2. GRANDProto300: status, science case, and prospects

Abstract

GRANDProto300, the mid-scale prototype of the GRAND experiment, is a planned radio array of 300 antennas over $200\, \rm km^{2}$ that will be deployed in the radio-quiet location of Xiao Dushan (China) by $\sim 2026$. The array will act as a test bench for the GRAND experiment and aim to achieve autonomous radio-detection and reconstructions of very inclined air showers in a large-scale array. GRANDProto300 will detect ultra-high-energy cosmic rays in the energy range $10^{16.5}-10^{18}\, \rm eV$ at a rate comparable to Auger. GRANDProto300 could also contain a ground particle array that would validate the performances of the radio detectors. We discuss the current status of the detector commissioning and the rich science case made possible by GRANDProto300, which covers the study of the Galactic-to-extragalactic transition, fast radio bursts and ultra-high-energy gamma-rays., Comment: 8 pages, 6 figures. Abstract

To detect ultra-high-energy neutrinos, experiments such as ARA and RNO-G target the radio emission these particles induce when cascading in the ice, using deep antennas in South Pole or in Greenland. One of the main backgrounds for such signals is the radio emission generated by cosmic-ray showers, either directly in the ice, or in the air and transmitted to the ice, which can both reach the deep antennas. The first detection of cosmic rays with deep antennas would thus validate this detection principle and allow us to calibrate the detectors. FAERIE, the Framework for the simulation of Air shower Emission of Radio for in-Ice Experiments, is a numerical tool that couples both CoREAS and GEANT4 Monte-Carlo codes to simulate the radio emission from cosmic-ray showers deep in the ice. Using this code, we will investigate cosmic-ray radio signatures and the possible implications on the design of a cosmic-ray veto., Comment: 8 pages, 5 figures. Abstract

The operation of upcoming ultra-high-energy cosmic-ray, gamma-ray, and neutrino radio-detection experiments, like the Giant Radio Array for Neutrino Detection (GRAND), poses significant computational challenges involving the production of numerous simulations of particle showers and their detection, and a high data throughput. GRANDlib is an open-source software tool designed to meet these challenges. Its primary goal is to perform end-to-end simulations of the detector operation, from the interaction of ultra-high-energy particles, through -- by interfacing with external air-shower simulations -- the ensuing particle shower development and its radio emission, to its detection by antenna arrays and its processing by data-acquisition systems. Additionally, GRANDlib manages the visualization, storage, and retrieval of experimental and simulated data. 5. Loss of coherence and change in emission physics for radio emission from very inclined cosmic-ray air showers

Abstract

Next-generation radio experiments such as the Radio Detector of the upgraded Pierre Auger Observatory and the planned GRAND and BEACON arrays target the detection of ultra-high-energy particle air showers arriving at low elevation angles. These inclined cosmic-ray air showers develop higher in the atmosphere than vertical ones, enhancing magnetic deflections of electrons and positrons inside the cascade. We evidence two novel features in their radio emission: a new polarization pattern, consistent with a geo-synchrotron emission model and a coherence loss of the radio emission, both for showers with zenith angle $\theta \gtrsim 65^{\circ}$ and strong enough magnetic field amplitude (typical strength of $B\sim 50\, \rm \mu T$). Our model is compared with both ZHAireS and CoREAS Monte-Carlo simulations. Our results break the cannonical description of a radio signal made of Askaryan and transverse current emission only, and provide guidelines for the detection and reconstruction strategies of next-generation experiments, including cosmic-ray/neutrino discrimination., Comment: 6 pages, 2 figures. 10. Particle content of very inclined air showers for radio signal modeling

Abstract

The reconstruction of very inclined air showers is a new challenge for next-generation radio experiments such as the AugerPrime radio upgrade, BEACON, and GRAND, which focus on the detection of ultra-high-energy particles. To tackle this, we study the electromagnetic particle content of very inclined air showers, which has scarcely been studied so far. Using the simulation tools CORSIKA and CoREAS, and analytical modeling, we explore the energy range of the particles that contribute most to the radio emission, quantify their lateral extent, and estimate the atmospheric depth at which the radio emission is strongest. We find that the distribution of the electromagnetic component in very inclined air showers has characteristic features that could lead to clear signatures in the radio signal, and hence impact the reconstruction strategies of next-generation radio-detection experiments., Comment: 22 pages, 15 figures, accepted for publication to Journal of Cosmology and Astroparticle Physics. Abstract

