Abstract Loss of ARID1A has been reported to drive the progression of lung adenocarcinoma, yet the underlying mechanism remains elusive. In this study, we performed secretome analysis to identify the key secreted proteins regulating lung adenocarcinoma progression. We showed that the VASN level was significantly elevated in the conditioned medium from ARID1A-depleted A549 and H1299 cells. Restoration of ARID1A in ARID1A-depleted lung adenocarcinoma cells prevented the upregulation and secretion of VASN. Clinical analysis demonstrated a negative correlation between ARID1A and VASN expression in ARID1A-mutated lung adenocarcinomas. The patients with ARID1A-mutated lung adenocarcinoma had significantly higher concentrations of serum VASN than healthy controls. Moreover, serum VASN concentrations were associated with TNM stage, lymph node metastasis, and overall survival of the patients with ARID1A-mutated lung adenocarcinoma. Functional studies indicated that VASN overexpression potentiated the proliferation, invasion, and tumorigenesis of lung adenocarcinoma cells. Antibody neutralization of VASN suppressed the aggressiveness of ARID1A-depleted lung adenocarcinoma cells both in vitro and in vivo. Addition of recombinant VASN protein promoted the proliferation and invasion of lung adenocarcinoma cells. Additionally, knockdown of Notch1 blocked the aggressive phenotype induced by recombinant VASN protein. In conclusion, our data uncover the role of VASN in mediating the progression of ARID1A-depleted lung adenocarcinoma and highlight VASN as a promising therapeutic target for this disease.

BackgroundCarpal tunnel syndrome (CTS) is a common symptom of nerve compression and a leading cause of pain and hand dysfunction. However, the underlying biological mechanisms are not fully understood. The aim of this study was to reveal the causal effect of circulating metabolites on susceptibility to CTS.MethodsWe employed various Mendelian randomization (MR) methods, including Inverse Variance Weighted, MR-Egger, Weighted Median, Simple Mode, and Weighted Model, to examine the association between 1,400 metabolites and the risk of developing CTS. We obtained Single-nucleotide polymorphisms (SNPs) associated with 1,400 metabolites from the Canadian Longitudinal Study on Aging (CLSA) cohort. CTS data was derived from the FinnGen consortium, which included 11,208 cases and 1,95,047 controls of European ancestry.ResultsThe results of the two-sample MR study indicated an association between 77 metabolites (metabolite ratios) and CTS. After false discovery rate (FDR) correction, a strong causal association between glucuronate levels (odd ratio (OR) [95% CI]: 0.98 [0.97–0.99], p FDR = 0.002), adenosine 5′-monophosphate (AMP) to phosphate ratio (OR [95% CI]:0.58 [0.45–0.74], p FDR = 0.009), cysteinylglycine disulfide levels (OR [95% CI]: 0.85 [0.78–0.92], p FDR = 0.047) and CTS was finally identified.ConclusionIn summary, the results of this study suggest that the identified glucuronate, the ratio of AMP to phosphate, and cysteinylglycine disulfide levels can be considered as metabolic biomarkers for CTS screening and prevention in future clinical practice, as well as candidate molecules for future mechanism exploration and drug target selection.

