2. Rethinking transitional justice through the lens of structural violence. Toward a new model of intervention for post-conflict societies.

La violenza di massa caratterizza l’epoca in cui viviamo, provocando morti, sparizioni forzate, deportazioni e gravi traumi individuali e collettivi. Il suo devastante impatto sociale non si esaurisce quando i conflitti armati, le aggressioni e i regimi oppressivi cessano: la violenza di massa costringe società profondamente lacerate a ideare strategie per fare i conti con il passato e guardare al futuro. In questo le assiste la giustizia di transizione, da intendersi come un insieme di processi e meccanismi volti ad accertare le responsabilità per i crimini occorsi e a promuovere la riconciliazione, la pacificazione e il cambiamento sociale. Più precisamente, la giustizia di transizione è chiamata a incentivare non solo la pace negativa, ovvero l’assenza di violenza diretta, ma anche la pace positiva, ossia l’assenza di violenza strutturale. Ciò implica la necessità di approfondire le dinamiche sociali che hanno portato al conflitto e, ancor prima, di comprendere il fenomeno della violenza collettiva nelle sue relazioni con la struttura sociale e con il sistema culturale. A partire da queste premesse, la tesi di dottorato esamina la nozione di violenza strutturale considerando le principali prospettive teoriche in tema di struttura sociale e di potere concepito in termini relazionali. In un successivo momento, delinea le caratteristiche che dovrebbero presentare gli interventi della giustizia di transizione per riuscire a destrutturare la violenza strutturale e promuovere la trasformazione sociale. In questo quadro, particolare attenzione è riservata alla necessità di assicurare la partecipazione attiva nei processi di pacificazione non solo alle vittime e alla società civile, ma anche ai perpetratori, spesso relegati a un ruolo marginale. A sostegno di ciò, sono presentati i risultati di una ricerca qualitativa relativa al ruolo che il ‘movimento della dissociazione’ ha svolto negli anni ’80 del secolo scorso nel contrasto al terrorismo italiano, nonché, Mass violence appears to be a defining characteristic of our time, causing deaths, enforced disappearances, deportations, and severe individual and collective trauma. Its devastating social impact does not cease with the end of armed conflicts, aggression, and oppressive regimes; instead, mass violence compels deeply fractured societies to devise strategies for dealing with the past and move forward. In this context, transitional justice plays a crucial role, involving processes and mechanisms to establish accountability for crimes, encourage reconciliation, and drive social change. More in detail, transitional justice goes beyond promoting negative peace – the absence of direct violence – to fostering positive peace – the absence of structural violence. This requires a thorough understanding of the social dynamics that led to the conflict and, even before that, a careful examination of mass violence in its relation to the social structure and cultural system. Building on this foundation, the dissertation delves into the concept of “structural violence,” by examining key theoretical perspectives on social structure and power conceived in relational terms. Subsequently, it outlines the crucial traits of transitional justice interventions capable of successfully dismantling structural violence and promoting social transformation. Within this framework, particular emphasis is placed on ensuring active participation in peacebuilding processes not only of victims and civil society but also of perpetrators, who are often marginalized in their roles. To support this assertion, qualitative research findings are presented, focusing on the role of the ‘dissociation movement’ in countering Italian terrorism during the 1980s and its contribution to fostering a more just and peaceful society. - Abstract
Background: Active relapsing–remitting (RR) and secondary progressive (SP) multiple sclerosis (MS) are currently defined as “relapsing MS” (RMS). The aim of this cross-sectional study was to assess drivers of treatment switches due to clinical relapses in a population of RMS patients collected in the Italian MS and Related Disorders Register (I-MS&RD). Methods: RRMS and SPMS patients with at least one relapse in a time window of 2 years before of data extraction were defined as RMS. Factors associated with disease-modifying therapy (DMT) switching due to clinical activity were assessed through multivariable logistic regression models in which treatment exposure was included as the last recorded DMT and the last DMT’s class [moderate-efficacy (ME), high-efficacy (HE) DMTs and anti-CD20 drugs]. Results: A cohort of 4739 RMS patients (4161 RRMS, 578 SPMS) was extracted from the I-MS&RD. A total of 2694 patients switching DMTs due to relapses were identified. Switchers were significantly (p < 0.0001) younger, less disabled, more frequently affected by an RR disease course in comparison to non-switcher patients. The multivariable logistic regression models showed that Alemtuzumab (OR 0.08, 95% CI 0.02–0.37), Natalizumab (0.48, 0.30–0.76), Ocrelizumab (0.1, 0.02–0.45) and Rituximab (0.23, 0.06–0.82) exposure was a protective factor against treatment switch due to relapses. Moreover, the use of HE DMTs (0.43, 0.31–0.59), especially anti-CD20 drugs (0.14, 0.05–0.37), resulted to be a protective factor against treatment switch due to relapses in comparison with ME DMTs. Conclusions: More than 50% of RMS switched therapy due to disease activity. HE DMTs, especially anti-CD20 drugs, significantly reduce the risk of treatment switch.