The Giant Radio Array for Neutrino Detection (GRAND) is an envisioned observatory of ultra-high-energy particles of cosmic origin, with energies in excess of 100 PeV. GRAND uses large surface arrays of autonomous radio-detection units to look for the radio emission from extensive air showers that are triggered by the interaction of ultra-high-energy cosmic rays, gamma rays, and neutrinos in the atmosphere or underground. In particular, for ultra-high-energy neutrinos, the future final phase of GRAND aims to be sensitive enough to discover them in spite of their plausibly tiny flux. Presently, three prototype GRAND radio arrays are in operation: GRANDProto300, in China, GRAND@Auger, in Argentina, and GRAND@Nancay, in France. Their goals are to field-test the design of the radio-detection units, understand the radio background to which they are exposed, and develop tools for diagnostic, data gathering, and data analysis. 13. Deep learning-based deconvolution for interferometric radio transient reconstruction

Abstract

Radio astronomy is currently thriving with new large ground-based radio telescopes coming online in preparation for the upcoming Square Kilometre Array (SKA). Facilities like LOFAR, MeerKAT/SKA, ASKAP/SKA, and the future SKA-LOW bring tremendous sensitivity in time and frequency, improved angular resolution, and also high-rate data streams that need to be processed. They enable advanced studies of radio transients, volatile by nature, that can be detected or missed in the data. These transients are markers of high-energy accelerations of electrons and manifest in a wide range of temporal scales. Usually studied with dynamic spectroscopy of time series analysis, there is a motivation to search for such sources in large interferometric datasets. This requires efficient and robust signal reconstruction algorithms. To correctly account for the temporal dependency of the data, we improve the classical image deconvolution inverse problem by adding the temporal dependency in the reconstruction problem. Then, we introduce two novel neural network architectures that can do both spatial and temporal modeling of the data and the instrumental response. Then, we simulate representative time-dependent image cubes of point source distributions and realistic telescope pointings of MeerKAT to generate toy models to build the training, validation, and test datasets. Finally, based on the test data, we evaluate the source profile reconstruction performance of the proposed methods and classical image deconvolution algorithm CLEAN applied frame-by-frame. In the presence of increasing noise level in data frame, the proposed methods display a high level of robustness compared to frame-by-frame imaging with CLEAN. The deconvolved image cubes bring a factor of 3 improvement in fidelity of the recovered temporal profiles and a factor of 2 improvement in background denoising., Comment: 15 pages, 19 figures. 19. Overview of radio experiments for UHE cosmic particles detection

Abstract

Radio-detection is a mature technique that has gained large momentum over the past decades. Its physical detection principle is mainly driven by the electromagnetic part of the shower, and is therefore not too sensitive to uncertainties on hadronic interactions. Furthermore its technical detection principle allows for a 100% duty cycle, and large surface coverage thanks to the low cost of antennas. Various detection methods of UHE particles now rely on the radio signal as main observable. For instance, ground based experiments such as AERA on the Pierre Auger Observatory or LOFAR detect the radio emission from air-showers induced by high-energy particles in the atmosphere; in-ice experiment such as ARA, IceCube, or ARIANNA benefits from a detection in denser media which reduces the interaction lengths; finally, balloon experiments such as ANITA allow for very sensitive UHE neutrino detection with only a few antennas. Radio-detection is now focused on building increasingly large-scale radio experiments to enhance the detector sensitivity and address the low fluxes at UHE. Abstract

Background & Aims: Tacrolimus has been associated with recurrence of primary biliary cholangitis (PBC) after liver transplantation (LT), which in turn may reduce survival. This study aimed to assess the association between the type of calcineurin inhibitor used and long-term outcomes following LT in patients with PBC. Methods: Survival analyses were used to assess the association between immunosuppressive drugs and graft or patient survival among adult patients with PBC in the European Liver Transplant Registry. Patients who received a donation after brain death graft between 1990 and 2021 with at least 1 year of event-free follow-up were included. Results: In total, 3,175 patients with PBC were followed for a median duration of 11.4 years (IQR 5.9–17.9) after LT. Tacrolimus (Tac) was registered in 2,056 (64.8%) and cyclosporin in 819 (25.8%) patients. Following adjustment for recipient age, recipient sex, donor age, and year of LT, Tac was not associated with higher risk of graft loss (adjusted hazard ratio [aHR] 1.07, 95% CI 0.92-1.25, p = 0.402) or death (aHR 1.06, 95% CI 0.90-1.24, p = 0.473) over cyclosporin. Abstract

