Abstract

Background: Globally, moderate wasting affects approximately 33 million children. Complex bidirectional interactions exist between wasting and infection in children. Children who experience both conditions have an increased risk of adverse outcomes including progression to severe wasting and mortality. Breaking the cycle between moderate wasting and infection could help improve growth and survival in these children. The NUTRIMAM trial will aim to investigate the efficacy of a 12-week regimen of three different nutritional interventions in at-risk young children (i.e., children who are moderately wasted and have one/more acute infections) on anthropometric recovery. Further, the study will explore whether recovery can be sustained with a post-intervention package that includes counseling and food vouchers. Sustaining anthropometric recovery beyond supplement administration will have important implications for programs. Methods: NUTRIMAM is a multi-country, multi-center individually randomized, open-label, trial in five countries including Bangladesh, India, Mali, Pakistan, and Tanzania. A total of 6360 moderately wasted children aged 6 to 24 months with acute illness will be enrolled at health centers. Children will be randomly allocated to receive one of three dietary supplements (locally available foods, ready-to-use supplementary foods, or microbiota-directed supplementary foods) for 12 weeks. Anthropometric recovery will be assessed over this period. Participants who recover will then be re-randomized to a post-recovery support intervention comprising either counseling and food vouchers or routine standard of care for recovered children for an additional 12 weeks to determine if this intervention facilitates sustained recovery at 24 weeks. Discussion: Children who are moderately wasted and have an infection are at higher risk of adverse outcomes. There are very few clinical trials that have been performed among children with moderate wasting with infectious illnesses to investigate if it is possible to break the undernutrition–infection cycle and thereby reduce the risk of nutritional deterioration to severe wasting or mortality and decrease the risk of acute infections. Abstract

Summary: Background: Effective methods of preventing and identifying childhood wasting are required to achieve global child health goals. Family mid-upper arm circumference (MUAC) programs train caregivers to screen their child for wasting with MUAC tapes. We assessed the effectiveness of a two-way short message service (SMS) platform (referred to as the Maternally Administered Malnutrition Monitoring System [MAMMS]) in western Kenya. Methods: In this individual-level randomised controlled trial in two rural countries in western Kenya, children (aged 5–12 months) were randomly allocated (1:1) to receive either standard care (SOC) or MAMMS. Randomisation method was permuted-block randomisation with a block size of 10. Eligible participants were children attending maternal child health clinics in the two counties whom had a MUAC between 12.5 and 14.0 cm. The MAMMS group received two MUAC tapes and weekly SMS reminders to screen their child's MUAC. The SOC group received routine community health volunteer services and additional quarterly visits from the study team. The primary analysis used a cox proportional hazards model to compare SOC and MAMMS time-to-diagnosis of wasting (MUAC <12.5 cm) confirmed by a health professional during 6-months follow-up. Secondary outcomes were days from enrolment to treatment initiation among children with wasting, proportion of all children with wasting who were identified by the two approaches (treatment coverage), mean MUAC at treatment initiation, and duration of wasting treatment. This trial was registered on ClinicalTrials.gov, NCT03967015., Findings: Between August 1, 2019 and January 31, 2022, 1200 children were enrolled, among whom the incidence of confirmed wasting was 37% lower in the MAMMS group (hazard ratio: 0.63, 95% CI: 0.42-0.94, p = 0.022). Among children with wasting, the median number of days-to-diagnosis was similar between study groups (MAMMS: 63 days [interquartile range (IQR): 23-92], SOC: 58 days [IQR: 22-94]). Treatment coverage in the MAMMS group was 83.3% (95% CI: 39.9-100.0) while coverage in the SOC group was 55.6% (95% CI: 22.3-88.9%, p = 0.300). Treatment duration and mean MUAC at treatment initiation were similar between groups., Interpretation: Family MUAC supported by SMS was associated with a 37% reduction in wasting among young children. Here, we show geospatial estimates of overweight and wasting prevalence among children under 5 years of age in 105 low- and middle-income countries (LMICs) from 2000 to 2017 and aggregate these to policy-relevant administrative units. Abstract