- Abstract
BACKGROUND: Gait impairments are common disabling symptoms of Parkinson's disease (PD). Among the approaches for gait rehabilitation, interest in robotic devices has grown in recent years. However, the effectiveness compared to other interventions, the optimum amount of training, the type of device, and which patients might benefit most remains unclear. OBJECTIVE: To conduct a systematic review about the effects on gait of robot-assisted gait training (RAGT) in PD patients and to provide advice for clinical practice. METHODS: A search was performed on PubMed, Scopus, PEDro, Cochrane library, Web of science, and guideline databases, following PRISMA guidelines. We included English articles if they used a robotic system with details about the intervention, the parameters, and the outcome measures. We evaluated the level and quality of evidence. RESULTS: We included twenty papers out of 230 results: two systematic reviews, 9 randomized controlled trials, 4 uncontrolled studies, and 5 descriptive reports. Nine studies used an exoskeleton device and the remainders end-effector robots, with large variability in terms of subjects' disease-related disability. CONCLUSIONS: RAGT showed benefits on gait and no adverse events were recorded. However, it does not seem superior to other interventions, except in patients with more severe symptoms and advanced disease.

- Abstract
BACKGROUND: Many robots are available for gait rehabilitation (BWSTRT and ORET) and their application in persons with SCI allowed an improvement of walking function.OBJECTIVE: The aim of the study is to compare the effects of different robotic exoskeletons gait training in persons with different SCI level and severity.METHODS: Sixty-two studies were included in this systematic review; the study quality was assessed according to GRADE and PEDro's scale.RESULTS: Quality assessment of included studies (n = 62) demonstrated a prevalence of evidence level 2; the quality of the studies was higher for BWSTRT (excellent and good) than for ORET (fair and good). Almost all persons recruited for BWSTRT had an incomplete SCI; both complete and incomplete SCI were recruited for ORET. The SCI lesion level in the persons recruited for BWSTRT are from cervical to sacral; mainly from thoracic to sacral for ORET; a high representation of AIS D lesion resulted both for BWSTRT (30%) and for ORET (45%). The walking performance, tested with 10MWT, 6MWT, TUG and WISCI, improved after exoskeleton training in persons with incomplete SCI lesions, when at least 20 sessions were applied. Persons with complete SCI lesions improved the dexterity in walking with exoskeleton, but did not recover independent walking function; symptoms such as spasticity, pain and cardiovascular endurance improved.CONCLUSION: Different exoskeletons are available for walking rehabilitation in persons with SCI. The choice about the kind of robotic gait training should be addressed on the basis of the lesion severity and the possible comorbidities.

Background: The recovery of walking after stroke is a priority goal for recovering autonomy. In the last years robotic systems employed for Robotic Assisted Gait Training (RAGT) were developed. However, literature and clinical practice did not offer standardized RAGT protocol or pattern of evaluation scales. Objective: This systematic review aimed to summarize the available evidence on the use of RAGT in post-stroke, following the CICERONE Consensus indications. Methods: The literature search was conducted on PubMed, Cochrane Library and PEDro, including studies with the following criteria: 1) adult post-stroke survivors with gait disability in acute/subacute/chronic phase; 2) RAGT as intervention; 3) any comparators; 4) outcome regarding impairment, activity, and participation; 5) both primary studies and reviews. Results: Sixty-one articles were selected. Data about characteristics of patients, level of disability, robotic devices used, RAGT protocols, outcome measures, and level of evidence were extracted. Conclusion: It is possible to identify robotic devices that are more suitable for specific phase disease and level of disability, but we identified significant variability in dose and protocols. RAGT as an add-on treatment seemed to be prevalent. Further studies are needed to investigate the outcomes achieved as a function of RAGT doses delivered.

14. Transitioning from dynamic security in Italian prisons: Assessing the influence of perceived insecurity on prison management

In the earlier part of the decade, italian prisons inplemented the open-cell regime (custodia aperta) and dynamic security, partly as a response to being convicted of overcrowding by the European Court of Human Rights. However, the Penitentiary Administration Circular No. 3693/6143 of 2022 significantly scaled back these interventions, (re)proposing the closed-cell regime and static security to govern prisons. This study aims to clarify the reasons that prompted the shift in the paradigm. It achieved this by first outlining the strategies for prison security management promoted in Italy in the last decades. Second, it investigates the recent departure from dynamic security by looking both at the ‘inside’ and ‘outside’ of prisons. Specifically, on the one hand, prison officers’ perspectives on dynamic security are considered to understand their views on this mode of surveillance. On the other hand, policies that respond to perceived insecurity in Italian society are examined to assess their impact on both the implementation of dynamic security and the return to static security. While the study acknowledges some officers’ resistance to dynamic security, it found that the return to static security can be traced primarily to the policies of the Italian government. They have involved an increase in the inmate population (of which a high percentage is foreign) and the enhancement of the neutralising (not rehabilitating) function of punishment; these factors likely prevented the establishment of positive relationships in prison, which are at the core of dynamic security.