IMPORTANCE:. Ventilator-associated pneumonia (VAP) frequently occurs in patients with cardiac arrest. Diagnosis of VAP after cardiac arrest remains challenging, while the use of current biomarkers such as C-reactive protein (CRP) or procalcitonin (PCT) is debated. OBJECTIVES:. To evaluate biomarkers’ impact in helping VAP diagnosis after cardiac arrest. DESIGN, SETTING, AND PARTICIPANTS:. This is a prospective ancillary study of the randomized, multicenter, double-blind placebo-controlled ANtibiotherapy during Therapeutic HypothermiA to pRevenT Infectious Complications (ANTHARTIC) trial evaluating the impact of antibiotic prophylaxis to prevent VAP in out-of-hospital patients with cardiac arrest secondary to shockable rhythm and treated with therapeutic hypothermia. An adjudication committee blindly evaluated VAP according to predefined clinical, radiologic, and microbiological criteria. All patients with available biomarker(s), sample(s), and consent approval were included. MAIN OUTCOMES AND MEASURES:. The main endpoint was to evaluate the ability of biomarkers to correctly diagnose and predict VAP within 48 hours after sampling. The secondary endpoint was to study the combination of two biomarkers in discriminating VAP. Blood samples were collected at baseline on day 3. Routine and exploratory panel of inflammatory biomarkers measurements were blindly performed. Analyses were adjusted on the randomization group. RESULTS:. Among 161 patients of the ANTHARTIC trial with available biological sample(s), patients with VAP (n = 33) had higher body mass index and Acute Physiology and Chronic Health Evaluation II score, more unwitnessed cardiac arrest, more catecholamines, and experienced more prolonged therapeutic hypothermia duration than patients without VAP (n = 121). In univariate analyses, biomarkers significantly associated with VAP and showing an area under the curve (AUC) greater than 0.70 were CRP (AUC = 0.76), interleukin (IL) 17A and 17C (IL17C) (0.74), macrophage colony-stimulating factor 1 (0.73), PCT (0.72), and vascular endothelial growth factor A (VEGF-A) (0.71). Multivariate analysis combining novel biomarkers revealed several pairs with p value of less than 0.001 and odds ratio greater than 1: VEGF-A + IL12 subunit beta (IL12B), Fms-related tyrosine kinase 3 ligands (Flt3L) + C–C chemokine 20 (CCL20), Flt3L + IL17A, Flt3L + IL6, STAM-binding protein (STAMBP) + CCL20, STAMBP + IL6, CCL20 + 4EBP1, CCL20 + caspase-8 (CASP8), IL6 + 4EBP1, and IL6 + CASP8. Best AUCs were observed for CRP + IL6 (0.79), CRP + CCL20 (0.78), CRP + IL17A, and CRP + IL17C. CONCLUSIONS AND RELEVANCE:. Our exploratory study shows that specific biomarkers, especially CRP combined with IL6, could help to better diagnose or predict early VAP occurrence in cardiac arrest patients.

Abstract

Summary: Background: Despite the increasing efficacy of chemotherapy (C), the 5-year survival rate for patients with unresectable colorectal liver metastases (CLM) remains around 10%. Liver transplantation (LT) might offer a curative approach for patients with liver-only disease, yet its superior efficacy compared to C alone remains to be demonstrated. Methods: The TransMet randomised multicentre clinical trial (NCT02597348) compares the curative potential of C followed by LT versus C alone in patients with unresectable CLM despite stable or responding disease on C. Patient eligibility criteria proposed by local tumour boards had to be validated by an independent committee via monthly videoconferences. Outcomes reported here are from a non-specified interim analysis. These include the eligibility of patients to be transplanted for non resectable colorectal liver metastases, as well as the feasibility and the safety of liver transplantation in this indication. Findings: From February 2016 to July 2021, 94 (60%) of 157 patients from 20 centres in 3 countries submitted to the validation committee, were randomised. Reasons for ineligibility were mainly tumour progression in 50 (32%) or potential resectability in 13 (8%). The median delay to LT after randomisation was 51 (IQR 30–65) days. Nine of 47 patients (19%, 95% CI: 9–33) allocated to the LT arm failed to undergo transplantation because of intercurrent disease progression. Three of the 38 transplanted patients (8%) were re-transplanted, one of whom (3%) died post-operatively from multi-organ failure. Interpretation: The selection process of potential candidates for curative intent LT for unresectable CLM in the TransMet trial highlighted the critical role of an independent multidisciplinary validation committee. After stringent selection, the feasibility of LT was 81%, as 19% had disease progression while on the waiting list. These patients should be given high priority for organ allocation to avoid dropout from the transplant strategy. Funding: No source of support or funding from any author to disclose for this work. Abstract