Abstract Aims The present study aimed to phenotype the cerebral structural and glucose metabolic alterations in patients with heart failure (HF) using simultaneous positron emission tomography (PET)/magnetic resonance (MR) and to investigate their relationship to cardiac biomarkers and cognitive performance. Methods and results Forty‐two HF patients caused by ischaemic heart disease (mean age 67.2 ± 10.4, 32 males) and 32 age‐ and sex‐matched healthy volunteers (mean age 61.3 ± 4.8, 18 males) were included in this study. Participants underwent simultaneous cerebral fluorine‐18 (18F) fluorodeoxyglucose PET/MR followed by cardiac MR scan, and neuropsychological scores were obtained to assess cognitive performance. The grey matter volume (GMV) and standardized uptake value ratio (SUVR) were calculated to examine cerebral structural and metabolic alterations. Cardiac biomarkers included cardiac MR parameters and cardiac serum laboratory tests. Background: Myocardial segmental motion is associated with cardiovascular pathology, often assessed through myocardial strain features. The stability of the motion can be influenced by myocardial fibrosis. This research aimed to explore the complexity metrics (CM) of myocardial segmental motion curves, observe their correlation with late gadolinium enhancement (LGE) transmural extension (TE), and assess diagnostic efficacy combined with segmental strains in different TE segments. Methods: We included 42 myocardial infarction patients, dividing images into 672 myocardial segments (208 remote, 384 viable, and 80 unviable segments based on TE). Radial and circumferential segmental strain, along with CM for motion curves, were extracted. Correlation between CM and LGE, as well as the potential distinguishing role of CM, was evaluated using Pearson correlation, univariate linear regression (F-test), multivariate regression analysis (T-test), area under curve (AUC), machine learning models, and DeLong test. ObjectiveTo investigate the protective mechanism of ganoderma acid A against α-amanitin-induced liver injury in mice. MethodsForty Kunming mice were divided into 4 groups intraperitoneally injected with solution (10 mL/kg) containing α-amanitin at concentrations of 0.0, 0.1, 0.2 and 0.3 mg/kg to establish intoxicated mouse model. Another sixty Kunming mice were assigned into a blank control group, a α-amanitin intoxicated group, and four α-amanitin intoxicated groups administered with ganoderma acid A at dosages of 5, 10, 20, 40 mg/kg every 6 hours totally 4 times; the blank control group and the α-amanitin intoxicated group were treated with equal amount of normal saline. All the mice were sacrificed 48 hours after the first treatment. Serum alanine transaminase (ALT), aspartate transaminase (AST), alkaline phosphatase (ALP) and glutamyl transferase (GGT) were detected and hepatic reactive oxygen species (ROS), interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α), cyclooxygenase-2 (COX-2), hepatic rat sarcoma (RAS) protein, nuclear factor kappa B (NF-κB), tumor necrosis factor receptor superfamily member 5 (TNFRSF5) protein were also determined. ResultsCompared with those of the blank control mice, significantly increased serum ALT, AST, ALP, GGT, hepatic expressions of ROS, IL-8, TNF-α, COX-2, rat sarcoma (RAS) protein (0.08 ± 0.02 vs. 12.55 ± 0.54), NF-κB (0.07 ± 0.01 vs. 10.58 ± 0.78), TNFRSF5 (0.09 ± 0.01 vs. 11.05 ± 0.83) protein were detected in the α-amanitin intoxicated mice (P < 0.05 for all). In comparison with the untreated α-amanitin intoxicated mice, the intoxicated mice administered with ganoderma acid A had significantly decreased serum ALT, AST, ALP, GGT and hepatic expressions of ROS, IL-8, TNF-α, COX-2 (all P < 0.05); the intoxicated mice with ganoderma acid A treatment also had dose-dependent decreased hepatic expressions of RAS protein (10.58 ± 0.89, 8.35 ± 0.73, 5.54 ± 0.44, and 3.74 ± 0.29 vs. 12.55 ± 0.54), NF-κB (8.53 ± 0.63, 6.82 ± 0.62, 4.24 ± 0.32, and 2.78 ± 0.21 vs. 10.58 ± 0.78), and TNFRSF5 (9.74 ± 0.78, 7.79 ± 0.71, 3.10 ± 0.34, and 2.81 ± 0.23 vs. 11.05 ± 0.83) (P < 0.05 for all). ConclusionGanoderma acid A can inhibit oxidative stress and inflammatory response to alleviate liver injury caused by α-amanitin in mice and the mechanism of the effect may be related to the activation of liver RAS/NF-κB/TNFRSF5 protein signaling pathway inhibited by ganoderma acid A.

Change detection based on optical image processing plays a crucial role in the field of damage assessment. Although existing damage scene change detection methods have achieved some good results, they are faced with challenges, such as low accuracy and slow speed in optical image change detection. To solve these problems, an image change detection approach that combines infrared polarization imaging with a fast principal component analysis network (Fast-PCANet) is proposed. Firstly, the acquired infrared polarization images are analyzed, and pixel image blocks are extracted and filtered to obtain the candidate change points. Then, the Fast-PCANet network framework is established, and the candidate pixel image blocks are sent to the network to detect the change pixel points. Finally, the false-detection points predicted by the Fast-PCANet are further corrected by region filling and filtering to obtain the final binary change map of the damage scene. Comparisons with typical PCANet-based change detection algorithms are made on a dataset of infrared-polarized images. The experimental results show that the proposed Fast-PCANet method improves the PCC and the Kappa coefficient of infrared polarization images over infrared intensity images by 6.77% and 13.67%, respectively. Meanwhile, the inference speed can be more than seven times faster. The results verify that the proposed approach is effective and efficient for the change detection task with infrared polarization imaging. The study can be applied to the damage assessment field and has great potential for object recognition, material classification, and polarization remote sensing.

