator (SOI) wafer using a straightforward single-etch process. It demonstrates the ability for wavelength division de-multiplexing across both TE and TM polarization states, ranging from 1530 to 1542 nm. Measurements reveal that the insertion loss for each channel is less than 3 dB. Additionally, the polarization-dependent loss measures at 1 dB. These experimental results underline the superior performance characteristics of the designed polarization-insensitive AWG. To the best of our knowledge, this is the first reported C-band silicon AWG that accomplishes polarization-wavelength synchronous demultiplexing. This development introduces a new perspective in the design and functionality of AWGs, highlighting the significant potential of this device in the realm of optical communication.

An increased level of reactive oxygen species (ROS) plays a major role in endothelial dysfunction and vascular smooth muscle cell (VSMC) proliferation during in-stent thrombosis and restenosis after coronary artery stenting. Herein, we report an electrospun core-shell nanofiber coloaded with 4-hydroxy-2,2,6,6-tetramethylpiperidine 1-oxyl (TEMPOL) and rapamycin (RAPA) that correspondingly serves as an ROS scavenger and VSMC inhibitor. This system has the potential to improve the biocompatibility of current drug-eluting stent (DES) coatings with the long-term and continuous release of TEMPOL and rapamycin. Moreover, the RAPA/TEMPOL-loaded membrane selectively inhibited the proliferation of VSMCs while sparing endothelial cells (ECs). This membrane demonstrated superior ROS-scavenging, anti-inflammatory and antithrombogenic effects in ECs. In addition, the membrane could maintain the contractile phenotype and mitigate platelet-derived growth factor BB (PDGF-BB)-induced proliferation of VSMCs. In vivo results further revealed that the RAPA/TEMPOL-loaded covered stents promoted rapid restoration of vascular endothelium compared with DES and persistently impeded inflammation and neointimal hyperplasia in porcine models.

During public health crises, the investigation into the modes of public emotional contagion assumes paramount theoretical importance and has significant implications for refining epidemic strategies. Prior research predominantly emphasized the antecedents and aftermath of emotions, especially those of a negative nature. The interplay between positive and negative emotions, as well as their role in the propagation of emotional contagion, remains largely unexplored. In response to this gap, an emotional contagion model was developed, built upon the foundational model and enriched from a complex network standpoint by integrating a degradation rate index. Stability analyses of this model were subsequently conducted. Drawing inspiration from topological structural features, an enhanced model was introduced, anchored in complex network principles. This enhanced model was then experimentally assessed using Watts-Strogatz’s small-world network, Barabási-Albert’s scale-free network, and Sina Weibo network frameworks. Results revealed that the rate of infection predominantly dictates the velocity of emotional contagion. The incitement rate and purification rate determine the overarching direction of emotional contagion, whereas the degradation rate modulates the waning pace of emotions during intermediate and later stages. Furthermore, the immunity rate was observed to influence the proportion of each state at equilibrium. It was discerned that a greater number of initial emotional disseminators, combined with a larger initial contagion node degree, can amplify the emotion contagion rate across the social network, thus augmenting both the peak and overall influence of the contagion.

Introduction Radical surgery including mediastinal lymph node dissection is the standard treatment for early-stage non-small cell lung cancer (NSCLC). About 50% lung nodules are pure ground glass or part-solid nodules, which are predominantly clinical stage IA NSCLC. Non-solid nodules rarely develop mediastinal lymph node metastasis.Method and analysis A phase III study was started in China to evaluate the non-inferiority in overall survival of spared mediastinal lymph node dissection compared with mediastinal lymph node dissection in stage IA NSCLC. A total of 1362 patients will be enrolled from 4 institutions in 2–3 years. The second endpoints are relapse-free survival and perioperative data, including duration of hospitalisation, duration of chest tube placement, operation time, blood loss.Ethics and dissemination This protocol has been reviewed and approved by the Clinical Research Review Board of Tianjin Medical University Cancer Institute and Hospital. The damage of cement slurry to the reservoir is directly related to the production of oil and gas wells, and is a problem that must be solved for the efficient and economic development of oil and gas reservoirs. According to the characteristics and field conditions of reservoir damage caused by cement slurry, this paper analyzes the main factors affecting the degree of reservoir damage, puts forward the technical index requirements of the evaluation device, and establishes the evaluation method of reservoir damage caused by cement slurry in case of loss of injection. The results show that the influence of temperature on the damage of the cement slurry layer is small, and the influence of pressure difference on the damage to the cement slurry layer is large; When the pressure difference is 22 Mpa, the penetration damage rate caused by cement slurry to the pore center without mud cake attachment is about 43%. After a day of core maintenance, the permeability damage rate will reach 61.2%; The damage of cement slurry to the pore micro-fractured core is very serious. After 1 day of maintenance, the damage to the core permeability after cement slurry solidification is as high as 83%, with an average of 76.5%; When the risk of leakage is low, the cement slurry system with low density and low water loss can be used for cementing operation. The filtrate of cement slurry should be better matched with the formation clay minerals, formation water, and drilling fluid filtrate. When the risk of leakage is high, high-density cement slurry, balanced pressure cementing, pressure control cementing, and other technologies to avoid leakage can be used for cementing operations. When the cementing conditions are more difficult, the cementing slurry system can be used to dissolve the cementing slurry system at a later stage. The research results provide theoretical data support for the study of reducing the damage of cement slurry to the reservoir.