Background and purpose: Clinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation. Method: Logistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number. Results: From March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p=0.005) and more in smoker patients (OR 1.39; p=0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p=0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p=0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p=0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p=0.05). Differences in frequencies between the three waves were found for flu-like symptoms (G1, p=0.024), joint or muscle pain (G2, p=0.013) and ageusia and anosmia (G5, p < 0.001). All cases should be referred to variants up to Delta. Conclusion: Several factors along with the choice of specific therapeutic approaches might have a different impact on the occurrence of some COVID-19 symptoms.

Historical masonry monuments, among which churches and religious buildings represent a conspicuous part in Europe, are prone to severe damage and even destruction in case of exceptional loadings as earthquakes. Seismic assessment and protection of such structures has thus become a high-priority goal as well as an exceptional challenge given the complex and interdisciplinary nature of the activities needed and the constraints represented by the safeguard of their outstanding cultural and historical values. In this paper, the majestic cathedral of St. Michael Arcangelo in the medieval village of Casertavecchia, Italy, is investigated from both historical and structural points of view. The church is the result of successive constructions, modifications and restorations, as well as transformations that occurred over the centuries. The construction of the cathedral started under the Norman domination in 1113, and the works were completed in 1153. The present study is addressed to the enhancement of the knowledge of this monument, through an in-depth historical analysis and a preliminary structural assessment by means of numerical modelling. Hence, documental research has been firstly performed to obtain information about its primary construction as well as any subsequent interventions occurring over time. An accurate geometric survey of the actual configuration has been carried out to detect the complex three-dimensional geometry of the structure. Furthermore, the structural constructive details and the degradation state of the structural elements have been surveyed and documented. Thanks to the collected data, a preliminary Finite Element model has been developed, with the aim of identifying the most vulnerable areas of the church and investigating its seismic response.

Large part of the historical heritage is characterised by the presence of timber elements for floors and roof, which need to sustain vertical load and comply with safety prescriptions of current building codes. One of the main stages in the assessment of historical timber structures is the evaluation of material properties. This is not a trivial task, as (i) timber is a complex material, (ii) the presence of defects strongly influences the overall bearing capacity of the element, and (iii) most times exhaustive destructive testing activities may not be feasible. In this respect, the use of resistographic non-destructive tests was proposed in past research as a means to estimate the strength of defect-free wood in longitudinal direction as well as a measure of the material local defectiveness. This approach is formalized in the present paper to derive a design procedure for the structural assessment of ancient timber structures. The procedure is applied to the case study of Croce di Lucca church in Naples. The evaluation of the bearing capacity of the roof structure by means of the proposed experimental methodology reveals significant structural issues in the system, in particular regarding the bottom tie beam.