Background: The aim of this study was to exploit integrated PET/MRI to simultaneously evaluate the morphological, component, and metabolic features of advanced atherosclerotic plaques and explore their incremental value. Methods: In this observational prospective cohort study, patients with advanced plaque in the carotid artery underwent 18F-FDG PET/MRI. Plaque morphological features were measured, and plaque component features were determined via MRI according to AHA lesion-types. Maximum standardized uptake values (SUVmax) and tissue to background ratio (TBR) on PET were calculated. Area under the receiver-operating characteristic curve (AUC) and net reclassification improvement (NRI) were used to compare the incremental contribution of FDG uptake when added to AHA lesion-types for symptomatic plaque classification. Results: A total of 280 patients with advanced plaque in the carotid artery were recruited. A total of 402 plaques were confirmed, and 87 of 402 (21.6%) were symptomatic plaques. 18F-FDG PET/MRI was performed a mean of 38 days (range 1–90) after the symptom. Increased stenosis degree (61.5% vs. 50.0%, p < 0.001) and TBR (2.96 vs. 2.32, p < 0.001) were observed in symptomatic plaques compared with asymptomatic plaques. The performance of the combined model (AHA lesion type VI + stenosis degree + TBR) for predicting symptomatic plaques was the best among all models (AUC = 0.789). The improvement of the combined model (AHA lesion type VII + stenosis degree + TBR) over AHA lesion type VII model for predicting symptomatic plaques was the highest (AUC = 0.757/0.454, combined model/AHA lesion type VII model), and the NRI was 50.7%. Conclusions: Integrated PET/MRI could simultaneously evaluate the morphological component and inflammation features of advanced atherosclerotic plaques and provide supplementary optimization information over AHA lesion-types for identifying vulnerable plaques in atherosclerosis subjects to achieve further stratification of stroke risk.

The Moon is the primary target of human deep space exploration. The internal structure of the Moon is an essential window for us to understand the formation and evolution of the Moon and the Earth-Moon system. Compared with other geophysical fields, the gravity field model of the Moon plays an important role in the investigating of its interior structure because of its global uniform coverage and continuous improvements on its resolution and accuracy. This paper addresses the scientific applications of the lunar gravity field to investigate the Moon's internal structure. The role played by the lunar gravity data in lunar internal structure study is discussed, including the crust, mantle and lunar core, with emphasis on the contribution of higher degree gravity field models to the study of shallow structures. The remaining problems associated with the internal structure's inversion using lunar gravity data are also analyzed in the paper. Finally, we looked forward to the development of lunar gravity detection techniques and the prospects for future applications of gravity field to study the lunar interior structure.

α-Amanitin is a representative toxin found in the Amanita genus of mushrooms, and the consumption of mushrooms containing α-Amanitin can lead to severe liver damage. In this study, we conduct toxicological experiments to validate the protective effects of Ganoderic acid A against α-amanitin-induced liver damage. By establishing animal models with different durations of Ganoderic acid A treatment and conducting a metabolomic analysis of the serum samples, we further confirmed the differences in serum metabolites between the AMA+GA and AMA groups. The analysis of differential serum metabolites after the Ganoderic acid A intervention suggests that Ganoderic acid A may intervene in α-amanitin-induced liver damage by participating in the regulation of retinol metabolism, tyrosine and tryptophan biosynthesis, fatty acid biosynthesis, sphingosine biosynthesis, spermidine and spermine biosynthesis, and branched-chain amino acid metabolism. This provides initial insights into the protective intervention mechanisms of GA against α-amanitin-induced liver damage and offers new avenues for the development of therapeutic drugs for α-Amanitin poisoning.

Wide-bandgap gallium nitride (GaN)-based semiconductors offer significant advantages over traditional Si-based semiconductors in terms of high-power and high-frequency operations. As it has superior properties, such as high operating temperatures, high-frequency operation, high breakdown electric field, and enhanced radiation resistance, GaN is applied in various fields, such as power electronic devices, renewable energy systems, light-emitting diodes, and radio frequency (RF) electronic devices. For example, GaN-based high-electron-mobility transistors (HEMTs) are used widely in various applications, such as 5G cellular networks, satellite communication, and radar systems. When a current flows through the transistor channels during operation, the self-heating effect (SHE) deriving from joule heat generation causes a significant increase in the temperature. Increases in the channel temperature reduce the carrier mobility and cause a shift in the threshold voltage, resulting in significant performance degradation. Moreover, temperature increases cause substantial lifetime reductions. Accordingly, GaN-based HEMTs are operated at a low power, although they have demonstrated high RF output power potential. The SHE is expected to be even more important in future advanced technology designs, such as gate-all-around field-effect transistor (GAAFET) and three-dimensional (3D) IC architectures. Materials with high thermal conductivities, such as silicon carbide (SiC) and diamond, are good candidates as substrates for heat dissipation in GaN-based semiconductors. However, the thermal boundary resistance (TBR) of the GaN/substrate interface is a bottleneck for heat dissipation. This bottleneck should be reduced optimally to enable full employment of the high thermal conductivity of the substrates. Here, we comprehensively review the experimental and simulation studies that report TBRs in GaN-on-SiC and GaN-on-diamond devices. The effects of the growth methods, growth conditions, integration methods, and interlayer structures on the TBR are summarized. This study provides guidelines for decreasing the TBR for thermal management in the design and implementation of GaN-based semiconductor devices.

