This prospective phase I trial aimed to determine the recommended dose of 3-day total marrow and lymphoid irradiation (TMLI) for a myeloablative conditioning regimen by increasing the dose per fraction. The primary end-point of this single-institution dose escalation study was the recommended TMLI dose based on the frequency of dose-limiting toxicity (DLT) ≤100 days posthematopoietic stem cell transplantation (HSCT); a 3 + 3 design was used to evaluate the safety of TMLI. Three dose levels of TMLI (14/16/18 Gy in six fractions over 3 days) were set. The treatment protocol began at 14 Gy. Dose-limiting toxicities were defined as grade 3 or 4 nonhematological toxicities. Nine patients, with a median age of 42 years (range, 35-48), eight with acute lymphoblastic leukemia and one with chronic myeloblastic leukemia, received TMLI followed by unrelated bone marrow transplant. The median follow-up period after HSCT was 575 days (range, 253-1037). Three patients were enrolled for each dose level. No patient showed DLT within 100 days of HSCT. The recommended dose of 3-day TMLI was 18 Gy in six fractions. All patients achieved neutrophil engraftment at a median of 19 days (range, 14-25). One-year overall and disease-free survival rates were 83.3% and 57.1%, respectively. Three patients experienced relapse, and no nonrelapse mortality was documented during the observation period. One patient died due to disease relapse 306 days post-HSCT. The recommended dose of 3-day TMLI was 18 Gy in six fractions. The efficacy evaluation of this regimen is currently being planned in a phase II study.

The availability of an in-situ monitoring and feedback control system during the implementation of metal additive manufacturing technology ensures that high-quality finished parts are manufactured. This study aims to investigate the correlation between the surface texture and internal defects or density of laser-beam powder-bed fusion (LB-PBF) parts. In this study, 120 cubic specimens were fabricated via application of the LB-PBF process to the IN 718 Ni alloy powder. The density and 35 areal surface-texture parameters of manufactured specimens were determined based on the ISO 25,178–2 standard. Using a statistical method, a strong correlation was observed between the areal surface-texture parameters and density or internal defects within specimens. In particular, the areal surface-texture parameters of reduced dale height, core height, root-mean-square height, and root-mean-square gradient demonstrate a strong correlation with specimen density. Therefore, in-situ monitoring of these areal surface-texture parameters can facilitate their use as control variables in the feedback system.

Chimeric antigen receptor (CAR) T-cell therapy has revolutionized the approach to patients with relapsed or refractory diffuse large B-cell lymphoma (r/r DLBCL). This study retrospectively analyzed patients treated with commercially available tisagenlecleucel at our hospital and evaluated its safety and effectiveness. Of the 21 patients evaluated, any grade and grade ≥3 cytokine release syndrome (CRS) occurred in 85.7% and 9.5% of the patients, respectively. A total of 66.7% received tocilizumab and 28.6% received glucocorticoids for the treatment of CRS. The complete response (CR) rate at 3 months was 61.9% (95% confidence interval [CI] 38.4-81.9). After a median follow-up of 6.3 months following CAR-T infusion, the progression-free survival (PFS) and overall survival rates at 6 months were 53.1% (95%CI 28.3-72.7) and 69.2% (95%CI 43.7-84.9), respectively. Severe cytopenia and hypogammaglobulinemia occurred frequently following CAR-T infusion. Eight patients (38.1%) had comorbidities that would have made them ineligible for leukapheresis in the JULIET trial. However, the presence of comorbidities at the time of leukapheresis had no significant effect on the rates of CR, PFS, and adverse events. Tisagenlecleucel for r/r DLBCL in the real-world setting showed high efficacy and manageable safety profile comparable with the pivotal trial.

A 60-year-old woman with myelodysplastic syndrome/myeloproliferative neoplasm-unclassifiable underwent unrelated bone marrow transplantation from a human leukocyte antigen (HLA) 8/8 allele-matched male donor. Neutrophil engraftment was achieved on day 29. Fluorescence in situ hybridization of sex chromosomes demonstrated complete donor chimerism. The red blood cell and platelet transfusion dependence continued, and the neutrophil count decreased gradually. Despite prolonged administration of broad-spectrum antibiotics for febrile neutropenia, blood cultures on days 46 and 58 returned positive for Stenotrophomonas maltophilia (SM). Contrast-enhanced computed tomography revealed multiple nodules of septic emboli in the lungs and kidneys, suggesting a disseminated SM infection. Antibiotic therapy was conducted based on antimicrobial susceptibility testing. However, the blood cell count failed to normalize and a secondary graft failure was diagnosed. A HLA-haploidentical peripheral-blood stem-cell transplantation from the patient's son was performed on day 134 after the initial transplantation. Neutrophil engraftment was achieved on day 11. Red blood cells and platelets were also engrafted. After the resolution of the SM bacteremia, the patient was discharged on day 63. The prognosis of the SM bacteremia with neutropenia is poor. Antibiotic treatment based on antimicrobial susceptibility testing and a second transplant from an HLA-haploidentical donor likely contributed to the successful outcome in this patient.