- Abstract
INTRODUCTION: There is growing evidence on the efficacy of gait robotic rehabilitation in patients with multiple sclerosis (MS), but most of the studies have focused on gait parameters. Moreover, clear indications on the clinical use of robotics still lack. As part of the CICERONE Italian Consensus on Robotic Rehabilitation, the aim of this systematic review was to investigate the existing evidence concerning the role of lower limb robotic rehabilitation in improving functional recovery in patients with MS. EVIDENCE ACQUISITION: We searched for and systematically reviewed evidence-based studies on gait robotic rehabilitation in MS, between January 1st, 2010 and December 31st, 2020, in the following databases: Cochrane Library, PEDro, PubMed and Google Scholar. The study quality was assessed by the 16-item assessment of multiple systematic reviews 2 (AMSTAR 2) and the 10-item PEDro scale for the other research studies. EVIDENCE SYNTHESIS: After an accurate screening, only 17 papers were included in the review, and most of them (13 RCT) had a level II evidence. Most of the studies used the Lokomat as a grounded robotic device, two investigated the efficacy of end-effectors and two powered exoskeletons. Generally speaking, robotic treatment has beneficial effects on gait speed, endurance and balance with comparable outcomes to those of conventional treatments. However, in more severe patients (EDSS >6), robotics leads to better functional outcomes. Notably, after gait training with robotics (especially when coupled to virtual reality) MS patients also reach better non-motor outcomes, including spasticity, fatigue, pain, psychological well-being and quality of life. Unfortunately, no clinical indications emerge on the treatment protocols. CONCLUSIONS: The present comprehensive systematic review highlights the potential beneficial role on functional outcomes of the lower limb robotic devices in people with MS. Introduction: Stroke is the third leading cause of adult disability worldwide, and lower extremity motor impairment is one of the major determinants of long-term disability. although robotic therapy is becoming more and more utilized in research protocols for lower limb stroke rehabilitation, the gap between research evidence and its use in clinical practice is still significant. The aim of this study was to determine the scope, quality, and consistency of guidelines for robotic lower limb rehabilitation after stroke, in order to provide clinical recommendations. EVidEncE acQuisition: We systematically reviewed stroke rehabilitation guideline recommendations between January 1, 2010 and october 31, 2020. We explored electronic databases (n.=4), guideline repositories and professional rehabilitation networks (n.=12). two independent reviewers used the appraisal of Guidelines for research and Evaluation (aGrEE) ii instrument, and brief syntheses were used to evaluate and compare the different recommendations, considering only the most recent version. EVIDENCE SYNTHESIS: From the 1219 papers screened, ten eligible guidelines were identified from seven different regions/countries. Four of the included guidelines focused on stroke management, the other six on stroke rehabilitation. robotic rehabilitation is generally recommended to improve lower limb motor function, including gait and strength. unfortunately, there is still no consensus about the timing, frequency, training session duration and the exact characteristics of subjects who could benefit from robotics. conclusions: our systematic review shows that the introduction of robotic rehabilitation in standard treatment protocols seems to be the future of stroke rehabilitation. however, robot assisted gait training (raGt) for stroke needs to be improved with new solutions and in clinical practice guidelines, especially in terms of applicability.

- Abstract
Background and purpose: In relapsing-remitting multiple sclerosis patients (RRMS) disability progressively accumulates over time. To compare the cumulative probability of 6-month confirmed disability-worsening events using a fixed baseline or a roving Expanded Disability Status Scale (EDSS) reference, in a real-world setting. Methods: A cohort of 7964 RRMS patients followed for 2 or more years, with EDSS scores recorded every 6 months, was selected from the Italian Multiple Sclerosis Register. The overall probability of confirmed disability-worsening events and of confirmed disability-worsening events unrelated to relapse was evaluated using as reference a fixed baseline EDSS score or a roving EDSS score in which the increase had to be separated from the last EDSS assessment by at least 6 or 12 months. Results: Using a fixed baseline EDSS reference, the cumulative probability of 6-year overall confirmed disability-worsening events was 33.2%, and that of events unrelated to relapse was 10.9% (33% of overall confirmed disability-worsening events). Using a roving EDSS, the proportions were respectively 35.2% and 21.3% (61% of overall confirmed disability-worsening events). Conclusions: In a real-world setting, roving EDSS reference scores appear to be more sensitive for detecting confirmed disability-worsening events unrelated to relapse in RRMS patients.

- Abstract
Importance: Availability of new disease-modifying therapies (DMTs) and changes of therapeutic paradigms have led to a general improvement of multiple sclerosis (MS) prognosis in adults. It is still unclear whether this improvement also involves patients with pediatric-onset MS (POMS), whose early management is more challenging. Objective: To evaluate changes in the prognosis of POMS over time in association with changes in therapeutic and managing standards. Design, Setting, and Participants: Retrospective, multicenter, observational study. Data were extracted and collected in May 2019 from the Italian MS Registry, a digital database including more than 59000 patients. Inclusion criteria were MS onset before age 18 years, diagnosis before January 2014, and disease duration of at least 3 years. Exclusion criteria were primary progressive MS, Expanded Disability Status Scale (EDSS) score of at least 8 one year after onset, unavailability of diagnosis date, and less than 2 EDSS score evaluations. Eligible patients were 4704 patients with POMS. According to these criteria, we enrolled 3198 patients, excluding 1506. Exposures: We compared time to reach disability milestones by epoch of MS diagnosis (<1993, 1993-1999, 2000-2006, and 2007-2013), adjusting for possible confounders linked to EDSS evaluations and clinical disease activity. We then analyzed the difference among the 4 diagnosis epochs regarding demographic characteristics, clinical disease activity at onset, and DMTs management. Main Outcomes and Measures: Disability milestones were EDSS score 4.0 and 6.0, confirmed in the following clinical evaluation and in the last available visit. Results: We enrolled 3198 patients with POMS (mean age at onset, 15.2 years; 69% female; median time to diagnosis, 3.2 years; annualized relapse rate in first 1 and 3 years, 1.3 and 0.6, respectively), with a mean (SD) follow-up of 21.8 (11.7) years. 30. The Judicial Implementation of the Right to The Truth: Some Thoughts on the Argentinian Experience of the Juicios por la Verdad

- Abstract
Safeguarding the right to the truth has become crucial in dealing with systematic violations of human rights. Especially in contexts of transition to democracy, telling the truth is considered of utmost importance for fighting against impunity and promoting peace. Nevertheless, scholars have paid little attention to the judicial implementation of this right and, in particular, to the value of judicial protection of the right to the truth. The article aims to fill this gap by discussing the Juicios por la Verdad (the Truth Trials), a unique experience promoted by the Argentinian civil society in the wake of the military dictatorship. Specifically, it investigates the impact of the judicial recognition of the right to the truth on both the victims’ lives and society’s attempt to come to terms with the past. The analysis shows that the right to the truth may serve as a tool for knowledge, acknowledgment, strengthening the rule of law and, to an extent, for justice.

