5. Optimal DaTQUANT Thresholds for Diagnostic Accuracy of Dementia with Lewy Bodies (DLB) and Parkinson's Disease (PD).

Abstract

Background: Quantitative thresholds are helpful to define an abnormal DaT SPECT in patients with suspected nigrostriatal degenerative diseases (NSDD). The optimal DaTQUANT threshold for diagnostic accuracy of DaT SPECT across combined movement and cognitive disorder populations has been previously described. Methods: We established optimal DaTQUANT thresholds that enhance the discrimination between dementia with Lewy bodies (DLB) and non-DLB dementia types, as well as between Parkinsonian syndromes (PS) and conditions not characterized by nigrostriatal degeneration (non-PS). Results: Data from a total of 303 patients were used in this retrospective analysis. Posterior putamen of the more affected hemisphere (MAH) was shown to be an accurate single-variable predictor for both DLB and PS and was comparable to the most accurate multi-variable models. Conclusions: Automated quantification with DaTQUANT can accurately aid in the differentiation of DLB from non-DLB dementias and PS from non-PS. Optimal thresholds for assisting a diagnosis of DLB are striatal binding ratio (SBR) ≤ 0.65, z-score ≤ −2.36, and a percent deviation ≤ −0.54 for the posterior putamen of the MAH. Optimal posterior putamen thresholds for assisting a diagnosis of PS are SBR ≤ 0.92, z-score ≤ −1.53, and a percent deviation ≤ −0.33, which are similar to our previously reported posterior putamen threshold values using a blended patient pool from multiple study populations. Abstract

In the human brain, iron is more prevalent in gray matter than in white matter, and deep gray matter structures, particularly the globus pallidus, putamen, caudate nucleus, substantia nigra, red nucleus, and dentate nucleus, exhibit especially high iron content. Abnormally elevated iron levels have been found in various neurodegenerative diseases. Additionally, iron overload and related neurodegeneration may also occur during aging, but the functional consequences are not clear. In this study, we explored the correlation between magnetic susceptibility--a surrogate marker of brain iron--of these gray matter structures with behavioral measures of motor and cognitive abilities, in 132 healthy adults aged 40-83 years. Latent variables corresponding to manual dexterity and executive functions were obtained using factor analysis. The factor scores for manual dexterity declined significantly with increasing age. Independent of gender, age, and global cognitive function, increasing magnetic susceptibility in the globus pallidus and red nuclei was associated with decreasing manual dexterity. This finding suggests the potential value of magnetic susceptibility, a non-invasive quantitative imaging marker of iron, for the study of iron-related brain function changes.

24. The Impact of Aging on Decision Making

Abstract

Aging is associated with many social, cultural, cognitive and neural changes. Given this, it should not be a surprise that decision making preferences and abilities also change as we age. However, it can be hard to determine whether the age-related changes we are seeing are due to biological or social factors. Here, I discuss three studies examining what exactly causes these age-related changes across several types of decisions. In chapter 1, I probe how decision-making changes as we get older through the use of both a novel task in the aging context, the Horizon task. The Horizon task allows us to measure exploration behavior in the context of a decision making game. Specifically, it allows us to decouple two kinds of exploration, directed and random exploration to show how aging impacts random, but not directed exploration. In Chapter 2, I use Transcranial Magnetic Stimulation (TMS) on older adults performing the Horizon task to reduce activity in the frontal pole to determine it’s role in supporting exploration. In Chapter 3, I used remote behavioral testing in a memory and decision-making task to expand our understanding of age-related decision-making alterations and explore the validity of remote testing methods for detecting age-related effects. Taken together, these studies show how age-related neural and cognitive changes impact decision making across tasks and contexts.

42. System Identification and Optimal Control of Complex Brain Networks

Abstract

The communicability between topologically associated brain regions may provide a globally connected architecture to support communication through coherence. In chapter 1 of the dissertation, connectivity statistics are discovered by inverting a generative model of dynamic functional connectivity (FC) based on the dynamic communicability between brain regions. Dynamic FC and communicability are related by a set of affine transformations that preserve the group structure of Hermitian positive-definite (HPD) matrices. Infinitesimal transformations are integrated over time on a manifold of HPD matrices, describing parametric geodesic curves that represent dynamic changes in communicability. A hierarchical geodesic regression model is fit to a dataset involving 30 participants who performed a modified Eriksen Flanker task, to learn within- and between- subject connectivity statistics. Differences in dynamic FC are highly predictive of trial characteristics, as a linear classifier can predict whether participants respond with left/right hand and whether the response is correct or incorrect. The remaining chapters of the dissertation present new methods that enable causal investigations of functional networks using non-invasive brain stimulation. Chapter 2 simulates a transcranial electrical stimulation (TES) experiment, using a closed-loop feedback controller to drive the system into a priori target states (i.e., activating or deactivating a set of target ROIs) by identifying an optimal stimulation policy that balances difference performance criteria, such as stimulation dose and state error. The optimal control algorithm minimizes an expected future cost function to reduce stimulation dose for participants while achieving the desired effect - driving the system into different functional networks such as the Default Mode Network (DMN) and Cognitive Control Network (CCN). 43. Midfrontal Theta As A Biomarker of Trait Worry: Insights from Resting State and a Reinforcement Learning Task

Abstract

Anxiety disorders account for billions in unnecessary medical costs and care (Keesler & Greenberg, 2002). Worry is a defining feature of Generalized Anxiety Disorder (one of the most common anxiety disorders) as well as a comorbid syndrome with many other psychopathologies (i.e., depression, PTSD, etc.). Given this the need to understand the pathophysiology of worry and its impact on brain function, in particular, is paramount in informing future intervention. The present study aimed to examine whether midfrontal theta electroencephalography (EEG) represented an adequate biomarker for understanding the pathophysiology of trait worry. Moreover, this study sought to examine the possible links between trait worry, midfrontal theta, and emotional contrast avoidance (the tendency to use worry to dampen and avoid negative emotional shifts). This research recruited a sample of college students stratified by trait worry level (low, mid, and high) to examine midfrontal theta activity at rest and during a reinforcement learning task. These findings indicate that increased midfrontal theta at rest is significantly predicted by both high trait worry and high contrast avoidance scores. In addition, this study replicated other behavioral findings that demonstrate midfrontal theta’s role in signaling for greater cognitive control during reinforcement learning tasks. Taken together the findings suggest midfrontal theta as a viable biomarker of trait worry. Future directions and limitations will be discussed.

Abstract

The purpose of the study is to investigate whether there are age-related differences in locomotion due to changes in presence of vision, optic flow speed, and lateral flow asymmetry using virtual reality technology. Gait kinematics and heading direction were measured using a three-dimensional motion analysis system. Although older and younger adults were affected differentially by the availability of vision, a greater dependence on optic flow information in older adults during walking was not found. Linear relations were observed between walking performance and flow speed as well as heading direction and flow asymmetry. The findings suggest that the ability to integrate optic flow information into the multimodal system for assessment of walking speed and heading direction is comparable in older and younger adults. Abstract

Two experiments were carried out to investigate contributions of voluntary manual exploration and vision to rod-bisection performance when the rod was placed in different spatial positions. Analysis showed both voluntary manual exploration and vision contributed to the accuracy of rod-bisection performance in healthy adults (Exp. 1:12 men, 13 women; Exp. 2:10 men, 11 women). Advantages of voluntary manual exploration were related to rod position and initial search direction. Results are discussed with regard to ecological approach to action and perception, two cortical systems (ventral and dorsal streams) for perception and control of actions, and the hemispheric activation hypothesis.