Introduction Myelodysplastic syndromes (MDS), a heterogeneous group of acquired clonal hematopoietic stem cell disorders, is characterized by peripheral cytopenia and has a high tendency to progress to acute myeloid leukemia (AML). Serum ferritin (SF) level is a well-known indicator of iron stores and inflammation. Elevated SF levels is common in patients with MDS and appear to be largely related to regular red blood cell (RBC) transfusions. However, some patients with MDS showed high SF levels at the time of diagnosis, even before receiving RBC transfusions, although the exact mechanisms is unclear. The impact of SF elevation without RBC transfusion has not been well characterized. In this study, we assessed cytogenetic, molecular, and clinical characteristics of patients with MDS and SF elevation. Methods Patients with newly diagnosed MDS who visited our hospital between Jan 2011 and Jan 2020 were included in this study. Patients were defined as high-SF-MDS when SF values were >350 ng/mL (patients with Results Among the 112 patients with MDS, SF value were available for 87 patients. This study analyzed 28 women and 59 men with a median age of 69 years (range: 28-87 years). The median follow-up period was 540 days (range: 3-3618 days). A total of 17 patients died during the observational period (until Apr 2021). Patients were diagnosed with MDS with single lineage dysplasia (MDS-SLD, n = 9), MDS with multilineage dysplasia (MDS-MLD, n = 26), MDS with ring sideroblasts with SLD (MDS-RS-SLD, n = 5), MDS-RS-MLD (n = 6), MDS with excess blasts 1 (MDS-EB1, n = 25), MDS-EB2 (n = 13), MDS with isolated del(5q) (n=1), and MDS, unclassifiable (MDS-U, n = 2). The median SF value was 263 ng/mL (range: 3-1,245 ng/mL); 31 patients were classified as high-SF-MDS and 56 were classified as low-SF-MDS. The Proportion of MDS subtypes differed significantly between the groups (P=0.004, Table1). Compared to low-SF-MDS, MDS-EB1/2 (51.6% vs. 39.3%) and MDS-RS-SLD/MLD (22.6% vs. 1.8%) was more common in high-SF-MDS. In the laboratory data analysis, high-SF-MDS patients showed low Hb levels, high RBC distribution width values, and high bone marrow RS percentages. In patients with high-SF-MDS, as well as MDS-RS, the presence of bone marrow RS was also observed in seven out of 11 patients with MDS-EB1/2 (median 8%, range 2-19%). Complex karyotype ( was more common in patients with high-SF-MDS (46.4% vs. 16.0%, P=0.007) than those with low-SF-MDS. Trisomy 8 and del (5q) were also frequently detected among those with high-SF-MDS. Patients with high-SF-MDS had worse 2-year overall survival (OS) than those with low-SF-MDS. To clarify the molecular characteristics of high-SF-MDS in more detail, NGS analysis was performed on 11 patients with high-SF-MDS whose samples were available. Ten gene mutations were detected in eight patients. Interestingly, five among 11 patients harbored TP53 mutation; the median variant allele frequency was 53.9% (range, 39.7-64.1%). Subsequently, SF3B1 gene mutation was detected in three patients; and two of them also harbored TP53 mutation. Other gene mutations, RUNX1, STAG2, SRSF2, ASXL1, ATM, DNMT3A, BCOR, and DDX41 were detected in one patient each. Figure 1 Figure 1. Disclosures No relevant conflicts of interest to declare.

A forest-stream trophic link was examined by stable carbon isotope analyses which evaluated the relationship of aquatic insects emerging from a stream to the diets of web-building spiders. Spiders, aquatic and terrestrial prey, and basal resources of forest and stream food webs were collected in a deciduous forest along a Japanese headwater stream during May and July 2001. The δ13C analyses suggested that riparian tetragnathid spiders relied on aquatic insects and that the monthly variation of such dependence is partly associated with the seasonal dynamics of aquatic insect abundance in the riparian forest. Similarly, linyphiid spiders in the riparian forest exhibited δ13C values similar to aquatic prey in May. However, their δ13C values were close to terrestrial prey in both riparian and upland (150 m away from the stream) forests during June to July, suggesting the seasonal incorporation of stream-derived carbon into their tissue. In contrast, araneid spiders relied on terrestrial prey in both riparian and upland forests throughout the study period. These isotopic results were consistent with a previous study that reported seasonal variation in the aquatic prey contribution to total web contents for each spider group in this forest, implying that spiders assimilate trapped prey and that aquatic insect flux indeed contributes to the energetics of riparian tetragnathid and linyphiid spiders.

Food webs driven by energy from the oxidation of methane are now recognized to be omnipresent in terrestrial and freshwater ecosystems (e.g. lakes, soils and peat bogs), as well as in deep-sea hydrothermal vents and cold seeps. However, the incorporation of methane-derived carbon into stream food webs has never been reported. Here we present the first circumstantial evidence from stable carbon and nitrogen isotope ratios that a stream food web composed of aquatic macro- invertebrates is partly sustained by methanotrophs or chemoautotrophs that gain carbon respired by methane-oxidizing bacteria. Methane-derived carbon seems to enter stream communities around anoxic habitats such as backwater pools and hyporheic zones. Because these reductive habitats exist in many streams, food webs partly sustained by methane-derived carbon are likely to be ubiquitous in lotic ecosystems.