Abstract Background Feature selection is a critical step for translating advances afforded by systems-scale molecular profiling into actionable clinical insights. While data-driven methods are commonly utilized for selecting candidate genes, knowledge-driven methods must contend with the challenge of efficiently sifting through extensive volumes of biomedical information. This work aimed to assess the utility of large language models (LLMs) for knowledge-driven gene prioritization and selection. Methods In this proof of concept, we focused on 11 blood transcriptional modules associated with an Erythroid cells signature. We evaluated four leading LLMs across multiple tasks. Next, we established a workflow leveraging LLMs. The steps consisted of: (1) Selecting one of the 11 modules; (2) Identifying functional convergences among constituent genes using the LLMs; (3) Scoring candidate genes across six criteria capturing the gene’s biological and clinical relevance; (4) Prioritizing candidate genes and summarizing justifications; (5) Fact-checking justifications and identifying supporting references; (6) Selecting a top candidate gene based on validated scoring justifications; and (7) Factoring in transcriptome profiling data to finalize the selection of the top candidate gene. Results Of the four LLMs evaluated, OpenAI's GPT-4 and Anthropic's Claude demonstrated the best performance and were chosen for the implementation of the candidate gene prioritization and selection workflow. This workflow was run in parallel for each of the 11 erythroid cell modules by participants in a data mining workshop. Module M9.2 served as an illustrative use case. The 30 candidate genes forming this module were assessed, and the top five scoring genes were identified as BCL2L1, ALAS2, SLC4A1, CA1, and FECH. Researchers carefully fact-checked the summarized scoring justifications, after which the LLMs were prompted to select a top candidate based on this information. GPT-4 initially chose BCL2L1, while Claude selected ALAS2. When transcriptional profiling data from three reference datasets were provided for additional context, GPT-4 revised its initial choice to ALAS2, whereas Claude reaffirmed its original selection for this module. Conclusions Taken together, our findings highlight the ability of LLMs to prioritize candidate genes with minimal human intervention. This suggests the potential of this technology to boost productivity, especially for tasks that require leveraging extensive biomedical knowledge.

Abstract

Autonomous radio-detection, i.e., detection of air-showers with standalone radio arrays, is one of the major technical challenges to overcome for the next generation astroparticle detectors. In this context, we study polarisation signatures of simulated radio signals to perform an identification of the associated air-showers initiated by cosmic-rays and neutrinos. We compare the two sources of radio emission (the charge excess and geomagnetic) and show that the former is almost negligible for inclined (zenith angle $>65^{\circ}$) cosmic-ray air-showers. This provides an efficient background rejection criterion at the DAQ level, based on the projection of the total electric field along the direction of the local magnetic field. This relevant quantity can be easily computed -- even in an online treatment -- for antennas measuring three orthogonal polarisations. Independently of the experimental antenna layout, we estimate that assuming a random polarisation of noise events, a rejection from $\approx 72\%$ (for a non favorable detector location) to $\approx 93\%$ (for a favorable location) of the noise induced events and a trigger efficiency of 87% (93%) with a $3\sigma$ ($5\sigma$) trigger threshold level should be achievable. We also show that neutrino-induced showers present a charge excess to geomagnetic signal ratio up to $\sim 10$ times higher than for cosmic ray showers. Although this characteristic makes the identification of neutrino-induced showers challenging via the method developed here, it provides an efficient criterion to perform an offline discrimination between cosmic-ray and neutrino primaries. Abstract

