Abstract

The LUX-ZEPLIN experiment recently reported limits on WIMP-nucleus interactions from its initial science run, down to $9.2\times10^{-48}$ cm$^2$ for the spin-independent interaction of a 36 GeV/c$^2$ WIMP at 90% confidence level. In this paper, we present a comprehensive analysis of the backgrounds important for this result and for other upcoming physics analyses, including neutrinoless double-beta decay searches and effective field theory interpretations of LUX-ZEPLIN data. We confirm that the in-situ determinations of bulk and fixed radioactive backgrounds are consistent with expectations from the ex-situ assays. The observed background rate after WIMP search criteria were applied was $(6.3\pm0.5)\times10^{-5}$ events/keV$_{ee}$/kg/day in the low-energy region, approximately 60 times lower than the equivalent rate reported by the LUX experiment., Comment: 25 pages, 15 figures

Abstract

The Research Libraries Group (RLG) hosted a symposium to explore opportunities for cooperative action--to take advantage of technology and to improve electronic access to information--particularly in the RLG context, and to develop strategies for making the most effective use of technology and electronic information in support of research and teaching missions of RLG member institutions. The program consisted of a keynote address, presentations on four main topics, six small group discussions, closing remarks, and a program wrap-up. In the keynote speech, "Knowledge Services in the Digitized World: Possibilities and Strategies" by Douglas E. Van Houweling, the role of the "new library" is defined, and fundamental questions are raised about the nature of the library business, the customers, and future needs. The first topic, "Local or Remote Access: Choices and Issues" by Nancy M. Cline, concerns selecting different modes of access to information and suggests strategies for making the right choices. In the second topic, "Building Xanadu: Creating the New Library Paradise," Jerry D. Campbell discusses a survey of faculty and students that illustrates the changing needs and expectations of users; highlights new opportunities for service through technology; explores the effects of new technologies on organizational structure and services; and suggests strategies for revitalizing library service. "Fair use" is examined in the third topic, "How Fair Is Use: When Publishing Is More Like Broadcasting?" by Kathryn M. Downing, in the context of promoting greater access to information. The copyright/licensing program used by the music industry is suggested as a possible model. The fourth topic, "Partners and Alliances" by Robert C. Berring, covers the need for librarians to form alliances with resourceful information vendors. Closing remarks were made by Kathleen Price in "Xanadu Revisited: Clothing the Emperor for the New Library Role in the Electronic Library Paradigm," and James Michalko presented the program wrap- up, "Challenges Facing the Research Libraries Group in the Coming Decade". Appendices provide cases in point (Project JANUS, the RiCE program, and Project TULIP) as well as a profile and list of symposium participants. (AEF)

Abstract

The European Intergroup for Childhood Non-Hodgkin Lymphoma (EICNHL) was established 25 years ago with the goal to facilitate clinical trials and research collaborations in the field both within Europe and worldwide. Since its inception, much progress has been made whereby major improvements in outcomes have been achieved. In this Review, we describe the different diagnostic entities of non-Hodgkin lymphoma in children and young adults describing key features of each entity and outlining clinical achievements made in the context of the EICNHL framework. Furthermore, we provide an overview of advances in biopathology with an emphasis on the role of biological studies and how they have shaped available treatments. Finally, for each entity, we describe future goals, upcoming clinical trials, and highlight areas of research that require our focus going forward.

Abstract

Data on the spectrum of second malignant neoplasms (SMNs) after primary childhood non-Hodgkin’s lymphoma (NHL) are scarce. One-hundred-and-eighty-nine NHL patients diagnosed in a 30 years period of 1980–2010 developing an SMN were retrieved from 19 members of the European Intergroup for Childhood NHL and/or the international Berlin-Frankfurt-Münster Study Group. Five subgroups of SMNs were identified: (1) myeloid neoplasms (n?=?43; 23%), (2) lymphoid neoplasms (n?=?51; 27%), (3) carcinomas (n?=?48; 25%), (4) central nervous system (CNS) tumors (n?=?19; 10%), and (5) “other” SMNs (n?=?28; 15%). In 37 patients (20%) preexisting disorders were reported with 90% having any kind of cancer predisposition syndrome (CPS). For the 189 primary NHL patients, 5-year overall survival (OS) after diagnosis of an SMN was 56?±?4%, being worst for patients with preexisting disorders at 28?±?8%. 23. Willingness to Pay for Anterior Cruciate Ligament Reconstruction.