Background: Blunt cerebrovascular injuries are rare and serious complications of trauma that can lead to cerebral vascular dissection or aneurysm. Improving awareness of blunt cerebrovascular injuries and using computed tomography angiography to pre-screen patients at high risk is recommended by current guidelines to prevent the occurrence of ischemic stroke complications. Case description: A 32-year-old male patient was admitted to the hospital following neck trauma accompanied by stroke symptoms. Imaging revealed intimal injury of the right common carotid artery and acute cerebral infarction. After an endarterectomy and repair, the vascular lumen obstruction resolved, blood flow was restored, and the patient's condition stabilized. Conclusion: Blunt cerebrovascular injury has been seriously neglected in clinical practice. Delayed or under-diagnosis of blunt cerebrovascular injury can result large strokes. Standardized treatment protocols, which include the screening and grading of blunt cerebrovascular injury, may reduce the risk of permanent neurological dysfunction, and even death, in patients.

Spontaneous internal carotid artery dissection (ICAD) is a rare disease and an important cause of stroke in young patients. The presentation of ICAD is variable and atypical. We have used three-dimensional T1-weighted volume isotropic turbo spin-echo acquisition to help diagnose ICAD. ICAD should be considered in young patients presenting with relevant symptoms in an emergency setting. We herein report ICAD with an unexplained onset in two patients. Neither had a history of large or small cervical trauma, but both had a history of hypertension. The first patient was a 33-year-old man who presented with upper extremity numbness and mobility impairment, and the second patient was a 40-year-old man with onset of visual impairment. There were no obvious triggers in either case. Both patients were diagnosed with ICAD by blood vessel examination. However, their prognoses differed. The first patient recovered after endovascular therapy, whereas the second patient was successfully managed with medical treatment. ICAD can cause different symptoms depending on where the dissection occurs. Clinicians must be aware of the different clinical manifestations of ICAD, make the correct diagnosis in a timely manner, make appropriate treatment plans according to the patient’s condition, and strive for timely recanalization of the blood vessels.

Phase-Change Materials (PCMs) are widely applied in dynamic optical modulation due to the dramatic changes in their complex refractive index caused by temperature variation. As the functionality varies, the application of a single PCM cannot meet the compact, efficient and broadband needs of optical modulators. In this work, we combine vanadium dioxide (VO2) and a chalcogenide (Ge2Sb2Te5 (GST) or In3SbTe2 (IST)) to obtain a VO2–GST/IST multiple-stack film that is optimized by a genetic algorithm. This film has a wide spectrum and high modulation properties with three self-switchable modes varied by temperature, including transmission, absorption and reflection. The optimal results are an average normal transmittance, absorbance, and reflectance of 0.76, 0.91, 0.86 in 3–5 μm and 0.72, 0.90, 0.90 in 8–14 μm under different temperature ranges. The film enhances the transmission and absorption properties due to the formation of anti-reflective coating and Fabry–Perot resonance. Compared with GST, the film maintains high reflectance due to the metal-like interface reflection of crystalline IST, which exhibits metallic properties. For different polarization states, the film demonstrates great directional insensitivity when the incidence angles vary from 0° to 60°. The designed self-adaptive multistage infrared radiative thermo-optic modulator has promising implications for optical fuse, fiber-optic communication and energy storage fields.

Abstract Background The aim of the study was to explore the relationship between vitamin D status and islet function in patients with type 2 diabetes mellitus. Methods The participants were recruited from Hebei General Hospital. Basic characteristics and blood indicators were collected after fasting overnight. The data were analyzed statistically using SPSS 22.0. Analysis of variance, a nonparametric test, or a trend Chi-square test was used for the comparisons. The association between 25-hydroxy vitamin D and modified homeostasis model assessment-β was assessed using multivariate ordinal logistic regression. Results One hundred seventy-four patients aged 26 to 79 years with type 2 diabetes mellitus were included in this study. Patients with vitamin D deficiency had a lower modified homeostasis model assessment-β level compared with those without vitamin D deficiency. There were differences in body mass index, diabetes course, glycosylated hemoglobin, fasting blood glucose, fasting blood C-peptide, triglyceride, and 25-hydroxy vitamin D among different modified homeostasis model assessment-β groups based upon the tertiles. 25-hydroxy vitamin D, as continuous or categorical variables, was positively related to modified homeostasis model assessment-β whether or not cofounding factors were adjusted. Conclusion There is an association between increased 25-hydroxy vitamin D levels and improvement in modified homeostasis model assessment-β function in patients with type 2 diabetes mellitus. Backgroundparenchymal hematoma (PH) is a severe complication of endovascular treatment (EVT) for acute basilar artery occlusion (ABAO). This study aimed to evaluate the incidence and predictors of PH after EVT for ABAO.MethodsUsing data from the Endovascular Treatment for Acute Basilar Artery Occlusion Study, we enrolled patients treated with mechanical thrombectomy from the BASILAR registry. PH was assessed in accordance with the Heidelberg Bleeding Classification. Logistic regression was used to identify predictors of PH.ResultsA total of 639 patients were included. Forty-eight patients (7.5%) were diagnosed with PH within 48 h of EVT. Ninety-day mortality was higher in patients with PH compared with those without (81.3 vs. 42.8%, P < 0.001). Favorable neurological outcomes (modified Rankin scale score, 0–3) rates was lower in patients with PH compared with those without (6.3 vs. 34.5%, P < 0.001). With a multivariate analysis, hypertension [odds ratio (OR) = 2.30, 95% confidence interval (CI) 1.04–5.08], pre-treatment National Institutes of Health Stroke Score (NIHSS, >25; OR = 3.04, 95% CI 1.43–6.45), and Neutrophil-to-lymphocyte ratio (NLR, >10; OR = 1.88, 95% CI 1.02–3.48) were associated with PH after EVT.ConclusionsPH occurred at a rate of 7.5% after EVT in patients with ABAO. Hypertension, higher baseline NIHSS, and higher NLR value increase the risk of PH after EVT for ABAO.