- Abstract
Background and purpose Some studies have shown that air pollution, often assessed by thin particulate matter with diameter below 2.5 mu g/m(3) (PM2.5), may contribute to severe COVID-19 courses, as well as play a role in the onset and evolution of multiple sclerosis (MS). However, the impact of air pollution on COVID-19 has never been explored specifically amongst patients with MS (PwMS). This retrospective observational study aims to explore associations between PM2.5 and COVID-19 severity amongst PwMS. Methods Data were retrieved from an Italian web-based platform (MuSC-19) which includes PwMS with COVID-19. PM2.5 2016-2018 average concentrations were provided by the Copernicus Atmospheric Monitoring Service. Italian patients inserted in the platform from 15 January 2020 to 9 April 2021 with a COVID-19 positive test were included. Ordered logistic regression models were used to study associations between PM2.5 and COVID-19 severity. Results In all, 1087 patients, of whom 13% required hospitalization and 2% were admitted to an intensive care unit or died, were included. Based on the multivariate analysis, higher concentrations of PM2.5 increased the risk of worse COVID-19 course (odds ratio 1.90; p = 0.009). Conclusions Even if several other factors explain the unfavourable course of COVID-19 in PwMS, the role of air pollutants must be considered and further investigated.

- Abstract
Background and purpose Clinical outcomes of multiple sclerosis (MS) patients affected by coronavirus disease 2019 (COVID-19) have been thoroughly investigated, but a further analysis on main signs and symptoms and their risk factors still needs attention. The objective of this study was to group together and describe based on similarity the most common signs and symptoms of COVID-19 in MS patients and identify all factors associated with their manifestation. Method Logistic and linear regression models were run to recognize factors associated with each pooled group of symptoms and their total number. Results From March 2020 to November 2021, data were collected from 1354 MS patients with confirmed infection of COVID-19. Ageusia and anosmia was less frequent in older people (odds ratio [OR] 0.98; p = 0.005) and more in smoker patients (OR 1.39; p = 0.049). Smoke was also associated with an incremental number of symptoms (OR 1.24; p = 0.031), substance abuse (drugs or alcohol), conjunctivitis and rash (OR 5.20; p = 0.042) and the presence of at least one comorbidity with shortness of breath, tachycardia or chest pain (OR 1.24; p = 0.008). Some disease-modifying therapies were associated with greater frequencies of certain COVID-19 symptoms (association between anti-CD20 therapies and increment in the number of concomitant symptoms: OR 1.29; p = 0.05). Differences in frequencies between the three waves were found for flu-like symptoms (G1, p = 0.024), joint or muscle pain (G2, p = 0.013) and ageusia and anosmia (G5, p < 0.001). All cases should be referred to variants up to Delta. Conclusion Several factors along with the choice of specific therapeutic approaches might have a different impact on the occurrence of some COVID-19 symptoms.

- Abstract
Background: The MuSC-19 project is an Italian cohort study open to international partners that collects data on multiple sclerosis (MS) patients with COVID-19. During the second wave of the pandemic, serological tests became routinely available. Objective: To evaluate the seroprevalence of anti-SARS-CoV-2 antibodies according to the use of disease-modifying therapy (DMT) in a subset of patients included in the MuSC-19 data set who had undergone a serological test. Methods: We evaluated the association between positive serological test results and time elapsed since infection onset, age, sex, Expanded Disability Status Scale score, comorbidities and DMT exposure using a multivariable logistic model. Results: Data were collected from 423 patients (345 from Italy, 61 from Turkey and 17 from Brazil) with a serological test performed during follow-up. Overall, 325 out of 423 tested patients (76.8%) had a positive serological test. At multivariate analysis, therapy with anti-CD20 was significantly associated with a reduced probability of developing antibodies after COVID-19 (odds ratio (OR) = 0.20, p = 0.002). Conclusion: Patients with MS maintain the capacity to develop humoral immune response against SARS-COV-2, although to a lesser extent when treated with anti-CD20 drugs. Overall, our results are reassuring with respect to the possibility to achieve sufficient immunization with vaccination.