Radio detection of air-showers is a mature technique that has gained momentum over the past decades. With increasingly large-scale experiments, massive air-shower simulations are needed to evaluate the radio signal at each antenna position. Radio Morphing was developed for this purpose. It is a semi-analytical tool that enables a fast computation of the radio signal emitted by any air-shower at any location, from the simulation data of one single reference shower at given positions. It relies on simple electromagnetic scaling laws of the radio emission (i.e., electric field) at the antenna level and then an interpolation of the radio pulse at the desired positions. We present here major improvements on the Radio Morphing method that have been implemented recently. The upgraded version is based on revised and refined scaling laws, derived from physical principles. It also includes shower-to-shower fluctuations and a new spatial interpolation technique, thanks to which an excellent signal timing accuracy of a fraction of nanosecond can be reached. This new implementation, provides simulated signals with relative differences on the peak-to-peak amplitude of ZHAireS simulations below 10\% (respectively 25\%) for 91\% (99\%) of antennas while the computation time was reduced by more than 2 orders of magnitude compared to standard simulations. This makes Radio Morphing an efficient tool that allows for a fast and accurate computation of air-shower radio signals. Abstract

Recurrent models have gained popularity in deep learning (DL) based video super-resolution (VSR), due to their increased computational efficiency, temporal receptive field and temporal consistency compared to sliding-window based models. However, when inferring on long video sequences presenting low motion (i.e. in which some parts of the scene barely move), recurrent models diverge through recurrent processing, generating high frequency artifacts. To the best of our knowledge, no study about VSR pointed out this instability problem, which can be critical for some real-world applications. Video surveillance is a typical example where such artifacts would occur, as both the camera and the scene stay static for a long time. In this work, we expose instabilities of existing recurrent VSR networks on long sequences with low motion. We demonstrate it on a new long sequence dataset Quasi-Static Video Set, that we have created. Finally, we introduce a new framework of recurrent VSR networks that is both stable and competitive, based on Lipschitz stability theory. We propose a new recurrent VSR network, coined Middle Recurrent Video Super-Resolution (MRVSR), based on this framework. Objectives Discontinuation or continuation of maintenance immunosuppressive therapy (MIST) after a severe lupus nephritis (LN) requires measuring the risk of relapse but reliable clinical and biological markers are lacking. The WIN-IgE study assesses the value of serum anti-dsDNA IgE autoantibodies as a biomarker for the prediction of relapse in severe LN.Methods WIN-IgE is an ancillary study of the WIN-Lupus study (NCT01284725), a prospective controlled clinical trial which evaluated the discontinuation of MIST after 2–3 years in class III or IV±V LN with active lesions. WIN-IgE included all patients with available serum collected at randomisation for continuation or discontinuation of MIST. In these sera, anti-dsDNA antibodies, IgE and IgG, were quantified by ELISA and compared between patients who experienced LN relapse and those who did not during the 24 months of follow-up.Results 52 patients were included, 25 in the MIST continuation group and 27 in the MIST discontinuation group, 12 experienced a biopsy-proven relapse of LN. Initial anti-dsDNA IgE antibodies levels were higher in patients with subsequent LN relapse. Anti-dsDNA IgG was not associated with relapse. Survival without LN relapse was lower in patients with anti-dsDNA IgE levels above vs below a threshold of 1.9 arbitrary units (p=0.019), particularly in the subgroup of patients randomised to discontinue MIST (p=0.002). In all patients, anti-dsDNA IgE above 1.9 arbitrary units had a positive predictive value of 0.8 for severe LN relapse.Conclusions These results suggest blood anti-dsDNA IgE as a non-invasive predictive marker of LN relapse.

Abstract Background of the study AML classification tools have been developed to stratify the risk at AML diagnosis. There is a need to evaluate these tools in the current therapeutic era. Cohort characteristics In this retrospective study, we compared five classifiers: ELN 2017, ELN 2022, ALFA classifier, Papaemmanuil et al. classifier, and Lindsley et al. classifier, in a real‐life cohort of 281 patients newly diagnosed for AML in Nice University Hospital. In our cohort median age was 68 years old, sex ratio was M/F 56%/44%, performance status was lower than 2 in 73.1% of patients, AML subtype was “De novo” in 71.5%, “secondary” in 22.4%, and “therapy‐related” in 6.0% of patients. Intensive chemotherapy was used in 53.0% of patients, and non‐intensive chemotherapy in 40.6% of patients. Molecular analysis was available in a large majority of patients and the main mutations found were NPM1 (22.7%), DNMT3A (17.4%), TP53 (13.1%), TET2 (12.4%), and FLT3‐ITD (12.4%). Abstract