The effect of Zn/Ca ratio on the corrosion behavior of Mg–3Zn-0.2Ca-1.0MgO (3ZX) and Mg–1Zn-0.2Ca-1.0MgO (ZX) was investigated on the as-extruded specimens. Microstructure observations revealed that the low Zn/Ca ratio led to the grain growth from 1.6 µm in 3ZX to 8.1 µm in ZX. At the same time, the low Zn/Ca ratio changed the nature of second phase from the existence of Mg-Zn and Ca2Mg6Zn3 phases in 3ZX to the dominated Ca2Mg6Zn3 phase in ZX. The local galvanic corrosion caused by the excessive potential difference was alleviated obviously due to the missing of MgZn phase in ZX. Besides, the in vivo experiment also showed that ZX composite exhibited a good corrosion performance and the bone tissue around the implant grew well.

Objectives: In the face of the complex and changing network security environment, how to fight against Advanced Persistent Threat (APT) attacks has become an urgent problem for the entire security community. The massive APT attack analysis reports and threat intelligence generated by security companies have significant research value. They can effectively provide the information of APT organizations, thereby assisting in the traceability analysis of network attack events. Aiming at the problem that APT analysis reports have not been fully utilized, and there is a lack of automation methods to generate structured knowledge and construct feature portraits of the hacker organizations, an automatic knowledge extraction method of APT attacks combining entity recognition and entity alignment is proposed. The proposed method can automatically extract entities from APT analysis reports and construct structured knowledge of the APT organization. Methods: An automatic extraction method of APT attack knowledge that integrates entity recognition and entity alignment is designed. Firstly, 12 entity categories are designed according to the characteristics of APT attacks. Then, lowercase conversion, data cleaning, and data annotation are performed on the corpus through the preprocessing layer, and the preprocessed APT text sequence is represented as a vector. Secondly, the Bert model is built to pre-train the annotated corpus, encode each word, and generate the corresponding word vector. Also, the BiLSTM model is constructed to capture long-distance and contextual semantic features. The attention mechanism is built to highlight key features and convert the vector sequence into an annotation probability matrix. Thirdly, the CRF algorithm is utilized to decode the relationship between the output predicted labels and generate the optimal label sequence. Finally, the entity alignment method based on semantic similarity and Birch is constructed, which can improve the quality of the extracted APT attack knowledge through knowledge matching and merging into the infobox of each APT organization. Results: In terms of entity recognition, the proposed APT attack entity recognition method is superior to the existing entity recognition methods (i.e., CRF, LSTM-CRF, GRU-CRF, BiLSTMCRF, CNN-CRF, and Bert-CRF). The experimental results of our method have been improved to a certain extent, whose precision, recall, and F1-score are 0.929 6, 0.873 3, and 0.900 6. Compared with CRF, the F1-score of the proposed model is increased by 14.32%. Compared with CNN-CRF, which integrates convolutional neural networks, the F1-score of the proposed model is increased by 6.92%. Compared with LSTM-CRF and BiLSTM-CRF, the F1-score of the proposed model is increased by 8.43% and 5.30%, respectively. Compared with GRU-CRF, the F1-score of this model is increased by 8.74%. Compared with Bert-CRF, the F1-score of this model is increased by 7.03%. In addition, the accuracy of the proposed model is 0.9004, which is 9.85% higher than the average of the other six models. Also, the proposed model's training process is more stable, and the entire curve converges faster, which can achieve higher accuracy with fewer training batches. The model's error converges faster in the training period, and the curve is smoother. Moreover, the proposed model has the best prediction effect on the "attack method" entity category, whose F1-score is 0.927 5. On the one hand, a large number of entities exist in this category. On the other hand, this category of entities widely exists in semantic-rich APT attack events and has the action characteristics of attack behavior, which leads to a better recognition effect of this category. In terms of entity recognition with small sample annotation, the proposed method's precision, recall, and F1-score are 0.780 0, 0.589 4, and 0.671 4, respectively. Compared with the CRF model, LSTM-CRF model,GRU-CRF model, BiLSTM-CRF model, CNN-CRF model, and Bert-CRF model, the F1-score values of the proposed model are improved by 27.42%, 18.78%, 23.62%, 13.25%, 14.88%, and 14.46%. This experiment fully demonstrates that the proposed method can perform pre-training on a small sample corpus through the Bert model, thereby improving the effect of entity recognition. In terms of entity alignment and knowledge fusion, the experiment automatically extracts named entities with the high frequency of various entity categories, which often exist in APT attack events. For example, common APT organizations include "APT29", "APT32", "APT28", and "Turla";common attack equipment includes "PowerShell", "Cobalt Strike", and "Mimikatz"; common attack methods include "Spearphishing", "C2", "Watering Hole Attack", and "Backdoor"; common vulnerabilities include "CVE-2017-11882", "CVE-2017-0199", and "CVE-2012-0158", etc. The proposed method combines the corpus titles and keywords to carry out entity fusion of APT organization names. Finally, the infobox of common APT organizations in this dataset is constructed, and the structured knowledge of each APT organization is formed. Also, the attack domain knowledge of APT28 and APT32 is shown in detail. Conclusions: According to the characteristics of APT attacks, an automatic extraction method of APT attack knowledge based on entity recognition and entity alignment is designed and implemented. This method can effectively identify APT attack entities, automatically extract advanced persistent threat knowledge under the condition of few-sample annotation, and generate structured feature portraits of common APT organizations, which will provide support for subsequent APT attack knowledge graph construction and attack traceability analysis.