For the large-scale network mobile edge computing and caching technology of future 6G mobile communications, firstly, the architectures and principles of mobile edge computing and caching in large-scale wireless networks were introduced, and the necessity and universality were clarified.Then, from the perspective of the five key issues in the mobile edge computing and caching enabled large-scale wireless network, including computing offloading, edge caching, multi-dimensional resource allocation, user association and privacy protection, the recent researches and further pointed out the future development trends and research directions were reviewed and analyzed.Finally, for the privacy preservation issue, a federated learning based privacy-preserving scheme was proposed.Simulation results show that the proposed scheme can simultaneously preserve user privacy and improve the quality of service.

Abstract Total hip arthroplasty and total knee arthroplasty are extensively used for the treatment of the end-stage degenerative joint diseases. Currently, periprosthetic bone loss is still the major cause of aseptic loosening, resulting in implant failures. Previous literature introduced some widely accepted protocols for the prevention and treatment of periprosthetic bone loss, but no guideline has been proposed. Denosumab, a human monoclonal immunoglobulin G2 (IgG2) antibody, can inhibit bone resorption by binding to the receptor activator of nuclear factor kappa-B ligand (RANKL). This article reviews the present findings and evidence concerning the effect of denosumab on the periprosthetic bone loss after total hip arthroplasty and total knee arthroplasty. Overall, the current evidence suggests that denosumab is a promising agent for the treatment of periprosthetic bone loss.

Abstract Background Currently, thoracoscopic lobectomy is widely used in clinical practice, and postoperative placement of ultrafine drainage tube has advantages of reducing postoperative pain and accelerating postoperative recovery in patients. This study aimed to investigate the feasibility and safety of placement of 8F ultrafine chest drainage tube after thoracoscopic lobectomy and its superiority over traditional 24F chest drainage tube. Methods A retrospective data analysis was conducted in 169 patients who underwent placement of 8F ultrafine chest drainage tube or 24F chest drainage tube with thoracoscopic lobectomy for lung cancer from January 2018 to December 2019. Propensity score matching (PSM) was used to reduce bias between the experimental group and the control group. After PSM, 134 patients (67 per group) were enrolled. The drainage time, the total drainage volume, postoperative hospital stay, postoperative pain score and postoperative complication of both groups were analyzed and compared. Results Compared to group B, group A had lower pain scores on postoperative days 1, 2 and 3 (3.72 ± 0.65point vs 3.94 ± 0.67point, P = 0.027; 2.72 ± 0.93point vs 3.13 ± 1.04point, P = 0.016; and 1.87 ± 0.65point vs 2.39 ± 1.22point, P = 0.005), shorter drainage time (4.25 ± 1.79d vs 6.04 ± 1.96d, P = 0.000), fewer drainage volume (1100.42 ± 701.57 ml vs 1369.39 ± 624.25 ml, P = 0.021); and shorter postoperative hospital stay (8.46 ± 2.48d vs 9.37 ± 1.70d, P = 0.014). Postoperative complications such as subcutaneous emphysema, pulmonary infection, atelectasis, chest tube reinsertion and intrathoracic hemorrhage showed no differences between both groups (P > 0.05). Conclusion Compared with 24F chest drainage tube, the application of an 8F ultrafine chest drainage tube after thoracoscopic lobectomy has significantly shortened the drainage time, reduced the total drainage volume, reduced the postoperative pain degree, shortened the hospital day, and effectively detected postoperative intrathoracic hemorrhage. So, it is considered as an effective, safe and reliable drainage method.