This essay analyses the normative sources that outline the right to the truth and considers the case law on the subject. By looking first at the Inter-American system, specific attention is paid to the perspective adopted by the Strasbourg Court in reading the search for truth within the framework of the ECHR system. Moreover, the essay investigates the potential of this right as a catalyst of institutional trust in mature democracies. The paper concludes by welcoming the attitude of the European Court of Human Rights.

Background: The introduction of oral disease-modifying therapies (DMTs) for relapsing–remitting multiple sclerosis (RRMS) changed algorithms of RRMS treatment. Objectives: To compare the effectiveness of treatment with dimethyl fumarate (DMF) and teriflunomide (TRF) in a large multicentre Italian cohort of RRMS patients. Materials and Methods: Patients with RRMS who received treatment with DMF and TRF between January 1st, 2012 and December 31st, 2018 from twelve MS centers were identified. The events investigated were “time-to-first-relapse”, “time-to-Magnetic-Resonance-Imaging (MRI)-activity” and “time-to-disability-progression”. Results: 1445 patients were enrolled (1039 on DMF, 406 on TRF) and followed for a median of 34months. Patients on TRF were older (43.5 ± 8.6 vs 38.8 ± 9.2years), with a predominance of men and higher level of disability (p < 0.001 for all). Patients on DMF had a higher number of relapses and radiological activity (p 38DMF = 3.83, CI = 1.11 to 13.23, p = 0.033). Both DMTs controlled similarly MRI activity and disability progression. Conclusions: Patients on DMF had higher relapse-free survival time than TRF group after the first 38 months ontherapy.

Purpose: To evaluate if jugular vein flow restoration in various venographic defects indicative of chronic cerebrospinal venous insufficiency (CCSVI) in multiple sclerosis (MS) patients can have positive effects on cerebral lesions identified using magnetic resonance imaging (MRI). Materials and Methods: The Brave Dreams trial ( ClinicalTrials.gov identifier NCT01371760) was a multicenter, randomized, parallel group, double-blind, sham-controlled trial to assess the efficacy of jugular venoplasty in MS patients with CCSVI. Between August 2012 and March 2016, 130 patients (mean age 39.9±10.6 years; 81 women) with relapsing/remitting (n=115) or secondary/progressive (n=15) MS were randomized 2:1 to venography plus angioplasty (n=86) or venography (sham; n=44). Patients and study personnel (except the interventionist) were masked to treatment assignment. MRI data acquired at 6 and 12 months after randomization were compared to the preoperative scan for new and/or >30% enlargement of T2 lesions plus new gadolinium enhancement of pre-existing lesions. The relative risks (RR) with 95% confidence interval (CI) were estimated and compared. In a post hoc assessment, venograms of patients who underwent venous angioplasty were graded as “favorable” (n=38) or “unfavorable” (n=30) for dilation according to the Giaquinta grading system by 4 investigators blinded to outcomes. These subgroups were also compared. Results: Of the 130 patients enrolled, 125 (96%) completed the 12-month MRI follow-up. Analysis showed that the likelihood of being free of new cerebral lesions at 1 year was significantly higher after venoplasty compared to the sham group (RR 1.42, 95% CI 1.00 to 2.01, p=0.032). Patients with favorable venograms had a significantly higher probability of being free of new cerebral lesions than patients with unfavorable venograms (RR 1.82, 95% CI 1.17 to 2.83, p=0.005) or patients in the sham arm (RR 1.66, 95% CI 1.16 to 2.37, p=0.005). Conclusion: Expanded analysis of the Brave Dreams data that included secondary/progressive MS patients in addition to the relapsing/remitting patients analyzed previously showed that venoplasty decreases new cerebral lesions at 1 year. Post hoc analysis confirmed the efficacy of the Giaquinta grading system in selecting patients appropriate for venoplasty who were more likely to be free from accumulation of new cerebral lesions at MRI.