Analyzing the factors influencing traffic congestion is essential for urban planning and coordinated development. Previous research frequently focuses on the internal aspects of traffic systems, often overlooking the impact of external factors on congestion sources. Therefore, this study utilizes a geospatial dataset and mobile signaling data, firstly applying the Fuzzy C-Means (FCM) algorithm to identify congested roads of different levels and trace the localization of travelers’ origins on regional congested roads. Furthermore, it employs the LightGBM method to study the influence of the built environment of various congestion sources on network-level congestion. The findings are as follows: (1) There is a positive correlation between traffic congestion and geographical location, with congestion predominantly caused by a few specific plots and demonstrating a concentrated trend in city centers. (2) Residential population density is the most critical factor, accounting for over 12% of the congestion contribution, followed by road density and working population density. (3) Both residential and working population densities show a non-linear positive correlation with congestion contribution, while the mixture of land use displays a non-linear V-shaped influence. Additionally, when residential population density is between 8000 and 11,000, it notably exacerbates congestion contribution. Significantly, by emphasizing land use considerations in traffic system analysis, these findings illuminate the intricate linkages between urban planning and traffic congestion, advocating for a more comprehensive approach to urban development strategies.

Double shell capsule can provide a potential low-convergence to fusion ignition at relatively low temperature (∼3 keV). One of the main sources of degrading double shell implosion performance is the low-mode asymmetries. Recently, the experiments on the evolution of low-mode asymmetries introduced by x-ray P2 drive asymmetry during double shell implosions were carried out on the SG facility, where the outer shell and inner shell shapes were measured through the backlit radiography, and the fuel shape near stagnation was measured by core x-ray self-emission imaging. The time-dependent x-ray flux symmetry was controlled by varying the inner cone fraction, defined as the ratio of the inner cone power to the total laser power, while keeping the drive temperature histories same across experiments. Both the hohlraum radiation and the capsule implosions were analyzed using a two-dimensional radiation-hydrodynamics code. Comparing the experimental radiographs and self-emission images to the simulations, it is found that the simulated outer shell, inner shell and hot spot shapes are in qualitative agreement with experiments, especially, the symmetry swings of the hot spot shape near stagnation are observed from both experimental and simulation results. Further, the effect of x-ray drive asymmetries on double shell implosion performance is preliminarily investigated using numerical simulations. We find that the azimuthal variations in radial velocity caused by drive asymmetries can generate azimuthal mass flow of the inner shell, thus kinetic energy of the inner shell would be not converted into fuel internal energy with high efficiency, and the mass-averaged ion temperature of the fuel at stagnation would be reduced.

BackgroundPhthalates (PAEs) are important synthetic substances in plastics, attracting much attention due to their potential effects on the cardiovascular system.MethodsIn this study, urine and blood samples from 39 individuals were collected in Tianjin, China. Phthalates and phthalate metabolites (mPAEs) were analyzed using gas chromatography-mass spectrometry (GC-MS) and high-performance liquid chromatography-mass spectrometry (HPLC-MS), respectively. The polymerase chain reaction (PCR) products from bisulfite-treated mitochondrial DNA (mtDNA) samples were analyzed using pyrosequencing technology.ResultsThe detection frequencies for 9 PAEs varied from 2.56 to 92.31%, and those for 10 mPAEs varied from 30.77 to 100%. The estimated daily intakes (EDIs) and cumulative risk of PAEs were calculated based on the experimental statistics of urinary PAEs and mPAEs. For PAEs, the HIRfD (hazard index corresponding to reference doses) values of 10.26% of participants and the HITDI (hazard index corresponding to tolerable daily intake) values of 30.77% of participants were estimated to exceed 1, suggesting a relatively high exposure risk. The mtDNA methylation levels in the MT-ATP8 and MT-ND5 were observed to be lower than in the MT-ATP6. Mono-ethyl phthalate (MEP) and MT-ATP8 were positively correlated with triglyceride levels (p < 0.05). Based on the association of PAEs, mtDNA methylation, and triglycerides, the mediating role of mtDNA methylation between PAEs and cardiovascular diseases (CVDs) was analyzed in this study, but no mediated effect was observed.ConclusionThe effects of PAE exposure on cardiovascular diseases (CVDs) should be investigated further.

BackgroundObesity is often accompanied by lower 25(OH)D levels, whereas these two parameters exhibit opposite effects on bone health. It is uncertain what are the effects of lower 25(OH)D levels in obesity on bone health in elderly Chinese people.MethodsA nationally representative cross-sectional analysis of China Community-based Cohort of Osteoporosis (CCCO) was performed from 2016 to 2021, which consisted of 22,081 participants. Demographic data, disease history, Body mass index (BMI), bone mineral density (BMD), the levels of the biomarkers of vitamin D status and those of bone metabolism markers were measured for all participants (N = 22,081). The genes (rs12785878, rs10741657, rs4588, rs7041, rs2282679 and rs6013897) related to 25(OH)D transportation and metabolism were performed in a selected subgroup (N = 6008).ResultsObese subjects exhibited lower 25(OH)D levels (p < 0.05) and higher BMD (p < 0.001) compared with those of normal subjects following adjustment. The genotypes and allele frequency of rs12785878, rs10741657, rs6013897, rs2282679, rs4588 and rs7041 indicated no significant differences among three BMI groups following correction by the Bonferroni’s method (p > 0.05). The levels of total 25(OH)D (ToVD) were significantly different among the GC1F, GC1S and GC2 haplotype groups (p < 0.05). Correlation analysis indicated that ToVD levels were significantly correlated with parathyroid hormone levels, BMD, risk of osteoporosis (OP) and the concentration levels of other bone metabolism markers (p < 0.05). Generalized varying coefficient models demonstrated that the increasing BMI, ToVD levels and their interactions were positively associated with BMD outcomes (p < 0.001), whereas the reduced levels of ToVD and BMI increased the risk of OP, which was noted notably for the subjects with reduced ToVD levels (less than 20.69 ng/ml) combined with decreased BMI (less than 24.05 kg/m2).ConclusionThere was a non-linear interaction of BMI and 25(OH)D. And higher BMI accompanied by decreased 25(OH)D levels is associated with increased BMD and decreased incidence of OP, optimal ranges exist for BMI and 25(OH)D levels. The cutoff value of BMI at approximately 24.05 kg/m2 combined with an approximate value of 25(OH)D at 20.69 ng/ml are beneficial for Chinese elderly subjects.