Une alimentation équilibrée est le socle sur lequel reposent des vies saines, des moyens de subsistance solides et durables et des économies prospères. En l’absence de ce socle, leur construction s’appuie sur des sables mouvants. Les enfants souffrant d’un retard de croissance ne peuvent grandir et se développer normalement et présentent des troubles cérébraux et un affaiblissement de leur système immunitaire. Les femmes sous-alimentées et en âge de procréer n’ingèrent pas suffisamment de fer pour alimenter leurs muscles. Beaucoup d’adultes présentent une surcharge pondérale si importante qu’ils sollicitent exagérément leur coeur et leurs poumons et sont susceptibles de déclencher un diabète, une hypertension et des maladies cardiovasculaires. La malnutrition entraîne une perte de 11 % du PIB résultant des pertes humaines, de mauvais apprentissages à l’école, d’une diminution des revenus au travail et d’absentéisme pour cause de maladie. Les mères sous-alimentées sont plus susceptibles de donner naissance à des bébés sous-alimentés, perpétuant ainsi une transmission intergénérationnelle de la pauvreté. La malnutrition affecte l’organisme, l’économie et menace l’avenir. Sa prévalence menace de nuire aux ambitions en matière de développement durable.

Abstract

Good nutrition is the bedrock upon which to build healthy lives, strong and resilient livelihoods, and thriving economies. In its absence, their construction takes place on quicksand. Stunted children fail to grow and develop properly, with impairment to brains and immune systems being particular problems. Malnourished women of childbearing age do not have enough iron in their blood to feed their muscles. Many adults carry so much excess weight they put their heart and lungs under extreme stress and may suffer from diabetes, hypertension, and heart disease. Malnutrition leads to 11 percent of gross national product being squandered as a result of lives lost, less learning in school, less earning in the workplace, and days lost to illness. And malnourished mothers are more likely to give birth to malnourished newborns, perpetuating the intergenerational transmission of poverty. Malnutrition corrodes the body, the economy, and the future. Ambitions for sustainable development are likely to be thwarted by its presence.

Una buena nutrición es fundamental para promover vidas saludables, medios de subsistencia sólidos y resistentes, y economías prósperas. Sin ella, estos empeños se asientan sobre terreno inestable. Los niños con retraso de crecimiento no pueden crecer ni desarrollarse adecuadamente, y padecen en especial problemas de daño cerebral y sistemas inmunes debilitados. Las mujeres malnutridas en edad fértil no tienen suficiente hierro en la sangre para alimentar sus músculos. Muchos adultos tienen tanto sobrepeso que someten a su corazón y sus pulmones a un estrés extremo y pueden sufrir de diabetes, hipertensión y enfermedad del corazón. La malnutrición conduce a un desperdicio del 11 por ciento del producto interno bruto como resultado de la pérdida de vidas, el menor aprendizaje en la escuela, la disminución de ingresos por trabajo y los días perdidos por bajas médicas. Por otra parte, las madres malnutridas son más susceptibles de dar a luz a bebés malnutridos, perpetuando así la transmisión intergeneracional de la pobreza. La malnutrición corroe el cuerpo, la economía y el futuro. Cuando está presente, los planes de desarrollo sostenible tienen una mayor probabilidad de verse frustrados.

A boa nutrição é fundamental para se construírem vidas saudáveis, meios de subsistência fortes e resilientes, e economias prósperas. Em sua ausência, a construção é feita em areia movediça. Crianças com atraso de crescimento não se desenvolvem adequadamente e, em especial, demonstram problemas de insuficiência cerebral e do sistema imunológico. Mulheres desnutridas em idade reprodutiva não têm ferro suficiente no sangue para alimentar seus músculos. Muitos adultos apresentam tanto excesso de peso que colocam seu coração e pulmões em estresse extremo e podem ter diabetes, hipertensão e doenças cardíacas. A desnutrição leva a 11 por cento do produto nacional bruto ser desperdiçado em consequência de mortes, menos aprendizado na escola, menor renda salarial e dias perdidos com doença. E as mães desnutridas estão mais propensas a dar à luz recém-nascidos desnutridos, perpetuando a transmissão intergeracional da pobreza. A desnutrição corrói o corpo, a economia e o futuro. As ambições de alcançar um desenvolvimento sustentável são prejudicadas com sua presença.

Abstract

At the 2013 Nutrition for Growth Summit in London, 96 signatories (governments, civil society organizations, donors, United Nations’ agencies, and businesses) agreed to support the creation of an annual report on global nutrition that would be authored by an independent expert group, in partnership with a large number of contributors. The first edition of this report, the Global Nutrition Report 2014, puts a spotlight on worldwide progress by the 193 member countries of the United Nations in improving their nutrition status, identifies bottlenecks to change, highlights opportunities for action, and contributes to strengthened nutrition accountability on country and global levels.