- Abstract
Masonry structures represent large part of the historical and cultural built heritage in Europe. The assessment of their structural behaviour is critical for the preservation and conservation of buildings, monuments and infrastructures especially in seismic areas. In particular, one relevant issue is the evaluation of damage and residual load-bearing capacity of historical masonry structures affected by earthquakes. Such evaluation is necessary to assess structural safety, in order to define posteriori actions such as evacuation, retrofitting or demolition. A successful technique aimed at better knowing the dynamical behaviour of a structure is Operational Modal Analysis (OMA), in which the modal properties of the system are identified based on vibration data collected when the structure is under operating conditions. In Structural Health Monitoring (SHM) analysis, any variation of such modal properties could be considered as an indicator of modifications induced in the structure, such as change in mass or stiffness due to environmental agents or development and propagation of damage. The identification of the location, the type and the severity of the damage entails the solution of a further inverse problem, in which modal data, possibly coupled to other sources of information, are exploited for the assessment of such damage. In this paper, a novel comprehensive procedure is proposed for the identification of damage in masonry structures after a seismic event. The procedure relies on the acquisition of operational vibration data before and after an earthquake. In a first phase, those data are analysed by means of Stochastic Subspace Identification (SSI) to estimate frequencies and modal shapes of the linear system in undamaged and damaged conditions respectively. Afterwards, such modal properties are used as input for the inverse problem of estimating material parameters for an approximate numerical model representing the structure. The proposed methodology is applied to a pseudo-experimental case study in which the acceleration data are provided as the output of a mesoscale numerical representation of a 3D masonry building. The same building is then modelled with a macroscale approach using a homogenous isotropic model, whose damaged and undamaged features are identified by means of inverse analysis against the SSI output. The results show the potential of the proposed approach, where simple and non-destructive measurements are coupled to advanced computational methodologies to provide a comprehensive framework for damage identification after seismic events.

- Abstract
The response of masonry structures under seismic loading is very complex, as it is governed by the anisotropic and quasi-brittle nature of masonry. In recent past, this has driven the development of mechanical descriptions at different scales of representation, including micro-, meso- and macro-scale models and multi-scale approaches. In general micro- and meso-scale descriptions enable an accurate prediction of crack patterns and damage-induced anisotropy, as they explicitly allow for masonry bond and the interaction between units and mortar joints. Nevertheless, at present their use is restricted to the simulation of the nonlinear response up to collapse of masonry components like walls and arches, but not entire building structures because of the excessive computational cost required. Thus, in the analysis of realistic masonry structures, macro-models representing masonry as a continuum material are usually employed as they are much more computationally efficient. However, accurate response predictions entails complex material formulations calibrated on the physical response at structural component level or on more expensive micro or mesoscale models. In most previous macroscale models, masonry is modelled by using 2D representations, focusing on the monotonic response up to collapse. Moreover, very few masonry macroscale models allow for strength and stiffness degradations under cyclic loading, which strongly affect the behaviour when masonry structures are subjected to earthquakes. Isotropic descriptions are often utilised in practice, as they are generally simpler and computationally robust. However, they may not be representative of masonry characterised by regular bond patterns. In this paper, a novel 3D material model for the nonlinear analysis of masonry structures under seismic loading is described. The model is based on uncoupled damage and plasticity, which guarantees computational efficiency and robustness at the local level. The elastic behaviour and yield surface account for the orthotropic behaviour linked to the specific regular bond pattern, where stiffness and strength degradation and opening-closure of cracks are accurately represented. The potential of the proposed macroscale modelling strategy is shown in several numerical examples on masonry walls and building structures under earthquake loading.

- Abstract
Tuned Mass Dampers (TMDs) are aimed at mitigating vibrations of a structure under seis-mic or wind excitation by tuning the characteristics of the device to control specific reso-nance frequencies of the structure. However, the vibration modes of a structure can be considerably altered by the interaction with soil, leading to a loss of efficiency of the de-vice. This paper shows the preliminary results of a study aimed at formulating a methodology for the design of TMDs accounting for soil-structure interaction. Taking as a reference an illustrative case study of a timber building equipped with a TMD, the results of a paramet-ric study on the effects of soil-structure interaction are presented. This is accomplished through finite elements simulations in which soil-structure interaction is described by dy-namic impedance functions, in order to have computationally efficient models to study the properties of the soil-structure system. The results are expressed in terms of non-dimensional performance curves of the TMD accounting for soil-structure interaction. The performance curve describes the progressive decay of the TMD efficiency as a function of soil-structure relative stiffness, highlighting the main features of the response of the soil-structure system. This also allowed a clear quantification of the relative contributions of soil stiffness and TMD to the attenuation of the structural displacements.