Aims: Altered lipid, energy metabolism and sleep disorders had been linked with coronary heart disease (CHD), however, the metabolic signatures and sleep rhythm in non-obstructive coronary atherosclerosis-CHD remain unclear. This pilot study aims to investigate the lipidome and central carbon metabolites profiles and associated sleep characteristics among CHD patients without traditional risk factors. Methods: From January to July 2021, 15 CHD patients and 15 healthy controls were randomly selected from the cardiology unit of Zhongshan Hospital, Shanghai. A total of 464 lipids and 45 central carbon metabolites (CCM) were quantified in blood plasma. Metabolic signatures were selected through orthogonal projections to latent structures discriminant analysis (OPLS-DA) and principal component analysis (PCA) was conducted to link the profiles of identified metabolites with CHD risk, sleep patterns, cardiometabolic traits and cardiac electrophysiologic parameters. Results: Using OPLS-DA, we identified 40 metabolites (variable influence on projection >1) that were altered in CHD patients, with 38 lipids, including 25 triacylglycerols (TAGs), 8 diacylglycerols (DAGs), being elevated and two CCM metabolites (i.e., succinic acid and glycolic acid) being reduced. Using PCA, four principal components (PCs) were identified and associated with increased risk of CHD. Specifically, one standard unit increasement in the PC that was characterized by high levels of DAG (18:1) and low succinic acid and the PC that was characterized by high levels of two sphingomyelins [SM (26:0) and SM (24:0)] was associated with 21% [odds ratio (OR) = 1.21, 95% CI: 1.02,1.43] and 14% (OR = 1.14,1.02,1.29) increased risk of CHD, respectively. Further regression analyses confirmed that the identified metabolites and the four PCs were positively associated with TG and ALT. Interestingly, glycolic acid was negatively associated with sleep quality and PSQI. Participants with night sleep mode tended to have a high level of the identified lipids, especially FFA (20:4). Conclusion: In the present pilot study, our findings provide clues on alterations of lipid and energy metabolism in CHD patients without traditional risk factors, with multiple triacylglycerols and diacylglycerols metabolites seemingly elevated and certain nonlipids metabolites (e.g., succinic acid and glycolic acid) decreased in cases. Considering the limit sample size, further studies are warranted to confirm our results.

PurposeTo propose and evaluate a comprehensive modeling approach combing radiomics, dosiomics and clinical components, for more accurate prediction of locoregional recurrence risk after radiotherapy for patients with locoregionally advanced HPSCC.Materials and methodsClinical data of 77 HPSCC patients were retrospectively investigated, whose median follow-up duration was 23.27 (4.83-81.40) months. From the planning CT and dose distribution, 1321 radiomics and dosiomics features were extracted respectively from planning gross tumor volume (PGTV) region each patient. After stability test, feature dimension was further reduced by Principal Component Analysis (PCA), yielding Radiomic and Dosiomic Principal Components (RPCs and DPCs) respectively. Multiple Cox regression models were constructed using various combinations of RPC, DPC and clinical variables as the predictors. As a new type of cantilever-type supporting structure, the double-row pile supporting combination system can improve the overall rigidity to maintain the safety and stability of the side of the foundation pit. The case of a double-row pile foundation pit support project in the Wanquan District of Zhangjiakou is taken as an example. Based on the existing double-row pile crown beam stiffness coefficient calculation method, the effect coefficient of the crown beam and the coupling beam is introduced to optimize and improve the effect of the coupling beam and the crown beam. The results show that (1) in the calculation of the double-row pile structure, the common lateral restraint effect of the crown beam and the connecting beam on the double-row supporting piles should be considered, and the rigid connection of the crown beam and the connecting beam should be taken as a whole to improve the rigidity coefficient of the double beam lateral support of a rectangular double-row pile. (2) The combined stiffness of the double-beam combined support system has a greater impact on the displacement of the pile top, and the displacement under a combined stiffness of 40～50 MN/m are close to the observed value. The calculated length of the crown beam and the introduced crown beam and connecting beam effect coefficient have a great influence on the combined stiffness of the double-beam composite support system. The combined stiffness decreases with the increase of the calculated length, and increases with the decrease of the effect coefficient. (3) The lateral support stiffness of the double-row piles under the double-beam composite support system is affected by the difference between the vertical and lateral displacements of the front and rear piles. The maximum lateral displacement of the front and rear piles is affected by the friction angle of the soil, the cohesion and the proportional coefficient of the horizontal resistance of the soil. Changing the tensile strength will not affect the displacement and deformation of the double-row piles. There is a displacement inflection point below the buried depth of the foundation pit and within the scope of the pile bottom. The displacement at the inflection point is equal under different friction angles and different cohesive forces.

Quantitative assessment of hand function can assist therapists in providing appropriate rehabilitation strategies, which plays an essential role in post-stroke rehabilitation. Conventionally, the assessment process relies heavily on clinical experience and lacks quantitative analysis. To quantitatively assess the hand motor function of patients with post-stroke hemiplegia, this study proposes a novel multi-modality fusion assessment framework. This framework includes three components: the kinematic feature extraction based on a graph convolutional network (HAGCN), the surface electromyography (sEMG) signal processing based on a multi-layer long short term memory (LSTM) network, and the quantitative assessment based on the multi-modality fusion. To the best of the authors’ knowledge, this is the first study of applying a graph convolution network to assess the hand motor function. We also collect the kinematic data and sEMG data from 70 subjects who completed 28 types of hand movements. Therapists first graded patients using traditional motor assessment scales (Brunnstrom Scale and Fugl-Meyer Assessment Scale) and further refined the patient’s motor assessment result by their experience. Then, we trained the HAGCN and LSTM networks and quantitatively assessed each patient based on the proposed assessment framework. Finally, the Spearman correlation coefficient (SC) between the assessment result of this study and the traditional scale are 0.908 and 0.967, demonstrating a significant correlation between the proposed assessment and the traditional scale scores. In addition, the SC value between the score of this study and the refined hand motor function is 0.997, indicating the “ceiling effect” of some traditional scales can be avoided.