Abstract Objectives The purpose of this study was to investigate the impact of single nucleotide polymorphisms (SNPs) in the ANGPTL4 gene and the SNP–SNP interactions on atherosclerotic ischemic stroke (IS) risk. Patients and methods A case-control study was conducted. A total of 360 patients with atherosclerotic IS and 342 controls between December 2018 and December 2019 from Longyan First Hospital affiliated to Fujian Medical University were included. A logistic regression model was used to examine the association between SNPs and atherosclerotic IS risk. Odds ratios (ORs) and 95% confidence intervals (95% CIs) were calculated. Generalized multifactor dimensionality reduction was employed to analyze the SNP-SNP interaction. Results Logistic regression analysis showed that atherosclerotic IS risk was significantly lower in carriers with the rs11672433-T allele than those with the CC genotype (CT+ TT vs. CC); adjusted OR, 0.005; 95% CI, 0.02–0.11. We found a significant 2-locus model (P = 0.0010) involving rs11672433 and rs4076317; the cross-validation consistency of this model was 10 of 10, and the testing accuracy was 57.96%. Participants with the CT or TT of rs11672433 and CC of rs4076317 genotype have the lowest atherosclerotic IS risk, compared to subjects with CC of rs11672433 and the CC of rs4076317 genotype, OR (95%CI) was 0.06(0.02–0.22), after covariates adjustment for gender, age, smoking and alcohol status, hypertension, Diabetes mellitus, TG, TC, HDL-C, LDL-C, Uric acid. Conclusions We found that rs11672433 was associated with decreased atherosclerotic IS risk; we also found that gene–gene interaction between rs11672433 and rs4076317 was associated with decreased atherosclerotic IS risk.

Abstract Objectives To investigate the association of eight variants of four matrix metalloproteinase (MMP) genes with ischemic stroke (IS) and whether interactions among these single nucleotide polymorphisms (SNPs) increases the risk of IS. Methods Among 547 patients with ischemic stroke and 350 controls, matrix‐assisted laser desorption/ionization time of flight mass spectrometry was used to examine eight variants arising from four different genes, including MMP‐1 (rs1799750), MMP‐2 (rs243865, rs2285053, rs2241145), MMP‐9 (rs17576), and MMP‐12 (rs660599, rs2276109, and rs652438). Gene–gene interactions were employed using generalized multifactor dimensionality reduction (GMDR) methods. Results The frequency of rs17576 was significantly higher in IS patients than in controls (p = .033). Logistic regression analysis revealed the AG and GG genotypes of rs17576 to be associated with a higher risk for IS, with the odds ratio and 95% confidence interval being 2.490 (1.251–4.959) and 2.494 (1.274–4.886), respectively. GMDR analysis showed a significant SNP‐SNP interaction between rs17576 and rs660599 (the testing balanced accuracy was 53.70% and cross‐validation consistency was 8/10, p = .0107). Logistic regression analysis showed the interaction between rs17576 and rs660599 to be an independent risk factor for IS with an odds ratio of 1.568 and a 95% confidence interval of 1.152–2.135. Conclusion An MMP‐9 rs17576 polymorphism is associated with increased IS risk in the Han Hakka population and interaction between MMP‐9 rs17576 and MMP‐12 rs660599 is associated with increased IS risk as well.

Infrared (IR) stealth plays a vital role in the modern military field. With the continuous development of detection technology, multi-band (such as near-IR laser and middle-IR) compatible IR stealth is required. Combining rigorous coupled wave analysis (RCWA) with Deep Learning (DL), we design a Ge/Ag/Ge multilayer circular-hole metasurface capable of multi-band IR stealth. It achieves low average emissivity of 0.12 and 0.17 in the two atmospheric windows (3~5 μm and 8~14 μm), while it achieves a relatively high average emissivity of 0.61 between the two atmospheric windows (5~8 μm) for the purpose of radiative thermal management. Additionally, the metasurface has a narrow-band high absorptivity of 0.88 at the near-infrared wavelength (1.54 μm) for laser guidance. For the optimized structure, we also analyze the potential physical mechanisms. The structure we optimized is geometrically simple, which may find practical applications aided with advanced nano-fabrication techniques. Also, our work is instructive for the implementation of DL in the design and optimization of multifunctional IR stealth materials.

Objective To evaluate the use of Ilizarov external mini-fixation in the treatment of Monteggia fractures (dislocation of the radial head with an associated fracture of the proximal ulna) in children. Methods Children with proximal ulnar fracture were included and underwent fracture reduction surgery with Ilizarov external mini-fixators, followed by immobilization of the supinated forearm with plaster. The reduction was evaluated intra-operatively using arthrography. Mackay criteria were used to evaluate clinical outcomes at follow-up. Results A total of 15 children were included in the study. Mackay efficacy was 100 %, indicating excellent outcomes using the Ilizarov external mini-fixator. Conclusion Use of the Ilizarov external mini–fixator is particularly suitable in the treatment of children with comminuted and compression fractures of proximal ulna. It is easy to operate, low invasive and is worthy of promotion.

Abstract Total knee arthroplasty (TKA) is regarded as the most effective surgery for patients with later-stage arthritis of the knee, but the postoperative pain management for functional improvement of the knew is still a challenging task. This review discusses the mechanism by which the selective cyclooxyenase-2 inhibitors, which reduce the peripheral and central sensitization, decrease pain after TKA. This review also covers the protocols, safety, efficacy, and progress of cyclooxyenase-2 inhibitors in pre-emptive analgesia.