Although in LOPD, as in other neuromuscular diseases, controlled low impact sub-maximal aerobic exercise and functional ability exercise can improve general functioning and quality of life, as well as respiratory rehabilitation, the bulk of evidence on that is weak and guidelines are lacking. To date, there is no specific focus on rehabilitation issues in clinical recommendations for the care of patients with Pompe disease, and standard practice predominantly follows general recommendation guidelines for neuromuscular diseases. The Italian Association of Myology, the Italian Association of Pulmonologists, the Italian Society of Neurorehabilitation, and the Italian Society of Physical Medicine and Rehabilitation, have endorsed a project to formulate recommendations on practical, technical, and, whenever possible, disease-specific guidance on rehabilitation procedures in LOPD, with specific reference to the Italian scenario. In this first paper, we review available evidence on the role of rehabilitation in LOPD patients, particularly addressing the unmet needs in the management of motor and respiratory function for these patients. - Abstract
Background and purpose: In relapsing-remitting multiple sclerosis patients (RRMS) disability progressively accumulates over time. To compare the cumulative probability of 6-month confirmed disability-worsening events using a fixed baseline or a roving Expanded Disability Status Scale (EDSS) reference, in a real-world setting. Methods: A cohort of 7964 RRMS patients followed for 2 or more years, with EDSS scores recorded every 6 months, was selected from the Italian Multiple Sclerosis Register. The overall probability of confirmed disability-worsening events and of confirmed disability-worsening events unrelated to relapse was evaluated using as reference a fixed baseline EDSS score or a roving EDSS score in which the increase had to be separated from the last EDSS assessment by at least 6 or 12 months. Results: Using a fixed baseline EDSS reference, the cumulative probability of 6-year overall confirmed disability-worsening events was 33.2%, and that of events unrelated to relapse was 10.9% (33% of overall confirmed disability-worsening events). Using a roving EDSS, the proportions were respectively 35.2% and 21.3% (61% of overall confirmed disability-worsening events). Conclusions: In a real-world setting, roving EDSS reference scores appear to be more sensitive for detecting confirmed disability-worsening events unrelated to relapse in RRMS patients.

- Abstract
Background: The introduction of oral disease-modifying therapies (DMTs) for relapsing–remitting multiple sclerosis (RRMS) changed algorithms of RRMS treatment. Objectives: To compare the effectiveness of treatment with dimethyl fumarate (DMF) and teriflunomide (TRF) in a large multicentre Italian cohort of RRMS patients. Materials and Methods: Patients with RRMS who received treatment with DMF and TRF between January 1st, 2012 and December 31st, 2018 from twelve MS centers were identified. The events investigated were “time-to-first-relapse”, “time-to-Magnetic-Resonance-Imaging (MRI)-activity” and “time-to-disability-progression”. Results: 1445 patients were enrolled (1039 on DMF, 406 on TRF) and followed for a median of 34 months. Patients on TRF were older (43.5 ± 8.6 vs 38.8 ± 9.2 years), with a predominance of men and higher level of disability (p < 0.001 for all). Patients on DMF had a higher number of relapses and radiological activity (p <.05) at baseline. Time-varying Cox-model for the event “time-to-first relapse” revealed that no differences were found between the two groups in the first 38 months of treatment (HRt < 38DMF = 0.73, CI = 0.52 to 1.03, p = 0.079). When the time-on-therapy exceeds 38 months patients on DMF had an approximately 0.3 times lower relapse hazard risk than those who took TRF (HRt>38DMF = 3.83, CI = 1.11 to 13.23, p = 0.033). Both DMTs controlled similarly MRI activity and disability progression. Conclusions: Patients on DMF had higher relapse-free survival time than TRF group after the first 38 months ontherapy.

- Abstract
The paper presents an effective strategy for creating realistic 3D microscale finite element meshes for masonry components with generic bond. Microscale masonry modelling, which considers separate representations for masonry units and mortar joints, offers an accurate description of masonry components providing that the actual masonry bond is properly represented. This may be problematic in the case of rubble masonry, where stone units are often randomly assembled and connected by highly irregular mortar joints. The proposed dis-cretisation strategy utilises a vectorisation procedure retrieving the basic geometry of a raster image of the analysed masonry component, which reveals the geometry of each stone block on the external face of the element. A Matlab script properly developed utilises proprietary image processing tools and considers the raster image as the input parameter, providing a numerical description of the external bond. It comprises the position of the vertex of the stone units with a specific level of simplification (e.g. number of vertexes for each unit) which influences the refinement of the finite element mesh. The developed script is linked to the automatic mesh generator Gmsh which creates 3D finite element meshes for both mortar and stone units. Numerical examples are presented, where nonlinear simulations of a rubble masonry test-window are performed using ADAPTIC, a nonlinear finite element code. The numerical results confirm the potential of the proposed meshing strategy to obtain accurate response predictions of rubble masonry components using microscale modelling.

Detailed mesoscale models enable realistic response predictions of masonry structures subjected to different loading conditions. The accuracy of the numerical predictions strongly depends upon the calibration of the model material parameters, which is usually conducted at the level of masonry constituents. However, especially for existing structures testing of individual components can be difficult or unreliable. In this work, an innovative approach for the calibration of a mesoscale masonry representation is proposed. It is based on the inverse analysis of the results of physical in situ tests performed using an innovative setup with flat-jacks. The post-processing inverse procedure comprises (i) metamodeling as a replacement of expensive nonlinear simulations, (ii) sensitivity analysis to reduce the parameters to identify to those which effectively control the recorded response, and (iii) optimisation by means of Genetic Algorithms to find the best fitting model parameter set. The potential of the proposed calibration procedure is shown considering the response of masonry components tested in laboratory following the proposed in-situ test.