Diffuse malignant peritoneal mesothelioma (DMPM) is an unusual and life-threatening locally invasive tumor. The morbidity and mortality of the disease are associated with progressive local effects in the abdominal cavity, such as abdominal distention, painful sensations, and early saturation with reduced oral intake, which eventually lead to intestinal obstruction and cachexia. Computed tomography (CT) has been widely used as a first-line diagnostic tool for DMPM. In addition, the most sensitive immunohistochemical markers of DMPM include WT 1, D2-40, and calmodulin. This paradigm has altered with the advancements in the immunohistochemical analysis of BRCA1-Associated Protein 1 (BAP1) the lack of BAP1 expression shows the diagnosis of malignancy. DMPM is resistant to conventional chemotherapies. Therefore, the gold standard for the treatment of DMPM is the combination of cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC). The overexpression of the phosphatidylinositol 3-kinase (PI3K)/AKT serine/threonine kinase 1 (AKT)/mammalian target of rapamycin (mTOR) signaling pathway drives the malignant phenotype of DMPM, thereby showing promising potential for the treatment of DMPM. The coordinated activities among multiple RTKs are directly involved in the biological processes of DMPM, suggesting that the combined inhibition of the PI3K and mTOR signaling pathways might be an effective measure. This treatment strategy can be easily implemented in clinical practice. However, the combined inhibition of ERBB1(HER1)/ERBB2 (HER2) and ERBB3 (HER3) requires further investigations. Thus, based on these, the discovery of novel targeted therapies might be crucial to improving the prognosis of DMPM patients.

During the COVID-19 pandemic, many buildings in northeast China have had clusters of infected cases in the vertical layout. There is speculation that vertical aerosol transmission occurs. The houses in northeast China are airtight, and range hoods may be used for a long period of time when cooking. The pathway and factors influencing vertical aerosol transmission are worth studying. To elucidate a viral aerosol transmission pathway, we selected a multistory apartment and a high-rise building in Changchun city, Jilin province, China, to conduct an in-depth investigation and on-site simulation experiments. According to epidemiological investigation information on infected cases, building structures, drainage, ventilation, etc., we used fluorescent microspheres to simulate the behaviors of infected people, such as breathing and flushing the toilet after defecation, to discharge simulated viruses and track and monitor them. The field simulation experiment confirmed the transmission of fluorescent microsphere aerosols to other rooms in two types of buildings using a vertical aerosol transmission pathway of toilet flush-sewage pipe-floor drain without a water seal. Our study showed that, in the absence of a U-shaped trap or floor drain water seal whether in a multistory apartment or high-rise residential building, there is a transmission pathway of “excretion of virus through feces-toilet flushing-sewage pipe-floor drain without water seal,” which will cause the vertical transmission of viral aerosol across floors during the COVID-19 pandemic. Moreover, the negative pressure generated by turning on the range hood when closing doors and windows increase aerosol transmission. Based on this negative pressure, prevention and control measures for residential buildings in northeast China during the COVID-19 pandemic were proposed.

Abstract Background Programmed cell death ligand 1 (PD‐L1) is widely known as an immune checkpoint molecule in tumor cells. Osteopontin (OPN) is expressed by both tumor cells and tumor‐associated macrophages (TAMs), and both autocrine and paracrine of OPN are considered to be involved in tumor metastasis, proliferation and immunosuppression. However, little is known about the relationship between OPN expressed in TAMs (TOPN) and PD‐L1 in non‐small cell lung cancer (NSCLC). Methods Tissue microarray was used to detect the expression of TOPN, TAMs and PD‐L1 by multiple quantitative fluorescence staining in 509 NSCLC patients undergoing complete pulmonary resection. The correlations between TOPN, PD‐L1 and clinicopathological data were analyzed. An in vitro coculture system was established to investigate the crosstalk between TOPN and neoplastic PD‐L1. In vivo, the intrinsic features of PD‐L1 in NSCLC xenografts were evaluated after being coinjected with OPN‐positive TAMs, and a series of key cytokines and chemokines were detected in the tumor microenvironment. Results A positive association between the TOPN and PD‐L1 expression in tumor tissues from 509 patients with NSCLC was verified. In addition, TOPN and PD‐L1 were independent prognostic factors for overall survival (OS) and disease‐free survival (DFS) of NSCLC patients. Moreover, TOPN upregulated PD‐L1 expression in NSCLC cells through the nuclear factor‐κB (NF‐κB) pathway in vitro TOPN induced the PD‐L1 expression promoted the tumor growth in tumor‐bearing mice, altering immune‐related cytokines and chemokines. Conclusions TOPN regulates PD‐L1 expression through the NF‐κB pathway in NSCLS, which is a potential independent biomarker and target for prognosis as well as immunotherapy.