Abstract

The outcomes of ACL reconstructions in terms of patient satisfaction and function are well known. Most orthopaedic surgeons feel that Medicare and other payors do not reimburse enough for this surgery. The purpose of this study is to determine how much patients are willing to pay for this surgery and compare it to reimbursement rates. Methods: We constructed a survey which described the function and limitations of an ACL deficient knee and the expected function of that knee after an ACL reconstruction. We then asked the volunteers how much they would be willing to pay for an ACL reconstruction if it were their knee. We also gathered data on the yearly earnings and Tegner activity level of the volunteers. In all, 143 volunteers completed the survey. We computed correlation coefficients between willingness to pay and both yearly earnings and Tegner activity level. Results: The average amount that the volunteers were willing to pay for an ACL reconstruction was $4,867.00. There was no correlation between yearly earnings and willingness to pay. The correlation coefficient was 0.34. There was a weak correlation between Tegner activity level and willingness to pay. This correlation coefficient was 0.81. The Medicare allowable rate for ACL reconstruction (CPT 29888) in the geographic area of the study was $1,132.00. Conclusion: The data demonstrates that patients are willing to pay much more than traditional payors for ACL reconstruction. These payors undervalue the benefit of this surgery to the patient. There is increasing pressure on orthopaedic surgeons to not participate in insurance plans that reimburse poorly. This places an increasing financial burden on the patient. This study suggests that patients may be willing to pay more for their surgery than their insurance plan and accept more of this burden. [ABSTRACT FROM AUTHOR]

25. CD56+ NK lymphomas:clinicopathological features and prognosis

Abstract

The surface molecule CD56 marks a category of malignant lymphoma of putative natural killer (NK) cell origin. We conducted a retrospective analysis of 24 cases of CD56+ NK lymphoma/leukaemia to define the clinicopathologic and prognostic features of this specific group of lymphomas. 56 cases of nasal lymphomas and 204 cases with an initial diagnosis of peripheral T-cell lymphoma were retrospectively analysed. To specifically examine lymphomas of putative NK origin, only those that were negative for surface expression of CD3 but positive for CD56 were analysed. 24 cases were identified. The initial predominant sites of involvement were nasal (n =18), palate (n = 1), nodal (n = 1) and multi-organ (n =4). Clinically, in patients with disease localized to one anatomical site (n = 20), most had symptoms confined to the nose, with a high percentage in early stage (I: 91%; IV: 9%). The marrow was not involved in any of these cases. However, patients with multi-organ involvement at presentation (n = 4) behaved differently. All presented acutely with pancytopenia, hepatosplenomegaly, and marrow infiltration with haemophagocytosis. A leukaemic phase was observed in one case. Anthracycline containing combination chemotherapy resulted in complete remission in 75% of patients with localized disease, but only in 25% with multi-organ involvement. The median survival of patients with localized disease was 12 months, compared with 2 months in the multi-organ group (P =0.06); the disease-free survival was significantly better in the former (P <0.01). The overall median survival of all patients was still poor at 11 months.

27. Coinfection of multiple strains of Epstein-Barr virus in immunocompetent normal individuals: reassessment of the viral carrier state

Abstract

This study reassesses the occurrence of Epstein-Barr virus (EBV) diversity and coinfection versus dominance of a single viral strain within immunocompetent normal carriers. Polymerase chain reaction analysis of several different polymorphic loci of the EBV genome was performed on collections of peripheral blood mononuclear cells and multiple lymphoid and epithelial tissues of the same individuals. Autopsy specimens from 15 individuals who died of causes unrelated to EBV infection served as normal viral carriers. Unexpectedly, coinfection of multiple distinct strains of EBV of the same type (usually type 1) and less frequently of both types 1 and 2 was found to be very high within individual viral carriers. These data indicate that coinfection with multiple EBV strains is much more prevalent in normal carriers than previously appreciated, which in turn has direct implications on EBV persistence, host–viral interaction and pathogenesis.