Abstract Total knee arthroplasty (TKA) is considered a cost-effective and efficacious treatment for patients with end-stage knee arthritis. Meanwhile, TKA has been regarded as one of the most painful orthopaedic surgeries. Pain control after TKA remains a challenging task. Many analgesic innovations are used to reduce the level of pain, but none has been proven to be the optimum choice till now. Multimodal analgesia incorporates the use of analgesic adjuncts with different mechanisms of action to enhance postoperative pain management. This approach is a preferable choice in relieving postoperative pain with minimum side effects. This paper aims to review pre-emptive analgesia for pain management in TKA. We reviewed the application of pre-emptive analgesia, its physiological mechanism, and the techniques.

Purpose: To investigate the characteristics of cervicocephalic spotty calcium (SC) and coronary atherosclerosis in patients with acute ischemic stroke (AIS) and to assess the predictive value of SC for coronary atherosclerosis using combined coronary and cervicocephalic CTA.Materials and Methods: Patients with AIS (n = 70) confirmed by brain MRI or CT and patients with asymptomatic carotid atherosclerosis (n = 58) confirmed by carotid ultrasonography were enrolled in our study. Subjects in both groups underwent combined coronary and cervicocephalic CTA. SC was used to evaluate cervicocephalic atherosclerosis. Coronary artery stenosis (CAS) ≥ 50% by segment and coronary artery calcium score (CACS) were used to evaluate coronary atherosclerosis. The SC frequency and the difference in coronary atherosclerosis between the two groups were compared, and the correlation between SC and coronary atherosclerosis was analyzed. Independent factors for CAS ≥ 50% were assessed via logistic regression analysis. Receiver operating characteristic curve analysis was performed to evaluate the added value of SC for predicting CAS ≥ 50%.Results: Both SC and the CACS were significantly higher in the Stroke group than in the Control group (total SC count: 6.83 ± 4.34 vs. 2.98 ± 2.87, P < 0.05; CACS: 477.04 ± 798.01 vs. 136.31 ± 205.65, P < 0.05). There were significant differences in the presence of CAS ≥ 50% (61.4 vs. 27.6%, P < 0.001). SC and coronary atherosclerosis were significantly correlated for both the CACS and CAS ≥ 50% (r = 0.746 and 0.715, respectively; P < 0.001). SC was an independent predictor for CAS ≥ 50%.Conclusion: SC correlates significantly with the CACS and could serve as an independent predictor of CAS ≥ 50% in patients with AIS, which suggests that combined cerebrovascular and cardiovascular assessments are of importance for such patients.

Climatic parameters fluctuate dynamically and their turbulences become more significant as the influence of the climate change increases. A robust model that is able to factor in the recent climate change for long-term climatic parameters forecasting is desired to strategically plan for future anthropogenic activities. In this study, two stochastic time series model, namely the seasonal auto-regressive integrated moving average (SARIMA) model and the artificial neural network (ANN) model are used to predict monthly mean temperature (Tmean), relative humidity (RH), wind speed (u) and pan evaporation (Epan) up to 12 months ahead. This study is conducted using data collected from three meteorological stations in the northern region of the Peninsular Malaysia. The stochastic models forecasted the Tmean with the highest accuracy, followed by RH, u and Epan. Besides, despite the increasing time step (from 1 to 12 months), the accuracy of the models remain consistent. However, both of the models are susceptible to the occurrence of extreme climates. In general, the SARIMA model performs better than the ANN model, probably attributed to its ability to consider the seasonality of the climatic data rather than depending solely on black-box computation.

Purpose. This study was conducted to investigate whether high body mass index (BMI) and triglycerides (TGs) were protective factors for reducing osteoporosis (OP) in patients with type 2 diabetes mellitus (T2DM). Participants and Methods. Seventy-nine patients (aged 20 to 81) with T2DM were included in the study. Basic information and blood indicators were collected. Bone mineral density was used to diagnose OP. Participants were grouped according to BMI (normal weight vs. overweight/obese participants), TG (normal TG vs. hypertriglyceridemia), and OP (non-OP vs. OP), and differences were compared between groups. Regression analysis was used to explore whether BMI or TG were independent factors affecting OP. Results. The proportions of OP in the overweight/obese and hypertriglyceridemic groups were significantly lower than those in the normal weight (30.0% vs. 69.0%; P=0.001) and normal TG (27.3% vs. 56.5%; P=0.010) groups. In the OP group, the BMI (24.8±3.4 vs. 26.6±2.5 kg/m2; P=0.009) was significantly lower than that in the non-OP group, and TG showed the same trend (1.30 (0.81) vs. 1.71 (1.1) mmol/L; P=0.020). Logistic regression in the crude model showed that the odds ratios (ORs) of OP in the overweight/obese and hypertriglyceridemic groups were 0.193 (95% CI: 0.071, 0.520) and 0.315 (95% CI: 0.119, 0.830) compared with those of the normal weight and normal TG groups. After adjusting for sex and smoking, the ORs were 0.204 (95% CI: 0.074, 0.567) and 0.242 (95% CI: 0.082, 0.709) for the overweight/obese and hypertriglyceridemic groups, respectively. After adjusting for all confounding factors, the ORs for these groups were 0.248 (95% CI: 0.083, 0.746) and 0.299 (95% CI: 0.091, 0.989), respectively. Conclusion. BMI and TG are independent protective factors against OP in patients with T2DM.