Tertiary lymphoid structures (TLS) existence is correlated with favorable prognosis in many types of cancer including non-small cell lung cancer (NSCLC). However, TLS formation and its relationship with treatment response remains unknown in NSCLC who received anti-PD-1 antibody plus chemotherapy as the neoadjuvant treatment (neoadjuvant chemoimmunotherapy). Here, we investigate TLS maturation and abundance in resectable NSCLC receiving neoadjuvant treatments. We retrospectively collected formalin-fixed paraffin embedded (FFPE) tissues from patients with resectable NSCLC (stage II–IIIA) from three cohorts based on treatment: naïve (N=40), neoadjuvant chemoimmunotherapy (N=40), and neoadjuvant chemotherapy (N=41). The TLS in tumor tissues was detected by immunohistochemical staining, and the differences in TLS maturation and abundance among different treatment groups were analyzed, as well as the relationship with pathological response and prognosis of patients. Multiplex immunofluorescence staining was used to explore the features of immune microenvironment. Higher major pathological response (MPR) rate and pathological complete response (pCR) rate were in the neoadjuvant chemoimmunotherapy group than in the neoadjuvant chemotherapy group (MPR: 45.0% vs 17.1%; pCR: 35.0% vs 4.9%). Among the three cohorts, neoadjuvant chemoimmunotherapy-treated NSCLCs displayed highest TLS maturation and abundance. Both the maturation and abundance of TLS were significantly correlated with MPR in both the neoadjuvant chemoimmunotherapy and the chemotherapy group. Patients with high maturation and abundance of TLS exhibited better disease-free survival (DFS) in all the three cohorts. TLS maturation was also an independent predictor for DFS in the neoadjuvant chemoimmunotherapy and treatment naïve group. Multiplex immunohistochemistry analysis using paired biopsy-surgery samples showed increased infiltration of CD8+T cell and decreased infiltration of M1 and M2 macrophages after neoadjuvant chemoimmunotherapy treatment in patients achieving MPR. There were no significant differences in features of immune cell infiltration for those with mature TLS achieving MPR when cross-compared across the three cohorts. These results demonstrate that TLS maturation is associated with MPR and an independent predictor for DFS in resectable neoadjuvant chemoimmunotherapy-treated NSCLC. The induction of TLS maturation may be a potential mechanism of action of neoadjuvant chemoimmunotherapy in resectable NSCLC.

BackgroundEarly revascularization of the culprit vessel is the most effective treatment for reducing the risk of mortality from acute STEMI with and without cardiogenic shock. However, the most recent trends and impact of multivessel percutaneous coronary intervention (PCI) during the index hospitalization on in-hospital outcomes are unknown.MethodsThe National Inpatient Sample was queried from October 2015 to 2019 for hospitalizations with STEMI. The impact of multivessel PCI on in-hospital outcomes of patients with and without cardiogenic shock was evaluated.ResultsOf 624,605 STEMI hospitalizations treated with PCI, 12.5% were complicated by cardiogenic shock. Among hospitalizations without cardiogenic shock, 15.7% were treated by multivessel PCI, which declined from 20.8% in 2015 to 13.9% in 2019 (Ptrend < 0.001). Multivessel and culprit-only PCI had similar rates of In-hospital mortality (2.4 vs. 2.3%, p = 0.027) and major adverse cardiac and cerebrovascular events (MACCE; 7.4 vs. 7.2%, p = 0.072). Among hospitalizations with cardiogenic shock, 22.1% were treated by multivessel PCI, which declined from 29.2% in 2015 to 19.4% in 2019 (Ptrend < 0.001). Multivessel PCI was associated with higher rates of in-hospital mortality (30.9 vs. 28.4%, p < 0.001) and MACCE (39.9 vs. 36.5%, p < 0.001) than culprit-only PCI.ConclusionThe frequency of multivessel PCI for STEMI with and without cardiogenic shock is declining. Multivessel PCI is associated with worse in-hospital outcomes for STEMI with cardiogenic shock but not for STEMI without cardiogenic shock.

IntroductionBiobased polymer blends have been recommended as an eco-friendly solution to abate plastic pollution in the environment. However, the formation of microplastics (MPs) by photodegradation of biobased polymer blends in the marine environment is still not well understood. In this study, we investigated the formation of MPs and the changes in the physicochemical properties of three types of biobased polymer blends after photodegradation in seawater.MethodsThe investigated materials included non-biodegradable polyethylene/ thermoplastic starch blends (PE/TPS) and polypropylene/thermoplastic starch blends (PP/TPS), as well as biodegradable polylactic acid/poly (butylene adipate-co-terephthalate)/thermoplastic starch blends (PLA/PBAT/TPS). The control groups were the corresponding neat polymers, including polyethylene (PE), polypropylene (PP), and polylactic acid (PLA).ResultsThe size distribution of the pristine and aged MPs indicated that the polymer blends were more likely to produce small-sized particles after photodegradation due to their poorer mechanical properties and lower resistance to UV irradiation than the neat polymers. Noticeable surface morphology alterations, including cracks, holes, and pits, were observed for polymer blends after photodegradation, while neat polymers were relatively resistant. After photodegradation, the attenuated total reflection Fourier transformed infrared spectroscopy (ATR-FTIR) spectrum of the polymer blends showed a significant decrease in the characteristic bands of thermoplastic starch (TPS), indicating depletion of their starch fractions. The C1s spectra of the polymer blends demonstrated that the aged MPs contained fewer -OH groups than the pristine MPs, further confirming the photodegradation of TPS. The molecular weight distribution curve of the polymer blends shifted significantly towards low molecular weight, suggesting the occurrence of chain scission during photodegradation. These results indicate that the polymer blends have a higher degree of photodegradation than neat polymers, and thereby generate more small-sized MPs than neat polymers. Photodegradation caused changes in the contact angle and surface charge of MPs derived from biobased polymer blends, which may affect the vector effects of MPs on any coexisting pollutants.DiscussionIn summary, polymer blends may pose a higher risk to the marine environment than neat polymers, and caution should be taken in promoting biobased polymer blends.