Very inclined extensive air showers (EAS), with both down-going and up-going trajectories, are particularly targeted by the next generation of extended radio arrays, such as GRAND. Methods to reconstruct the incoming direction, core position, primary energy and composition of showers with these specific geometries, remain to be developed. Towards that goal, we present a new reconstruction procedure based on the arrival times and the amplitudes of the radio signal, measured at each antenna station. This hybrid reconstruction method, harnesses the fact that the emission is observed, at the antenna level, far away from the emission region, thus allowing for a point-like emission description. Thanks to this assumption, the arrival times are modelled following a spherical wavefront emission, which offers the possibility to reconstruct the radio emission zone as a fixed point along the shower axis. From that point the amplitude distribution at the antenna level is described through an Angular Distribution Function (ADF) taking into account at once all geo-magnetic asymmetries and early late effects as well as additional signal asymmetries featured by very inclined EAS. Le visage de la mobilité urbaine a évolué rapidement dans les dernières années dans un contexte de diversification des innovations en mobilité. Notre analyse s'intéresse à l'accueil, exprimé au travers de la couverture médiatique, des services de mobilité partagée et en libre-service à Montréal. Au travers de sept cas d'études (Communauto, Bixi, Auto-mobile, Car2Go, Bixi électrique et les vélos (Jump) et trottinettes en libre-service sans ancrage (Lime et Bird)), nous cherchons à comprendre comment la presse dépeint ces innovations. Pour chaque cas, une fenêtre temporelle de trois ans, commençant un an avant son introduction, est identifiée. Trois quotidiens majeurs à Montréal servent à former notre corpus d'articles (n=179), tous publié entre 1997 et 2021. Cette analyse renseigne aussi les dynamiques d'intégrations d'une innovation au secteur sociotechnique de la mobilité quotidienne. Trois périodes ressortent de la couverture. 1) Un traitement initial largement descriptif des innovations au cas par cas, précède à 2) un traitement comparatif des cas, qui fait écho à une vision politique en faveur de la complémentarité des modes, malgré certaines complexités d'intégration. Les perceptions positives des nouveaux modes font place à 3) une approche plus critique aux véhicules sans ancrage s'inscrivant pourtant dans la même mouvance, mais produisant un lot d'enjeux distincts. L'origine québécoise d'une innovation et la nature motorisée de l'innovation semblent avoir une influence positive sur sa réception et son intégration à la ville. Our analysis focuses on the reception, expressed through early media coverage, of shared and self-service mobility services in Montreal. Using seven case studies (Communauto, Bixi, Auto-mobile, Car2Go, electric Bixi and bicycles (Jump) and self-service scooters without anchoring (Lime and Bird)), we seek to understand how the press depicts these innovations. For each case, a three-year time window starting one year before its introduction is identified. Three major daily newspapers in Montreal are used to form our corpus of articles (n=179), published between 1997 and 2021. This analysis also informs the dynamics of integration of an innovation in the sociotechnical sector of daily mobility. Three periods emerge from the coverage. 1) An initial and largely descriptive treatment of innovations on a case-by-case basis, precedes 2) a comparative treatment of cases, which echoes a political vision in favor of the complementarity of transport modes, despite integration complexities. The positive perceptions of the new modes give way to 3) a more critical approach to emerging dockless systems that, while being part of the same movement, produce a variety of distinct issues. The local origin of an innovation and the motorized nature of the innovation seem to positively influence its reception and its integration into the city. Abstract

Abstract Personalized medicine plays an important role in treatment optimization for COVID-19 patient management. Early treatment in patients at high risk of severe complications is vital to prevent death and ventilator use. Predicting COVID-19 clinical outcomes using machine learning may provide a fast and data-driven solution for optimizing patient care by estimating the need for early treatment. In addition, it is essential to accurately predict risk across demographic groups, particularly those underrepresented in existing models. Unfortunately, there is a lack of studies demonstrating the equitable performance of machine learning models across patient demographics. To overcome this existing limitation, we generate a robust machine learning model to predict patient-specific risk of death or ventilator use in COVID-19 positive patients using features available at the time of diagnosis. We establish the value of our solution across patient demographics, including gender and race. In addition, we improve clinical trust in our automated predictions by generating interpretable patient clustering, patient-level clinical feature importance, and global clinical feature importance within our large real-world COVID-19 positive patient dataset. We achieved 89.38% area under receiver operating curve (AUROC) performance for severe outcomes prediction and our robust feature ranking approach identified the presence of dementia as a key indicator for worse patient outcomes. We also demonstrated that our deep-learning clustering approach outperforms traditional clustering in separating patients by severity of outcome based on mutual information performance. Finally, we developed an application for automated and fair patient risk assessment with minimal manual data entry using existing data exchange standards.
