Abstract

Current remote patient monitoring (RPM) systems are fully reliant on the Internet. However, complete reliance on Internet connectivity is impractical in rural and remote environments where modern infrastructure is often lacking, power outages are frequent, and/or network connectivity is sparse (e.g. rural communities, mountainous regions of Appalachia, American Indian reservations, developing countries, and natural disaster situations). This paper proposes augmenting intermittent Internet with opportunistic communication to leverage the social behaviors of patients, caregivers, and community members to facilitate out-of-range monitoring of patients via Bluetooth 5 during intermittent network connectivity in rural communities. The architecture is evaluated for Owingsville, KY using U.S. Census Bureau, the National Cancer Institute's, and IPUMS-ATUS sample data, and is compared against a delay tolerant RPM case that is completely disconnected from the Internet. 15. Predicting the Initial Lapse Using a Mobile Health Application after Alcohol Detoxification

Abstract

The prediction and prevention of the initial lapse--which is defined as the first lapse after a period of abstinence--is important because the initial lapse often leads to subsequent lapses (within the same lapse episode) or relapse. The prediction of the initial lapse may allow preemptive intervention to be possible. This dissertation reports on a predictive modeling study of the initial alcohol lapse after patient left residential care. The data were collected via a mobile health application, Addiction-Comprehensive Health Enhancement Support System (A-CHESS). A-CHESS was designed to offer ongoing support to alcohol addiction patients who have returned to their own community after completing inpatient treatment programs. Patients may access A-CHESS services at any time anywhere to help them cope with the recovery. In addition to the first chapter as the introduction of the problems and context, this dissertation consists of the other three chapters; each chapter presents a study to address different challenges faced in the development of such a predictive model in A-CHESS. The first challenge is the validation of a survey instrument used in the A-CHESS Weekly Check-in; the second challenge is the study of A-CHESS use behavior before the initial lapse; and the third challenge is to develop a comprehensive predictive model of the initial lapse. The steps taken in this dissertation to address these challenges have been fruitful. The major findings are practical and can be implemented in A-CHESS. Abstract

A meta-analysis of oncology papers from around the world revealed that cancer patients who lived more than 50 miles away from hospital centers routinely presented with more advanced stages of disease at diagnosis, exhibited lower adherence to prescribed treatments, presented with poorer diagnoses, and reported a lower quality of life than patients who lived nearer to care facilities. Connected health approaches-or the use of broadband and telecommunications technologies to evaluate, diagnose, and monitor patients beyond the clinic-are becoming an indispensable tool in medicine to overcome the obstacle of distance.

Abstract

The purpose of this study was to investigate the nature and effects of exchanging emotional support via a smartphone-based support group for patients with alcohol dependence. Of the 349 patients who met the Diagnostic and Statistical Manual of Mental Disorders (4th ed.) criteria for alcohol dependence, 153 patients participated in the discussion group within the Addiction-Comprehensive Health Enhancement Support System, a smartphone application aimed at reducing relapse. This was developed to prevent problem drinking by offering individuals in recovery for alcohol dependence automated 24/7 recovery support services and frequent assessment of their symptom status as part of their addiction care. The results showed that receiving emotional support from health care providers improved coping self-efficacy. Giving emotional support and receiving emotional support from health care providers acted as a buffer, protecting patients from the harmful effects of emotional distress on risky drinking. Clinicians and researchers should use the features of smartphone-based support groups to reach out to alcoholic patients in need and encourage them to participate in the exchange of emotional support with others.