Ring-and-hub network structure is very common in the real world, while that neighboring rings may sometimes share nodes or line segments makes this structure irregular. In this paper, we modify Dorogovtsev-Mendes model and its subsequent models to analytically estimate the average distance between nodes on the same ring in irregular ring-and-hub networks. In order to observe the accuracy of our modified model, we develop an algorithm to generate irregular ring-and-hub networks by computer. Then, we compare the analytic estimates with the practical values on those computer-generated and real networks. The results show that our modified model actually estimates the average shortest distance of its rings when only straight and U-shape paths between the start and end points are allowed. The accuracy of estimates for innermost several rings can be acceptable.

Abstract Background The jailed balloon technique is widely used for coronary bifurcation lesions, but a residual risk of SB occlusion remains, necessitating SB rewiring and further interventions, including balloon inflation or stenting, which may result in failure and SB loss. This study introduced a novel modified technique of small side branch (SB) protection, namely, double kissing inflation outside the stent (DKo) technique, for coronary bifurcations without the need for SB rewiring. Methods We performed the DKo technique in consecutive patients in our center from 1/2019 to 12/2019. The procedure was as follows. We inserted a guide wire into both branches followed by proper preparation. The SB balloon was simultaneously inflated with main vessel (MV) stenting. The SB balloon remained in situ until it was kissing inflated with postdilation of the bifurcation core, which is different from traditional strategies. The proximal optimization technique was performed with a short noncompliant balloon strictly not exceeding the bifurcation. Rates of SB loss and in-hospital outcomes were evaluated. Results The technique was successfully performed in all 117 enrolled patients without any rewiring or SB loss. The mean lesion lengths of the MV and SB were 38.3 ± 19.9 mm and 11.7 ± 7.1 mm, respectively. On average, 1.5 ± 0.6 stents were used per patient, while the mean pressure of the SB balloon was 7.4 ± 3.1 atm. Coastal environments, ecologically fragile zones, are subjected to great human pressures, particularly, xenobiotic pollutants such as microplastics (MPs) and trace metals. Yet, the impact of anthropogenic intervention on the spatial patterns of MPs in different coastal environmental compartments of Qingdao, a city located in the west Yellow Sea, is still unclear. Therefore, the spatial distribution, characteristics, and diversity of MPs (≥ 50 µm) in seawater, sediment, and sand samples collected from 10 zones intervened by different anthropogenic activities in Qingdao coastal environment were investigated. The abundance of MPs was 93.1 ± 63.5 items/m3 in seawater, which was 4577 ± 2902 items/kg in sediments and 3602 ± 1708 items/kg in the beach sands. A spatial analysis indicated that the distribution characteristics of MPs, including abundance, color, and type, greatly varied among the zones with different extent of human activities. The highest abundance of MPs in the seawater was detected in the abandoned aquafarm, followed by harbors, beaches, estuary, sewage discharge areas, operational aquafarm, and rural areas, whereas the highest MP abundance in the sediments followed the order of harbor, sewage discharge, estuary, abandoned aquafarm, beaches, rural area, and operational aquafarm. The highest MP abundance in the scenic and recreational beach sands was, respectively, in the intertidal and supratidal zone. The transparent chlorinated polyethylene fragments with the relatively small size of 50–100 μm were the dominant MPs in the coastal environment. The higher physicochemical characteristic diversity in terms of size, color, shape, and type of MPs in the aquafarms, harbors, and recreational beach than those of the other zones, illustrated the higher complexity and diversity of MP pollution sources in these zones. These results jointly indicated that aquaculture, navigation, and tourism mainly determined MP spatial distribution patterns in the coastal environment of Qingdao. These results also extend the understanding of the inventory and fate of MPs in coastal environment, thus providing important data to establish effective strategies for abating MP pollution in marine ecosystems.

BackgroundCurrent guidewires for transradial coronary angiography had defects of passage difficulty or branch injury. This study sought to investigate the safety and efficiency of a novel method of active knuckle-angle 0.035-inch hydrophilic guidewire in transradial coronary angiography.MethodsPatients undergoing a transradial coronary procedure in our team from August 2015 to June 2020 were retrospectively investigated. We compared the demographic and interventional characteristics of 1,457 patients receiving advancement of unmodified guidewires (Traditional group) and 1,322 patients receiving advancement of the knuckle guidewire (Knuckle group). Afterwards we included 239 patients and randomized them according to a random number table to either the unmodified or the knuckle guidewire to further confirm the efficiency and safety of knuckle guidewire advancement.ResultsIn the retrospective analysis, unwilling passage of guidewire into branches occurred more in the Traditional group than in the Knuckle group (9.5 vs. 0.08%, p < 0.001). Two patients in the Traditional group experienced guidewire-associated perforation. One patient was treated with covered stent for internal mammarian artery perforation, while the other was managed with compression for brachial branch perforation. In the randomized controlled study, unwilling passage of guidewire also occurred more in the Traditional group (10.8 vs. 1%, p < 0.001). Median duration of guidewire advancement from the sheath to aortic root significantly decreased from 33 seconds in the Traditional group to 21 seconds in the Knuckle group.ConclusionActive knuckle angle guidewire represented a novel method to prevent unwilling passage and associated perforation with efficiency improvement and a reduction in radiation exposure.