The flux of emerging aquatic insects from streams can provide a significant energy subsidy to riparian web-building spiders. However, despite the high temporality of aquatic insect emergence, the effects of such aquatic insect dynamics on spider distribution are poorly understood. To examine the relationship, the aquatic insect flux from a headwater stream in a northern Japanese deciduous forest was experimentally manipulated by using a greenhouse-type covering, during May to July. Under natural conditions, the aquatic and terrestrial insect abundances dramatically decreased and increased from May through July, respectively. The experimental reduction of aquatic insect flux depressed the density of horizontal orb weavers (Tetragnathidae) in both May and June, but not in July when aquatic insects were scarce, indicating a temporal limitation on spider distribution by aquatic insect flux. In contrast, the densities of both vertical orb weavers (Araneidae) and sheet weavers (Linyphiidae) were unaffected by the manipulation throughout the study period. These various responses, differing among months or spider guilds, may be attributed to the degree of specialization for aquatic prey in the spiders and their mobility in response to aquatic insect flux. The experimental results provided direct evidence that the temporal dynamics of aquatic insect flux, as well as spider characteristics, were primary factors determining the distributional patterns of riparian web-building spiders.

Preparing targeted cells for medical applications from human induced pluripotent stem cells (hiPSCs) using growth factors, compounds, or gene transfer has been challenging. Here, we report that human induced hepatic lineage-oriented stem cells (hiHSCs) were generated and expanded as a new type of hiPSC under non-typical coculture with feeder cells in a chemically defined hiPSC medium at a very high density. Self-renewing hiHSCs expressed markers of both human embryonic stem cells (hESCs) and hepatocytes. Those cells were highly expandable, markedly enhancing gene expression of serum hepatic proteins and cytochrome P450 enzymes with the omission of FGF-2 from an undefined hiPSC medium. The hepatic specification of hiHSCs was not attributable to the genetic and epigenetic backgrounds of the starting cells, as they were established from distinct donors and different types of cells. Approximately 90% of hiHSCs autonomously differentiated to hepatocyte-like cells, even in a defined minimum medium without any of the exogenous growth factors necessary for hepatic specification. After 12 days of this culture, the differentiated cells significantly enhanced gene expression of serum hepatic proteins (ALB, SERPINA1, TTR, TF, FABP1, FGG, AGT, RBP4, and AHSG), conjugating enzymes (UGT2B4, UGT2B7, UGT2B10, GSTA2, and GSTA5), transporters (SULT2A1, SLC13A5, and SLCO2B1), and urea cycle-related enzymes (ARG1 and CPS1). In addition, the hepatocyte-like cells performed key functions of urea synthesis, albumin secretion, glycogen storage, indocyanine green uptake, and low-density lipoprotein uptake. The autonomous hepatic specification of hiHSCs was due to their culture conditions (coculture with feeder cells in a defined hiPSC medium at a very high density) in self-renewal rather than in differentiation. These results suggest the feasibility of preparing large quantities of hepatocytes as a convenient and inexpensive hiPSC differentiation. Our study also suggests the necessity of optimizing culture conditions to generate other specific lineage-oriented hiPSCs, allowing for a very simple differentiation.

We previously reported that the inertia force (IF) of the blood flowing out of left ventricle (LV) during late systolic phase produces greater LV elastic recoil force and brings faster LV relaxation. Echodynamography (ALOKA) is a novel technique that enables us to obtain the flow velocity vector from conventional color Doppler velocity data on the range of interest at any phase of cardiac cycle without angle dependency. We investigated whether the flow velocity vector obtained at LV outflow tract (LVOT) in late systole had a relation with IF and invasively obtained LV relaxation parameters. Method: Study subjects were consecutive 26 patients who underwent diagnostic cardiac catheterization and conventional color Doppler imaging on the same day. Color Doppler image was acquired in the apical 3-chamber view. The analyses for flow velocity vector were performed offline using an echo image analyzer. The late systolic velocity of the blood flowing was obtained as an average value of the L VOT (V LVOT −late systole). LV pressure was obtained using a catheter-tipped micromanometer. From the recorded pressure waves, first derivative of LV pressure (dP/dt) and a time constant τ of LV pressure decay during isovolumic relaxation were calculated. From LV pressure-dP/dt relationships (phase loop), IF was determined. Results: A significant positive correlation was observed between the V LVOT -late systole and IF (r=0.75, pLVOT -late systole also had significant correlations both with peak negative dP/dt (r=0.49, p Conclusion: This study indicates that the V LVOT -late systole, which may be a noninvasive substitute for IF, has significant correlations with the parameters of LV relaxation. Echodynamography is a new useful technique to estimate LV early diastolic function.

During the remelting reaction, belite crystals with Al/Fe ratio 1 produced a liquid which readily spread on the lamellae with a resulting high rate of reaction. At 1450°C the distribution coefficient of Al and Fe atoms between the belite and the interstitial liquid ((Al/Fe)Belite/(Al/Fe)Liquid) was about 0.8.