The aim of present work is study the effcet of Al-Nb-B _2 O _3 -CuO system reaction products on microstructure and properties of A356 alloy. B _2 O _3 powder, Al powder, CuO powder and Nb powder were used as raw materials to form Al–Nb–B _2 O _3 –CuO reaction system, and A356 alloy was refined by in situ reaction and near-liquidus casting. The size, phase, morphology and distribution of in situ reaction products were observed and analysed by optical microscope, x-ray diffractometer, scanning electron microscope and transmission electron microscope. The products of the Al–Nb–B _2 O _3 –CuO reaction system are mainly Al _3 Nb, NbB _2 and Al _2 O _3 . The ratio of the prefabricated block is 8:1:1:2 . When the prefabricated block was added at 2.5 wt%, the particle size of NbB _2 , Al _3 Nb and Al _2 O _3 is 0.5–1 μ m. The microstructure of A356 alloy is fine and uniform. The average grain size is refined to less than 20 μ m. Compared with untreated A356 alloy, the wear rate is reduced from 2.091% to 0.882%, and the average hardness of the alloy is increased from 41.3HRC to 58.2HRC.

The M site is occupied by 2/3 of TaV and 1/3 of CuII, whereby the average M—S distances are slightly longer than those of ordered BaTaS3. Additional second toe Weil osteotomy has been performed to reduce transfer metatarsalgia as some studies have highlighted the presence of second toe pain after isolated HV correction. 1, 2 There is a paucity of literature evaluating residual second toe pain after these procedures . We hypothesize that combined Scarf/Weil osteotomy will result in less residual second toe pain while achieving good functioning outcome. The aim of this study was to evaluate the presence of second toe pain after isolated scarf osteotomy versus combined Scarf/Weil osteotomy. Methods: Between January 2007 and June 2012, 439 patients underwent a scarf osteotomy for symptomatic hallux valgus at a tertiary hospital. Patients who underwent isolated scarf osteotomy were matched 1:1 to patients who had both Scarf and second toe Weil osteotomy. The following functional outcome scores were prospectively collected preoperatively and postoperatively at 6 and 24 months: American Orthopaedic Foot and Ankle Society (AOFAS) Hallux Metatarsophalangeal-Interphalangeal (MTP-IP) Scale, Visual Analog Scale (VAS), SF36-Physical Component Score (PCS) and Mental Component Score (MCS). Radiological outcomes such as the Hallux Valgus Angle (HVA) and Intermetatarsal Angle (IMA) were measured. Statistical analysis was performed using SPSS® 20.0 and significance was defined as a p-value = 0.05. The Student’s unpaired t-test was used to compare the radiological and functional outcomes between the two groups of patients, while the paired t-test was used to analyze improvement in pre- and postoperative outcomes within each group. Laboratory-created samples of methane hydrate (MH)-bearing media are a necessity because of the rarity and difficulty of obtaining naturally-occurring samples. The hypothesis that the inevitable heterogeneity in the phase saturations of the laboratory samples may lead to unreliable and non-repeatable results provided the impetus for this study, which aimed to determine the conditions under which maximum uniformity can be achieved. To that end, we designed four experiments involving different multi-stage cooling regimes (in terms of their duration and number of stages) to induce MH formation under excess-water conditions. In the absence of direct visualization capabilities, we analysed the experimental results by means of numerical simulation, which provided high-resolution predictions of the spatial distributions of the phase saturations in the cores and enabled the estimation of the parameters controlling the kinetic MH-formation behaviour through history-matching. Analysis of the numerical results indicated that, under the conditions of the experiments and with the design of the reactor, significant heterogeneities in phase saturation distributions were observed in all cases, leading to the conclusion that it is not possible to obtain cores with uniform phase saturation. Additionally, contrary to expectations, heterogeneities increased with the number of cooling stages and the duration of cooling, and this was attributed to imperfect insulation of the upper part of the reactor. A set of simulations involving perfect insulation of the reactor top confirmed the validity of this assumption: (a) predicting the formation of high-uniformity MH-bearing cores that became more homogeneous as the number of cooling stages and the length of the cooling period increased; and (b) providing important information for the improvement of the standard design of the experimental apparatus for the laboratory creation of MH-bearing cores using the excess water method.