This study focuses on the components and levels of polycyclic aromatic hydrocarbons (PAHs) and their derivatives (MPAHs and OPAHs) in plasma samples from 19 oil workers, pre- and post-workshift, and their exposure–response relationship with mitochondrial DNA (mtDNA) methylation. PAH, MPAH, OPAH, and platelet mtDNA methylation levels were determined using a gas chromatograph mass spectrometer (GC-MS) and a pyrosequencing protocol, respectively. The total plasma concentrations of PAHs in mean value were, respectively, 31.4 ng/mL and 48.6 ng/mL in pre- and post-workshift, and Phe was the most abundant (13.3 ng/mL in pre-workshift and 22.1 ng/mL in post-workshift, mean value). The mean values of total concentrations of MPAHs and OPAHs in the pre-workshift were 2.7 ng/mL and 7.2 ng/mL, while in the post-workshift, they were 4.5 ng/mL and 8.7 ng/mL, respectively. The differences in the mean MT-COX1, MT-COX2, and MT-COX3 methylation levels between pre- and post-workshift were 2.36%, 5.34%, and 0.56%. Significant (p < 0.05) exposure–response relationships were found between PAHs and mtDNA methylation in the plasma of workers; exposure to Anthracene (Ant) could induce the up-regulation of the methylation of MT-COX1 (β = 0.831, SD = 0.105, p < 0.05), and exposure to Fluorene (Flo) and Phenanthrene (Phe) could induce the up-regulation of methylation of MT-COX3 (β = 0.115, SD = 0.042, p < 0.05 and β = 0.036, SD = 0.015, p < 0.05, respectively). The results indicated that exposure to PAHs was an independent factor influencing mtDNA methylation.

A low-clearance tunnel portal in the shallow-buried, joint-developed, broken, and loose surrounding rock slope deposit may cause safety issues during construction. In this study, the Guanyin Mountain Tunnel of the Chong-Ai expressway was taken as a case study, and the characteristics of the loose and broken surrounding rocks, their low clearance, and shallow buried bias were comprehensively studied. The three-dimensional numerical model of the Guanyin Mountain tunnel portal section was constructed by the Rhino, AutoCAD, and FLAC 3D software, and the whole construction process of the tunnel portal was simulated. Under the conditions of loose and broken surrounding rocks, the surrounding rock deformation, surface settlement, and slope stability at the portal of the shallow buried tunnel with a small clear distance during the construction of the center diaphragm (CD) method and circular reserved core soil method were studied. The following conclusions are drawn. During the simulated excavation of the tunnel, the maximum surface settlement is 10.74 mm, which meets the requirement of the specification. When the left tunnel is excavated, the surrounding rock deformation of the right arch shoulder should be carefully considered. The maximum deformation value can reach 14.314 mm. After excavation, the deformation rate of the right tunnel is large, and initial support should be installed in time. Since the stratum rock at the portal of the tunnel is strongly weathered, the uplift value of the arch bottom is large and gradually decreases along the axial direction. The tunnel arch bottom and arch foot are plastic areas prone to tensile damage. Therefore, it is imperative to strengthen the inverted arch support of the tunnel in the strongly weathered rock stratum. The excavation sequence of the tunnel portal section adopts the method of excavating the left tunnel first and then excavating the right tunnel, which is more conducive to ensuring the slope stability.

There has been extensive research on antibiotics exposure in adults by biomonitoring, but the biological mechanisms and potential risks to human health remain limited. In this study, 102 adults aged 26–44 years in Tianjin were studied and 23 common antibiotics in urine were analyzed by Liquid chromatography-mass spectrometry (LC-MS). All antibiotics were detected in urine, with an overall detection frequency of 40.4% (the detection frequencies of phenothiazines, quinolones, sulfonamides, tetracyclines, and chloramphenicol were 77%, 54%, 24%, 28%, and 49%, respectively.). Ofloxacin and enrofloxacin had the highest detection frequencies (85% and 81%), with median concentrations of 0.26 (IQR: 0.05–1.36) and 0.09 (IQR: 0.03–0.14) ng/mL, respectively. Based on health risk assessment, the predicted estimated daily exposures (EDEs) ranged from 0 μg/kg/day to 13.98 μg/kg/day. The hazard quotient (HQ) values of all the antibiotics except ofloxacin and ciprofloxacin were bellow one, which are considered safe. For all blood samples, the mitochondrial DNA (mtDNA) methylation levels in the MT-ATP6 (ranging between 3.86% and 34.18%) were slightly higher than MT-ATP8 and MT-ND5 (ranging between 0.57% and 9.32%, 1.08% and 19.62%, respectively). Aim to investigate the effect of miR-206 on the growth and metastasis of breast cancer stem cells and clarify the precise mechanism of miR-206 on EVI-1-mediated CALR expression in driving malignant phenotype. Our results showed that miR-206 mimics suppressed CALR expression, inhibited the proliferation and metastasis ability of breast cancer stem cells and finally induced cellular apoptosis. Over-expression of CALR could effectively attenuate the cytotoxic effect of miR-206. Further studies demonstrated that EVI-1 could be served as a key regulator of miR206-mediated CALR expression. Elevation of EVI-1 can reverse the function of miR-206 on induction of CALR.

We aimed to study the relationship between pneumocephalus on non-contrast CT (NCCT) and post-operative cerebrospinal fluid leakage (p-CFL) after endoscopic transsphenoidal sellar and suprasellar tumor surgeries. Data from patients who underwent endoscopic treatment for sellar or suprasellar tumors from January 2018 to March 2020 were consecutively collected and reviewed. The NCCT pneumocephalus (NP) was measured the first day after operation and the first day after the expansive sponge was extracted. p-CFL was determined according to post-operative clinical symptoms, high resolution CT and glucose test, and expert consensus. Of the 253 patients enrolled in this study, 32 (12.6%) had p-CFL. Compared with patients without p-CFL, patients with p-CFL had a higher occurrence of intra-operative CFL, a longer operation time, a higher rate of pneumocephalus on first-day NCCT after operation (i.e., first-day NP), and a higher rate of NP volume change between two NCCT measurements (referred to as the NP change) (all p < 0.05). In multivariate regression analysis, first-day NP was independently associated with p-CFL occurrence [odds ratio (OR)=6.395, 95% confidence interval (CI)=2.236-18.290, p=0.001).
